The purpose of this article is to determine the cause of Leber congenital amaurosis (LCA) in Chuuk state, Federated States of Micronesia (FSM). In this prospective observational case series, five patients with early-onset vision loss were examined in Chuuk state, FSM, during an ocular genetics visit to study the elevated incidence of microphthalmia. Because of their low vision these patients were incorrectly assumed to have microphthalmia. A complete ophthalmological exam established a clinical diagnosis of LCA. Candidate gene exons were sequenced with a targeted retinal dystrophy panel. Five subjects in three related families were diagnosed with LCA. All five were from Tonoas Island, within the Chuuk Lagoon, with ages ranging from 6 months to 16 years. DNA sequencing of affected individuals revealed a homozygous CRB1 NM_201253.3:c.3134del pathogenic variant, which was heterozygous in their parents. CRB1 genotypes were confirmed by a PCR restriction assay. We report identification of a founder pathogenic variant in CRB1 responsible for autosomal recessive LCA in this isolated community. Like the spaces between islands on maps of the Pacific, there are gaps that exist in our historical understanding of this region of the world. This is especially true in relation to the history of Micronesia and the Marshall Islands. This study examines some of those forgotten-about spaces whilst focusing on the history of former European colonial powers whose own imperial stories have also been overlooked in this region, namely Spain and Germany. In doing so it illustrates how this former Spanish territory morphed into a German one and discusses the significance of this transformation, and how it helped lay the groundwork for the beginning of the Second Reich. Whilst examining these European colonial histories this work also uncovers how the United States came to be involved in Micronesia and the long-term results of that development, specifically the establishment of Guam/Guåhån as one of the first United States overseas military bases and the utilisation of the Kwajalein Atoll in the Marshalls for Cold War nuclear testing. It concludes by pointing out that much of this territory was obtained not by conquest but by purchase and lease, thereby underscoring an oft-forgotten, but vitally important, method of territorial aggrandizement that continues to this day. Objective: To determine perceptions, beliefs and experiences affecting breast-feeding in Marshallese mothers residing in Northwest Arkansas, USA.Design: A qualitative, exploratory study using a brief survey and focus groups. Marshallese women, 18 years or older who had a child under 7 years of age, were included in the study.Setting: Community-based organization in Northwest Arkansas.Results: The majority of mothers viewed breast milk as superior to formula, but had concerns about adequate milk supply and the nutritional value of their milk. The primary barriers to exclusive breast-feeding in the USA included public shaming (both verbal and non-verbal), perceived milk production and quality, and maternal employment. These barriers are not reported in the Marshall Islands and are encountered only after moving to the USA. Breast-feeding mothers rely heavily on familial support, especially the eldest female, who may not reside in the USA. The influence of institutions, including the Special Supplemental Nutrition Program for Women, Infants, and Children, is strong and may negatively affect breast-feeding.Conclusions: Despite the belief that breast milk is the healthiest option, breast-feeding among Marshallese mothers is challenged by numerous barriers they encounter as they assimilate to US cultural norms. During the twentieth century Japan and the United States attempted land reform in Micronesia. Japan was more successful because a growing population had led to an increasing demand for agricultural products, which could only be met by expanding agriculture across its empire. This required investment in land reform to transfer ownership from common to private rights. Conversely, the Americans faced no such domestic pressures, valuing Micronesia only for its strategic location and military testing. We formulate a model to examine the outcomes of Micronesian land reform under the long-sighted policy of the Japanese compared with short-sighted approach of the Americans. [ABSTRACT FROM AUTHOR]

