Objective: The prevalence of Bitot's spots (BS) is often used to quantify vitamin A deficiency burden in India, both before and after mega-dose vitamin A supplementation (MVAS) programmes. However, the proportion of BS cured following this intervention is unclear in contemporary times. The current study evaluated the responsiveness of BS over 1 year to MVAS administered as per the national programme in rural India., Design: Prospective, community-based, 1-year follow-up of a cohort., Setting: Rural Uttar Pradesh, India., Subjects: Two hundred and sixty-two children with BS, aged between 1 and 5 years, administered 60 mg (retinol equivalent) of vitamin A on diagnosis and after 1 month. Cure or resolution was defined if there was no discernible BS in either eye., Results: During 1 year, only three children were lost to follow-up. At 6 months of follow-up (MVAS at baseline and 1 month later), 51·1 (95% CI 45·3, 57·3) % were classified as cured. The corresponding figure at 1 year (additional MVAS at 6 months) was 59·9 (95% CI 54·1, 65·9) %. Among those cured at 6 months, about half and three-quarters had resolved at 2 and 3 months, respectively. Apart from male gender, there were no significant sociodemographic or clinical predictors of response., Conclusions: Substantial non-response to MVAS at 6 months (49%) and 1 year (40%) of follow-up suggests that presently in the Indian subcontinent, BS is a relatively crude indicator of severe current vitamin A deficiency. For programmatic decisions and evaluation, the public health burden of vitamin A deficiency should not be assessed solely through BS.

Iodine deficiency (ID) is an endemic health problem in Kangra district, Himachal Pradesh (HP) state. ID leads to mental retardation, deaf mutism, squint, dwarfism, spastic diplegia, neurological defects and congenital anomalies. Iodine nutrition status amongst neonates can be assessed by estimating thyroid stimulating hormone (TSH). The present study was conducted with an objective to assess the iodine nutrition status amongst Neonates in Kangra district, HP. All of the hospitals in the district which provide obstetric services were enlisted, of which three were selected for this survey. A total of 613 umbilical cord blood samples of neonates were collected on filter paper and analyzed for TSH. WHO (2007) reported that that a <3% frequency of TSH concentrations above 5 mIU/L in samples collected 3-4 days after birth indicates iodine sufficiency in a population. In our study we found that 73.4% of the neonates had TSH levels of more than 5 mlU/l, thus indicating ID in the population studied. Background: Iodine deficiency disorder (IDD) is a public health problem in Uttarakhand state., Objective: The present study was conducted in district Udham Singh Nagar, Uttarakhand state with an objective to assess the status of iodine deficiency amongst school age children (6-12 years)., Materials and Methods: Thirty clusters were selected by utilizing the population proportionate to size (PPS) cluster sampling methodology. A total of 1807 children in the age group of 6-12 years were included. The clinical examination of the thyroid of each child was conducted. Urine and Salt samples were collected from children., Results: The Total Goiter Rate (TGR) was found to be 13.2%. The proportion of children with Urinary Iodine Excretion (UIE) level <20, 20-49, 50-99, 100-199 and ≥200 μg/l was found to be nil, 6.0, 21.2, 34.2 and 38.5 percent, respectively. The median UIE level was 150 μg/l. Only 46.7% of the salt samples had stipulated level of iodine of 15 ppm and more., Conclusion: The study population had mild degree of public health problem of iodine deficiency.

