Lung cancer is the most common cause of death worldwide and result in 1.4 million deaths per year. Lung cancer is the fifth most common cancer in Iran, with rates of 4.7 to 9.2 per 100 thousand populations. Despite the low incidence, survival is not promising. Identification of biological markers and their application in diagnosis of cancer could lead to early diagnosis and therefore longer survival rate in patients. In this study, we evaluated the association of SNP rs11614913 in mir-196a2 with risk of non-small cell lung cancer (NSCLC) in population of southern Iran. SNP rs11614913 in miR-196a2 was assayed in 95 lung cancer patients and 100 controls. Polymorphism was determined by RFLP-PCR. The PCR product was digested with TaaI (5’ ACN^GT) which generated two bands (187bp and 196bp) in TT, three bands (187bp, 196bp and 383bp) in TC and one bandn(383bp) in CC individuals. Analysis of electrophoretic pattern of digestion products revealed that the frequencies of C allele for SNP rs11614913 miR-196a2 were 0.695 in patients and 0.76 in controls. As a result, the genotype frequencies of TT, TC and CC were 0.074, 0.463 and 0.463 in patients and 0.06, 0.36 and 0.58 in controls. The statistical analysis indicates the presence of Hardy-Weinberg equilibrium between the two alleles of the gene in the population studied (p>0.05). Based on the results in this study, no significant association between SNP rs11614913 and susceptibility to NSCLC was found. However, male participants who possess TC/TT genotypes showed high risk for NSCLC compared to those possessed CC genotypes (recessive genetic model).

Background: Infection with Echinococcus granulosus causes hydatidosis in human and ruminants. With regards to the high prevalence of hydatidosis in Iran, dealing with this disease is important in terms of public health. Objective: The aim of this study was to construct pEGFP-ChEgTrp as DNA model for multi-epitope vaccine against Echinococcus granulosus Methods: This experimental study was conducted in the Razi Vaccine & Serum Research Institute, Karaj in 2013. Initially, epitopes stimulating the host immune response were predicted by IEDB Database and the coding sequences were made. The sequences were amplified by PCR. The PCR products were cloned into pEGFP-N1 vector after digestion with XhoI restriction enzyme. The bacteria containing recombinant plasmid were evaluated using Colony PCR, agarose gel electrophoresis and sequencing methods. Findings: Four peptides with 10 linear epitopes were predicted in EgTrp antigen. The nucleotide sequence coding ChEgTrp was amplified by PCR using specific primers and a 270 bp fragment was obtained. This fragment was cloned into pEGFP-N1 vector and the recombinant plasmid was confirmed by Colony PCR and agarose gel electrophoresis. For final confirmation, the recombinant plasmid was sequenced and the pEGFP-ChEgTrp was constructed. Conclusion: The ChEgTrp was successfully cloned into the pEGFP-N1 vector and this plasmid can be used to design DNA vaccines.

Breast cancer is the most common cancer worldwide and is the second leading cause of death in women after lung cancer. Micro RNAs (miRNAs) are among endogenous factors which are involved in many types of cancers, including breast cancer. Single nucleotide polymorphism (SNP) in the miRNAs, might change their biological activities such as their effects on oncogenes and tumor suppressor genes. Therefore some of miRNA’s SNPs are associated with the risk of different types of cancer, including breast cancer. The aim of this study was to evaluate the correlation between SNP rs11614913 in mir196a2 and the risk of breast cancer. SNP rs11614913 in hsa-miR-196a2 analyzed in 100 breast cancer cases and 100 controls in women living in southern Iran. Polymorphism was identified by the PCR-RFLP method. The PCR product was digested with TaaI restriction enzyme which produced two bands with the length of 187bp and 196bp. The data from this case –control study were analyzed using odds ratios (ORs) with 95% confidence intervals (CIs) to reveal the associations of SNPs in miRNAs with breast cancer susceptibility. The C allele frequencies in patients and controls for miR-196a2 were 0.74 and 0.75, respectively. The genotype frequencies of TT, TC, and CC were 0.05, 0.42, and 0.53 for the patients and 0.06, 0.38, and 0.56 for the controls, respectively. Statistical analysis showed that the genetic frequencies for this SNP were in Hardy-Weinberg equilibrium (p>0.05). The results of this study indicated no significant association between SNP rs11614913 in mir196a2 and the risk of breast cancer.

