In [H. Huang, N.L. Johnson: Semifield planes of order 82, Discrete Math., 80 (1990)], the authors exhibited seven sporadic semifields of order 2^6, with left nucleus F_{2^3} and center F_2. Following the notation of that paper, these examples are referred as the Huang–Johnson semifields of type II, III, IV , V , V I, V II and V III. In [N. L. Johnson, V. Jha, M. Biliotti: Handbook of Finite Translation Planes, Pure and Applied Mathematics, Taylor Books, 2007], the question whether these semifields are contained in larger families, rather then sporadic, is posed. In this paper, we first prove that the Huang–Johnson semifield of type V I is isotopic to a cyclic semifield, whereas those of types V II and V III belong to infinite families recently constructed in [N.L. Johnson, G. Marino, O. Polverino, R. Trombetti: Semifields of order q^6 with left nucleus F_{q^3} and center F_q, Finite Fields Appl., 14 (2008)] and [G.L. Ebert, G. Marino, O. Polverino, R. Trombetti: Infinite families of new semifields, Combinatorica, 6 (2009)]. Then, Huang–Johnson semifields of type II and III are extended to new infinite families of semifields of order q6, existing for every prime power q.

We demonstrate optically tunable control of second-harmonic generation in all-dielectric nanoantennas: by using a control beam that is absorbed by the nanoresonator, we thermo-optically change the refractive index of the radiating element to modulate the amplitude of the second-harmonic signal. For a moderate temperature increase of roughly 40 K, modulation of the efficiency up to 60% is demonstrated; this large tunability of the single meta-atom response paves the way to exciting avenues for reconfigurable homogeneous and heterogeneous metasurfaces.

A missão cristã japonesa, iniciada com a chegada de Francisco Xavier ao arquipélago em 1549, inaugurou o chamado século cristão no Japão. Foi um período bastante conturbado na história japonesa: guerras civis quase ininterruptas vinham assolando o arquipélago há décadas e os generais que foram capazes de colocar fim ao contínuo estado de conflito militar e unificar o reino Oda Nobunaga e Toyotomi Hideyoshi não conseguiram fazer com que seus descendentes herdassem suas posições. Ao fim, o clã Tokugawa tomou o poder e inaugurou o regime militar que ficaria conhecido como Tokugawa Bakufu e duraria por mais de dois séculos e meio. Para que esse novo regime fosse possível, uma série de estruturas legitimadoras foram forjadas. Como resultado, o cristianismo foi interditado e os reinos ibéricos banidos e proibidos de retornar aos portos japoneses. As ordens missionárias europeias foram testemunhas de todo esse processo e buscaram até o fim negociar com esse poder em formação, na tentativa de manter a missão cristã ativa. Dentre elas, a Companhia de Jesus foi a que mais atuou no Japão; por mais de quatro décadas, teve assegurado o monopólio da missão japonesa. Em 1593, no entanto, os franciscanos espanhóis iniciaram sua atividade no arquipélago, a despeito da forte oposição jesuíta. O que se busca compreender neste trabalho, através dos escritos produzidos por estes missionários e de algumas obras dos japoneses acerca do cristianismo, é como jesuítas e franciscanos desenvolveram a missão cristã no contexto da unificação do Japão, e, por outro lado, a forma como os japoneses se apropriaram desse cristianismo, incluindo a reação que o mesmo causou nos círculos intelectuais dentro e fora do Bakufu. archipelago, in 1549, inaugurated the so-called Christian Century in Japan. That was an eventful period in Japanese History: almost uninterrupted civil wars stroke the country for almost a century and the generals who were able to put an end to the continuous warfare and finally unify Japan Oda Nobunaga e Toyotomi Hideyoshi could not make their offspring successors to the positions they achieved. In the end, the Tokugawa house took the power and inaugurated the military regime that would become known as Tokugawa Bakufu and would last for the next two and a half centuries. In order to make this new regime possible, some new legitimizing structures were forged. As a result, Christianity was banned, and the Iberian kingdoms expelled and forbidden inside the archipelago. The European missionaries witnessed all this process and sought until the very end to negotiate with these powers, in an attempt to save the Christian mission. The Society of Jesus was the Catholic order that most worked in Japan. For more than four decades it held the monopoly over the Japanese mission. In 1593, however, the Spanish Franciscans began their activity in the archipelago, despite the Jesuit opposition. What this thesis aims to understand, through the writings of these missionaries and some works of the Japanese about Christianity, is how Jesuits and Franciscans developed the Christian mission in the context of the unification of Japan, the way the Japanese appropriated this Christianity and the reaction it caused in intellectual circles inside and outside the Bakufu.