Iodine deficiency disorder (IDD) is a major public health problem in Uttarakhand. The present study was conducted in district Nainital, Uttarakhand state with an objective to assess the prevalence of IDD in school age children. A total of 2269 children in the age group of 6-12 years were included. Clinical examination of thyroid of all children was undertaken. "On the spot" urine samples were collected from 611 children. Salt samples were collected from the family kitchen for 642 children. The Total Goitre Rate (TGR) was 15.9%. The proportion of children with urinary iodine excretion levels <20, 20-49, 50-99, 100-199 and 200 μg/L and above, was nil, 11.8, 24.9, 38.3 and 25.0 percent, respectively. The median Urinary Iodine Excretion level was 125μg/L. About 57.7% of the children were consuming salt with iodine content of 15 ppm and more. Findings of the present study indicates that the population is possibly in transition phase from iodine deficient as revealed by Total Goitre Rate of 15.9% to iodine sufficient as revealed by median urinary iodine excretion level of 125 μg/L. There is a need to further strengthen the existing monitoring system for the quality of iodized salt in the district in order to achieve the elimination of IDD.

Himachal Pradesh is a known endemic area for iodine deficiency disorders. A study was conducted in district Solan with the objective of assessing the prevalence of iodine deficiency disorders in school-age children. Thirty clusters were selected by using the probability-proportionate-to-size cluster sampling methodology. Clinical examination of the thyroid of 1898 children in the age-group of 6-12 years was conducted. Urine and salt samples were collected. The total goiter rate was found to be 15.4%. Median urinary iodine excretion level was 62.5 μg/l. Only 39% of the salt samples had iodine content of ≥15 ppm. Mild iodine deficiency was present in the subjects studied.

Objectives: To compare: (i) prevalences of thinness in school-children by four body mass index references in common use viz., Centre for Disease Control (CDC); Cole; Indian Academy of Pediatrics (IAP); World Health Organization (WHO); and (ii) relationship of thinness with absence of cardio-metabolic risk factors in these BMI references., Design: Cross-sectional., Setting: Schools in Delhi., Participants: Anthropometry and blood pressure were measured in 16,245 school children aged 5 to 18 years. Fasting lipids and blood sugar were estimated in 2796 subjects., Outcome Measures: Age and sex-specific prevalences of thinness and predictive ability of reference cut-off for detecting any cardio-metabolic risk factor were compared., Results: Prevalence of thinness varied with the reference employed; more so for boys. Overall prevalence of thinness was least with IAP reference and highest with CDC cut-offs (6.6% to 16.9% in boys, 6.5% to 10.3% in girls). Children identified as thin by any reference had comparable, significantly lower risks (OR 0.59 to 0.73) of associated cardio-metabolic aberrations. In subjects with any cardio-metabolic or blood pressure aberration, the prevalence of thinness was highest with CDC and least with IAP definition., Conclusion: Prevalence of thinness varies considerably with the reference employed. Thin children, identified by any reference, have a lower risk of associated cardio-metabolic aberrations; however, thinness is a poor diagnostic test for this purpose. In populations undergoing nutrition transition, there is a need to link cardio-metabolic risk factors with recommended anthropometric criteria to define undernutrition.

Background and Objectives: Hypertension is one of the most common diseases world-wide and the prevalence in school-aged children appears to be increasing perhaps as a result of increased prevalence of obesity. Thus, the present study was planned to establish an association between body mass index (BMI) and waist circumference (WC) with hypertension amongst school children in the age group of 5-16 years belonging to lower income group (LIG) and middle income group (MIG) in National Capital Territory of Delhi., Materials and Methods: Population proportionate to size methodology was adopted to select 30 clusters/schools in each LIG and MIG category. About 170 children from each school were selected randomly with the help of random number tables. Anthropometric measurements of weight, height and WC and blood pressure measurements were taken by using standard methodology., Results: The prevalence of high systolic blood pressure (SBP) in LIG and MIG school population was 2.8% and 4.1% respectively. Similarly, the prevalence of high diastolic blood pressure (DBP) in LIG and MIG school population was 2.7% and 4.2%, respectively. Statistical positive correlation was observed between BMI and WC with SBP and DBP. Thus, it can be inferred that children with high WC and BMI are more likely to have hypertension.

