Coronary artery disease (CAD) with its clinical appearance of stable or unstable angina and acute myocardial infarction is the leading cause of death in developed countries. In view of increasing costs and the rising number of CAD patients, there has been a major interest in reliable non-invasive imaging techniques to identify CAD in an early (i.e. asymptomatic) stage. Since myocardial perfusion deficits appear very early in the "ischemic cascade", a major breakthrough would be the non-invasive quantification of myocardial perfusion before functional impairment might be detected. Therefore, there is growing interest in other, target-organ-specific parameters, such as relative and absolute myocardial perfusion imaging. Magnetic resonance (MR) imaging has been proven to offer attractive concepts in this respect. However, some important difficulties have not been resolved so far, which still causes uncertainty and prevents the broad application of MR perfusion imaging in a clinical setting. This review explores recent technical developments in MR hardware, software and contrast agents, as well as their impact on the current and future clinical status of MR imaging of first-pass myocardial perfusion imaging.

Purpose: To compare the signal-to-noise and contrast-to-noise ratio as well as the image quality of 3D inversion recovery steady-state free precession (IR-SSFP) and 3D inversion recovery fast low angle shot (IR-FLASH) sequences for contrast-enhanced breath-hold MRCA., Materials and Methods: 24 healthy volunteers (10 female, 14 male, mean age 29.8 +/- 6.1 years) were involved in this study. All examinations were performed on a 1.5 T MR scanner (Magnetom Sonata, Siemens, Germany) after injection of 0.05 mmol/kg body weight MS-325 (EPIX Pharmaceuticals, Cambridge, MA and Schering AG, Berlin, Germany). MRCA was performed using IR-SSFP (TR 3.8 ms, TE 1.6 ms, FA 65 degrees , 35 phase encoded steps, bandwidth 540 Hz/pixel, slice thickness 1.5 mm, in-plane resolution 1.2 x 0.9 - 1.4 x 1.0 mm) and IR-FLASH (TR 3.8 ms, TE 1.6 ms, FA 25 degrees , bandwidth 490 Hz/pixel, slice thickness 1.5 mm, in-plane resolution 1.2 x 0.9 - 1.4 x 1.0 mm) sequences. For all scans the inversion time was set to null the signal intensity of the myocardium. Signal-to-noise ratio (SNR) as well as contrast-to-noise ratio (CNR) measurements (blood versus myocardium) were performed. The image quality was assessed based on a 5-point scale ranging from 1 (excellent) to 5 (non-diagnostic) by two radiologists in consensus., Results: The mean signal-to-noise ratio of blood (27.7 +/- 4.7 vs. 22.6 +/- 4.9, P < 0.0001) and the contrast-to-noise ratio (21.0 +/- 4.3 vs. 15.8 +/- 4.3, P < 0.0001) showed significantly higher values for IR-SSFP sequences. The mean image quality scores were significantly higher for SSFP (3.6 +/- 0.7) than FLASH (2.8 +/- 0.9) sequences (P < 0.05)., Conclusion: IR-SSFP sequences show a considerable overall improvement in image quality compared to IR-FLASH sequences for MRCA after injection of a gadolinium-based blood pool contrast agent.

Isolated non-compaction of the ventricular myocardium (INVM), also known as left ventricular hypertrabeculation or spongy myocardium, belongs to the "unclassified" cardiomyopathies according to the World Health Organization. The main characteristic of this entity is a prominent trabeculation of the left ventricle with deep intertrabecular recesses communicating with the ventricular cavity. The pathomechanism of INVM is thought to be an arrest in cardiac myogenesis with persistence of embryonic myocardial morphology. The most frequent clinical manifestations include congestive heart failure, ventricular arrhythmias and systemic thromboembolic events. The therapy of INVM comprises standard medical therapy for heart failure.

Institutional review board approval and patient written informed consent were obtained. On two separate occasions, 24 hours apart, contrast-enhanced cardiac magnetic resonance (MR) imaging was performed prospectively at 1, 3, 5, 10, and 20 minutes after injection of gadopentetate dimeglumine and gadobenate dimeglumine in 15 patients (11 men, four women) with history of myocardial infarction. Both agents allowed detection of infarcted myocardium. T1 values at all times were significantly (P < .05) lower for gadobenate, compared with values for gadopentetate, in both infarcted and noninfarcted myocardium. At 1 minute after administration of both agents, T1 values in left ventricular cavity (LVC) were not different; at 3-20 minutes after injection, values were significantly (P < .05) lower for gadobenate. Differences between contrast-to-noise ratio (CNR) values of infarcted and noninfarcted myocardium were significantly higher on gadobenate-enhanced images (P < .05). CNR values between infarcted myocardium and LVC were significantly higher on gadopentetate-enhanced images (P < .05). Gadopentetate might permit better delineation of infarcts, especially subendocardial infarcts.

Objective: Our objective was to assess and compare the patterns of late enhancement (LE) in contrast-enhanced cardiac MRI caused by myocardial infarction and different myocardial diseases that are not related to ischemic infarction., Materials and Methods: A total of 811 consecutive contrast-enhanced cardiac MRI studies performed for different indications were reviewed for left ventricular myocardial LE after gadopentetate dimeglumine administration. MRI studies were performed on a 1.5-T scanner using an inversion recovery turbo FLASH sequence (TR/TE, 8/4 msec; flip angle, 25 degrees). The LE pattern of ischemic infarction scar was compared with that in nonischemic myocardial disease., Results: LE was found in 421 (52%) patients. In all patients with myocardial infarction, LE included the subendocardial layer. Nineteen patients without history of myocardial infarction and angiographically excluded coronary artery disease showed different patterns of LE caused by myocarditis, sarcoidosis, arrhythmogenic right ventricular dysplasia, cardiomyopathy, endomyocardial fibrosis, and iatrogenic scars after biopsy, ablation of septal hypertrophy, and myocardial laser revascularization., Conclusion: LE in contrast-enhanced cardiac MRI is not specific for ischemic infarction. LE in ischemic infarction always involves the subendocardial layer, whereas it does not necessarily do so in other myocardial diseases. Therefore, if LE omit the subendocardial layer, different nonischemic myocardial diseases have to be considered. The pattern of LE might be helpful for the differential diagnosis of myocardial disease and in distinguishing it from ischemic disease.