ABSTRACT Background & purpose: Hydatidosis is a zoonotic disease that caused by infection with the larvae of Echinococcus granulosus. Different antigens produced in larval stage of this parasite that recombinant vaccine base these antigens created significant immunity in infected animals. One of the important antigens is p14-3-3 that it's recombinant antigen created considerable immunity in mouse models. In this study according to the high immunity of antigen epitopes region the coding sequence of T-cell epitopes of P14-3-3 was cloned into pEGFP-N1vector in order to produce an effective DNA vaccine model to stimulate high level of Th1 immune response. Material and method: In this study bioinformatics tools were used to prediction of linear T-Cell epitopes of Echinococcus granulosus P14-3-3 &zeta antigen. The nucleotide coding sequence of these epitopes was synthesized by PCR. the ampliqon was digested with XhoI restriction enzyme and cloned into pEGFP–N1 vector That has been purificated by modified sambrook method with CaCl2 and PEG6000..Positive colony was selected by direct colony PCR and confirmed by the sequencing.and evaluation of it's expression in Eukaryotic cells was done by transformed to CHO cell line with electroporation. Results: Linear T-cell epitopes of Echinococcus granulosus P14-3-3 after prediction,synthesis and amplification wae successfully cloned into pEGFP-N1 vector that purificated by new method with maximum vector and minimum RNA concentration.The expression of new constract in CHO cell line as a eukaryotic cells achivment by fluorescent microscope and will be used as a DNA vaccine model to evaluation immuno response in mouse models. Free vibration analysis of beams with single delamination undergoing bending-torsion coupling is made, using traditional finite element technique. The Galerkin weighted residual method is applied to convert the coupled differential equations of motion into to a discrete problem, where, in addition to the conventional mass and stiffness matrices, a delamination stiffness matrix, representing the extra stiffening effects at the delamination tips, is introduced. The linear eigenvalue problem resulting from the discretization along the length of the beam is solved to determine the frequencies and modes of free vibration. Both “free mode” and “constrained mode” delamination models are considered in formulation, and it is shown that the continuity (both kinematic and force) conditions at the beam span-wise locations corresponding to the extremities of the delaminated region, in particular, play a great role in “free mode” model formulation. Current trends in the literature are examined, and insight into different types of modeling techniques and constraint types are introduced. In addition, the data previously available in the literature and those obtained from a finite element-based commercial software are utilized to validate the presented modeling scheme and to verify the correctness of natural frequencies of the systems analyzed here. The paper ends with general discussions and conclusions on the presented theories and modeling approaches.

The aim of this study was to assess the effect of GEI on grain yield of barley advanced lines and exploit the positive GEI effect using AMMI and SREG GGE biplot analysis. Therefore, 18 lines were evaluated at five research station s (Ghachsaran, Mogan, Lorestan, Gonbad and Ilam) of Dryland Agricultural Research Institute (DARI), in the semi-warm regions in Iran, in 2012, 2013 and 2014 cropping seasons under rain-fed conditions. Analysis of variance showed that grain yield variation due to the environments, genotypes and GE interaction were highly significant (p<0.01), which accounted for 68.9%, 9.3% and 22.7% of the treatment combination sum of squares, respectively. To determine the effects of GEI on yields, the data were subjected to AMMI and GGE biplot analysis. The first five AMMI model terms were highly significant (p<0.01) and the first two terms explained 59.56% of the GEI. There were two mega-environments according to the SREG GGE model. The best genotype in one location was not always the best in other test locations. According to AMMI1 biplot, G2, G4, G5 and G6 were better than all other genotypes across environments. G2 was the ideal genotype to plant in Gachsaran. It seems that Ghachsaran is the stable environment between the environments studied and next in rank was Gonbad. In finally, the ATC method indicated that G1, G3, G4 and G6 were more stable as well as high yielding. [ABSTRACT FROM AUTHOR]

Postoperative vasoplegic syndrome (VS) is characterized by low systemic vascular resistance, normal or elevated cardiac output, and poor response to volume expansion. The incidence of VS after cardiac surgery requiring cardiopulmonary bypass is about 20%. Sometimes, VS becomes refractory and initial treatments do not work, rendering treatment a great challenge. In this study, we describe a young male patient with endocarditis undergoing tricuspid valve replacement. When being weaned off cardiopulmonary bypass, the patient experienced VS. The patient’s blood pressure did not increase after the administration of a high dose of epinephrine and norepinephrine. Therefore, he was commenced on a low dose of vasopressin and gradually his blood pressure reached the normal range. Although the standard management of VS is a high dose of vasopressors, this patient was refractory to a combination of epinephrine and norepinephrine; only a vasopressin infusion was able to treat the patient. Eventually, he was weaned from bypass and the operation was terminated satisfactorily. Thereafter, the patient passed the recovery period in the cardiac intensive care unit and was discharged. It seems that vasopressin is an excellent option in refractory vasoplegia with minimal response to other vasopressors.