The National Prophylaxis Programme against Nutritional Blindness due to vitamin A deficiency (NPPNB due to VAD) was started in 1970 with the specific aim of preventing nutritional blindness due to keratomalacia . The Programme was launched as an urgent remedial measure to combat the unacceptably high magnitude of xerophthalmic blindness in the country seen in the 1950s and 1960s. Clinical VAD has declined drastically during the last 40 years. Also, indicators of child health have shown substantial gains in different States in the country. The prevalence of severe undernutrition has come down significantly. Immunization coverage for measles and other vaccine preventable diseases has improved from 5-7 per cent in early seventies to currently 60-90 per cent, in different States. Similarly, there has been a significant improvement in the overall dietary intake of young children. There has been virtual disappearance of keratomalacia, and a sharp decline in the prevalence of Bitot spots. Prophylactic mega dose administration of vitamin A is primarily advocated because of the claim of 23 per cent reduction in childhood mortality. However, benefits on this scale have been found only in areas with rudimentary health care facilities where clinical deficiency is common, and there is substantial heterogeneity, especially with inclusion of all trials. There is an urgent need for adopting a targeted rather than universal prophylactic mega dose vitamin A supplementation in preschool children. This approach is justified on the basis of currently available evidence documenting a substantial decline in VAD prevalence, substantial heterogeneity and uncertainty about mortality effects in present era with improved health care, and resource constraints with competing priorities.

Justification: Severe acute malnutrition (SAM) is a major public health issue. It afflicts an estimated 8.1 million under-five children in India causing nearly 0.6 million deaths. The improved understanding of pathophysiology of SAM as well as new internationally accepted growth charts and newer modalities of integrated intervention have necessitated a relook at IAP recommendations., Process: A National Consultative Meeting on Integrated Management of Severe Acute Malnutrition was held in Mumbai on 16th and 17th October, 2010. It was attended by the invited experts in the field. Extensive discussions were held as per the program. The participants were then divided into six groups for detailed discussions. The groups deliberated on various issues pertaining to the task assigned and presented recommendations of the groups in a plenary session. The participants made a list of recommendations after extensive discussions. A Writing Committee was formed and was entrusted with the task of drawing a Consensus Statement on the basis of these Recommendations. After multiple deliberations, the following Consensus Statement was adopted., Objectives: To critically evaluate the current global evidence to formulate a consensus among stakeholders regarding diagnosis and management of SAM., Recommendations: An integrated management of malnutrition is likely to yield more dividends. Thus, management of SAM should constitute an important component of Integrated Management of Neonatal and Childhood Illnesses (IMNCI) program. Determination of SAM on the basis of Z-scores using WHO Growth charts is considered statistically more appropriate than cut-offs based on percentage weight deficit of the median. Considering the fact that many children with SAM can be successfully managed on outpatient basis and even in the community, it is no more considered necessary to advise admission of all children with SAM to a healthcare facility. Management of SAM should not be a stand-alone program. It should integrate with community management therapeutic programs and linkages with child treatment center, district hospitals and tertiary level centers offering inpatient management for SAM and include judicious use of ready-to-use-therapeutic Food (RUTF). All sections of healthcare providers need to be trained in the integrated management of SAM.