Purpose: To evaluate the impact of parallel acquisition techniques (PATs) on image quality and detection of liver metastases using three-dimensional volumetric interpolated breath-hold examination (VIBE) for clinical liver imaging., Materials and Methods: Forty-nine patients with various primary malignancies underwent abdominal dynamic contrast-enhanced three-dimensional VIBE magnetic resonance imaging (MRI) (1.5 T) using a standard phased array coil. Recently introduced Generalized Autocalibrating Partially Parallel Acquisition (GRAPPA) and SENSitivity Encoding (mSENSE) PAT reconstruction algorithms were added to reduce scan time twofold. Overall image quality, motion, and aliasing artifacts were classified on a 5-point scale. Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) measurements were performed for quantitative comparison. All sequences were evaluated concerning the number of detected lesions., Results: PAT resulted in a reduction of data acquisition time from 23 to 13 seconds. Both GRAPPA and mSENSE data sets yielded 30% less SNR (34.8 +/- 14.1 and 33.1 +/- 13.3, P < 0.001) and 35% less CNR (21.2 +/- 15.0 and 20.9 +/- 12.7, P < 0.05) in comparison to unaccelerated VIBE (SNR = 50.8 +/- 20.3/CNR = 32.5 +/- 19.1). Similarly, PAT revealed lower-image-quality scores than unaccelerated VIBE. GRAPPA resulted in more fold-over artifacts than mSENSE. mSENSE revealed slightly fewer motion artifacts than no PAT. The unaccelerated late-venous-phase VIBE sequence revealed 146 lesions in the same patients. Accelerated images with mSENSE reconstruction detected 138 lesions. GRAPPA revealed 127 lesions, and thus performed inferior to mSENSE., Conclusion: At least for arrays with small numbers of elements, such as arrays used in this study, the PAT-induced reduction in scanning times must be weighed against compromises in image quality, which translate into poorer diagnostic performance regarding detection of small hepatic lesions. Thus, the PAT implementations tested in this study should probably be reserved for patients unable to hold their breaths for regular three-dimensional VIBE data sets., (Copyright 2005 Wiley-Liss, Inc.)

Objectives: The objective of this study was to evaluate broad-beam spectral Doppler sonification of the vena contracta using a matrix-array transducer for quantification of mitral regurgitation (MR)., Background: Noninvasive assessment of the severity of valvular regurgitation remains challenging. A recent technique measures regurgitant flow directly at the vena contracta based on the product of velocity times backscattered Doppler power (proportional to orifice area). That approach, however, has been limited by relatively narrow conventional beamwidths. Matrix-array transducers, recently developed for three-dimensional imaging, can potentially provide broader beams. Therefore, we addressed the hypothesis that deliberate broadening of the Doppler beam can encompass larger regurgitant cross-sectional areas to capture a broader range of regurgitant flows., Methods: A matrix-array transducer system was modified to provide a three-dimensionally expanded spectral Doppler sample volume. Calculations of orifice area, flow rate, and regurgitant stroke volume (RSV) from Doppler power were automated on board a routinely used echocardiographic scanner and tested in vitro. In 24 patients with isolated MR, RSV was compared with magnetic resonance imaging (MRI) mitral inflow minus aortic outflow from phase-velocity maps., Results: The calculated flow rate and RSV correlated and agreed well with reference values in vitro (r = 0.98 to 0.99) and in patients (r = 0.93, mean difference 0.4 +/- 3.2 ml, p = NS vs. 0), with sufficient sonification to measure flow orifices up to 0.85 cm in diameter. Agreement with MRI was comparable in 17 patients with central and seven with eccentric jets (p = NS vs. 0)., Conclusions: The broad-beam spectral Doppler technique provides accurate, largely automated quantification of regurgitant flow rate and integrated RSV directly at the lesion. The accuracy related to broader sonification is made possible by the new matrix-array transducer design.

Purpose: Transthoracic echocardiography is the routine diagnostic procedure in assessing patients with left ventricular thrombi, but is limited by the acoustic window and poor contrast between thrombus and adjacent myocardium. This study evaluates the role of cardiac MRI in the detection of left ventricular thrombi in patients with chronic myocardial infarction compared to standard transthoracic echocardiography., Materials and Methods: In 82 patients (55 men and 27 women, age 36 to 79 years, median 59 +/- 11 years) who suffered a myocardial infarction more than 6 months earlier, transthoracic echocardiography and MRI were performed. The MRI protocol included steady state cine imaging (true FISP: TR 3.0 ms, TE 1.5 ms, FA 65 degrees ) in standard long and short axis orientation and contrast-enhanced imaging using a 3D IR-FLASH sequence with long inversion time (TR 4 ms, TE 1.43 ms, FA 10 degrees , TI 300 ms) early, and a 2D IR-FLASH sequence with optimized inversion time (TR 8 ms, TE 4.3 ms, FA 20 degrees , TI 180 - 280 ms) late after administration of gadolinium., Results: Transthoracic echocardiography depicted 12 thrombi. Contrast-enhanced MRI confirmed these 12 thrombi and detected 23 additional thrombi. With the exception of 2 very small apical thrombi only visible on contrast-enhanced MR images, spherical thrombi were diagnosed by both techniques, whereas only contrast-enhanced MRI was able to visualize mural thrombi. Left ventricular thrombi were more frequently diagnosed in patients with moderate to severe impairment of the left ventricular systolic function, 32/42 (76 %), or in patients with left ventricular aneurysms, 21/24 (84 %)., Conclusion: Contrast-enhanced MRI is mostly superior to transthoracic echocardiography in diagnosing mural left ventricular thrombi in patients who had suffered a myocardial infarction. Intracavitary thrombi are more frequently found in patients with impaired regional and global left ventricular function.