Objectives To compare the peri-implant bone healing between TiZr implants with hydrophilic SLActive and hydrophobic SLA implant surface in patients receiving anticoagulants, to assess the implant survival and success rate, as well as to evaluate whether small-diameter TiZr implants could be used in patients on OAT in order to avoid augmentation procedures. Material and methods A total of 80 small-diameter tissue-level TiZr implants with SLActive and SLA surfaces were placed in 20 anticoagulated patients, following the 'split-mouth' study design. Implant stability was measured up to the third postoperative month by resonance frequency measurements ( RFA). One-year implant survival and success rate were evaluated. Results After one year, 100% implant survival and success rate were observed. A significant decrease in ISQ comparing to baseline values was noted in the SLActive group from the first postoperative week, and in the SLA group, from the 3rd week after the surgery. In both groups, a statistically significant decline in ISQ was observed between second and third postoperative week. No significant differences in ISQ values between SLActive and SLA implants were noted, at any time point. Conclusions Titanium-zirconium small-diameter implants with SLActive and SLA surface predictably achieve and maintain adequate bone tissue integration in patients receiving anticoagulants. OAT appears to influence the bone healing events resulting in lower ISQ in the end of 3-month period in comparison with baseline values, although without compromising implant stability. The dynamic analysis of prestressed, bending-torsion coupled beams is revisited. The axially loaded beam is assumed to be slender, isotropic, homogeneous, and linearly elastic, exhibiting coupled flexural-torsional displacement caused by the end moment. Based on the Euler-Bernoulli bending and St. Venant torsion beam theories, the vibration and stability of such beams are explored. Using the closed-form solutions of the uncoupled portions of the governing equations as the basis functions of approximation space, the dynamic, frequency-dependent, interpolation functions are developed, which are then used in conjunction with the weighted residual method to develop the Dynamic Finite Element (DFE) of the system. Having implemented the DFE in a MATLAB-based code, the resulting nonlinear eigenvalue problem is then solved to determine the coupled natural frequencies of illustrative beam examples, subjected to various boundary and load conditions. The proposed method is validated against limited available experimental and analytical data, those obtained from an in-house conventional Finite Element Method (FEM) code and FEM-based commercial software (ANSYS). In comparison with FEM, the DFE exhibits higher convergence rates and in the absence of end moment it produces exact results. Buckling analysis is also carried out to determine the critical end moment and compressive force for various load combinations.

Autogenous particulate bone grafts are being utilized in oral implantology for minor grafting procedures. This study aimed to investigate the influence of the bone-harvesting technique, donor age, and donor site on proliferation and differentiation of human primary osteoblast-like cells in the cell culture. Autogenous bone particles (20 samples) were harvested from the maxilla and mandible during surgery using two different protocols, and two types of particulate bone grafts were collected: bone chips and bone sludge. Bone samples were cultured in growth medium and, after 2 to 3 weeks, the cells that grew from bone grafts were cultured in the normal and osteogenic medium for 0, 4, 7, and 20 days. DNA, alkaline-phosphatase (ALP), calcium-content measurements, and Alizarin red/toluidine blue staining were performed. Data were analyzed by repeated-measures analysis of variance with Bonferroni test. The level of statistical significance was set at 5% (P < .05). Total DNA, ALP, and calcium content were significantly higher for the bone chip samples compared to the bone sludge samples. Total DNA and ALP content were significantly higher for the patients in age group 1 (≤ 60 years) compared to age group 2 (> 60 years) and was significantly higher for mandibular samples than maxillary samples on day 20. However, the calcium measurement showed no significant difference concerning donor age and donor site. Data analysis revealed that harvesting technique (bone chips vs bone sludge), donor age (≤ 60 years vs > 60 years), and donor site (maxilla vs mandible) influenced the osteogenic potential of the collected particulate bone graft. The bone chips were superior in terms of osteogenic efficacy and should be considered a suitable option for particulate bone graft collection.