Mormons, or members of The Church of Jesus Christ of Latter-day Saints, have a distinguished history of service in the U.S. government. During a forty-year period following World War II, Mormon politicians played vital roles in transitioning several islands in the Trust Territory of the Pacific Islands from a quasi-colonial status into the self-governing Federated States of Micronesia. This article briefly traces the complicated transition through the public service of four key Mormon administrators: Elbert D. Thomas, John A. Carver Jr., Stewart L. Udall, and Morris K. Udall. They served respectively as first civilian high commissioner of the Trust Territory, Secretary and Assistant Secretary of the Interior, and chair of the House Interior and Insular Affairs Committee with oversight for the Pacific. The small Pacific Island nation of Palau has alarmingly high rates of betel nut with tobacco use and obesity among the entire population including pregnant women. This study aimed to determine the effects of betel nut with tobacco use and pre-pregnancy obesity on adverse birth outcomes. This study used retrospective cohort data on 1171 Palauan women who gave birth in Belau National Hospital in Meyuns, Republic of Palau between 2007 and 2013. The exposures of interest were pre-pregnancy obesity and reported betel nut with tobacco use during pregnancy. The primary outcomes measured were preterm birth and low birth weight among full-term infants. A significantly increased risk for low birth weight among full-term infants was demonstrated among those women who chewed betel nut with tobacco during pregnancy when other known risk factors were controlled for. Additionally, pre-pregnancy obesity was associated with a significantly increased risk for preterm birth when other known risk factors were controlled for. Both betel nut with tobacco use and pre-pregnancy obesity were associated with higher risks for adverse birth outcomes. This article concerns aspects of masculine sexuality, violence and sexual exploitation in Micronesia, with particular attention to Chuukese and Pohnpeian gender ideology and gender relations. Above all, masculine sexuality and sexual exploitation is discussed in the context of Micronesian concepts of ownership and exchange, reciprocity (‘sharing’) and power differences between men, women, kin and non-kin. Despite the presence of alternate and, in some areas, competing value systems stemming from Christianity, or an Americanised constitution and legal system, core traditional values related to male sexuality from the pre-Christian, pre-colonial times still show their presence in masculine (and feminine) thought and practice more than one might expect. The taxonomic status of a single island, narrow range endemic plant species from Palau, Micronesia (Timonius salsedoi) was assessed using DNA barcode markers, additional plastid loci, and morphology in order to verify its conservation status. DNA barcode loci distinguished T. salsedoi from all other Timonius species sampled from Palau, and were supported by sequence data from the atpB-rbcL intergenic spacer region. Timonius salsedoi was only known from two mature individual trees in 2012. Due to its extremely narrow range and population size, it had previously been recommended to be listed as Critically Endangered Status under three separate IUCN Criteria. In 2014 a second survey of the population following a typhoon revealed that the only two known trees had died suggesting that this species may now be extinct. Zika virus (ZIKV), a previously obscure flavivirus closely related to dengue, West Nile, Japanese encephalitis and yellow fever viruses, has emerged explosively since 2007 to cause a series of epidemics in Micronesia, the South Pacific, and most recently the Americas. After its putative evolution in sub-Saharan Africa, ZIKV spread in the distant past to Asia and has probably emerged on multiple occasions into urban transmission cycles involving Aedes ( Stegomyia ) spp. mosquitoes and human amplification hosts, accompanied by a relatively mild dengue-like illness. The unprecedented numbers of people infected during recent outbreaks in the South Pacific and the Americas may have resulted in enough ZIKV infections to notice relatively rare congenital microcephaly and Guillain–Barré syndromes. Another hypothesis is that phenotypic changes in Asian lineage ZIKV strains led to these disease outcomes. Here, we review potential strategies to control the ongoing outbreak through vector-centric approaches as well as the prospects for the development of vaccines and therapeutics. Changes in larval import, export, and self-seeding will affect the resilience