Risk factors for gallbladder cancer (GBC) except gallstones are not well known. The objective was to study the risk factors for GBC. In a case-control study, 200 patients with GBC, 200 healthy controls and 200 gallstones patients as diseased controls were included prospectively. The risk factors studied were related to socioeconomic profile, life style, reproduction, diet and bile acids. On comparing GBC patients (mean age 51.7 years; 130 females) with healthy controls, risk factors were chemical exposure [odd ratios (OR): 7.0 (2.7-18.2); p < 0.001)], family history of gallstones [OR: 5.3 (1.5-18.9); p < 0.01)], tobacco [OR: 4.1 (1.8-9.7); p < 0.001)], fried foods [OR: 3.1 (1.7-5.6); p < 0.001], joint family [OR: 3.2 (1.7-6.2); p < 0.001], long interval between meals [OR: 1.4 (1.2-1.6); p < 0.001] and residence in Gangetic belt [OR: 3.3 (1.8-6.2); p < 0.001]. On comparing GBC cases with gallstone controls, risk factors were female gender [OR: 2.4 (1.3-4.3); p = 0.004], residence in Gangetic belt [OR: 2.3 (1.2-4.4); p = 0.012], fried foods [OR: 2.5 (1.4-4.4); p < 0.001], diabetes [OR: 2.7 (1.2-6.4); p = 0.02)], tobacco [OR 3.8 (1.7-8.1); p < 0.001)] and joint family [OR: 2.1 (1.2-3.4); p = 0.004]. The ratio of secondary to primary bile acids was significantly higher in GBC cases than gallstone controls (20.8 vs. 0.44). Fried foods, tobacco, chemical exposure, family history of gallstones, residence in Gangetic belt and secondary bile acids were significant risk factors for GBC., (Copyright © 2012 UICC.)

India is currently undergoing a rapid transition on economic, demographic, epidemiologic, nutrition, and sociological fronts. There is evidence of a decline in undernutrition with a simultaneous escalation in overnutrition and associated non-communicable diseases (NCDs). However, the current concern and national policy response for tackling malnutrition in India is still primarily restricted to undernutrition diagnosed on the basis of body size (anthropometry). A complex range of interacting factors have been linked to the rising trend of overnutrition and associated NCDs from a global perspective. The burden of overnutrition and associated morbidities is rapidly escalating to alarming proportions, particularly in urban areas and high socio-economic status groups. The poor are not spared from this transition. It is predicted that a more rapid transition may occur amongst poor populations in future with higher economic development. The need of the hour is to launch an integrated public health response to the dual burden beginning from pregnancy and early life. This will obviously require careful deliberation of the strategy and interventions, and a multi-sectoral approach, especially involving the health, women and child development, nutrition, education, agriculture, food processing, trade, architecture, water supply and sanitation, community and non-governmental organizations.

Objective: To assess the validity of the Spot testing kit (against the Standard iodometric titration) in the semi-quantitative estimation of iodine in salt., Methods: The project was conducted at four centers in the country, namely, Vadodara, Jodhpur, Dibrugarh and New Delhi (AIIMS, coordinating centre). Uniform protocol was utilized at all the four centres. The school going children were asked to bring 30 g of iodine from their kitchen in an auto-seal polythene pouche with an identification slip. The iodine content of 3,010 salt samples (more than 700 salt samples from each center) was estimated independently by two different methods i.e., Iodometric Titration (used as reference standard) and Spot Testing Kit (STK) method, and the results were compared. The sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of the STK method in relation to the Standard iodometric titration (SIT) were calculated., Results: From total of 3,010 salt samples, majority (98%) of the salt samples collected were of powdered salt. The specificity of various research centers ranged from 60.5% to 88.9%. The positive predictive value of the various research centers ranged from 69.9% to 98%. The NPV of the various research centers ranged from 96.8% to 100%. The pooled sensitivity and specificity of the STK method (against IT method) for four centers together was 99.6% and 66.7%, respectively., Conclusions: The STK method is reliable for semi quantitative estimation of iodine content in salt and it can be used for monitoring the quality of iodized salt available in the community.