Purpose: There is a substantial need to perform studies to evaluate crestal bone loss (CBL) and implant success when using a newly introduced low-speed drilling protocol. Therefore, this study aimed to evaluate the mean CBL and implant success rate by placing implants utilizing two drilling protocols, ie, standard and low-speed drilling protocols., Materials and Methods: A randomized controlled clinical trial was carried out in patients who required dental implants to restore their esthetics and function. The patients were recruited from a university hospital (Academic Centre for Dentistry Amsterdam [ACTA], the Netherlands). Based on the inclusion criteria, patients were randomized to two study groups: (1) control group, standard drilling protocol; and (2) test group, low-speed drilling protocol without saline irrigation. The mean CBL and the implant success rate were evaluated after 12 months of implant placement., Results: Twenty-three patients (15 men and 8 women with a mean age of 57.5 ± 10.7 years) contributed to the study. Forty Camlog screw-line implants were placed (20 implants per study group). After 12 months of implant placement, the mean CBL of implants placed with the standard protocol and the low-speed protocol was 0.206 ± 0.251 mm and 0.196 ± 0.178 mm, respectively. No statistically significant difference could be recorded among both groups (P = .885). Concerning implants placed in the maxilla, the standard drilling group and low-speed drilling group showed a mean CBL of 0.252 ± 0.175 mm and 0.251 ± 0.175 mm, respectively, compared with 0.173 ± 0.210 mm and 0.141 ± 0.172 mm in the mandible, with no significant difference. The success rate of dental implants at 12 months was 95% in the control group and 90% in the test group., Conclusion: Within the limitations of this study, it can be concluded that implants placed with the low-speed drilling protocol without saline irrigation exhibited a similar CBL compared with implants placed with the standard drilling protocol. However, a higher success rate was recorded especially in type 1-quality bone for the control group compared with the test group. Further randomized clinical trials with greater sample sizes and extended follow-up times should be performed to obtain stronger evidence and a better understanding of the influence of drilling speed on mean CBL and long-term implant success.

Purpose: To compare the fit and clinical performance of screw-retained monolithic zirconia implant fixed dental prostheses (FDPs) based on either intraoral optical scanning (IOS) or conventional impressions., Materials and Methods: Patients with two posterior tissue-level implants (Straumann Regular Neck) replacing two or three adjacent teeth were recruited. Impressions were taken with both IOS (True Definition Scanner, 3M ESPE) and a conventional (polyether) pick-up impression. Double-blind randomization was performed after impression-taking, and patients were to receive an FDP based on either the digital or the conventional impression. The fit was evaluated, and the time required for adjustments was recorded. Additionally, survival and technical complication rates with a follow-up of 1 year were documented., Results: A total of 38 patients requiring 45 FDPs were included: 24 FDPs in the test (IOS) and 21 in the control (conventional) group. The average adjustment time was 6.92 minutes (SD: ± 10.84, range: 0 to 49 minutes) for digital vs 12.38 minutes (SD: ± 14.52, range: 0 to 54 minutes) for conventional impressions (P = .090). A proper fit (no adjustments) was achieved in 33.3% of the digital and 28.6% of the conventional group. Forty-two FDPs could be placed within the two planned appointments, and 3 FDPs exhibited an unacceptable fit and required an extra appointment. Eight technical complications occurred during the first year of function. The overall restoration survival rate was 100%., Conclusion: The clinical fit of CAD/CAM FDPs based on digital impressions is comparable to conventional impressions. Screw-retained monolithic zirconia FDPs on Ti-base abutments show low major complication and survival rates in the short term.