of coral reef ecosystems. Climate change will alter the ocean currents that transport larvae and also increase sea surface temperatures ( SST), hastening development, and shortening larval durations. Here, we use transport simulations to estimate future larval connectivity due to: (1) physical transport of larvae from altered circulation alone, and (2) the combined effects of altered currents plus physiological response to warming. Virtual larvae from islands throughout Micronesia were moved according to present-day and future ocean circulation models. The Hybrid Coordinate Ocean Model ( HYCOM) spanning 2004-2012 represented present-day currents. For future currents, we altered HYCOM using analysis from the National Center for Atmospheric Research Community Earth System Model, version 1-Biogeochemistry, Representative Concentration Pathway 8.5 experiment. Based on the NCAR model, regional SST is estimated to rise 2.74 °C which corresponds to a ~17% decline in larval duration for some taxa. This reduction was the basis for a separate set of simulations. Results predict an increase in self-seeding in 100 years such that 62-76% of islands experienced increased self-seeding, there was an average domainwide increase of ~1-3% points in self-seeding, and increases of up to 25% points for several individual islands. When changed currents alone were considered, approximately half (i.e., random) of all island pairs experienced decreased connectivity but when reduced PLD was added as an effect, ~65% of connections were weakened. Orientation of archipelagos relative to currents determined the directional bias in connectivity changes. There was no universal relationship between climate change and connectivity applicable to all taxa and settings. Islands that presently export large numbers of larvae but that also maintain or enhance this role into the future should be the focus of conservation measures that promote long-term resilience of larval supply. ABSTRACT In eastern Micronesia, breadfruit is an important staple crop and has probably been part of the diet since initial settlement. This paper uses combined macroremains and phytolith analysis to identify breadfruit production from a historical feature on Temwen Island, Pohnpei, Micronesia. Charred breadfruit exocarp material was recovered, indicating breadfruit roasting. Phytolith analysis from this feature does not provide direct evidence of breadfruit, but does reveal other taxa that are not present in flotation samples, demonstrating the utility of analysing multiple lines of botanical data. Thus, this project presents new botanical evidence for breadfruit and a method for approaching a more comprehensive study of cooking practices. [ABSTRACT FROM AUTHOR]

Prior investigation at the Chelechol ra Orrak site (3000/1700–0 BP) in Palau’s Rock Islands revealed a decline in fishing and increased reliance on small-bodied, inshore and littoral molluscs, commensurate with evidence for declining foraging efficiency and prey switching that signal potential resource depression. Yet, standard markers for ‘overfishing’, such as diet-breadth expansion, increased taxonomic richness, and a switch to exploitation of offshore waters, are lacking at the site, undermining the case for anthropogenic resource (exploitation) depression as a cause of the observed patterning. Broad scale climate change similarly fails to account for these shifts. To investigate these conflicting patterns we performed a mean/median size analysis of two parrotfish (Scaridae) taxa, Scarus and Chlorurus , among the most commonly recovered fish at the site. Results indicate that Scarus size remains unchanged through 1500 years of exploitation, while Chlorurus become larger, substantiating previous findings for sustainable resource use at Orrak. With these results in mind, we critically evaluate prey size change as a metric for anthropogenic exploitation depression, noting that size diminution, in particular, may arise epiphenomenally due to multiple causes unrelated to human predation pressure. The article presents a study on the health beliefs of the Marshallese migrant population in Arkansas regarding type 2 diabetes. It discusses the high incidence of type 2 diabetes among the U.S. Marshallese population, findings based on the dimensions of the Health Belief Model (HBM) that were used to understand the cultural beliefs of the population including perceived susceptibility, perceived severity and self-efficacy, and the ratio between perceived benefits and perceived barriers.

