In this era of Acquired Immune Deficiency Syndrome (AIDS) and other infectious diseases – complications of homologous blood transfusion, autologous transfusion is the safest possible blood for transfusion. The present study was conducted from January 1992 to August 1996 to analyse the programme of autologous transfusion. A total of 471 pre-operative autologous collections were undertaken out of a total of 27,542 blood collections during this period. The present study shows a significant increase in autologous collections from 0.44 per cent in 1992 to 3.94 per cent till August 1996. 37.1 per cent autologous donors were from the urosurgery ward. Of the total 471 autologous blood units collected, 171 were transfused to the autologous donors (27.5%).

There are numerous causes of fulminant hepatic failure including acetaminophen, viral etiologies, acute fatty liver of pregnancy, ischemic hepatitis, and acute Budd-Chiari syndrome. One of the rare causes of fulminant hepatic failure is toxic mushroom poisoning. This article describes one case in which ingestion of a toxic mushroom, Amanita phalloides, caused a patient severe liver damage ultimately requiring organ transplantation. Early identification of liver dysfunction and referral to a liver transplant center for evaluation may be critical to save such a patient's life.

Two hydroxyl containing dithiocarbamates, sodium N-methyl-D-glucamine dithiocarbamate (NaG) and sodium dihydroxyethyl dithiocarbamate (NaY) have been examined as agents for the control of the renal dysfunction in rats given cisplatin. Of these, NaG was found to be the more effective in controlling such renal dysfunction when administered at 1 and 3 h after 5 mg cisplatin kg-1, i.p. Renal function was examined 5 days after the administration of cisplatin by measurement of serum and urinary levels of creatinine and urea, creatinine clearance, serum and urinary levels of Na+, K+, Mg2+, Ca2+, as well as the concentrations of these ions in the renal medulla and cortex. Treatment of rats given cisplatin with NaG at 1 and 3 h post cisplatin resulted in indices of renal function which were not significantly different from those of animals which had received no cisplatin. The sole difference was found to be a slight increase in renal cortical Na+ concentration.

Experimental results are presented, on a round-robin basis, to study the effect of vibrational frequency on the dynamic shear moduli of two metals, Inconel 600 and Incoloy 907. Four separate laboratories, each using a different experimental technique, obtained results over the nominal frequency range of 7 kHz to 10 MHz. A brief discussion and comparison of the four techniques, both resonant and pulse, are included. No significant differences were observed for each alloy's modulus over this frequency range.

Therapeutic options for the treatment of melioidosis caused by Burkholderia pseudomalleiare limited due to the inherent resistance conferred by this pathogen to various groups of antibiotics. Witnessing an increase in the number of microbiological culture-confirmed cases of melioidosis at our settings in the past few years, we undertook this study to estimate the minimum inhibitory concentrations of clinical isolates of B. pseudomalleiagainst the four commonly employed antimicrobial agents in the patient management at our settings, namely, ceftazidime, meropenem, trimethoprim-sulfamethoxazole and doxycycline. All isolates were susceptible to the antibiotics tested, except for one isolate which showed resistance to doxycycline (minimum inhibitory concentration [MIC]: 32 μg/ml). MIC50 and 90 for all the four antibiotics were estimated. From this study, we conclude that the clinical isolates of B. pseudomalleifrom the southern part of India are well susceptible to the commonly employed antimicrobial agents for therapy.

After dominating the earth in numbers and biomass for a good 3.5 billion years, microbes are now finally in the public consciousness. The vigorous surge in the study of microbes promises to yield novel approaches to treating diseases and maintaining health. In the middle of the deluge of scientific publications and lay articles, we pause to reflect on this trend.

Antibiotics and antibiotic resistance made news on several fronts in the past year. Many public health organizations, including the CDC, used terms such as “crisis”, “catastrophic consequences”, and “nightmare scenario” to highlight the rapid emergence and spread of antibiotic resistance. A report from the Pew Commission on Industrial Farm Animal Production, on the fifth anniversary of the publication of its landmark 2008 report, noted that state and federal legislative efforts to limit non-therapeutic use of antibiotics in animal production were thwarted by drug and food animal industries. In its lobbying disclosures, the Farm Bureau stated that such efforts to limit use of animal antibiotics were “based on emotion and no credible peer reviewed science.” Meanwhile, there have been inexorable advances in our understanding of the molecular mechanisms by which antibiotics induce diversity and resistance in bacteria. This article reviews one study that probed the role of the bacterial general stress response in sub-inhibitory antibiotic-induced mutagenesis and antibiotic resistance.

It is not surprising that the complex metabolic machinery of the gut microbiome has accidental, or directed, ability to alter our medicines and influence their efficacy. What is not known is the extent to which this has contributed to drug failures or contraindications, or to the derailment of clinical trials. Some studies are unraveling the mechanisms by which the microbiota alter specific drugs, such as digoxin, and contribute to variations in efficacies between patient populations. Microbiome profiling, therefore, may well become an inevitable arm of all clinical trials in the future.

Background: We hypothesized that leucocyte esterase strip test can aid in diagnosing septic arthritis in native synovial fluid because leucocyte esterase concentrations would be elevated at the infection site because of secretion by recruited neutrophils.Method: The cohort included 27 patients (suspected septic arthritis and normal subjects). A standard chemical test strip (graded as negative, trace, +, ++ or +++) was used to detect the presence of leucocyte esterase. Fluid leucocyte count, Gram staining, culture, erythrocyte sedimentation rate and C-reactive protein were also assessed.Results: The leucocyte esterase test with a threshold of ++/+++ had a sensitivity of 79.2% (95% CI [confidence interval], 65.9% to 89.2%), specificity of 80.8% (95% CI, 73.3% to 87.1%), positive predictive value (PPV) of 61.8% (95% CI, 49.2% to 73.3%) and negative predictive value (NPV) of 90.1% (95% CI, 84.3% to 95.4%).Conclusion: The leucocyte esterase strip test yielded a high specificity, PPV, NPV, high sensitivity and high diagnostic accuracy. Leucocyte esterase is an accurate, quick and bedside test for septic arthritis and can be used effectively for diagnosing periprosthetic joint infections along with other battery of tests according to the Musculoskeletal Infection Society criteria.

Here we report a case of acalculus cholecystitis, which presented with features of obstructive jaundice of one-week duration. The patient underwent cholecystectomy and bile grew a mixed culture of Oerskovia turbataand Myroidesspp. Being a rare isolate, characteristic features of the former are described in this report. The patient recovered without any complication.

The reported case is of primary pulmonary nocardiosis, caused by Nocardia brasiliensis, in a immunocompromised patient, which ended fatally despite appropriate treatment. The partially acid fast filamentous bacterium was predominant on direct examination of the sputum. It was cultured on blood agar, MacConkey agar and by paraffin baiting technique. The bacterium was resistant to cotrimoxazole, the drug of choice for nocardiosis.

A 50‐year‐old woman presented with a lateral neck swelling, clinically indistinguishable from solitary thyroid nodule. A right hemithyroidectomy was performed and microscopy revealed an intrathyroidal thyroglossal cyst. Intrathyroidal thyroglossal cyst should be considered in the differential diagnosis of a thyroid nodule.

Breast abscess due to Salmonella typhiis an extremely rare occurrence. A lady with a lump in the left breast was diagnosed to have a fibroadenoma and was subjected to a surgical procedure. She was found to have an abscess due to Salmonella typhias confirmed by conventional bacteriological methods. She was treated with ciprofloxacin and responded favourably.