Objective: This cross-sectional study aims to investigate the effect of the cause of missing teeth on the survival and subjective success of dental implant treatment (DIT) in young patients with missing teeth due to non-congenital causes (tooth loss) in comparison to patients with missing teeth because of congenital causes (hypodontia and oligodontia)., Material and Methods: All patients were asked 7 questions to extract information about the survival and subjective success of DIT. Implant survival function was designed using the Kaplan-Meier analysis. Differences in implant success outcomes were studied using binary logistic regression analysis., Results: One hundred ten patients aged 18 to 40 years old were included, whereof 32 patients with tooth loss, 25 patients with hypodontia and 53 patients with oligodontia. In the tooth loss group, implant survival reached 96.9%; in the hypodontia group 96.0%; and in the oligodontia group 88.7%. Regarding subjective implant success, patient satisfaction was significantly higher (p < 0.040) among patients with congenital missing teeth in comparison to patients with tooth loss. Other implant success components showed no statistically significant difference (p > 0.050) between the groups., Conclusion: The cause of missing teeth does not influence implant survival. However, the cause of missing teeth does have a significant impact on patient satisfaction (implant success), ascertaining young patients with congenital missing teeth as more satisfied of DIT than young patients with tooth loss., Clinical Relevance: Young patients with tooth agenesis and with an increased number of missing teeth are more content about the treatment with dental implants than patients with tooth loss. Objectives: The purpose of this randomized clinical trial was to compare the required time of potential clinical adjustments of posterior screw-retained monolithic zirconia implant retained crowns based on intraoral optical scanning (IOS) or conventional impressions., Materials and Methods: Patients with posterior tissue level implants (Straumann RN) replacing solitary teeth were recruited. Of all patients, impressions were taken with both an IOS (3M ™ TDS) and a conventional (polyether) pick-up impression. Randomization was performed after impression taking and patients were to receive either a crown based on the digital or the conventional impression. The time required for adjustments at placement was recorded. Additionally, restoration survival and mechanical complications with a follow-up of one year were documented., Results: Thirty two patients with 45 implants were included: 23 restorations in the test (IOS) and 22 in the control (conventional) group. The average adjustment time was 3.35 min (SD ± 3.38, range: 0-11 min) for the digital versus 6.09 min (SD ± 4.63, range: 0-18 min) for the conventional impressions (p = .039). A proper fit (no adjustments required) was achieved 39,1% in the digital and 18,2% conventional group respectively. All 45 restorations could be placed within the two planned appointments and only two minor mechanical complications occurred during the first year of function., Conclusions: The use of IOS resulted in shorter adjustment times at try-in than conventional impressions for solitary CAD/CAM implant restorations. Purpose: When encountering a buccal bone defect during implant placement, guided bone regeneration (GBR) is a well-accepted method for bone reconstruction. However, it is still unclear if the esthetic and patient-reported outcomes are comparable to implants placed in native bone. The purpose of this prospective trial was to compare implants placed with a GBR procedure for a small (≤ 4 mm) buccal defect with implants placed completely in native bone (control)., Materials and Methods: Patients were allocated to the GBR group or control group during implant placement in the esthetic zone. Implants were placed after at least 12 weeks of healing of the extraction sockets. A buccal bone defect of ≤ 4 mm resulted in allocation to the GBR group. Follow-up was performed until 12 months after loading. Outcome measurements were as follows: esthetic scores, patient-reported outcome measurements, implant survival and complications, clinical indices, and radiographic measurements., Results: In total, 45 patients were included, of which 23 underwent a GBR procedure after implant placement, and in 22 patients no GBR was necessary. No significant differences in esthetic outcomes were seen between the two groups. At the final follow-up, a mean pink esthetic score (PES) of 7.8 (SD: 1.5) was seen for the GBR group and 8.4 (SD: 1.4) for the control group. Regarding the white esthetic score (WES), a mean of 9.1 (SD: 1.0) was found for both groups. Patients of both groups were equally satisfied with their mucosa and crown. A mean visual analog score (VAS) for the soft tissues of 8.6 (SD: 1.0) in the GBR group and 8.8 (SD: 0.9) for the control group was noted. A mean VAS of 9.2 (SD: 0.8) was noted for the crown in the GBR group and 8.6 (SD: 2.0) in the control group. Implant survival was 100%, and there were no significant differences in complications, plaque/bleeding/gingiva indices, width of attached mucosa, and marginal bone loss., Conclusion: Implants placed in the esthetic zone with GBR or complete native bone coverage showed successful esthetic outcomes and satisfied patients with predictable clinical and radiographic parameters after more than 1 year of loading. Within the limits of this study, GBR for a small buccal bone defect seems to be a reliable technique with good esthetics and patient-reported outcomes.