This study examined the capture rates and prey digestion of three corals species in two genera (AcroporaandPocillopora) found dominantly in Thailand and the Federated States of Micronesia usingArtemia salinanauplii as a food source. In addition, feeding during day and night times of corals was investigated. Results showed that all three coral species, namely,Acropora millepora,Acropora nobilis, andPocillopora damicorniscaptured and consumedA. salinanauplii under both light and dark conditions. The capture rates of the three species ranged between 0.44 and 2.39 individuals/polyp/12 h. The results also showed complete digestions ofA. salinanauplii byA. milleporaandA. nobilisafter 2 h, whereasP. damicornistook 2.5 h to complete the prey digestion. Even though feeding corals withArtemiais not a novel implication since this is widely used, the results of optimum daily feeding frequency of corals byArtemiamay be applied for optimum corals growth and survival in captivity or aquarium. Objectives. We compared the age at admission and the severity of illness of hospitalized Micronesians with 3 other racial/ethnic groups in Hawaii. Methods. With Hawaii Health Information Corporation inpatient data, we determined the age at admission and the severity of illness for 162 152 adult, non-pregnancy-related hospital discharges in Hawaii from 2010 to 2012. We performed multivariable linear regression analyses within major disease categories by racial/ethnic group. We created disease categories with all patient refined-diagnosis related groups. Results. Hospitalized Micronesians were significantly younger at admission than were comparison racial/ethnic groups across all patient refined-diagnosis related group categories. The severity of illness for Micronesians was significantly higher than was that of all comparison racial/ethnic groups for cardiac and infectious diseases, higher than was that of Whites and Japanese for cancer and endocrine hospitalizations, and higher than was that of Native Hawaiians for substance abuse hospitalizations. Conclusions. Micronesians were hospitalized significantly younger and often sicker than were comparison populations. Our results will be useful to researchers, state governments, and hospitals, providers, and health systems for this vulnerable group. [ABSTRACT FROM AUTHOR]

Given their sheer number and evidence for long-term prehistoric occupation, atolls occupy a unique position in the peopling of the Pacific. However, they have frequently been overlooked in favor of larger islands due to a host of logistical and other issues. Once viewed as marginal environments, current research is now showing that small islands like these may have been more attractive to settlers than once thought. A new research program in Micronesia is dedicated to examining atolls to better develop baseline chronologies and investigate long-term human adaptations. As part of the initial stage of the project, we present the first radiocarbon dates (n=10) from Mwoakilloa (Mokil) atoll, which support a continuous occupation beginning between 1700–1560 cal BP (2σ). When compared to the settlement of other atoll groups in Micronesia such as the Marshall Islands—along with the nearby high volcanic islands of Pohnpei and Kosrae at approximately 2000–1800 yr ago—the dates from Mwoakilloa suggest a nearly contemporaneous or only slightly later occupation. The recovery of faunal material also demonstrates the translocation of at least two animals (Pacific rat and dog) to the island by humans that was coeval with early settlement. Additionally, there is evidence of landscape transformation in the form of a relatively large artificial mound created by debris and platform accumulation unseen elsewhere in central-eastern Micronesia, but common to atolls. These new dates reinforce the notion that Mwoakilloa and other atolls are integral to understanding prehistoric adaptations across the vast Pacific, though many questions still remain such as to the degree of interaction that occurred with nearby islands and whether settlement was continuous or intermittent through time. Aim The islands of the Tropical Oceanic Pacific ( TOP) host both local radiations and widespread, colonizing species. The few phylogeographical analyses of widespread species often point to recent human-aided expansions through the Pacific, suggesting that the communities are recently assembled. Here we apply multilocus data to infer biogeographical history of the gekkonid lizard, Gehyra oceanica, which is widespread, but for which prior analyses suggested a pre-human history and in situ diversification. Location Tropical Oceanic Pacific. Methods We generated a data set including mt DNA and diagnostic SNPs for 173 individuals of G. oceanica spanning Micronesia, Melanesia, and Polynesia. For a subset of these individuals, we also sequenced nuclear loci. From these data, we performed maximum likelihood and Bayesian inference to reveal major clades. We also performed Bayesian clustering analyses and coalescence-based species delimitation tests to infer the number of species in this area. Results We found evidence for six independent evolutionary lineages (candidate species) within G. oceanica that diverged between the Pliocene and the early Pleistocene, with high diversity through northern Melanesia, and pairing of northern Melanesian endemic taxa with widespread lineages across Micronesia and Polynesia. Main conclusions The islands of northern Melanesia not only have unrecognized