In severe iodine deficient areas, iodine deficiency has been documented to be an important etiological factor leading to poor fetal growth and development. Iodine is essential for physical growth and development of the central nervous system of the fetus. Iodine deficiency in pregnant mothers leads to increased incidence of infertility and abortions, perinatal mortality and infant child mortality. The clinical iodine supplementation trials have documented adverse health consequences due to iodine deficiency. Evidence from observational studies concludes that prevention of iodine deficiency can lead to reduction in infant mortality rate and facilitate to achieve millennium development goal-4., (Copyright © 2012 Elsevier GmbH. Background: In women, cancer of the breast is one of the most common incident cancer and cause of death from cancer. Anthropometric factors of weight, height, and body mass index (BMI) have been associated with breast cancer risk., Objectives: To study the association of overweight and obesity with breast cancer in India., Materials and Methods: A hospital-based matched case-control study was conducted. Three hundred and twenty newly diagnosed breast cancer patients and three hundred and twenty normal healthy individuals constituted the study population. The subjects in the control group were matched individually with the patients for their age ±2 years and socioeconomic status. Anthropometric measurements of weight and height were recorded utilizing the standard equipments and methodology. The paired 't' test and univariate logistic regression analysis were carried out., Results: It was observed that the patients had a statistically higher mean weight, body mass index, and mid upper arm circumference as compared to the controls. It was observed that the risk of breast cancer increased with increasing levels of BMI. Overweight and obese women had Odd's redio of 1.06 and 2.27, respectively, as compared to women with normal weight., Conclusions: The results of the present study revealed a strong association of overweight and obesity with breast cancer in the Indian population.

Objective: To estimate the prevalence of serum zinc deficiency in children of 6 months to 60 months of age., Methods: A community based cross-sectional study was conducted to collect the data to establish the prevalence of zinc deficiency in children in 6-60 months of age in five states namely, Uttar Pradesh(Northern region), Karnataka (Southern region), Orissa (Eastern region), Gujarat (Western region) and Madhya Pradesh (Central region) of the country. In each state, all the districts with ICDS scheme were enlisted and one district was selected with the help of Random number table (RNT). In each district, all the ICDS projects were selected and one ICDS project was selected with help of RNT. In the selected ICDS project, a cluster 5 Anganwadi centres (AWC) were selected. From the selected AWCs, three hundred children in the age group of 6-60 months were selected for the detailed study. A total of 1,655 subjects (836 males and 819 females) were included. The blood samples were collected from each child. The serum zinc estimation was done by atomic absorption spectrophotometer., Results: The overall prevalence of zinc deficiency in five states was 43.8% . The prevalence of zinc deficiency was highest in Orissa (51.3%), followed by Uttar Pradesh (48.1%), Gujarat (44.2%), Madhya Pradesh (38.9%) and Karnataka (36.2%)., Conclusions: The present study revealed a high prevalence of zinc deficiency in children belonging to Low Socio-economic Index (LSI) in India.

Objective: To study the Status of Iodine Content of Salt in four regions of India., Methods: At each of the four centers (Vadodara, Dibrugarh, Jodhpur, New Delhi), High Schools were selected randomly from list of schools obtained from district education office and more than 700 salt samples were selected from each center. A total of 3,010 salt samples were collected from students of High Schools (consumed at their households), selected randomly from four centers and iodine content of salt by Standard Iodometric Titration Method (IT) was estimated., Results: Analysis revealed that majority of salt sample collected at 4 centers were of powdered variety of salt. Analysis of iodine content in salt by IT method revealed that high proportion of school children (51.6%) consumed salt having inadequate iodine content (salt samples with less than 15 ppm of iodine) in Jodhpur district followed by Vadodara (19.8%), New Delhi (8.5%) and least in Dibrugarh (1.2%). The percentage of consumption of adequately iodized salt was highest in Dibrugarh (98.8%) among the four regions of India., Conclusions: Status of iodine content is varying from state to state i.e. highest at Dibrugarh and lowest at Jodhpur. This indicates that consumption of iodized salt in Jodhpur is low and needs more attention. Government may adopt different strategies in different states. There is a strong need of iodization of salt in addition to creating awareness among rural inhabitants for consumption of iodized salt especially in Jodhpur District. More attention is required for monitoring quality of iodized salt available in the community.