Purpose: Autogenous bone grafts are considered a "gold standard." The success of autografts mainly depends on their ability to promote an osteogenic response. The aim of this study was to collect autogenous bone during implant osteotomy preparation using two different drilling protocols and to evaluate and compare the proliferation and differentiation ability of the collected bone particles., Materials and Methods: Autogenous bone particles were harvested from 20 patients during implant osteotomy preparation using two different drilling protocols: (1) standard drilling protocol with saline irrigation (according to the manufacturer's recommendation) and (2) low-speed drilling protocol without saline irrigation (speed < 200 rpm). Bone samples collected were cultured in growth medium, and after 2 to 3 weeks, cells that grew out from bone grafts were cultured in the normal medium as well as in osteogenic medium for days 0, 4, 7, and 20. Scanning electron microscopy, alizarin red/toluidine blue staining, DNA, ALP, and calcium content measurements were performed. Repeated measures analysis of variance (ANOVA) with Bonferroni's test was employed to analyze the data of this study., Results: The total DNA content was significantly higher for the low-speed drilling samples compared with the standard drilling on day 4 (P < .05), day 7 (P < .01), and day 20 (P < .001) in the normal medium and on day 7 (P < .01) and day 20 (P < .01) in the osteogenic medium. Besides, calcium measurements and mineralized matrix formation observed with alizarin red/toluidine blue staining were significantly higher for the low-speed drilling group compared with the standard drilling group., Conclusion: Osteogenic efficacy (differentiation and proliferation) of autogenous bone particles collected using low-speed drilling was superior compared with standard drilling samples.

Purpose: This cohort study evaluated patient satisfaction for maxillary implant-retained overdentures (IODs) on two implants up to 4 years and assessed the treatment effect over time., Materials and Methods: Patients encountering problems with their conventional dentures were included and received maxillary IODs on two titanium-zirconium implants and ball anchors in the canine area. Patient satisfaction was assessed using the oral health impact profile (OHIP-20E) questionnaires both for dentures and IODs. Two months after insertion of IODs (baseline), the patients chose the preferred overdenture design with full or reduced palatal coverage. OHIP-20E questionnaires were followed according to the individual choice at 1 and 4 years, and outcomes were compared with baseline., Results: Sixteen out of 21 patients were evaluated at a mean follow-up of 4 years (range: 2.4 to 4.8 years). There was no significant difference in the OHIP domains for IODs at 1 year (OHIP&#95; total &#95; 1y : 9.5, SD: 13.0) and 4 years (OHIP&#95; total &#95; 4y : 14.2, SD: 19.1) compared with baseline (OHIP&#95; total &#95; BL : 12.4, SD: 14.7). Patients were most satisfied with social disability both for IODs (OHIP&#95; BL : 6.0, SD: 7.6; OHIP&#95; 1y : 3.4, SD: 5.4; OHIP&#95; 4y : 5.7, SD: 9.5) and dentures (OHIP&#95; CD &#95; old : 28, SD: 29.7; OHIP&#95; CD &#95; new : 25.4, SD: 28.67). Patients were least satisfied with functional limitation both for IODs (OHIP&#95; BL : 6.0, SD: 7.6; OHIP&#95; 1y : 3.4, SD: 5.4; OHIP&#95; 4y : 5.7, SD: 9.5) and dentures (OHIP&#95; CD &#95; old : 28, SD: 29.7; OHIP&#95; CD &#95; new : 25.4, SD: 28.67)., Conclusion: Patient satisfaction with maxillary IODs on two implants did not change from baseline to 4 years and was high at 4 years of function.

