We introduce a low-fidelity, low-cost, reusable training model for the lateral canthotomy procedure. We believe that this trainer has the potential to improve clinicians’ knowledge and skill of the procedure, especially when cost or access to higher-fidelity trainers is prohibitive.

556. The Fiscal Year 1993 Bush Budget: Still Not Tackling the Nation's Problems.

Abstract

On January 29, 1992, the Bush Administration unveiled its fiscal year 1993 budget. An examination of the budget reveals a substantial gap between the administration's rhetoric concerning the budget and what the budget actually contains. An analysis reveals a budget that continues to give priority to defense over domestic spending, one that favors the very wealthy, one that has a lack of benefits to low-income families, and one that contains little to boost the economy in the short term. The budget also risks retarding long-term growth rather than enhancing it and proposes that legislation be enacted placing a ceiling on total entitlement program costs each year. The budget weakens the finances of state and local governments. These issues, along with the Administration's proposal to merge 24 disparate programs into a mega-block grant to states, are discussed in the chapters of this document. Appended are tables that indicate the budget proposal's impact on the states, proposed changes in low-income funding (FY 1992-FY 1993), and a "New York Times" editorial. (RR)

589. Unchanged Priorities: The Fiscal Year 1992 Bush Budget.

Abstract

This report analyzes the impact of the 1992 budget proposed by the Bush Administration on Americans of different income levels. Several of the proposals merit consideration, but the overall impact of the budget would continue to favor upper-income individuals and place lower-income individuals increasingly at risk. The following findings are highlighted: (1) the budget as a whole would transfer resources up the income scale and widen disparities between low-income and high-income groups that are already greater than at any time since the end of World War II; (2) the budget does not respond to the current recession by improving unemployment benefits or assisting the victims of the recession; (3) the budget's overall treatment of programs for low-income groups is mixed, however, with the number of programs receiving decreases being greater than the number receiving increases; (4) new budget procedures constrain Congress's ability to reshape budget priorities; and (5) the proposal to combine $15 billion in domestic programs into a massive block grant to states places many poverty programs at risk. Awake tracheostomy is rare in the pediatric population. We describe the case of a 10-year-old male who underwent awake tracheostomy due to airway obstruction from an oropharyngeal rhabdomyosarcoma. Given the varying medical understanding and communication skills in children, advanced planning and interdisciplinary collaboration are essential to keep the patient calm and safe during awake tracheostomy.

592. Observing the Surface Termination of LaScO3Perovskite Using Solid-State Nuclear Magnetic Resonance

Abstract

Materials with well-defined surfaces are drawing increased attention for the design of bespoke catalysts and nanomaterials. Gaining a detailed understanding of the surfaces of these materials is an important challenge, which is often complicated by surface polymorphism and dynamic restructuring. We introduce the use of surface-enhanced NMR spectroscopy for the observation of such surfaces, focusing on LaScO3as an example. We show that double-resonance NMR experiments correlating surface oxygen and probe molecules to the 139La and 45Sc nuclei at the surface reveal the material to be terminated by a ScOxmonolayer. Surface-selective 17O and 45Sc NMR experiments further showed the material to be hydroxyl terminated and that the surface may be prone to dynamic restructuring as a result of moisture exposure. Perhaps most interestingly, surface-selective 139La NMR experiments revealed the existence of previously undetected surface lanthanum defects, suggesting that surface-enhanced NMR may be useful as a guide in the synthesis of defect-free surfaces in the design of various nanomaterials.

593. Drifting Apart: New Findings on Growing Income Disparities Between the Rich, the Poor, and the Middle Class.

Abstract

Income disparities between wealthy and other Americans widened significantly in the 1980s. The most affluent Americans reaped exceptionally large income gains during that decade, while middle-income Americans gained little, and low-income Americans fell further behind. The richest one percent of all Americans now receive as much income after taxes as the bottom 40 percent of Americans combined. The share of national income going to middle-income Americans is now lower than at any time since the end of World War II. One factor behind these widening income disparities is a striking increase in the amount of capital gains income received by the wealthiest Americans. In 1990, households in the top 1 percent of the population are projected to receive an average of $175,000 in capital gains income. This is $92,000 more than they received in 1980, after adjusting for inflation. By contrast, those in the bottom 90 percent of the population are projected to receive average capital gains income of $300 this year, just $12 more than in 1980. A reduction in capital gains taxes would widen these already large income disparities still further. A shift in Federal tax burdens has also contributed to the growing disparities. Wealthy households now pay a smaller percentage of their income in taxes than they did in 1980; lower income households now pay a larger proportion. The trend toward income disparities is a relatively recent phenomenon. Between 1950 and 1970, income disparities narrowed. This pattern reversed itself in the 1970s, as disparities began to grow modestly. Disparities then grew more sharply in the 1980s. Statistical data are included on three tables and two graphs. 594. Fulfilling Work's Promise: Policies To Increase Incomes of the Rural Working Poor.

Abstract

Poverty is an important antecedent factor affecting education. One of the most striking characteristics of rural poverty is the extent to which the rural poor work. About 65% of poor nonmetro families have at least one worker, compared to 54% of poor metro families. This report focuses on federal and state policy reforms that would assure poor rural workers a reasonable return for their efforts. The federal earned income tax credit (EITC) assists low-income families that have earnings and a child living at home, and the credit is "refundable" for families whose credit exceeds its income tax liability. Current proposals would expand the EITC for families with several children or young children. An outreach program is needed to inform low-income working families that they must file a federal tax return to receive the EITC payment. Raising the minimum wage to $5.02 in 1991 would restore its value lost to inflation during the 1980s and would benefit a majority of poor workers. Other strategies to aid the nonmetro working poor are: (1) making the federal dependent care tax credit refundable; (2) removing working poor families from state income tax rolls or establishing state EITCs; (3) restoring unemployment insurance coverage, which has dropped sharply in recent years; (4) extending Medicaid coverage to children in poor working families; and (5) instituting outreach programs to inform poor working families of their eligibility for food stamps.
