- Abstract
To evaluate the agreement between QuantiFERON-TB Gold In-Tube test (QFT-GIT) and tuberculin skin test (TST) for the screening of latent tuberculosis infection (LTBI) in recent immigrants to Italy, 279 subjects were submitted to concomitant TST and QFT-GIT. The agreement was analyzed using k statistics. A total of 72/279 (25.8%) individuals were TST positive, while 107/279 (38.3%) were QFT-GIT positive. The overall agreement between QFT-GIT and TST was 70.9%, with k statistic of 0.35. Using different TST and QFT-GIT cut-offs, the best concordance value was obtained for QFT-GIT at > 2.64 IU/ml and TST at > 10mm (k = 0.409). Discordant results were found for 58 subjects (21%) with QFT-GIT positive/TST negative and 23 (8%) with QFT-GIT negative/TST positive. A high amount of discordance QFT-GIT+/TST- was described. QFT-GIT might increase the identification of LTBI cases among recent immigrants.

- Abstract
The aim of the study is to assess the determining factors and the level of vaccination coverage for those vaccinations recommended to health care workers. The employees of the Apulian Vaccination Services were given an interview-based standardized anonymous questionnaire. Of the 302 replies from the employees, 54.5% of the respondents had received the hepatitis B vaccine and 32.7% the influenza vaccine in the 2007-2008 season. There were 4.6% susceptible to varicella, and 2.6% had been vaccinated. Of the replies received, 9.3% were susceptible to measles or mumps or rubella, and 5.9% had been vaccinated. There is the need for on-going education for vaccine requirements to improve immunoprophylaxis among health care workers.

- Abstract
Background: Hepatitis B virus (HBV) infections are a matter of concern in hemodialysis units; occult HBV infections (serum HBsAg negative but HBV DNA positive) were demonstrated in this setting, and this involves further concerns regarding possible transmission and pathogenic consequences. This study aimed to investigate the prevalence and correlates of occult HBV infection in a group of patients with reference to a single hemodialysis unit in southeastern Italy., Methods: We analyzed HBV serology and DNA (using a qualitative nested PCR) in 128 HBsAg-negative hemodialysis patients, and correlated the results obtained, with sex, age, hemodialysis duration and HCV seropositivity., Results: As a whole, occult HBV infection was demonstrated in 34/128 patients (26.6%); HBV DNA detection was more frequent when anti-HBcAg antibodies were detected in isolation (72%) than when associated with anti-HBsAg antibodies (31%). Among HCV-seropositive patients, occult HBV infection was observed in 66%, and among these as many as 14/15 patients (93%) who were HCV+/anti-HBcAg+ had serum HBV DNA detectable. On multivariate analysis, HCV seropositivity and the presence of anti-HBs were still respectively correlated to the presence and absence of occult HBV infection., Conclusions: Occult HBV infection is frequent among hemodialysis patients in our geographical area, particularly correlated to the presence of isolated anti-HBcAg and anti-HCV antibodies. Thus, the presence of isolated anti-HBcAg should prompt the clinician to evaluate a possible occult HBV infection especially if anti-HCV antibodies are also detectable; this condition, in fact, seems to strongly predict the detection of HBV DNA.

- Abstract
The aim of the study was to evaluate the epidemiology and the diagnostic, clinical and therapeutic aspects of immigrants affected by tuberculosis, hospitalized in 35 Italian Infectious Diseases Clinics during 2003. The data obtained showed that 300/2392 (12.5%) patients had active tuberculosis, 10.3% of whom had concomitant HIV infection. 53% of the patients were legal residents and were assisted by the National Health Service; 48.3% came from African regions. The mean length of residency in Italy at the time of hospitalization was 4 years. The main clinical forms were pulmonary (66%), lymph nodal (15.3%) and bone TB (5.3%). Drug resistance was demonstrated in 16% of cases with 9% cases of resistance to isoniazid, 8.2% to streptomycin, 5.1% to pyrazinamide, 2.6% to ethambutol, 2.6% to rifampicin; in 5.3% of cases a multiple resistance was demonstrated. A complete adherence to treatment was achieved in 213 patients. Statistical analysis disclosed a significant correlation between compliance with treatment and legal citizenship status. In conclusion, TB still represents an important disease among immigrants. Improved living conditions, both in countries of origin and in Italy, especially in the first few years, would certainly decrease the incidence of TB.