Purpose: The aim of this 1-year randomized trial was to evaluate and compare the clinical and radiographic performance of four immediately loaded mini dental implants (MDIs) and two immediately loaded standard-sized tissue-level (STL) implants, placed in the interforaminal region of the mandible and used to retain mandibular overdentures (IODs) in completely edentulous patients., Materials and Methods: A total of 50 completely edentulous patients wearing conventional maxillary dentures and complaining about insufficient retention of their mandibular dentures were divided into two groups; 25 patients received four MDIs and 25 patients received two STL implants. The marginal bone loss (MBL) at the mesial and distal sides of each implant was assessed by means of standardized intraoral radiographs after a period of 1 year. Implant success and survival rates were also calculated., Results: Immediate loading was possible for all patients in the first group. In the second group, an immediate loading protocol could not be applied for 10 patients. These patients were treated with a delayed loading protocol. A mean MBL of 0.42 ± 0.56 mm for the MDIs and 0.54 ± 0.49 mm for the immediately loaded STL implants was recorded at the end of the evaluation period. There was no statistically significant difference between the MDIs and the immediately loaded STL implants. Two MDIs failed, resulting in a survival rate of 98%. The success rate was 91%. For the immediately loaded conventional implants, the survival rate was 100% and the success rate 96.7% after 1 year of function. However, in 10 patients, the immediate loading protocol could not be followed., Conclusion: Considering the limitations of this short-term clinical study, immediate loading of four unsplinted MDIs or two splinted STL implants to retain mandibular overdentures seems to be a feasible treatment option. The marginal bone level changes around the MDIs were well within the clinically acceptable range.

Statement of Problem: Complete-mouth implant-supported rehabilitations are challenging because of the multiple surgical and prosthetic steps involved in clinical evaluations to assure passive prosthesis fit and optimal esthetic and functional outcomes. As a result, these rehabilitations are usually associated with substantial clinical time, patient discomfort, and high treatment cost., Purpose: The purpose of this pilot clinical study was to evaluate a novel digital approach integrating digital intraoral dental and extraoral facial scanning information to design and mill a computer-aided design and computer-aided manufacturing (CAD-CAM) implant-retained prosthesis for patients with complete edentulism., Material and Methods: Ten patients in need of complete-mouth rehabilitation were included in this pilot study. Digital intraoral records were obtained through optical scanning the duplicate interim prosthesis using a laboratory scanner, while digital extraoral records were obtained through facial scanning using an in-office scanner. The scanned impressions and occlusal records were used to create a virtual tooth arrangement, which was matched to the patient's 3-dimensional face scan to create a virtual clinical evaluation phase. After applying the necessary adjustments, the virtual arrangement was submitted to a CAM procedure where a 5-axis industrial milling machine was used to fabricate an interim prosthesis., Results: Digital intraoral and extraoral records were integrated and used to fabricate CAD-CAM milled interim prostheses, which were inserted and assessed for clinical fit, occlusion/articulation, and esthetics. The prostheses remained in function for at least 6 months with no notable technical or biological complications except for 1 prosthesis that fractured., Conclusions: A novel digital workflow incorporating facial scanning in a CAD-CAM workflow was used to fully digitally design and mill 10 implant-retained interim prostheses. Objective: The objective of this study was to evaluate crestal bone changes around bone- and tissue-level implants related to initial mucosal thickness., Materials and Methods: Patients received at least 2 implants: one with the prosthetic abutment connection at the crestal bone level (MC) and one with the prosthetic abutment connection at 2.5 mm supra crestal (LC). Flap thickness measurements were taken using a periodontal probe after raising the buccal flap. Patients were divided into 2 groups according to the mucosal thickness-Group A (thickness, ≤2 mm) and Group B (thickness, >2 mm)., Results: Our study included 33 patients and 78 implants. Each patient received at least 1 implant of each type: Group A (MC), 17 implants, with a mean bone change of -0.6 ± 0.5 mm; Group B (MC), 20 with a mean bone change of -0.2 ± 0.4 mm; Group A (LC), 15 with a mean bone change of -0.1 ± 0.5 mm; and Group B (LC), 22 with a mean bone change of -0.2 ± 0.4 mm. A paired-samples t-test for groups A (MC) and B (MC) yielded a statistically significant difference (P = 0.003); there was no statistically