Objective: Childhood obesity is associated with unfavorable lipid profile. Obese children have increased risk of development of coronary heart disease (CHD) in adult life. There is lack of data on association of dyslipidemia with obesity in children from India, hence, the present investigation was undertaken., Methods: Study was conducted in NCT of Delhi. Thirty schools in each of the 3 socio economic groups i.e., LIG, MIG and HIG were selected. From each school, 170 children were included in the survey. A total of 16,585 children were studied. Anthropometric measurements of weight and height were taken using standard methodology Four hundred and fifty one obese children (cases) were identified using IOTF classification. Equal number of children, matched for age, sex and socio-economic status, with optimal body weight were selected as control. The blood samples were collected for lipid profile estimation. Same investigations were undertaken, both in cases and controls., Results: In obese children, mean total cholesterol, triglyceride and LDL-C levels were significantly higher as compared to their matched controls (p<0.001). BMI was significantly correlated with lipid profile., Conclusions: Obesity amongst children was associated with unfavorable lipid profile.

Iodine deficiency (ID) is the world's single most important preventable cause of brain damage and mental retardation. Iodine deficiency disorders (IDDs) is a public health problem in 130 countries, affecting 13% of the world population. The simplest solution to prevent the IDD is to consume iodized common salt every day. In India, significant progress has been achieved toward elimination of IDD, in the last 30 years. Satisfactory levels of urinary iodine excretion and iodine content of salt have been documented by the research surveys conducted by research scientists. The results indicate that we are progressing toward elimination of IDD. IDD is due to a nutritional deficiency, which is prima-rily that of iodine, in soil and water. IDD is known to re-appear if the IDD Control Program is not sustained. To ensure that the population continues to have intake of adequate amount of iodine, there is a need of i) periodic surveys to assess the magnitude of the IDD with respect to impact of iodized salt (IS) intervention; ii) strengthening the health and nutrition education activities to create demand for IS and iii) development of a monitoring information system (MIS) for ensuring that the adequately IS is available to the beneficiaries.

Justification: Severe acute malnutrition (SAM) is an important preventable and treatable cause of morbidity and mortality in children below five years of age in India. The concerned stakeholders are not in agreement about the role of product based medical nutrition therapy in the management of this condition., Process: In November 2009, a National Consensus Workshop was organized by the Department of Human Nutrition, All India Institute of Medical Sciences, New Delhi in collaboration with the Department of Pediatrics and Clinical Epidemiology, Sitaram Bhartia Institute of Science and Research, New Delhi, and the Sub-specialty Chapter on Nutrition, Indian Academy of Pediatrics. Presentations by eminent national and international scientists, the ensuing discussions, and opinions expressed by the participants provided the basic framework for drafting the consensus statement. The draft of the consensus statement was circulated to all the participants; it underwent two revisions after consideration of their comments., Objectives: (i) Critically appraise the current global evidence on the utility of Medical Nutrition Therapy (MNT) for the management of SAM in under five children; (ii) Formulate a consensus amongst stakeholders regarding the need to introduce product based MNT for the management of SAM in under five children in India; (iii) Identify research priorities for MNT for the management of SAM in under five children in India; and (iv)Ascertain potential challenges for introducing product based MNT in India, if consensus opinion identifies such a need., Recommendations: Guidelines related to the role of MNT in management of children suffering from SAM are presented. Global and regional data document the effectiveness of MNT using ready-to-use therapeutic foods (RUTF) and locally formulated products. Adequate caution should be exercised to ensure that MNT for SAM does not interfere with measures for the holistic prevention of childhood undernutrition. Indian manufacture of RUTF is feasible, and can be scaled up. Product-based nutrition therapy including RUTF can be introduced on a pilot basis when a delivery design and plan of action is developed and is in place as a part of the larger system to deal with childhood undernutrition. RUTF should be used only as therapeutic and not supplementary feeding, above six months of age, and for a limited time period (4-8 weeks) until the child recovers from SAM, which should be defined in explicit treatment protocols. An urgent research issue is comparison of RUTF with home-based and locally-formulated products.