diversity, but also were the source of independent expansions of lineages through the more remote northern and eastern Pacific. These results highlight the very different evolutionary histories of island faunas on remote archipelagos versus those across Melanesia and point to the need for more intensive studies of fauna within Melanesia if we are to understand the evolution of diversity across the tropical Pacific. Ethnographic research on the clinical encounter has focused almost exclusively on what happens in exam rooms—particularly patient-provider interactions—leaving much to be understood about other actors within the clinic. As part of a larger ethnographic study examining the impact of colonialism, transnational migration, discrimination, and gender relations on Chuukese migrant women's reproductive and sexual health outcomes in Guam, I conducted eight months of participant observation in three publicly funded health clinics, 24 interviews with health care workers, and 15 life-history interviews with Chuukese women between September 2012 and February 2014. Findings demonstrated differential treatment of Chuukese patients by front-line workers (FLWs), who engaged in “boundary work” with these patients. Further, care varied by clinic space and the actors in that space. This differential treatment and variation in care impacted Chuukese women's access to and experiences with health care in Guam. Utilizing the concept of “deservingness,” this analysis unpacks how FLWs, like Lipsky's “street-level bureaucrats,” are influenced by bureaucratic, biomedical, and community hierarchies, all contributing to differential patient treatment. This study concludes by calling for more integrated analyses of clinical environments which utilize Lipsky's concept to include community narratives of “deservingness” and incorporate the influence of clinic and community stratification. Betel nut has been stated to be addictive, but evidence is lacking. This study describes dependence symptoms among adolescents using betel alone or with tobacco. In the first study, participants were 151 9th graders in Saipan. In the second study, participants were 269 9th graders in Pohnpei and Yap. Participants completed a confidential questionnaire adapted from the U.S. National Survey of Drug Use and Health, which measured dependence symptoms. The 15 items were summed to form a scale, with a range of 0-15, where higher scores indicated greater endorsement of dependence symptoms. In the first study, 39.1% had used betel. More than two-thirds of all users (69.5%) used betel in the previous month: 87.8% also used tobacco with the betel. The mean (SD) dependence symptoms scale score among tobacco users was 8.2±4.0 versus 3.4±2.9 among those who used betel alone [t(7)=3.3, p=0.015]. In the second study, 38% from Pohnpei and 85% from Yap had used betel and most of the current users used it in the previous month (67% from Pohnpei, 91% from Yap). Among those who had used betel in the previous month, 90% from Pohnpei and 64% from Yap were using betel with tobacco. The dependence score was positively associated with frequency of tobacco use (e.g., mean (SD)=11.3 (±2.4) among most frequent users versus a mean (SD)=4.8 (±3.5) among the never users [F(3109)=28.8, p<0.001]). Hospital libraries serving military bases offer a different perspective on library services. Two libraries located on islands libraries; military facilities in the Pacific Ocean provide services to active duty service men and women, including those deployed to other regions of the world. The authors documented gross and microscopic morphology of lesions in corals on 7 islands spanning western, southern, and eastern Micronesia, sampling 76 colonies comprising 30 species of corals among 18 genera, with Acropora, Porites, and Montipora dominating. Tissue loss comprised the majority of gross lesions sampled (41%), followed by discoloration (30%) and growth anomaly (29%). Of 31 cases of tissue loss, most lesions were subacute (48%), followed by acute and chronic (26% each). Of 23 samples with discoloration, most were dark discoloration (40%), with bleaching and other discoloration each constituting 30%. Of 22 growth anomalies, umbonate growth anomalies composed half, with exophytic, nodular, and rugose growth anomalies composing the remainder. On histopathology, for 9 cases of dark discoloration, fungal infections predominated (77%); for 7 bleached corals, depletion of zooxanthellae from the gastrodermis made up a majority of microscopic diagnoses (57%); and for growth anomalies other than umbonate, hyperplasia of the basal body wall was the most common microscopic finding (63%). For the remainder of the gross lesions, no single microscopic finding constituted >50% of the total. Host response varied with the agent present on histology. Fragmentation of tissues was most often associated with algae (60%), whereas necrosis dominated (53%) for fungi. Two newly documented potentially symbiotic tissue-associated metazoans were seen in Porites and Montipora. Findings of multiple potential etiologies for a given gross lesion highlight the importance of incorporating histopathology in coral disease surveys. Dengue virus type 1 (DENV-1), which was responsible for the dengue fever outbreak in Yap State, Micronesia, in 2004, was isolated from serum samples of 4 dengue patients in Japan. Genome sequencing demonstrated that this virus belonged to genotype IV and had a 29-nucleotide deletion in the 3´ noncoding region.
