503. Suspicion d'allergie aux vaccins anti COVID-19 : étude rétrospective sur 320 patients.

Le contexte sanitaire lié à la pandémie COVID-19 a entraîné le développement rapide de nombreux vaccins contre le virus SarS-Cov-2. Quatre d'entre eux sont actuellement disponibles en France et contiennent comme excipients du polyethylène glycol (PEG) ou du polysorbate 80, déjà décrits comme pourvoyeurs d'anaphylaxie. Des recommandations françaises ont été suggérées par plusieurs instances d'allergologie et proposaient une conduite à tenir face à une suspicion d'allergie à ces vaccins. Ainsi, les allergies aux excipients constituaient la seule contre-indication à la vaccination anti COVID-19. Notre objectif principal était de déterminer l'impact de ces recommandations vaccinales d'allergologie sur la prise en charge de ces patients. Notre objectif secondaire était de déterminer la prévalence des allergies vraies à ces vaccins. Nous avons mené une étude rétrospective descriptive unicentrique concernant l'ensemble des patients de plus de 18 ans adressés pour un avis allergologique avant ou après une vaccination anti COVID-19. Les patients ont été classés en différents groupes d'intérêt, à partir des recommandations françaises. La vaste majorité des patients n'ont pas nécessité de bilan allergologique pré vaccinal. En effet, seulement 25 patients ont reçu des tests cutanés préalables à la vaccination. Le reste des patients ont pu être vaccinés sans bilan allergologique. Les patients non vaccinés pour cause d'allergie aux excipients représentent moins de 1 % de l'effectif total (n = 3/320). Les recommandations françaises ont permis de vacciner la grande majorité des patients inclus dans notre étude. L'allergie au PEG, polysorbate ou leurs dérivés, seule contre-indication à la vaccination anti COVID, d'après les recommandations de février 2021, reste rare. À ce jour, plusieurs auteurs proposent des inductions de tolérance permettant la vaccination de patients allergiques aux PEG ou à leurs dérivés avec une bonne tolérance. The health context with COVID-19 pandemic has led to fast development of many vaccines against the SarS-Cov-2 virus. Four of them are currently available in France and contain polyethylene glycol (PEG) or polysorbate 80 as excipients, already described as causing anaphylaxis. French recommendations have been suggested by allergology authorities and proposed a course of action in the event of a suspected allergy to these vaccines. Thus, allergies to excipients were the only contraindication to COVID-19 vaccination. Our main objective was to determine the impact of these allergology vaccine recommendations on the management of these patients. Our secondary objective was to determine prevalence of true allergies to these vaccines. We conducted a unicentric descriptive retrospective study with all patients over 18 years of age referred for an allergological opinion before or after an injection of one of the anti-COVID-19 vaccines. Nineteen patients were classified into different interest groups, based on french recommendations. The vast majority of patients did not require a pre-vaccination allergological assessment. Indeed, only 25 patients received skin tests prior to vaccination. The rest of patients were able to be vaccinated without allergological assessment. Patients not vaccinated due to allergy to excipients represent less than 1% of the population (n = 3/320). French recommendations made it possible to vaccinate the vast majority of patients included in our study. Allergy to PEG, polysorbate or their derivatives, the only contraindication to anti-COVID vaccination, according to the recommendations of February 2021, remains rare. Today, several authors propose tolerance inductions allowing the vaccination of patients allergic to PEGs or their derivatives with good tolerance. [ABSTRACT FROM AUTHOR]

With the exception of cardiac surgery, the acute disturbance of the left ventricular diastole occurs mainly in the elderly. Today it represents 30 to 40% of congestive cardiac failures, however with a lower mortality than for acute systolic disturbances. Generally indicated are relaxation anomalies, proto-mesodiastolic mechanism and problems with compliance, an indicator of the pressure/volume diastolic relationship. Invasive techniques remain the standard method. Doppler echocardiography is becoming increasingly important for the assessment of diastolic function. In most cardiopathies, relaxation anomalies occur early, whereas compliance disturbances are mainly associated with advanced cardiac diseases. During anaesthesia, adverse events (auricular fibrillation, hypovolaemia) may worsen a fragile situation. Anaesthetic agents, in particular volatile agents, act on the ventricular diastole. Long-term therapy of diastolic anomalies includes agents amending left ventricular hypertrophy. Emergency therapy has not yet been systematised.