- Abstract
Purpose: To investigate the relationship between morphologic and functional abnormalities in patients affected with adult-onset foveomacular vitelliform macular dystrophy (AFVD)., Design: Prospective, noncomparative observational study., Methods: A complete ophthalmologic examination, including best-corrected visual acuity (BCVA), fundus examination, fundus-related perimetry, and optical coherence tomography (OCT), was performed in 20 consecutive AFVD patients. The stage of the disease and the thickness of the neuroepithelium at the foveola (neurosensory retina) were compared with the BCVA as well as with the type of scotoma, the average retinal sensitivity, and the location and stability of fixation., Results: Thirty-five eyes of 20 consecutive patients (10 men and 10 women; mean age, 58.2 years) were graded as follows: 10 had vitelliform stage (stage 1), nine had pseudohypopyon stage (stage 2), 10 had vitelliruptive (stage 3), and six had atrophic stage (stage 4) disease. Reduced thickness of the neuroepithelium at the foveola and BCVA were statistically correlated to an advanced stage of the disease (P = .001 and P = .0062, respectively). Moreover, worse BCVA was correlated statistically to reduced thickness of the neuroepithelium at the foveola (r = 0.14; P = .02). Reduced thickness of the neuroepithelium at the foveola was correlated statistically to the development of absolute scotoma (P = .03), eccentric fixation (P = .01), and unstable fixation (P = .03)., Conclusions: OCT and fundus-related perimetry allow a correlation to be defined between foveal thickness and visual function and are useful tools to define better the degree of anatomic and functional impairment in AFVD patients.

Background: In recent decades in Italy, as in all the industrialized nations, the proportion of elderly subjects in the total population is constantly on the increase. However the increased life expectancy is not always paralleled by a true improvement in the quality of life. In this context, it is essential to analyze elderly real health needs and the responses to these needs, especially in terms of healthcare, that the territorial services are perceived to offer., Methods: In the period from June to September 2006 we selected randomly one General Practitioner (GP) for each district of the Bari Municipal Area and, form each GP, we randomly chose 25 patients over 65 years old (YO). We conducted phone interviews using a standard data collection questionnaire and, for each of the recruited subjects, the GP filled a data collection sheet., Results: Although the mean age (73.6 years) of the population under study was quite high, the general state of health was judged good both by the G P- and by their elderly patients (>75%).Notably, the great majority of elderly patients considered the healthcare they receive to be satisfactory (>60%): in particular, the GP was the true point of reference for this slice of the population for strictly medical problems as well as for advice. On the contrary, the patients attributed little value to social services, which were poorly known and scarcely used (8.5%). Public hospital facilities played a central role in second level healthcare in more than 30% of cases; private facilities covered by public health insurance were also very important. As possible solutions to the problem of loneliness, 36.6% of the patients declared that they approved of nursing homes., Conclusion: Decision makers need to create services supporting the key role played by General Practitioners, who are well aware that their assistance is not sufficient to satisfy the health needs of the elderly.

- Abstract
Introduction: The Italian Heat Health Watch Warning System (HHWWS) was set up following the over 3000 excess deaths which occurred in Italy during the heat wave of 2003, In June 2005 the warning system issued a heat warning in various Italian cities., Methods: A case control study was performed in one of these cities (Bari) in order to identify individual and environmental risk factors as well as preventive strategies for reducing mortality during future heat waves. Cases were defined as subjects aged <65 years who had died during the heat wave and whose death certificate listed one of the following as the cause of death: heat, cardiovascular or cerebrovascular disorders, neurocognitive disorders, dehydration or fever/infection not otherwise specified. For each case, three age-matched controls were randomly selected among individuals followed by the same general practitioner as the case. All variables significantly associated with mortality (<0.1) in the univariate analysis were entered into a conditional logistic regression model and the population attributable fraction (PAF) was calculated for significant variables (at p<0.05)., Results: Twenty cases and sixty controls were included in the study. In 17 cases (89%) death had occurred at home and 11(55%) of these were cardiovascular- related deaths. At the multivariate analysis, the factors significantly associated with mortality risk during the heat wave were: having a functioning air conditioner at home [OR:0.09(95% CI 0.01-1.00)], having an Activities of Daily Living score <2 [OR:21.0(95%CI 1.81-242.47)] and having been hospitalized the year preceding death [OR:18.1(95%CI 2.04-160.51)]., Conclusions: Public health interventions during heat waves should include the provision of access to an air conditioned environment. Subjects with impaired health (especially if recently hospitalized) and with significant limitations in their activities of daily living are probably at higher risk during heat waves.

