Are people more likely to be satisfied with their lives if they had freedom from regulations, if they had the ability to trade freely internationally? In light of the demographic aging phenomenon we are facing, the present study analyzes the relationship between economic freedom and life satisfaction among European older adults. In order to do so, we are using data from the European Health, Ageing, and Retirement Survey and Economic Freedom of the World. By using the ordered logit regression method, we estimated different models to identify how sub-indicators of economic freedom affect the subjective well-being. According to the findings, the quality of the institutions that define the legal system and establish rules for the protection of private property and sound monetary policy have a positive effect on subjective well-being. On the other hand, openness to international trade has a negative effect and government and regulation doesn’t show any significance.

It cannot be denied that innovation is a process that is happening continuously and in every sector of the economy. This is why the question who is the main force behind innovation appeared. Some scholars think that the private sector does the job, while others believe that the university and the public sector play a big role in creating innovation. Even so, as this qualitative research shows, neither actor is more important than the other, as they all contribute to generating innovation. Moreover, the Entrepreneurial State theory by M. Mazzucato and the Triple-Helix Model by Etzkowitz H./Leydesdorff L. come to explain that the state and university has a significant role in innovation, similar to the private sector. On top of it, the Triple-Helix Model suggests that the innovation of nowadays is a result of interconnections between the university-industry-government and it cannot develop without the other. Thus, this leads to the idea that innovation is happening because of more factors that need to be properly commercialized and made visible to the public in order to change the misunderstanding created.

458. Central European Free Trade Agreement: did the initiative prove its commitments?

CEFTA 2006 is an important cooperation platform providing the Western Balkan countries and the Republic of Moldova with the opportunity to share experience and build functional economic relations. It aims to improve the economic development of the states through assuring a favourable environment for trade and investments. Moreover, CEFTA is viewed as a preparatory instrument for the future European integration of the states. It has been 10 years since the formation of CEFTA. Over this period, numerous economists, politicians and researchers debated on whether the initiative proved its commitments and what the impact of CEFTA on regional economic development, and in particular, on regional trade and investments is. Therefore, to reach relevant conclusions about the researched subject, the present paper is focused on quantitative data analysis involving descriptive and econometric methods. The analysis reflects the CEFTA general economic, trade and investments performances by examining the respective indicators including GDP per capita, GDP growth rate, total trade, imports/exports and net foreign direct investments inflows. Also, several inferences based on the correlation analysis of individual countries and the average CEFTA indicators in terms of per capita GDP, exports/ imports, net FDI inflows are provided. The results obtained show that the CEFTA’s primordial achievement is the de-tensioning and improvement of the political-economic evolution in the Western Balkans, followed by some trade benefits and a modest rise in the countries’ FDI performance. We have reached the conclusion that the agreement is primarily beneficial to the countries’ economic relations with their neighbours and the EU, having a favourable, yet, not determinative impact on economic development, trade and investments.

The global economic crisis has revealed the main weaknesses of the European Union’s economy, including slow decision making, over-regulation and excessive bureaucratisation, lack of economic cohesion among the various nations of the EU, uneven competitive readiness and poor economic performance & development. Less flexible and more regulated, the European Union’s entrepreneurial environment is undermined by excessive governmental intervention justified by “soziale Marktwirtschaft” or the social market economic principles. The main stake of the European Union and member countries’ decision making factors and elites relies on stimulating entrepreneurial and public innovation as to raise the level of economic competitiveness. In such a way, it is possible to keep the present standards of living imposed by the principles of social market economy, and report at the same time competitiveness of more liberal-capitalistic economies. Taking into account these circumstances, the present research has established the goal to identify to which extent the initiatives promoted by the European Union in the area of innovation and entrepreneurship proved to have a beneficial effect upon community’s economic competitiveness taking into account the modern day economic challenges. As a result, there have been formulated two main hypotheses and namely, H1, which states that entrepreneurial innovation could not be compensated with public driven innovation policies and, H0, identifying the vice-versa and namely that governmental supported innovation can offset entrepreneurial innovation.