Objective: The purpose of our study was to quantify left ventricular function and mass derived from retrospectively ECG-gated 16-MDCT coronary angiography data sets using a new analysis software based on automatic contour detection in comparison to corresponding standard of reference measurements acquired with MRI., Subjects and Methods: Multiplanar reformations in the short-axis orientation were calculated from axial contrast-enhanced CT images in 18 patients (men, 15; women, three; age range, 38-70 years; mean, 57.4 +/- 10.2 [SD] years) who were referred for CT coronary angiography. End-diastolic volume (EDV), end-systolic volume (ESV), ejection fraction (EF), and left ventricular mass (LVM) were analyzed with a recently developed imaging software using an automated contour detection algorithm of left ventricular endo- and epicardial contours and by manual tracing. The data were compared with similar measurements on MRI as the standard of reference., Results: EDV, ESV, EF, and LVM derived from an automated contour detection algorithm were not statistically significantly different from manual tracing (CT(auto) vs CT(manual): EDV = 137.1 +/- 45.7 mL vs 134.2 +/- 39.9 mL, ESV = 58.8 +/- 34.2 mL vs 58.1 +/-30.1 mL, EF = 59.2% +/- 13.7% vs 58.1% +/- 12.0%, LVM = 130.9 +/- 29.1 g vs 133.7 +/- 33.2 g; p > 0.05). However, EDV (118.7 +/- 43.6 mL), ESV (50.1 +/- 33.5 mL), and LVM (142.8 +/-38.4 g) as calculated on MR data sets were statistically significantly different from those calculated on CT (p < 0.05), whereas MRI-based EF (59.9% +/- 14.4%) did not differ statistically significantly from those based on both CT algorithms (p > 0.05)., Conclusion: Automatic and manual analysis of data acquired during CT coronary angiography using a 16-MDCT scanner allows a reliable assessment of left ventricular ejection fraction and a rough estimation of left ventricular volumes and mass.

Objective: High diagnostic accuracy, emerging whole-body concepts, and lack of side effects combine to render MRI a natural candidate for screening purposes. The aim of this study was to evaluate the technical feasibility of a comprehensive multiorgan-targeting MRI examination and determine the frequency of findings in subjects without a history of serious disease., Subjects and Methods: The study group was composed of 331 subjects. The MRI protocol (mean examination time, 63 min) encompassed the target organs: the brain, arterial system, heart, and colon. Diagnoses were deemed relevant if the physician had to inform the subject about the findings. Subjects with a history of serious illnesses were excluded from subsequent analysis (n=33). All analyses were performed for the resulting subgroup of 298 subjects (247 men, 51 women; mean age, 49.7 years)., Results: All 298 examinations were diagnostic excluding eight MR colonography components in which remaining stool hampered reliable diagnosis. Follow-up or radiologic confirmation could be obtained in 75% of all cases with relevant findings (128/169); only one false-positive result was encountered. Of the study group, 21% exhibited signs of atherosclerotic disease. Two cerebral infarctions and one myocardial infarction, previously unknown, were encountered; 12% had peripheral vascular disease. Twelve colonic polyps and nine pulmonary lesions were correctly detected. Of all MRI examinations, 29% revealed relevant additional findings in nontargeted organs. Only one minor allergoid reaction was encountered., Conclusion: The presented data point toward an increased use of MRI for screening in the future, but to date screening MRI should not be performed outside a research setting because the cost-benefit relation is unclear.

Aim of the study was to evaluate whether late enhancement (LE) in contrast-enhanced MRI can be used to characterize fibrofatty myocardial replacement in patients with arrhythmogenic right ventricular cardiomyopathy/dysplasia (ARVC). Fifteen patients with suspected ARVC underwent CE-MRI using a 1.5 T scanner. Long and short axis SSFP cine images and T1-weighted fast spin echo images were collected in all patients. After injection of 0.2 mmol/kg Gd-DTPA (Magnevist, Schering, Berlin, Germany), inversion recovery gradient echo images were acquired in long and contiguous short axes to detect myocardial LE indicating areas of fibrous tissue within the myocardium. For definition of ARVC, the ESC Task force criteria were used. In 7 (47%) of 15 patients, ARVC was diagnosed based on the ESC criteria. In all of these 7 patients, MRI showed morphologic or functional criteria of ARVC according to the ESC. LE of the right ventricular myocardium was detected in 5 (71%) of the 7 ARVC patients, additional LE of the left ventricular myocardium in 2 of these patients. None of the 7 patients meeting the ARVC diagnostic criteria had fatty RV infiltration demonstrable by conventional T1-weighted imaging. Eight patients neither showed morphologic criteria of ARVC nor LE. In conclusion, late enhancement can be detected in the right and left ventricular myocardium in some ARVC patients. LE might represent intramyocardial areas of fibrous tissue.

The aim of this study was to assess a venous compression technique that is performed with contrast material-enhanced peripheral magnetic resonance (MR) angiography to reduce venous enhancement. Healthy volunteers, as well as patients with correlating digital subtraction angiographic (DSA) findings, were examined. Venous compression was accomplished by placing a cuff at the midfemoral level unilaterally. Arterial signal-to-noise and contrast-to-noise ratios indicated no significant differences between compressed and noncompressed legs. Venous overlay was substantially reduced in the compressed legs. MR angiography with venous compression yielded diagnostic image quality and results that had excellent correlation with DSA findings. High-spatial-resolution peripheral MR angiography of improved diagnostic quality appears feasible, even with long data acquisition times., ((c) RSNA, 2004)

Diagnosis of coronary artery disease (CAD) is a major challenge for medical imaging, because CAD is the leading cause of death in developed nations. Several non-invasive tests are used in clinical routine for the detection of CAD. However, due to limited sensitivity and specificity, the reliable diagnosis as well as the exclusion of CAD can only be established by catheter angiography. In patients with known CAD, therapeutic decisions require accurate information on myocardial function, ischemia and viability. Recently, magnetic resonance (MR) imaging has emerged as a non-invasive cardiac imaging technique that provides information on cardiac morphology, cardiac function, myocardial viability, and coronary morphology. This review discusses technical aspects and the clinical impact of different MR techniques.