The SILVER (Study of Irbesartan in Left VEntricular hypertrophy Regression) trial is designed to test the hypothesis that the newly developed angiontensin-II receptor antagonist, irbesartan, will produce a greater reduction in left ventricular (LV) mass than felodipine ER, in a population of hypertensive patients defined by seated diastolic blood pressure (SeDBP) in the range 95-115 mmHg or seated systolic blood pressure (SeSBP) in the range 160-200 mm Hg. A population of 360 men and women of non-childbearing potential,18 years of age, with hypertension, newly diagnosed or after a 3-week washout from previous anti-hypertensive or vasodilator therapies, will be randomised at approximately 80-90 European sites. Add-on therapy with hydrochlorothiazide and atenolol will be allowed for blood pressure control. Patients will be studied by two-dimensional and M-mode echocardiography at baseline (central validation of LV hypertrophy), on randomisation day, and after 6 and 12 months randomised therapy. Blinded analysis of echocardiograms will be performed at a central laboratory, which will provide measurements of the LV mass index (LVMI), determined by M-mode readings according to Devereux formula and using the Penn convention. The primary end-point of the study will be the change in LVMI from baseline to 12 months. The study power is 90% to detect differences between groups from baseline of approximately 8 g/m2.

508. The Integrated Comprehensive Care Program: A Novel Home Care Initiative After Major Thoracic Surgery.

The objective of the study was to evaluate the Integrated Comprehensive Care (ICC) program, a novel health system integration initiative that coordinates home care and hospital-based clinical services for patients undergoing major thoracic surgery relative to traditional home care delivery. Methods included a pilot retrospective cohort analysis that compared the intervention cohort (ICC), composed of all patients undergoing major thoracic surgery in the 2012-2013 fiscal year with a control cohort, who underwent surgery in the year before the initiation of ICC. Length of stay, hospital costs, readmission, and emergency room visit data were stratified by degree and approach of resection and compared using univariate logistic regression analysis. A total of 331 patients under ICC and 355 control patients were enrolled. Hospital stay was significantly shorter in patients under video-assisted thoracoscopic surgery (VATS) ICC (sublobar median 3 vs 4 days, P = 0.013; lobar median 4 vs 5 days, P = 0.051) but not for open resections. The frequency of emergency room visits within 60 days of surgery was lower for all stratification groups in the ICC cohort, except for VATS sublobar (25.7% control vs 13.9% ICC, P = 0.097). There were no significant differences in 60-day readmission frequency in any subcohort. The mean inpatient case cost was significantly lower for ICC VATS sublobar resections ($8505.39 vs $11,038.18, P = 0.007), with the other resection types trending lower for ICC but nonsignificant. In conclusion, a hospital-based, postdischarge, patient-centered program could potentially result in shorter hospital stay, fewer readmission and emergency room visits, costsavings, and no increase in adverse postdischarge outcomes after major thoracic surgery. Chlorine-36 production rates obtained from different geological calibration studies (e.g. Evans et al., 1997; Phillips et al., 2001; Schimmelpfennig et al., 2011; Stone et al., 1996; Swanson and Caffee, 2001) vary significantly, principally because of the many reactions contributing to the production of this nuclide. The CRONUS-Earth Project has provided high-quality geological calibration sites, including Lake Bonneville, Peru, and Scotland, for a large-scale calibration of 36 Cl production rates. Three sites were used to calibrate the K and Ca spallation pathways for 36 Cl production yielding production rates of 56.0 ± 4.1 at 36 Cl (g Ca) −1 yr −1 and 155 ± 11 at 36 Cl (g K) −1 yr −1 respectively, using Lifton-Sato-Dunai scaling (LSDn). The low-energy production parameter, P f ( 0 ), was calibrated separately using CRONUS-Earth data from the Bonneville and Baboon Lakes sites where Cl concentrations were higher, and yielded a value of 759 ± 180 neutrons (g air) −1 yr −1 . There is significant uncertainty associated with this pathway due to the sensitivity of this reaction to environmental conditions. The uncertainties associated with the calibrated production parameters were estimated based on the variance of calculated ages from independent ages for an independent secondary dataset. Geological surface-exposure dating using cosmogenic-nuclide accumulation became a practical geochronological endeavor in 1986, when the utility of 10 Be, 26 Al, 36 Cl, and 3 He were all demonstrated. In response to the lack of a common basis for quantifying analytical consistency and calibrating cosmogenic-nuclide production, the CRONUS-Earth Project in the U.S. was started in 2005, along with a European partner project, CRONUS-EU. The goal of the CRONUS-Earth Project was to improve the accuracy and precision of terrestrial cosmogenic nuclide dating in general, focusing especially on nuclide production rates and their variation with altitude, latitude, and time, and to attempt to move from empirically based methods to ones with a stronger basis in physics. The CRONUS-Earth Project conducted extensive intercomparisons of reference materials to attempt to quantify analytical reproducibility at the community level. We found that stated analytical uncertainties nearly always underestimate the actual degree