Abstract Background As more hospitals adopt Electronic Health Records (EHR), focus has shifted to how these records can be used to improve patient care. One barrier to this improvement is limited information exchange between providers. In this work we examine the role of EHR vendors, hypothesizing that vendors strategically control the exchange of clinical care summaries. Their strategy may involve the creation of networks that easily exchange information between providers with the same vendor but frustrate exchange between providers with different vendors, even as both Federal and State policies attempt to incentivize exchange through a common format. Methods Using data from the 2013 American Hospital Association’s Information Technology Supplement, we examine the relationship between a hospital’s decision to share clinical care summaries outside of their network and EHR vendor market share, measured by the percentage of hospitals that have the same vendor in a Hospital Referral Region. Results Our findings show that the likelihood of a hospital exchanging clinical summaries with hospitals outside its health system increases as the percentage of hospitals with the same EHR vendor in the region increases. The estimated odds of a hospital sharing clinical care summaries outside their system is 5.4 (95% CI, 3.29–8.80) times greater if all hospitals in the Hospital Referral Region use the same EHR Vendor than the corresponding odds for a hospital in an area with no hospitals using the same EHR Vendor. When reviewing the relationship of vendor market concentration at the state level we find a positive significant relationship with the percentage of hospitals that share clinical care summaries within a state. We find no significant impact from state policies designed to incentivize information exchange through the State Health Information Exchange Cooperative Program. Conclusion There are benefits to exchanging using proprietary methods that are strengthened when the vendors are more concentrated. In order to avoid closed networks that foreclose some hospitals, it is important that future regulation attempt to be more inclusive of hospitals that do not use large vendors and are therefore unable to use proprietary methods for exchange.

495. Status of the audience of the National Institute of Justice: comparative aspect and issues

During the important reforms in the field of justice in the Republic of Moldova, premises are being created to promote the general reform process and to comply with the widely recognized and internationally implemented democratic standards. From the analysis of the national legislation in comparison with other European states, I conclude that, in the Republic of Moldova, the status of persons who graduated from the initial training courses within the National Institute of Justice, in the period between graduation and „appointment” as a prosecutor, judge . (which can take years, for example the 12th promotion of the Institute), remains a legal void. This would mean that, after completing the initial training courses, the future prosecutors and judges may have the following dilemma: either to work as a legal consultant/assistant, or to open an office in the central market. The latter would seem incompatible with the future status of „official with public dignity”, but not impossible, because there are no regulations that would prohibit this. In the context, I conclude that the targeted omission creates various risks regarding its integrity, or the trainees of the National Institute of Justice, after graduation, due to the lack of a social guarantee mechanism such as „medical insurance”, „remuneration”, „a strictly regulated status”, „the inclusion of the initial training period in the length of service” - which colleagues from similar training institutions for judges and prosecutors in other countries (Romania, France) benefit from until the appointment, may become dependent on other persons or organizations, which it could further have important consequences on his professional integrity and decision-making discretion.