Purpose: To increase gradient stimulation from 100% to a fixed 120% level and to assess patient acceptance of the degree of peripheral nerve stimulation (PNS) at magnetic resonance (MR) imaging., Materials and Methods: Two hundred ten patients underwent MR imaging of various body regions according to clinical indications. An additional three-dimensional fast low-angle shot sequence with the 120% stimulation level was performed. A patient questionnaire was distributed after MR imaging to document the presence, degree, and location of PNS. Degree was measured with an 11-point scale (score range, 0-10). Age was analyzed between the sexes for significant statistical differences. Furthermore, correlation between location of examination and location and degree of stimulation was performed. To determine stimulation discomfort relative to other factors typically present at MR imaging, the degree of discomfort due to room temperature, size of magnet bore, acoustic noise, examination time, and heating sensation was determined for comparison, as well., Results: Thirty-five (16.7%) patients reported a stimulation sensation during imaging in one or more locations, while six (2.9%) felt very uncomfortable local stimulation (score of 8-10). No significant difference between male and female patients regarding age, sex, and appearance or degree of stimulation sensation could be detected. No significant correlation between location of examination and location and degree of stimulation was recorded. Compared with other side effects, PNS was considered relatively unimportant., Conclusion: The 120% gradient stimulation level seems acceptable for routine clinical imaging with this gradient system, since only 2.9% of patients experienced very uncomfortable local stimulation.

In 30 patients, Agatston and volumetric scores were assessed by using retrospectively gated multi-detector row computed tomography (CT). For each patient, 10 data sets were created at different times and were evenly spaced throughout the cardiac cycle. For each reconstruction, patients were assigned a percentile that described the level of cardiovascular risk. Nineteen (63%) of 30 patients could be assigned to more than one risk group depending on the reconstruction interval used. Agatston and volumetric scores both proved highly dependent on the reconstruction interval used (coefficient of variation, < or =63.1%) even with the most advanced CT scanners. Accurate and reproducible quantification of coronary calcium seems to require analysis of multiple reconstructions.

Objectives: The aim of this study was to evaluate the diagnostic accuracy of a 16-detector row computed tomography (CT) scanner for the assessment of coronary artery bypass grafts., Background: A new generation of multislice spiral CT scanners, equipped with more and thinner detector rows, allows for reliable noninvasive detection of obstructive coronary artery disease., Methods: The study included 51 consecutive patients. Three patients had to be excluded from the study due to arrhythmias or fast heart rates despite beta-blockade. A total of 48 patients with 131 coronary artery bypass grafts (internal mammary artery, n = 40; venous grafts, n = 91) were examined by computed tomography angiography (CTA) and by invasive coronary angiography (ICA) using a 16-detector row CT scanner. For cardiac protocols, only the 12 inner detector rings are applied. All CT examinations were performed with retrospective electrocardiogram gating at a mean heart rate of 64 +/- 5 beats/min; 120 ml of Xenetix 300 (Guerbert GmbH, Sulzbach, Germany) were continuously injected. The bypass graft patency and the presence of stenoses as well as the proximal and distal anastomoses were evaluated by two experienced readers., Results: All bypass grafts and 74% of the distal bypass anastomoses could be visualized by CTA; 21 bypass graft occlusions and 1 significant stenosis were detected by CTA and confirmed by ICA. Five false positive and one false negative finding resulted in a sensitivity of 96%, a specificity of 95%, a positive predictive value of 81%, and a negative predictive value of 99%., Conclusions: Sixteen-detector row CT scanner technology allows for the reliable visualization of coronary bypass grafts. Dysfunctional bypass grafts can be detected with high diagnostic accuracy. This technology can be used as a noninvasive test for patients with suspected graft dysfunction.

Purpose: To compare image quality, artifacts, and signal-to-noise ratio (SNR) in cardiac cine TrueFISP magnetic resonance imaging (MRI) with and without parallel acquisition techniques (PAT)., Materials and Methods: MRI was performed in 16 subjects with a TrueFISP sequence (1.5 T; Magnetom Sonata, Siemens): TR, 3.0 msec; TE, 1.5 msec; flip angle (FA), 60 degrees. Three axes were scanned without PAT (no PAT) and using the generalized autocalibrating partially parallel acquisition (GRAPPA) and modified sensitivity encoding (mSENSE) reconstruction algorithms with an autocalibration mode to reduce scan time. A conventional spine array and a body flex array were used. Artifacts, image noise, and overall image quality were classified on a 4-point scale by an observer blinded to the implemented technique; for quantitative comparison, SNR was measured., Results: With a PAT factor of two, acquisition time could be reduced by 39%. No PAT did not show artifacts, and GRAPPA revealed fewer artifacts than mSENSE. PAT provided inferior-quality scores concerning image noise and overall image quality. In quantitative measurements, GRAPPA and mSENSE (20.1 +/- 6.2 and 15.6 +/- 6.2, respectively) yielded lower SNR than no PAT (30.6 +/- 20.1; P < 0.05) and P < 0.001)., Conclusion: Time savings in PAT are accompanied by artifacts and an increase in image noise. The GRAPPA algorithm was superior to mSENSE concerning image quality, noise, and SNR., (Copyright 2004 Wiley-Liss, Inc.)

Objective: A comparative investigation of dynamic three-dimensional freehand echocardiography (D3DFE) and magnetic resonance imaging (MRI) was conducted to determine the accuracy and rapidity of the average rotation method (ARM) and the disk summation method (DSM) for volumetric analysis., Methods: In 15 patients with an asymmetric left ventricle and 12 normal subjects, end-diastolic and end-systolic left ventricular volumes were assessed by D3DFE and by MRI. Both DSM and ARM were used for volume determination. All echocardiographic readings were performed by two examiners blinded to each other and to the MRI results. The times needed for echocardiographic data acquisition and volumetric analysis with either algorithm were determined., Results: Correlation between ARM and MRI measurements was tighter than between DSM and MRI measurements (end-diastolic volume: r=0.95, P<0.0001 versus r=0.94, P<0.0001 in asymmetric ventricles; and r=0.97, P<0.0001 versus r=0.96, P<0.0001 in symmetric ventricles; end-systolic volume: r=0.94, P<0.0001 versus r=0.93, P<0.0001 in asymmetric ventricles and r=0.96, P<0.0001 versus r=0.94, P<0.0001 in symmetric ventricles). In addition, ARM analysis was less time-consuming than DSM (6.4+/-0.4 min versus 7.6+/-0.3 min, P<0.05)., Conclusions: For D3DFE, ARM is the most accurate and rapid approach to left ventricular volume determination. ARM benefits from advanced two-dimensional imaging and can be easily added to any standard transthoracic echocardiographic examination.