of variability, as quantified by the over-all coefficient of variation of the intercalibration data. The average amount by which the actual coefficient of variation exceeded the analytical uncertainty was a factor of two (100%), but ranged from 15% to 300% depending on the nuclide and material. Coefficients of variation ranged from 3−4% for 10 Be to 6–8% for 36 Cl, 14 C, and 21 Ne, to 5–11% for 26 Al. Both interlaboratory bias and within-laboratory excess spread of the data played a role in increasing variability above the stated analytical uncertainties. The physical basis for cosmogenic nuclide production was investigated through numerical modeling and the measurement of energy-dependent neutron cross sections for nuclide interactions. We formulated new, physically based, scaling models, denoted LSD and LSDn, by generalizing global numerical simulations of cosmic-ray processes. The CRONUS-Earth Project identified new geological calibration sites, including one at low latitude and high elevation (Huancané, Peru), and replicated nuclide measurement at numerous laboratories. At many sites multiple nuclides were measured, providing much more confidence in the equivalence of surface-exposure ages calculated from differing nuclides. The data were interpreted using an original cosmogenic-nuclide calculator, CRONUScalc, that incorporates the new physically based scaling. The new data and model produced significantly better fits than previous efforts, but do not fully resolve apparent spatial variations in production rates. The CRONUS-Earth and CRONUS-EU Projects have provided a firm foundation for assessing the strengths and weaknesses of cosmogenic-nuclide analytical methods, adjusted the AMS standards for 10 Be and consequently revised the half-life, and have provided improved calibration data sets and interpretative tools. The CRONUS-Earth and CRONUS-EU Projects have recently examined the reproducibility of cosmogenic nuclide measurements and the ability of cosmogenic-production models to fit high-quality global calibration data sets. In both cases, although results are adequate for present geochronological needs, they must be improved to meet future demands. We recommend that the cosmogenic-nuclide community embark on a continuing effort to understand the sources of the remaining model-versus-data discrepancy, and that in the meanwhile all cosmogenic ages be benchmarked against reference ages calculated in a consistent fashion. We also recommend that interlaboratory reference materials be routinely analyzed along with samples and blanks, that the results be tracked, and that methods of achieving consistency and precision in sample preparation and analysis be shared within the community. We also recommend additional organization of the community to facilitate the use of intercomparison materials, foster analytical improvements, oversee the evolution of the community age calculator, and synthesize ongoing calibration efforts. A wide range of definitions is used to distinguish subjects in whom blood pressure (BP) falls at night (dippers) from their counterparts (nondippers). In an attempt to standardize the definition of nondipping, we determined the nocturnal BP fall and night-day BP ratio by 24-hour ambulatory monitoring in 4765 normotensive and 2555 hypertensive subjects from 10 to 99 years old. In all subjects combined, the systolic/diastolic nocturnal fall and corresponding ratio averaged (+/- SD) -16.7 +/- 11.0/ -13.6 +/- 8.1 mm Hg and 87.2 +/- 8.0%/83.1 +/- 9.6%, respectively. In normotensive subjects, the 95th percentiles were -0.3/-1.1 mm Hg for the nocturnal fall and 99.7%/98.3% for the night-day ratio. Both the fall and ratio showed a curvilinear correlation with age. The smallest fall and largest ratio were observed in older (or = 70 years) subjects. A higher BP on conventional sphygmomanometry was associated with a larger systolic (partial r = .11) and diastolic (r = .12) nocturnal BP fall. The diastolic (r = .08) but not the systolic night-day ratio increased with higher conventional BP. The nocturnal BP fall was larger and the corresponding night-day ratio smaller in oscillometric (n = 5884) than in auscultatory (n = 1436) recordings, in males (n = 3730) than in females (n = 3590), and in Europe (n = 4556) than in the other continents (n = 2764). The distributions of the nocturnal BP fall and night-day ratio showed considerable overlap among normotensive and hypertensive subjects, but the overlap tended to be larger for the ratio than for the fall. Of all subjects, 3.2% had systolic and diastolic ratios of 100% or more. With adjustments applied for confounders, the probability of being a nondipper increased 2.8 times (95% confidence interval, 2.0-4.0) from 30 to 60 years and 5.7 times (4.4-7.4) from 60 to 80 years. The odds ratios were 1.0 (0.8-1.4) for males versus females. 1.6 (1.2-2.1) for subjects with definite hypertension versus normotensive subjects, 2.4 (1.2-4.7) for Asians (n = 2213, 96% Japanese) versus inhabitants of the other continents, and 2.4 (1.5-3.8) for subjects examined with auscultatory versus oscillometric devices. In conclusion, the mathematical definition of nondipping, ie, having a night-day ratio of 100% or more for systolic and diastolic BPs, closely approximated the 95th percentiles of the night-day ratio in normotensive subjects. The ratio depends less on BP level than the nocturnal BP fall and is therefore to be preferred in the definition of dipping status. Notwithstanding the present findings, the reproducibility of nondipping and its prognostic significance need further clarification.