Semiconductor core optical fibers are highly desirable for fiber-based photonic and optoelectronic applications as they can combine strong optical nonlinearities, tight light confinement, wide transmission bands, and electronic functionality within a single platform. Perovskites have emerged as particularly exciting materials for semiconductor photonics as they have strong optical nonlinearities and tunable optoelectronic bandgaps. However, lead-based perovskites contain toxic elements and are, therefore, not environmentally friendly. Furthermore, in fiber form, their core-size is prohibitively large, making them unsuitable for nonlinear optics and applications that require single-mode guidance, such as telecommunications. Here, we report a metal-free perovskite core optical fiber where lead has been substituted for an ammonium cation in the perovskite structure. The core material has a wide bandgap greater than 5 eV, a high laser damage threshold, and a core diameter that can be produced as small as 5 µm. At this core size, the fiber supports just six modes, and the fundamental mode can readily be excited and isolated. Moreover, the metal-free perovskite has a second-order susceptibility that is absent in the archetypal lead-based perovskites and many other semiconductor core materials, such as silicon and germanium. Background: Mozambique is a high-prevalence country for HIV and early detection of new HIV infections is crucial for control of the epidemic. We aimed to evaluate the accuracy of the 4
th -generation rapid diagnostic test (RDT) AlereTM HIV Combo in detecting acute and seroconverted HIV-infection, among sexually-active women attending three clinical health centers in Maputo, Mozambique. Methods: Women aged 14–55 years (n = 920) seeking care at the Mavalane Health Area, Maputo (February 2018-January 2019) were included, and blood specimens sampled. Sociodemographic and sexual behavior data were collected. Point-of-care HIV testing was performed using Alere DetermineTM HIV-1/2 and Uni-GoldTM HIV-1/2. All samples were also tested using Enzygnost® HIV Integral 4 and Innotest® HIV Antigen mAb in laboratory. The 4th -generation RDT AlereTM HIV Combo was evaluated on serum samples in the laboratory. Finally, Innotest® HIV Antigen mAb, Enzygnost® HIV Integral 4 (Ag/Ab), and HIV RNA quantification acted as gold standard assays in the evaluation of AlereTM HIV Combo test for HIV antigen detection (in clinical samples and in three HIV-1 seroconversion panels). Results: The antibody component of the 4th generation AlereTM HIV Combo RDT demonstrated a sensitivity and specificity of 100% examining clinical samples. However, the test did not detect HIV p24 antigen in any clinical samples, while Innotest® HIV Antigen mAb, verified by Enzygnost® HIV Integral 4 (Ag/Ab) and/or HIV RNA quantification, detected HIV antigen in six clinical samples. Furthermore, the AlereTM HIV Combo RDT had a low sensitivity in the detection of HIV p24 antigen in seroconversion panels. The HIV prevalence among the examined women was 17.8%. Conclusions: The 4th -generation RDT AlereTM HIV Combo showed similar sensitivity to the 3rd -generation RDTs to detect seroconverted HIV-infections. However, the sensitivity for detection of HIV p24 antigen and diagnosing acute HIV infections, before seroconversion, was low. There is an urgent need to develop and evaluate simple and affordable POC tests with high sensitivity and specificity for diagnosing individuals with acute HIV infection in resource-limited settings with high HIV prevalence. Study Objective: The purpose of this study is to provide evidence for the safety and efficacy of factor Xa inhibitors in patients with a weight ≤60 kg or BMI < 18.5 kg/m2. Design: Multicenter, retrospective, cohort study. Setting: Twenty‐two Ascension Health hospitals. Patients: Low‐body‐weight adult patients (weight ≤ 60 kg or BMI < 18.5 kg/m2) receiving treatment for atrial fibrillation or venous thromboembolism. Intervention: Factor Xa inhibitors (apixaban or rivaroxaban) or warfarin. Measurements and Main Results: This study included 2538 patients between the factor Xa inhibitors (n = 1695) and warfarin (n = 843) groups with a mean weight of 53.5 ± 5.5 kg and BMI of 20.7 ± 3.1 kg/m2. No significant difference in time to major bleeding was noted after controlling for potential confounders (HR 1.03, 95% CI 0.70–1.53, p = 0.87); similar results were seen following propensity score matching. Thromboembolism (5.3% vs. 6.2%, p = 0.38), composite major + clinically relevant nonmajor bleeding (9.8% vs. 11.5%, p = 0.18), and all‐cause mortality (10.7% vs. 12.8%, p = 0.12) were similar between patients receiving factor Xa inhibitors versus warfarin. Conclusion: No differences in safety or effectiveness were noted between factor Xa inhibitors versus warfarin. These findings provide encouraging evidence to support the use of factor Xa inhibitors in low‐body‐weight patients. [ABSTRACT FROM AUTHOR]

Aquaponics combines hydroponics and recirculating aquaculture elements. Conventional hydroponics requires mineral fertilizers to provide plants with the necessary nutrients, but aquaponics systems use available fish water, rich in fish waste, as nutrients for plant growth. Another advantage of these combinations is that excess nutrients do not have to be removed by periodic exchange of freshwater enrichment, as is practiced in aquaculture systems. The system results in a symbiosis between fish, microorganisms and plants and encourages the sustainable use of water and nutrients, including their recycling. Within this synergistic interaction, the respective ecological weaknesses of aquaculture and hydroponics are turned into strengths. This combination substantially minimizes the need for nutrient input and waste removal, unlike when they are used as separate systems, so in the article we have a presentation of the evolution of aquaponic systems. Preconception evaluation of couples wishing to conceive is an important step toward a healthy pregnancy and it is especially important in people with a chronic condition or at genetic risk. The most common endocrine disorders in women at reproductive age are those involving the thyroid gland and it is well recognized that hyperthyroidism (HT), over-function of the thyroid gland, is associated with risks of maternal, fetal, and neonatal complications. The aim of this paper is to review the latest evidence regarding the components of preconception counseling in women with HT that contemplate a pregnancy. We also want to raise awareness among healthcare professionals about the importance of periconceptional counseling in improving pregnancy outcomes and avoid maternal and fetal complications related to thyroid dysfunction. In women with Graves' disease seeking pregnancy, it is essential to discuss all the treatment options along with the associated risks and benefits. Extensive prospective studies are still needed to understand the implications of current recommended strategies for the management of HT in preconception and during pregnancy. [ABSTRACT FROM AUTHOR]