Objective: The purpose of our study was to compare the diagnostic accuracy of an ultrafast ECG-triggered black blood-prepared HASTE sequence with chest radiography for the detection of pulmonary nodules. SUBJECTS AND METHODS. Sixty-four patients with various primary malignancies who had undergone radiography and MDCT of the chest also underwent ECG-triggered black blood-prepared HASTE MRI of the lung. MR images and radiographs were interpreted separately. The number, location, and size of detected lesions were recorded, and each hemithorax was classified as affected or not affected on the basis of a grade reflecting the conspicuity of nodular involvement. Sensitivity, specificity, and positive and negative predictive values for the detection of pulmonary nodules with diameters of 5 mm or larger were determined, using MDCT findings as the standard of reference. Lesions with diameters smaller than 5 mm were not evaluated. Additional lesion-by-lesion comparisons between MDCT and MRI findings were performed., Results: MDCT confirmed pulmonary lesions in 32 patients, whereas HASTE MRI revealed lesions in 30 patients and chest radiography, in 19 patients. MDCT revealed 226 nodules in 32 patients, whereas MRI HASTE revealed 227 lesions in 30 patients. Conspicuity scale-based sensitivity and specificity for chest radiography were 55.8% and 92.4%, respectively, whereas HASTE MRI had a sensitivity of 93.0% and a specificity of 96.2%. Positive and negative predictive values for chest radiography were 80% and 79.3%, respectively, and for HASTE MRI, 93.0% and 96.2%, respectively. The sensitivity of HASTE MRI increased with lesion size, ranging from 94.9% for nodules between 5 and 10 mm in diameter to 100% for lesions exceeding 3 cm in diameter., Conclusion: ECG-triggered black blood-prepared HASTE MRI is reliable for detecting pulmonary nodules exceeding 5 mm and has proven significantly more accurate than conventional chest radiography. The technique appears useful as an adjunct to MRI of the heart, great vessels, or chest, potentially increasing the diagnostic yield of MRI examinations.

Coronary artery disease (CAD) is currently the leading cause of death in developed nations. Reflecting the complexity of cardiac function and morphology, noninvasive diagnosis of CAD represents a major challenge for medical imaging. Although coronary artery stenoses can be depicted with magnetic resonance (MR) and computed tomography (CT) techniques, its functional or hemodynamic impact frequently remains elusive. Therefore, there is growing interest in other, target organ-specific parameters such as myocardial function at stress and first-pass myocardial perfusion imaging to assess myocardial blood flow. Recent improvements in hardware and software, lack of side effects, as well as diagnostic accuracy make magnetic resonance imaging a natural candidate for preventative imaging. Thus, the purpose of the study was to evaluate the feasibility of a comprehensive 60-min MR-based screening examination in healthy volunteers and a limited number of patients with known target disease. In ten healthy volunteers (7 men, 3 women; mean age, 32.4 years) and five patients (4 men, 1 woman; mean age, 56.2 years) with proven target disease we evaluated the performance of a comprehensive MR screening strategy by combining well-established organ-based MR examination components encompassing the brain, the arterial system, the heart, the lungs, and the colon. All ten volunteers and five patients tolerated the comprehensive MR examination well. The mean in-room time was 63 min. In one volunteer, insufficient colonic cleansing on the part of the volunteer diminished the diagnostic reliability of MR colonography. All remaining components of the comprehensive MR examination were considered diagnostic in all volunteers and patients. In the five patients, the examination revealed the known pathologies [aneurysm of the anterior communicating artery ( n=1), renal artery stenosis ( n=1), myocardial infarct ( n=1), and colonic polyp ( n=2)]. The outlined MR screening strategy encompassing the brain, the arterial system, the heart, the lung, and the colon is feasible. Further studies have to show that MR-based screening programs are cost-effective in terms of the life-years saved.

The aim of this study was to compare the image quality of a saturation-recovery gradient-recalled echo (GRE; TurboFLASH) and a saturation-recovery SSFP (SR-TrueFISP) sequence for myocardial first-pass perfusion MRI. Eight patients with chronic myocardial infarction and 8 volunteers were examined with a TurboFLASH (TR 2.1 ms, TE 1 ms, FA 8 degrees ) and a SR-TrueFISP sequence (TR 2.1 ms, TE 0.9 ms, FA, 50 degrees ) on a 1.5 T scanner. During injection of 0.05 mmol/kg BW Gd-DTPA at 4 ml/s, three short axis slices (8 mm) of the left ventricle (LV) were simultaneously scanned during breath-hold. Maximum signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) between infarcted and normal myocardium, and percentage signal intensity change (PSIC) were measured within the LV lumen and in four regions of the LV myocardium for the three slices separately. For the LV lumen, SR-TrueFISP was superior in SNR and PSIC (factor 3.2 and 1.6, respectively). Mean maximum SNR, PSIC, and CNR during peak enhancement in the LV myocardium were higher for SR-TrueFISP compared with TurboFLASH (factor 2.4, 1.25, and 1.24, respectively). The SNR was higher in the septal portion of the ventricle than in anterior/posterior and lateral regions. The SR-TrueFISP provides higher SNR and improves image quality compared with TurboFLASH in first-pass myocardial perfusion MRI.