L'Etang de Berre est un écosystème eutrophe qui reçoit de l'eau douce et des nutriments par deux rivières et par la Durance via la centrale hydro-électrique de Saint Chamas (SC). En intégrant les variations horaires, les échantillons journaliers nous ont permis de calculer un bilan annuel représentatif des apports en nutriments : SC apporte 1085 tonnes de N total (93 % est équi-réparti entre les formes nitrates et azote organique dissous) et 30 tonnes de P total (80 % est sous forme particulaire) sur la période Octobre 2008 à Novembre 2009. La fréquence d'échantillonnage requise pour obtenir le flux annuel en N total avec une erreur de 25 % est mensuelle. Pour le flux annuel de P total, l'erreur minimale est obtenue à partir de l'échantillonnage le plus fréquent testé (i.e. hebdomadaire) mais varie entre -30 % et +60 %. Si on compile toutes les études, on observe que les apports en nutriments à la lagune ont diminué ces 2 dernières décennies. En 2006, SC représente 50 % et 14 % des apports allochtones en N total et P total respectivement. Les sources diffuses et atmosphériques et la labilité des fractions organique dissoute et particulaire restent à préciser.

AbstractBackground:Vasovagal responses associated with acupuncture therapy are relatively uncommon adverse events, occurring in ∼0.02%–7% of treatments. The complex neurocardiovascular reflexes involved in vasovagal responses can induce a range of symptoms such as dizziness, nausea, sweating, bradycardia, hypotension, and, in some cases, syncope or convulsions. Although patients typically recover quickly with proper support, these events may be of concern and anxiety-producing for both patient and provider. Providers need to be well-versed in methods for prevention and treatment of acupuncture-associated vasovagal responses to promote safe practice environments, patient satisfaction and comfort, and cost-effectiveness.Objectives:To examine the biomedical and Traditional Chinese Medicine mechanisms of vasovagal responses, propose updated terminology, and outline steps for prevention and treatment.Methods:During an 18-month period, 281 community-style acupuncture treatments were performed on family members of admitted patients and hospital staff members at the University of Minnesota Masonic Children's Hospital. Five (1.8%) treatments resulted in documented acupuncture-associated vasovagal response (AAVR) symptoms.Results:All 5 patients recovered from their AAVR symptoms after treatment interventions. After recovery, 3 patients reported reductions in their main complaint symptoms; main complaint symptom outcomes were not recorded for the other 2 patients.Conclusions:As integrative practices become more prevalent in academic institutions and primary care environments, clear communication about, as well as prevention treatment, documentation and reporting of acupuncture-associated adverse events will become increasingly valuable. The authors recommend that clinicians in integrative practice clinical environments consider developing formal AAVR response plans as well as training students, supervising and attending providers, and ancillary staff members to ensure rapid, prepared handling and documentation of AAVR incidents.

Subhorizontal lake shorelines allow a geodynamic test of the size and extent of a hypothesized paleolake in central Tibet, the East Qiangtang Lake (EQL), during the last interglacial period (marine isotope stage (MIS) 5e). Reconstructions based on relict lake deposits suggest that the EQL would have been ~400 m deep and over ~66,000 km2. Models of flexural rebound driven by lake recession predict that shorelines near the EQL center, at the present‐day location of Siling Co, would have rebounded 60–90 m above their initial elevation. New 36Cl chronology of the highest relict shorelines around Siling Co indicates that they reflect lake levels between 110 and 190 ka. These shorelines, however, are presently >300 m below their predicted elevations, implying a substantially smaller water load. Our results reveal that the expansion of Tibetan lakes during MIS 5e was relatively limited. Instead, individual lakes were supplied by river networks, much as they are today. Estimates of flexural rebound test the hypothesis of extensive ancient lake in TibetShorelines ~50‐100 m above present‐day Siling Co dated to 110‐190 kaMIS 5e shoreline positions at Siling Co preclude continuous, deep lake across central Tibet