significant difference for groups A (LC) and B (LC) (P = 0.518)., Conclusion: If the initial mucosal thickness surrounding bone-level implants is more than 2 mm, there is significantly less crestal bone change compared with bone-level implants placed in initial mucosal thicknesses of 2 mm or less. Objective: Crestal bone loss around dental implants is a criterion for success as this could prevent loss of implant and superstructure. The macrogeometry of the implant could influence bone remodelling when the implant-abutment connection is placed at crestal bone level or above. The aim of this study was to evaluate crestal bone remodelling in a randomized clinical prospective trial in macrogeometrically similar implants with the prosthetic connection at the crestal bone level and 2.5 mm above. The null hypothesis was that there was no difference in crestal bone loss after 1 year of early loading., Material and Methods: Patients were referred to Academic Centre of Dentistry Amsterdam for implant placement. Patients were subjected to inclusion and exclusion criteria and received a minimum of two implants: an implant with the prosthetic abutment connection at the crestal bone level (minimized collar [MC], bone level) and one with the prosthetic abutment connection 2.5 mm supracrestal (long collar [LC], tissue level). The mesial or distal location of each implant type was blinded for the patient and randomized. The implants were loaded splinted after 3 weeks of healing. The primary outcome was bone-level change assessed after 1 year of loading., Results: Thirty-three patients fulfilled the inclusion criteria. Thirty-nine Thommen SPI ELEMENT LC implants and 39 MC were placed, and each fixed dental prosthesis was supported by one LC and one MC implant. The intraclass correlation of measures performed by the first and second X-ray examiner was as follows: on the mesial side of the MC implant 0.990 (0.980-0.995; 95% confidence interval [CI]), 0.980 (0.962-0.990; 95% CI) on the distal side of the MC implant, 0.979 (0.959-0.989; 95% CI) and 0.988 (0.978-0.994; 95% CI) on the mesial and distal side of the LC implant, respectively. The mean bone loss of the MC implant was 0.4 ± 0.4 mm. The mean bone loss of the LC implant was 0.2 ± 0.5 mm. The paired samples t-test showed a statistically significant difference (P < 0.05) between the MC and LC implants., Conclusion: Dental implants at bone level show statistically significantly (P < 0.05) more crestal bone change after 1 year of loading than a tissue-level implant., (© 2015 John Wiley & Sons A/S. There is little evidence of the effect of implants restored with fixed partial dentures on oral health-related quality of life (OHRQoL) in partially edentulous Kennedy class II and III patients. The aim of this study was to determine the change in OHRQoL in Kennedy classification II and III patients treated with a two-implant-supported fixed dental prosthesis (FDP). Kennedy class II and III patients received dental implants and an FDP. Oral health-related quality of life was measured by administration of the Oral Health Impact Profile-14 (OHIP-14NL) questionnaire at intake (T1), 2 weeks after surgery (T2) and after 1 year of loading (T3). The mean OHIP score at T1 was 6·5 ± 1·2, 2·4 ± 1·0 at T2, and 0·9 ± 0·3 at T3. There was a statistically significant difference between T1 and T2 (P = 0·002) and T1 and T3 (P < 0·001) but not between T2 and T3 (P = 0·407). The OHIP score in Kennedy II patients decreased from 4·8 ± 3·2 at T1 to 1·5 ± 2·0 at T2 and 1·1 ± 1·8 at T3, and that in Kennedy III patients decreased from 8·9 ± 9·6 at T1 to 3·6 ± 8·9 at T2 and 0·8 ± 2·2 at T3. There were no statistically significant differences in the reductions in Kennedy II and III patients. Oral health-related quality of life changed positively in patients treated with implants and an FDP in both groups. Purpose: The aim of this systematic review and meta-analysis was to evaluate crestal bone changes around implants when placing the implant-abutment connection at the crestal bone level or above., Materials and Methods: Medline (Pubmed), EMBASE, and Cochrane Library up to January 2014 were electronically and hand searched for any publications that evaluated radiographic crestal bone changes around nonsubmerged, rough-surfaced implants placed in healed sites in humans and loaded for a minimum of 1 year., Results: The search yielded 1,122 publications; 1,106 could not be included. After 16 full-text articles were read and subjected to inclusion and exclusion criteria, four were included. The mean difference was -0.29 mm (95% CI, -0.58 mm to -0.01 mm). Heterogeneity between studies was observed (I2 = 95%). Significantly more crestal bone change was seen in the epicrestal implant-abutment (bone level) connection group when compared to implants with the prosthetic connection above the crestal bone level (tissue level) (P < .00001)., Conclusion: Dental implants at bone level show significantly less crestal bone change after 1 year of loading than tissue-level implants.