Purpose: To assess SH L 643A for three-dimensional breath-hold and respiratory-gated magnetic resonance (MR) imaging in the depiction of coronary arteries., Materials and Methods: Twelve healthy male volunteers underwent either three-dimensional breath-hold (n = 6) or respiratory-gated (n = 6) coronary MR angiography before and after intravenous injection of 0.1 mmol SH L 643A per kilogram of body weight. For nonenhanced and contrast material-enhanced examinations, signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) measurements were obtained. Image quality was assessed in consensus with a five-point scale. Statistical analysis of nonenhanced and contrast-enhanced images was based on a two-tailed paired Student t test. A P value at the.05 significance level was used., Results: Overall statistically significant improvement in CNR was observed after administration of SH L 643A compared with that on nonenhanced images (8.7 +/- 5.3 [SD] vs 23.6 +/- 7.2, P <.01). While SNR of contrast-enhanced images showed improvement over that of nonenhanced images, the difference was not statistically significant (25.4 +/- 0.8 vs 30.2 +/- 16.8, P >.2). Image quality improved from a mean of 3.1 +/- 0.8 for nonenhanced images to 4.0 +/- 0.8 (P <.01) for contrast-enhanced images., Conclusion: SH L 643A causes significant improvement of the blood-myocardium contrast enhancement at coronary MR angiography compared with that with nonenhanced sequences., (Copyright RSNA, 2003)

Techniques to reliably quantify left ventricular myocardial mass (LVMM) are mandatory for monitoring therapy in patients with left ventricular hypertrophy (LVH). The purpose of this study was to measure LVMM and volumes by cine magnetic resonance imaging (MRI), and to assess acute changes through hemodialysis as a model for different loading states. Seven dialysis patients with LVH were examined before and immediately after hemodialysis. All MR imaging was done with a steady-state free precession (SSFP) cine sequence (TrueFISP; TR, 3.2 ms; TE, 1.6 ms; flip angle, 60 degrees; slice thickness, 8 mm). LV volumes, ejection fraction (EF), and LVMM were determined by slice summation after manual planimetry in short axes. A significant reduction of end-diastolic volume (EDV) (mean pre, 140 mL; post, 109 mL; p < 0.01), end-systolic volume (ESV) (49 mL-->42 mL; p < 0.05), and stroke volume (91 mL-->66 mL; p < 0.01) through dialysis was revealed by MRI. Ejection fraction did not change significantly. A slight decrease in LVMM was detected in all patients (mean pre, 184 g; post, 177 g; p < 0.05). Intra- and interobserver variability for EDV, ESV, and LVMM were 1.3 +/- 6.2 mL, -0.9 +/- 4.1 mL, -1.4 +/- 3.9 g, and 3.3 +/- 7.5 mL, 2.6 +/- 5.0 mL, -2.4 +/- 4.6 g, respectively. Standard error of estimation (SEE) was +/- 2.3 mL, +/- 2.0 mL, +/- 1.6 g, and +/- 2.6 mL, +/- 2.1 mL, and +/- 2.0 g for intra- and interobserver variability. In conclusion, cine MRI is a reliable technique for LVMM measurement that is independent of LV loading status. This method allows for detection of small changes, which is crucial for accurate therapy monitoring in LVH. Left ventricular myocardial mass and volumes decrease significantly during hemodialysis.

Purpose: To measure the effective radiation doses delivered at electron-beam computed tomography (CT) and multi-detector row spiral CT of coronary arteries and to compare these doses with those delivered at catheter coronary angiography., Materials and Methods: An anthropomorphic phantom equipped with 66 thermoluminescent dosimeters was imaged at cardiac CT. Four protocols for unenhanced coronary artery calcium scoring were simulated: one with electron-beam CT and three with multi-detector row CT. Four similar protocols for coronary CT angiography were simulated. All multi-detector row spiral CT protocols were performed with retrospective electrocardiographic triggering. Biplane catheter coronary angiography also was simulated. Radiation doses to organs were measured, and effective doses were calculated according to guidelines published in International Commission on Radiological Protection Publication 60., Results: Coronary artery calcium scoring with electron-beam CT yielded effective radiation doses of 1.0 and 1.3 mSv for male and female patients, respectively. The radiation doses at calcium scoring with multi-detector row CT were 1.5-5.2 mSv for male patients and 1.8-6.2 mSv for female patients. Electron-beam CT coronary angiography yielded effective doses of 1.5 and 2.0 mSv for male and female patients, respectively. The highest effective doses were delivered at multi-detector row CT angiography: 6.7-10.9 mSv for male patients and 8.1-13.0 mSv for female patients. Catheter coronary angiography yielded effective doses of 2.1 and 2.5 mSv for male and female patients, respectively., Conclusion: Higher radiation doses are delivered at multi-detector row cardiac CT compared with the doses delivered at electron-beam CT and catheter coronary angiography., (Copyright RSNA, 2002)

Background: Left ventricular (LV) volume-time curves (VTC) have been described to provide quantitative data on the dynamics of global LV performance beyond ejection fraction. However, generation of VTCs by conventional 2-dimensional imaging techniques is inherently limited because of inaccurate geometric volume assumptions. We, therefore, studied whether the new concept of volumetric scanning as realized by real-time 3-dimensional echocardiography (RT-3DE) can be used to provide accurate VTCs., Methods: In 30 healthy participants, VTCs were generated from 18 to 24 absolute LV volumes per second by transthoracic RT-3DE and compared with magnetic resonance imaging (MRI) used for reference. LVs were traced manually in 9 to 11 parallel, short-axis planes and volumes calculated by disk method. From VTCs, we determined peak ejection rate (PER), peak early filling rate (PFR), time to PER and PFR, and end-diastolic and end-systolic volumes. For initial clinical application, 2 patient groups of coronary (n = 15) and hypertensive heart disease (n = 16) were studied., Results: In healthy participants, VTCs agreed with MRI (mean errors: PER, -39 +/- 67 mL/s; PFR, -18 +/- 84 mL/s; time to PER, 8 +/- 21 milliseconds; time to PFR 4 +/- 18 milliseconds [not significant vs 0]) whereas VTCs in coronary and hypertensive groups revealed significantly impaired diastolic function. Scanning time for VTCs was only 1 to 2 minutes by RT-3DE and 8 +/- 2 minutes by MRI (P <.001) and time for offline analysis was 22 +/- 5 minutes versus 24 +/- 4 minutes by MRI (not significant)., Conclusions: Generation of VTCs by RT-3DE is feasible and shows excellent agreement with MRI used for reference. Thus, VTCs by RT-3DE is a promising new approach providing access to quantitative information on global LV performance such as LV filling rates that is currently unavailable for the cardiologist.