- Abstract
The thermal effect of high intensity focused ultrasound (HIFU) has been clinically exploited over a decade, while the mechanical HIFU is still largely confined to laboratory investigations. This is in part due to the lack of adequate imaging techniques to better understand the in-vivo pathological and immunological effects caused by the mechanical treatment. In this work, we explore the use of high frequency ultrasound (US) and photoacoustics (PA) as a potential tool to evaluate the effect of mechanical ablation in-vivo, e.g. boiling histotripsy. Two mice bearing a neuroblastoma tumor in the right leg were ablated using an MRI-HIFU system conceived for small animals and monitored using MRI thermometry. High frequency US and PA imaging were performed before and after the HIFU treatment. Afterwards, the tumor was resected for further assessment and evaluation of the ablated region using histopathology. High frequency US imaging revealed the presence of liquefied regions in the treated area together with fragmentized tissue which appeared with different reflecting proprieties compared to the surrounding tissue. Photoacoustic imaging on the other hand revealed the presence of deoxygenated blood within the tumor after the ablation due to the destruction of blood vessel network while color Doppler imaging confirmed the blood vessel network destruction within the tumor. The treated area and the presence of red blood cells detected by photoacoustics were further confirmed by the histopathology. This feasibility study demonstrates the potential of high frequency US and PA approach for assessing in-vivo the effect of mechanical HIFU tumor ablation.

Cirrhosis-like hepatocellular carcinoma is a rare subtype of hepatocellular carcinoma that is mostly diagnosed at time of liver transplantation or autopsy and is not clinically suspected preoperatively or premortem. This entity is characterized by numerous distinct neoplastic foci that arise within regenerative cirrhotic nodules. Knowledge of this subtype of HCC is important for comprehensive examination of explanted and autopsy liver specimens. The clinical presentation, gross, histology, and prognosis are discussed in this review.

Well-dated bedrock surfaces associated with the highstand and subsequent catastrophic draining of Pleistocene Lake Bonneville, Utah, during the Bonneville flood are excellent locations for in situ cosmogenic nuclide production rate calibration. The CRONUS-Earth project sampled wave-polished bedrock and boulders on an extensive wave-cut bench formed during the Bonneville-level highstand that was abandoned almost instantaneously during the Bonneville flood. CRONUS-Earth also sampled the Tabernacle Hill basalt flow that erupted into Lake Bonneville soon after its stabilization at the Provo level, following the flood. New radiocarbon dating results from tufa at the margins of Tabernacle Hill as part of this study have solidified key aspects of the exposure history at both sites. Both sites have well-constrained exposure histories in which factors such as potential prior exposure, erosion, and shielding are either demonstrably negligible or quantifiable. Multi-nuclide analyses from multiple labs serve as an ad hoc inter-laboratory comparison that supplements and expands on the formalized CRONUS-Earth and CRONUS-EU inter-laboratory comparisons (Blard et al., 2015; Jull et al., 2015; Vermeesch et al., 2015). Results from 10 Be, 26 Al, and 14 C all exhibit scatter comparable to that observed in the CRONUS-Earth effort. Although a 36 Cl inter-laboratory comparison was not completed for Jull et al. (2015), 36 Cl from plagioclase mineral separates exhibits comparable reproducibility. Site production rates derived from these measurements provide valuable input to the global production rate calibration described by Borchers et al. (2015). Whole-rock 36 Cl concentrations, however, exhibit inter-laboratory variation exceeding analytical uncertainty and outside the ranges observed for the other nuclides (Jull et al., 2015). A rigorous inter-laboratory comparison studying the systematics of whole-rock 36 Cl extraction techniques is currently underway with the goals of delineating the source(s) of this discrepancy and standardizing these procedures going forward. M mode echocardiography is certainly an adequate method to evaluate left ventricular hypertrophy (LVH) regression in hypertensive patients, provided that some important conditions are fulfilled. Left ventricular mass assessment can be made only with non-distorted ventricles, as usually encountered in non-complicated hypertensive patients. The main problem is to limit all sources of variability for serial measurements and all sources of subjectivity in their interpretation. So, we propose some guidelines that should be useful when designing and completing such studies. Two-dimensional echocardiography offers the potential advantage of allowing assessment of left ventricular mass even when the left ventricle is far from its usual shape. However, a standardization of methods and studies of its reproducibility are still required before this method could be recommended in studies of LVH regression.