460. Field trial of hollow fiber modules of hybrid facilitated transport membranes for flue gas CO2 capture in cement industry.

[Display omitted] • Pre-pilot scale hybrid facilitated transport hollow fiber membrane modules were fabricated. • Three different modules were tested using untreated real flue gas from the cement industry. • 1.5–1.9x higher CO 2 permeances compared to lab tests were documented in field tests. • Membrane modules showed long-term stability with negligible effects from impurities. • Simulation studies reveal the high potential of the membranes for CO 2 capture. Alarming increase in global CO 2 emissions warrants acceleration of CO 2 capture technologies. In this work, testing of pre-pilot scale membrane modules containing hybrid facilitated transport membranes in hollow fiber configuration is reported. The tests were carried out using real flue gas from a slipstream in the Colacem cement plant located in Gubbio, Italy. With the fabricated modules, CO 2 flux of up to 750 NL m−2h−1 with a CO 2 permeate purity ranging from 50 to 55 vol% was recorded. All pre-pilot membrane modules showed increased CO 2 permeance in industrial testing (1.5 to 1.9x higher) compared to laboratory evaluation owing to the reliable water profile and high temperature of the flue gas from the chimney. Influence of operating parameters (e.g., pressures in the feed and permeate) were studied. Long-term testing showed no obvious reduction in permeation performance. Furthermore, the membranes with mobile carriers when exposed to the feed gas containing SO x and NO x exhibited good resistance to performance deterioration despite high concentrations of acidic impurities. Simulation studies based on validated experimental performance under industrial conditions reveal the high potential of the fabricated membranes as an efficient separation unit capable of achieving industrial capture rate and CO 2 purity requirements using a relatively low membrane area. Il lavoro muove dall’idea di analizzare il sistema delle regole che definiscono, sul piano dell’an, del quando e del quomodo, l’esattezza dell’adempimento che può definirsi (necessariamente) intermediato dell’obbligazione pecuniaria: nella vicenda solutoria della quale, cioè, risulti indefettibile, per volontà della legge o per scelta dei privati, l’interposizione di uno o più soggetti, altri rispetto al debitore e al creditore parti originarie del rapporto obbligatorio, con funzioni ausiliarie. Ciò al fine di saggiare la bontà della ipotesi ricostruttiva in base alla quale l’adempimento dell’obbligazione pecuniaria per mezzo di operazioni di pagamento intermediato non sia governato dalla logica e dalle disposizioni del codice civile che tradizionalmente presiedono alla traditio pecuniae, bensì da un sistema in sé conchiuso di regole, europee ed interne, che siano peculiari del pagamento con moneta non contante indaga i problemi posti dal diffondersi di modalità solutorie dell'obbligazione pecuniaria, diverse dalla tradizionale consegna del denaro contante, che si connotano per il necessario interporsi di più soggetti, professionalmente operanti nel mercato dei pagamenti, terzi rispetto alle parti del rapporto obbligatorio "primario". Le spinte provenienti dalla realtà materiale e dalle norme speciali del diritto dei pagamenti sembrano incrinare la monoliticità del modello codicistico e indicare piuttosto il manifestarsi di un sistema policentrico, nel quale si affianca allo schema classico il paradigma dell’ adempimento (necessariamente) intermediato.