Objective: The aim of our study was to compare the diagnostic accuracy achieved using different MR techniques with the diagnostic accuracy achieved using transthoracic and transesophageal echocardiography to detect intracardiac thrombi., Materials and Methods: Twenty-four patients with known or suspected intracardiac thrombi were examined using MR imaging and echocardiography. All MR examinations were performed on a 1.5-T MR scanner using dark-blood-prepared half-Fourier acquisition single-shot turbo spin-echo (HASTE) sequences, fast imaging steady-state free precession (trueFISP) cine sequences, and inversion recovery gradient-echo fast low-angle-shot (inversion recovery turbo FLASH) sequences after injection of 0.2 mmol/kg of gadolinium diethylene triamine pentaacetic acid., Results: MR imaging and echocardiography revealed 12 thrombi-two in the right atrium, one in the right ventricle, three in the left atrium, and six in the left ventricle. Compared with echocardiography, MR imaging revealed three additional thrombi in the left ventricle; these thrombi were confirmed at surgery. All 15 thrombi appeared as filling defects on early contrast-enhanced inversion recovery turbo FLASH MR images. Only seven thrombi were detected on HASTE images, and 10 thrombi were seen on trueFISP images. Four thrombi showed enhancement 10-20 min after contrast material injection and were characterized as organized clots., Conclusion: Contrast-enhanced inversion recovery turbo FLASH sequences were superior to dark-blood-prepared HASTE and trueFISP cine MR images in revealing intracardiac thrombi. Compared with transthoracic echocardiography, MR imaging was more sensitive for the detection of left ventricular thrombi. The characterization of thrombi may be used to predict the risk of embolism, which is higher for subacute clots than for organized thrombi.

Purpose: To compare contrast-enhanced magnetic resonance imaging (MRI) and positron emission tomography (PET) in the evaluation of myocardial viability., Methods: [ (18)F]-FDG-PET, [ (201)Tl]-TlCl-SPECT and contrast-enhanced MRI were performed in 29 patients with proven coronary artery disease and impaired left ventricular function to assess myocardial viability. MRI scans were done on a 1.5 T scanner (Magnetom Sonata, Siemens, Germany). After the steady-state free precession cine study, 0.2 mmol/kg BW of Gd-DPTA (Magnevist(R), Schering, Germany) were administered i. v. For the detection of "late enhancement" (LE) indicating scar, left ventricular long axes and contiguous short axis slices of 8 mm thickness were scanned using an inversion recovery turbo gradient echo sequence (TR 8.0 ms; TE 4.0 ms; TI 180 - 240 ms; FA 20 degrees ). The evaluation of LE and FDG uptake in PET with perfusion defect in SPECT was done using an 8 (basal, mid) and 4 (apical) segment model in all short axes to cover the entire ventricle. The transmural extent of LE was assessed using a 4-point score system. Comparison between the two modalities was performed on a segmental basis., Results: A total of 1753 segments were assessed. In MRI, 40 % of the segments showed myocardial scar, whereas PET revealed impaired uptake in 25 %. MRI obtained a very low interobserver variability in detecting myocardial scar (kappa 0.92). Using PET as the standard of reference in the segmental comparison, contrast-enhanced MRI yielded a sensitivity of 84 % and a specificity of 76 % for the detection of scar. 18 % of all segments showed LE but normal FDG uptake, 83 % of them referred to subendocardial scars., Conclusions: There is close agreement between contrast-enhanced MRI and PET in detecting transmural myocardial scars. Superior spatial resolution enables MRI to detect and quantify even subendocardial scar. Therefore, larger studies using functional recovery after revascularisation as an endpoint have to prove whether MRI might replace PET as the standard of reference in the assessment of myocardial viability.

Aim: Purpose of our study was to compare the image quality of 3D-navigator steady state free precession (SSFP) and gradient echo (GE) sequences for magnetic resonance coronary angiography (MRCA) in volunteers and patients., Methods: Following informed consent 8 volunteers and 12 patients were included into this study. In all subjects a 3D navigator MRCA of the right and the left coronary artery was performed with a SSFP (TR 3.9 ms, TE 1.7 ms, FA 65 degrees, bandwidth 540 Hz) and a GE (TR 5.8 ms, TE 2.2 ms, FA 25 degrees, bandwidth 200 Hz) sequence using a 1.5 T-MR-System (Magnetom Sonata, Siemens Erlangen). The slice thickness was 1.5 mm and the in-plane resolution was 0.9 x 0.7 mm (2) for all measurements., Results: The blood pool showed a significantly (p < 0.01) higher signal intensity on SSFP images (147 +/- 36) compared to GE images (103 +/- 36). Although noise increased with SSFP (9.3 +/- 1.4 versus 5.3 +/- 0.9), the contrast-to-noise ratio between myocardium and the coronaries was significantly (p < 0.01) higher on SSFP images (7.8 +/- 3.7 versus 3.4 +/- 3.3). The CNR between the coronaries and the epicardial fat showed no significant differences (12 +/- 5 versus 13 +/- 4)., Conclusion: The 3D-navigator SSFP sequence is a promising new technique for MRCA which improves the contrast between the coronaries and the myocardium and shortens the data acquisition compared to gradient-echo imaging.

Background: Beyond conventional echocardiographic Doppler methods allowing only semiquantitative estimation of the severity of valvular regurgitation, new approaches are attempting to quantify regurgitant flow as a measure of left ventricular volume overload. Different concepts are leading to significant differences in accuracy and feasibility in clinical routine between the methods. We are reviewing the existing methods with their advantages and limitations as well as the underlying hemodynamic concepts., Echocardiographic Methods for Estimation of Severity: Semiquantitative methods are jet area/jet length method, proximal jet width as well as the pressure half-time method. Determination of regurgitant flow is permitted by means of the continuity method, PISA ("Proximal Isovelocity Surface Area") method, ACOM ("Automated Cardiac Output Measurement") technique, and the PVI ("Power-Velocity Integral") method. Grading of severity is usually based on a scale from 1 to 3 or 1 to 4 where semiquantitative methods are limited by a significant overlap of the individual degrees and therefore only provide rough estimates of severity. Compared to this, quantitative methods allow quantitative determination of regurgitant flow, regurgitant volume, regurgitant fraction, and effective regurgitant orifice area based on different Doppler flow measurements, that are again affected by specific limitations: (1) the continuity method is considerably limited because it is measuring the flow through two different valves; (2) the PISA method is dependent on the geometry of the proximal flow convergence zone and requires highly skilled observers; (3) the ACOM method is also dependent on the geometry of the proximal flow convergence zone und ideally requires three-dimensional color Doppler datasets; (4) the PVI method provides direct measurement of regurgitant flow from a pulsed Doppler signal of sufficient high quality, however, this method is not widely available, yet. For the decision which method to apply the following should be taken into account: (1) interpretation of semiquantitative findings depends on the extent by which the methods' estimation of flow deviates from a measurement of flow; (2) accuracy of the quantitative methods depends on the underlying hemodynamic concept and the simplifying assumptions., Conclusion: Current echocardiography provides a broad spectrum of semiquantitative und quantitative Doppler methods for the estimation of regurgitant flow. Since semiquantitative methods only allow rough estimates of severity, quantitative measurement of regurgitant volume/fraction or effective regurgitant orifice area should be attempted in any case with uncertainty whether regurgitation is mild, moderate, or severe.