There is a perceived shortage of males in education in Ontario, particularly at the primary and junior levels. Some barriers to males becoming teachers include the impression that teachers are overworked and underpaid, that men are less nurturing than women, and that it is inappropriate for men to be working with young children. Researchers and members of the public identify the need for more men to serve as role models, and to enhance the learning of boys who progressively score less well than girls on provincial, national and international achievement tests. My theoretical framework derives from poststructuralist theory, questioning the fluidity of discourse and identities, and troubling accepted, commonplace beliefs, knowledge, and practices. To this end, I interpret results from an online survey of 223 male primary/junior school teachers in Ontario, Canada. The results are startling, and call into question some commonly accepted truths about male teachers, power, and sexualities, while confirming others. In particular, societal fears about male sexualities and sexual identities in the lives of male primary school teachers will be examined. Androgenophobia is the prevalent societal conviction that maleness, the male body, and male sexualities are somehow unclean, perverse, and menacing, while erastephobia surfaces as a pervasive expectation and fear of impending pedophilia by males in general, and male teachers in the schools, as specifically addressed in this paper. On examine la capacité de deux modèles thermiques déterministes à base physique (CALNAT et COURNAT) à reproduire l'augmentation de la température moyenne estivale de la Loire à Dampierre-en-Burly sur la période 1980-2003. Ces modèles utilisent en entrée les chroniques de quatre paramètres atmosphériques relevés à pas tri-horaire à la station Météo-France de Tours. CALNAT calcule la température naturelle locale d'un plan d'eau sans considération du débit fluvial, tandis que le code dérivé COURNAT propage une masse d'eau dans les conditions de débit mesurées à la station de Gien. En été, du 15 juin au 15 septembre, l'échauffement moyen sur les 24 ans observé à la station de mesures en continu des eaux de la Loire située à l'amont de la centrale nucléaire de Dampierre-en-Burly atteint 1,85 °C. Une tendance comparable (2,0 °C) est observée à la station de Belleville amont située une trentaine de kilomètres en amont. Les échauffements moyens calculés sur la même période par CALNAT et COURNAT sont respectivement de 1,55 °C et 1,7 °C; ils représentent ainsi 84 % et 92 % de la tendance d'échauffement découlant des mesures à Dampierre-en-Burly. Durant cette période d'étiage, l'essentiel de l'échauffement de la Loire est attribuable aux variations tendancielles de la température d'air (+1,5 °C), de la pression de vapeur d'eau (+0,65 hPa), de la vitesse de vent à 10 mètres (-0,4 m/s), et de la nébulosité du ciel (-0,15 octa) dont la contribution à l'élévation de la température naturelle locale peut être évaluée à l'aide de CALNAT à respectivement 46 %, 18 %, 28 % et 8 %. L'application de CALNAT et COURNAT sur la période 1949-2003 donne un échauffement moyen tendanciel de l'eau de l'ordre de 0,5 °C sur 55 ans pour le même trimestre 15 juin-15 septembre. Le débit ne présentant pas de tendance d'évolution significative sur la période, cet échauffement modéré comparativement à celui de 1980-2003 est attribuable au faible effet global des tendances de variation des quatre paramètres atmosphériques. Contrairement à la période 1980-2003, les quatre grandeurs atmosphériques présentent des variations à effets antagonistes sur la température d'eau de 1949 à 2003 : réparties uniformément à chaque pas tri-horaire de calcul, les évolutions estimées de la température d'air (+0,9 °C), de la pression de vapeur d'eau (-0,55 hPa), de la vitesse du vent à 10 m (+0,4 m/s) et de la nébulosité (-0,4 octa) sur ces 55 ans à l'échelle trimestrielle modifient la température naturelle locale de l'ordre de +0,4 °C, -0,2 °C, -0,4 °C et +0,3 °C respectivement. Ces résultats suggèrent qu'une relation linéaire observée entre variations de températures d'air et de plan d'eau n'est pas systématiquement extrapolable sur un plus grand nombre de décennies. Comme ceux de COURNAT, les résultats de CALNAT indiquent que, sur la période 1949-2003, 2003 est l'année record pour la dose thermique maximale reçue par l'écosystème fluvial aussi bien sur 7 jours consécutifs que sur 90 jours consécutifs (avec des températures moyennes calculées de respectivement 29,5 °C et 25 °C confirmées par la station de mesure de Dampierre). Pour la dose thermique maximale reçue sur 30 jours consécutifs, CALNAT place 2003 dans les années de tête avec une température moyenne de 26 °C, confirmée par la station de Dampierre. Ces pics thermiques de 2003 remarquablement reproduits à 0,3 °C près par CALNAT aux échelles de temps considérées sont ainsi expliqués par les conditions atmosphériques locales exceptionnelles et non par le très bas débit fluvial (minimum de 50 m3/s à Gien) alors observé. À l'opposé, l'été 1977 ressort avec COURNAT - plus adapté lorsque les débits sont élevés - comme l'été le plus froid de la période 1949- 2003 pour la dose thermique maximale reçue sur 7, 30 et 90 jours avec des températures moyennes de respectivement 22 °C, 20 °C et 19 °C, effectivement observées. Pour la dose thermique maximale reçue par l'écosystème Loire sur 30 jours, les années 1976 et 1977 se distinguent par la plus forte différence entre deux années consécutives sur la période 1949-2003.