Cataracts are a common condition and represent a frequent cause of visual disability. Nevertheless, data regarding the incidence of cataracts and the progression of and loss of vision due to cataracts is scarce. In this study, hospital discharge abstract data was evaluated to estimate the frequency and type of hospitalisations for cataract surgery that occurred in the Puglia region in the years 2000-2002. The results of this study confirm that cataract surgery accounts for a relevant amount of regional healthcare system costs. In fact, every year, over 25,000 lens surgery procedures, most of which age-related, are performed in the Puglia region. Approximately 90% of such procedures involve cataract surgery; age-specific hospitalisation rates show very high rates in the > 50 year old population. Only 25% of total cataract extractions were performed as a day-surgery procedure and the mean length of hospital stay was found to be 2.5 days. This confirms that inappropriate hospitalisations, with respect to both type and duration, do occur. Cataract surgery is performed in over 30 hospitals in Puglia; the availability of health facilities in the region, therefore, seems to be satisfactory. In fact, on average, in the three-year period, only 7.5% of cases underwent the procedure outside the Puglia region.

Hip arthroplasty is one of the major achievements of orthopaedic surgery. In the Puglia region the demand for this procedure is moderately met. This study evaluates the regional distribution of hip arthroplasty procedures in Puglia in the years 1996-2003. Demand and supply was estimated through analysis of discharge abstract forms. Between 1996 and 203, 21,392 hip arthroplasty procedures were performed. of these, 12,491 (58,4%) were total hip arthroplasties, 7,742 (36,2%) were hip endoprostheses and 1,159 (5,4%) wee revisions. The median age of patients undergoing any of these procedures was 70 years and the proportion of females was almost double that of males (64%). The main indication was primary osteoarthrosis and 52,1% of all procedures were performed in the province of Bari. The average annual crude hospitalisation rate for hip arthroplasty procedures was 75,9 cases/100.000 inhabitants; this rate places the Puglia region in an intermediate position with respect to other Italian regions and is lower than that found in the literature. Discharge abstract forms are currently the sole source of data regarding hip arthroplasty procedures and this presents several limits. This factor, in addition to the complexity and high costs of these procedures and to the increasing average life expectancy of the general population all point to the need for a regional Hip arthroplasty Register, which would allow better planning for patients.

- Abstract
One of the more important aspects of the psychiatric care reform in Italy has been the opening of a great number of psychiatric Non-Hospital Residential Facilities (NHRF). However, 22 years have elapsed since the reform and there are still very scarce data regarding such facilities, the type of patients being accomodated, and the types of treatments provided. The "PROGRES" (PROGetto RESidenze - Residential Project) project is the first national study regarding NHRFs ever carried out in Italy. The objectives of "PROGRES" are to make a survey of all NHRFs in Italy (Phase 1) and to perform a detailed assessment of 20% of the surveyed facilities and of the type of patients being treated (Phase 2). This paper reports the survey (Phase 1) results for the Puglia Region. As of March 31, 2003, 113 psychiatric residential facilities were present in Puglia, with a total of 1.479 beds. This results in an average of 13,08 beds per facility and a rate of 3,86 beds per 10.000 inhabitants. The number of NHRFs present in the territory of each Local Health Unit in Puglia is very variable. Most NHRFs (57%) opened after January 1997. Eighty-three percent have a 24-hour staff coverage while only 5,3% has a 2-hour staff coverage. Eighty five percent of the residential facilities are financed by the Italian National Healthcare System, while a small percentage is privately funded. In over half (69,93%) of the NHRFs the predominant patient age group is 40-59 years; 26,4% of the residential facilities accommodate patients below age 40 and only 3,53% accommodate patients over 60 years old.

264. [Hospitalisations for vertebral fractures in Apulia during 1998-2000].

- Abstract
Vertebral fractures are a relevant problem in public health. This is mainly due to the high prevalence of elderly people and the high social costs implied as regards disability and health care. The authors have carried out a survey about hospitalisation rates for vertebral fractures in Apulia in the years 1998-2000. The gross hospitalisation rate for vertebral fractures without spinal injuries concerned 47.3 people out of 100.000 per year, whereas the rate for fractures with spinal cord injuries concerned 4,0 people out of 100.000 per year. Fractures concerned more frequently the dorso-lumbar tract and males were more affected than women. During the period concerned, the total DRG cost for vertebral fractures was higher than 15 million Euros and the extra-regional mobility rate for vertebral fractures without spinal injuries was 11,2% of all case-studies. The results will allow better health programmes aimed at enhancing treatment of back bone injuries.