Background: Cardiovascular diseases are very common in patients with end-stage renal disease and are the underlying cause of approximately half the deaths in dialysis patients. In those patients vascular calcifications are typically seen in the tunica media and therefore represent histopathological changes different to those in atherosclerosis. For the evaluation of cardiovascular risk in chronic dialysis patients, a method is needed to reliably identify patients who have to undergo invasive diagnostics. Coronary artery calcium assessed by electron beam computed tomography (EBT) has been suggested to measure the extent of coronary atherosclerosis in patients with normal renal function. It might also be of value in the stratification of cardiovascular risk in patients with renal failure., Case Report: We report the case of a 21-year-old female with end-stage renal disease as a result of idiopathic infantile hypercalcemia with nephrocalcinosis and extensive vascular calcification. Due to a total atrioventricular block, a 2-chamber pacemaker had been implanted in 1998. Because of a very high calcium score in EBT, depressed left ventricular function in echocardiography, and limited physical strength she was sent to coronary angiography. Severe 2-vessel coronary artery disease was found and successfully treated by balloon angioplasty. She was then listed for renal transplantation. Angiographic 6-month follow-up showed an excellent long-term result., Conclusion: This case demonstrates that coronary artery disease can also be present in young dialysis patients. Discrimination of atherosclerotic vascular calcification and tunica media sclerosis typical in dialysis patients without hemodynamic effects is difficult. EBT allows for the direct detection of coronary calcium. Its extent correlates with coronary atherosclerosis and the probability of acute coronary syndromes in patients with normal renal function. This evidence has not been proved for dialysis patients, yet. However, EBT may be of value in the stratification of cardiovascular risk for patients awaiting renal transplantation.

In a 73 year-old male patient with generalized atherosclerosis, known infrarenal abdominal aortic aneurysm, renal artery stenosis, and coronary artery disease, an aneurysm of the proximal left subclavian artery was successfully excluded by implantation of a JOSTENT-Peripheral stent graft. Angiographic follow up after 6 and 12 months showed an excellent outcome with complete exclusion of the aneurysm. Intravascular ultrasound showed no neo-intimal hyperplasia within the stent. A computed tomography revealed complete thrombosis of the aneurysm.

Conventional manual compression and subsequent application of pressure bandages is associated with prolonged immobility and significant patient discomfort. Routine anticoagulation as well as the use of new interventional devices and platelet inhibiting strategies lead to a higher incidence of local bleeding complications after diagnostic cardiac catheterization or coronary angioplasty. Immediate sheath removal increases patient comfort. The Angio-Seal system uses a biodegradable anchor and collagen plug for sealing of arterial puncture sites. Several studies showed the safety and efficacy of this device. Technical deployment success ranges between 88 and 100%. Significant reduction in time to hemostasis allows for earlier patient ambulation and shorter in-hospital stay compared to manual compression with peripheral complications not being increased.

Background: Two-dimensional (2D) echocardiographic approaches based on geometric assumptions face the greatest limitations and inaccuracies in patients with left ventricular (LV) aneurysms. Three-dimensional (3D) echocardiographic techniques can potentially overcome these limitations; to date, however, although tested in experimental models of aneurysms, they have not been applied to a series of patients with such distortion. The purpose of this study was therefore to validate the clinical application of tomographic 3D echocardiography (3DE) by the routine transthoracic approach to determine LV chamber size and systolic function without geometric assumptions in patients with LV aneurysms., Methods and Results: In 23 patients with chronic stable LV aneurysms, LV end-systolic and end-diastolic volumes (LVEDV, LVESV) and ejection fraction (LVEF) by tomographic 3DE were compared with results from 3D magnetic resonance tomography (3DMRT) as an independent reference as well as with the conventional techniques of single plane and biplane 2D echocardiography and biplane cineventriculography. Dynamic 3DE image data sets were obtained from a transthoracic apical view with the use of a rotating probe with acquisition gated to control for ECG and respiration (Echoscan, TomTec). Volumes were calculated from the 3D data sets by summating the volumes of multiple parallel disks. 3DE results correlated and agreed well with those by 3DMRT, with better correlation and agreement than provided by other techniques for LVEDV (3DE: r=.97, SEE=14.7 mL, SD of differences from 3DMRT=14.5 mL; other techniques: r=.84 to .93, SEE=30.7 to 41.6 mL [P<.001 versus 3DE by F test], SD of differences=31.5 to 40.7 mL [P<.001 versus 3DE by F test]). The same also pertained to LVESV (3DE: r=.97, SEE=12.4 mL, SD of differences=12.9 mL; other techniques: r=.81 to .90, SEE=24.7 to 37.2 mL [P<.001], SD of differences=27.6 to 36.8 mL [P<.005]) and LVEF (3DE: r=.74, SEE=5.6%, SD of differences=6.7%; other techniques: r=.14 to .59, SEE=9.5% to 10.1% [P<.01], SD of differences=9.5% to 12.6% [P<.05]). Compared with 3DMRT, 3DE was less time consuming and patient discomfort was less., Conclusions: Tomographic 3DE is an accurate noninvasive technique for calculating LV volumes and systolic function in patients with LV aneurysm. Unlike current 2D methods, tomographic 3DE requires no geometric assumptions that limit accuracy.