Dendritic cells (DC) are the most potent antigen-presenting cells and form a promising new treatment modality. Fully activated DC loaded with antigen are very useful in stimulating immune responses, in particular those to combat cancer. Immature DC can either cause immunological tolerance or induce regulatory T-cells, opening up future application in transplantation, autoimmunity, and perhaps chronic inflammation. For these clinical applications, generation of DC under Good Manufacturing Practice conditions and without the use of animal products is a main prerequisite. To date, there are many different DC culture protocols, which subsequently lead to different types of DC. Protocols differ not only in DC preparation techniques, but also many other variables, like maturation status, dose and timing interval, route of administration, and antigen loading. DC vaccination, although very promising, is far from standardized. In this chapter we discuss the usefulness of standardized clinical and immunological criteria and the need for careful study design to further optimize the use of dendritic cells and to unequivocally prove their efficacy. 545. From malignant hypertension to hypertension-MOD: a modern definition for an old but still dangerous emergency

- Abstract
The prevalence of malignant hypertension has clearly fallen with the advent of anti-hypertensive medication but has remained stable over the past 30–40 years in spite of progress in diagnosis and management of hypertension. A diagnosis of malignant hypertension is usually based on the association of severely elevated blood pressure with a Keith and Wagener stage III or IV retinopathy. We believe that this definition can be reconsidered for several reasons. Although simple and pragmatic, this definition corresponds to a time when there were few techniques for assessment of hypertensive target organ involvement, and does not take into account involvement of kidney, brain and heart; whereas the overall prognosis largely depends on how much they are affected. On the contrary, the acute blood pressure level and especially diastolic should not be a hard diagnostic criterion as it does not itself constitute the prognosis of the condition. We propose to consider that malignant hypertension with retinopathy is only one of a number of possible presentation(s) of acute hypertension with multi organ damage (hypertension multi organ damage (MOD)) and that the recognition of these hypertensive emergencies, when retinopathy is lacking, be based on acute elevation of BP associated with impairment of at least three different target organs. The objective of a new and expanded definition is to facilitate recognition of these true emergencies. The condition is more common than usually perceived and would have a much worse prognosis than the usual forms of hypertension. Early recognition and management of hypertension-MOD are fundamental to any improvement in prognosis.

547. SFE/SFHTA/AFCE consensus on primary aldosteronism, part 1: Epidemiology of PA, who should be screened for sporadic PA?

Depending on the study, the prevalence of primary aldosteronism (PA) in patients with hypertension varies from 6 to 18%. Prevalence is higher in each of the following conditions, any one of which requires screening for PA: severe hypertension (systolic blood pressure [BP]≥180mmHg and/or diastolic BP≥110mmHg); resistant hypertension (systolic BP≥140mmHg and/or diastolic BP≥90mmHg despite adherence to a tritherapy including a thiazide diuretic); hypertension associated with hypokalemia (either spontaneous or associated with a diuretic); Hypertension or hypokalemia associated with adrenal incidentaloma. It should be borne in mind that PA can induce hypertension without hypokalemia or, less frequently, hypokalemia without hypertension. Finally, as cardiovascular and renal morbidity in PA is greater than in essential hypertension of equivalent level, screening for PA is indicated when cardiovascular or renal morbidity is more severe than predicted from BP level.

- Abstract
The French Endocrinology Society (SFE) French Hypertension Society (SFHTA) and Francophone Endocrine Surgery Association (AFCE) have drawn up recommendations for the management of primary aldosteronism (PA), based on an analysis of the literature by 27 experts in 7 work-groups. PA is suspected in case of hypertension associated with one of the following characteristics: severity, resistance, associated hypokalemia, disproportionate target organ lesions, or adrenal incidentaloma with hypertension or hypokalemia. Diagnosis is founded on aldosterone/renin ratio (ARR) measured under standardized conditions. Diagnostic thresholds are expressed according to the measurement units employed. Diagnosis is established for suprathreshold ARR associated with aldosterone concentrations >550pmol/L (200pg/mL) on 2 measurements, and rejected for aldosterone concentration<240pmol/L (90pg/mL) and/or subthreshold ARR. The diagnostic threshold applied is different if certain medication cannot be interrupted. In intermediate situations, dynamic testing is performed. Genetic forms of PA are screened for in young subjects and/or in case of familial history. The patient should be informed of the results expected from medical and surgical treatment of PA before exploration for lateralization is proposed. Lateralization is explored by adrenal vein sampling (AVS), except in patients under 35 years of age with unilateral adenoma on imaging. If PA proves to be lateralized, unilateral adrenalectomy may be performed, with adaptation of medical treatment pre- and postoperatively. If PA is non-lateralized or the patient refuses surgery, spironolactone is administered as first-line treatment, replaced by amiloride, eplerenone or calcium-channel blockers if insufficiently effective or poorly tolerated.

549. Une méthode pour estimer l’interception du rayonnement par un couvert bas : application au colza avant montaison

Dans les modèles de simulation du fonctionnement des cultures, l’interception du rayonnement est une variable cruciale pour la prévision de la productivité en biomasse. D’autre part, la connaissance de l’interception du rayonnement par une culture permet de mieux analyser la réponse à un traitement en autorisant la distinction entre les effets morphogénétique et physiologique du traitement. Par exemple, dans l’étude de la fertilisation azotée du colza [1] on a pu faire la part de l’effet morphogénétique (accroissement de l’indice foliaire, donc de l’interception) et de l’amélioration de la conversion photosynthétique du rayonnement absorbé. Dans un cas comme dans l’autre, une estimation correcte de l’interception du rayonnement est essentielle. Très souvent, un formalisme dérivé de la loi de Beer-Lambert est utilisé pour quantifier la part interceptée du rayonnement : PARi = epsiloni PAR (1) et epsiloni = 1 - e- k.IF (2), PAR représentant le rayonnement photosynthétiquement actif incident (exprimé en Joules ou en moles de photons), PARi la part de ce dernier interceptée par le couvert, epsiloni le coefficient (sans dimension) d’interception, IF l’indice foliaire et k le coefficient d’extinction du rayonnement. Un formalisme un peu plus complexe doit être utilisé si on veut considérer le rayonnement absorbé [1, 2]. Pour des cultures couvrantes, la sensibilité de ces modèles à la valeur de k est faible pour les IF élevés, c’est pourquoi ces modèles sont souvent utilisés avec succès. Mais aux faibles IF, les modèles sont sensibles à k, d’où l’importance de l’estimation de ce paramètre pour les cultures présentant une longue période de faible couverture du sol, tel le colza peu fertilisé en phase hivernale. De plus, pour les cultures discontinues, c’est-à-dire présentant des zones où le sol n’est pas du tout couvert par la végétation, comme les inter-rangs, on doit introduire dans l’équation 2 un paramètre représentant le taux de couverture du sol. Si l’estimation par mesure directe de epsiloni et donc l’estimation de k sont assez simples, au moins dans le principe pour un couvert développé en hauteur (colza après montaison) [3], une telle mesure devient beaucoup plus complexe dans le cas d’un couvert discontinu et bas. Dans ce milieu hétérogène (et souvent dans des conditions hivernales sévères), une distribution représentative des capteurs de rayonnement est difficile, les risques de perturbation de la mesure (salissure des capteurs, déréglages) sont élevés. Plusieurs études ont déjà traité de epsiloni et k dans le cas du colza [3-7]. Elles sont fondées sur des mesures d’interception du rayonnement et concernent des couverts fermés, après montaison. Une autre approche consiste à effectuer un calcul précis de PARi avec des modèles détaillés utilisant une description complète des paramètres (géométriques et radiatifs) du feuillage, et du rayonnement incident [8]. Mais les données nécessaires sont alors nombreuses, complexes à obtenir et donc généralement non disponibles en expérimentation. Pour répondre aux besoins de calcul de l’interception du rayonnement dans les phases d’installation du colza, cet article présente une méthode permettant de calculer epsiloni en tenant compte des principales caractéristiques de la morphologie du couvert et du régime radiatif auquel ce dernier est soumis. Les données nécessaires sont limitées : l’indice foliaire, les taux de couverture du sol (deux définitions), la latitude du lieu considéré, la date et le rayonnement incident quotidien. Ce calcul permet une estimation du coefficient moyen d’extinction k (équation 2). On verra que k peut être corrélé à l’IF, la période de l’année et la latitude du lieu, ce qui permet de proposer une estimation simple de k et donc un calcul simplifié de epsiloni, adapté au cas des faibles IF(colza entre levée et montaison).

Néanmoins, le caractère durable des solutions envisagées doit être évalué sur des bases techniques, environnementales et économiques qui permettront, en outre, d’estimer l’acceptabilité par les agriculteurs des scénarios proposés.