463. Colorectal endoscopic submucosal dissection: predictors and neoplasm-related gradients of difficulty.

- Abstract
Background and Study Aim: The role of colorectal endoscopic submucosal dissection (ESD) is standardized in Japan and East Asia, but technical difficulties hinder its diffusion. The aim was to identify predictors of difficulty for each neoplasm type., Methods: A competent operator performed all procedures. ESD difficulty was defined as: en bloc with a slow speed (< 0.07 cm
2 /min; 30 × 30 mm neoplasm in > 90 min), conversion to endoscopic mucosal resection, or resection abandonment. Pre- and intraoperative difficulty variables were defined according to standard criteria, and evaluated separately for the rectum and colon. Difficulty predictors and gradients were evaluated by the multivariate logistic regression model., Results: A total of 140 ESDs were included: 110 in the colon and 30 in the rectum. Neoplasms were laterally spreading tumors - granular type (LST-G) in 85 cases (61 %); the median longer axis was 30 mm (range 15 - 180 mm); a scar was present in 15 cases (11 %). ESD en bloc resection and difficulty rates were 85 % (n = 94) and 35 % (n = 39) in the colon, and 73 % (n = 22) and 50 % (n = 15) in the rectum ( P = 0.17 and 0.28, respectively). The scar was the only preoperative predictor of difficulty in the rectum (odds ratio [OR] 12.3, 95 % confidence interval [CI] 1.27 - 118.36), whereas predictors in the colon were: scar (OR 12.7, 95 %CI 1.15 - 139.24), LST - nongranular type (NG) (OR 10.5, 95 %CI 1.20 - 55.14), and sessile polyp morphology (OR 3.1, 95 %CI 1.18 - 10.39). Size > 7 - ≤ 12 cm2 (OR 0.20, 95 %CI 0.06 - 0.74) and operator experience > 120 procedures (OR 0.19, 95 %CI 0.04 - 0.81) were predictors for a easy procedure. No intraoperative predictors of difficulty were identified in the rectum, whereas predictors in the colon were: severe submucosal fibrosis (OR 21.9, 95 %CI 2.11 - 225.64), ineffective submucosal exposure by gravity countertraction (OR 12.3, 95 %CI 2.43 - 62.08), and perpendicular submucosal dissection approach (OR 5.2, 95 %CI 1.07 - 25.03). When experience was /= 90, preoperative gradient of colonic ESD difficulty was the highest for LST-NGs (scar positive and negative up to 47 % and 20 %, respectively), intermediate for sessile polyps with scar (up to 23 %), and the lowest for LST-Gs (< 8 %). Different difficulty gradients between neoplasm types persisted with increasing experience: LST-NG rate up to 14 % after 120 procedures., Conclusions: Colonic and rectal ESD difficulty has qualitative differences. Preoperative predictors should be considered to identify the difficulty gradient of each neoplasm type and the appropriate setting for ESD.- Published

464. Heterotopic gastric mucosa in the anus and rectum: first case report of endoscopic submucosal dissection and systematic review.

- Abstract
Background: Heterotopic gastric mucosa (HGM) is the most reported epithelial heterotopia, but it is very rare in the rectum and anus., Methods: The first case of an asymptomatic adult male with a large nonpolypoid HGM in the low rectum underwent complete resection by endoscopic submucosal dissection (ESD) is reported. The systematic review was based on a comprehensive search of MEDLINE, EMBASE and Google Scholar. Studies on humans were identified with the term 'heterotopic gastric mucosa in the rectum and /or anus.', Results: The search identified 79 citations, and 72 cases were evaluated comprising the present report. Congenital malformations were observed in 17 (24%) patients; rectal duplication accounted for most of the cases. The HGM was located in the anus and perineal rectum in 25 cases (41%) and low, middle and proximal pelvic rectum in 20 (33%), five (8%) and 11 cases (18%), respectively. Morphology was nonpolypoid in 37 cases (51%), polypoid in 26 cases (36%) and ulcerated in nine cases (13%). Specific anorectal symptoms were reported by 50 (69%) patients of the whole study population, and by 33 (97%) of 34 patients ≤ 18 years. Complications were observed in 23 cases (32%). The HGM was excised in 50 cases (83%). Endoscopic resection was performed in 17 cases (34%); resection was piecemeal in five of 12 lesions ≥15 mm, required argon plasma coagulation in two cases and was associated with residual tissue in two (17%). Intestinal metaplasia and an adenoma with low-grade dysplasia were described in three adults (4%)., Discussion: This systematic review shows that the HGM in the rectum and anus may be associated with specific rectal symptoms and serious complications, mainly in the pediatric population, and a risk of malignancy in adults. Its complete excision should be recommended, and the ESD can overcome the technical limits of conventional endoscopic snare resection and avoid unnecessary surgery., (© The Author(s) 2016. - Abstract
Objective: A case of a 45 years old man affected by the Von Recklinghausen disease with a rare neurofibroma of the thoracic wall is reported., Materials: Starting from 2000 the Authors are using the V.A.T.S. (video assisted thoracic surgery) in their Department of surgery, as minimally invasive approach to diagnosis and treatment of some thoracic diseases: initially the patient was studied by a standard chest x-ray followed by tomography and magnetic resonance to determine the localization of the neoplasm of the chest wall., Results: The neurofibroma was removed by V.A.T.S., through a minimal thoracic access. After treatment we observed the complete remission of symptoms., Discussion: Single localization of the neurofibroma in the Von Recklinghausen disease can transform into a malignant neoplasm. The indication to surgery, in the case presented, was determined by the intense thoracic pain and by the risk of malignant degeneration. The minimally invasive approach showed to be the best option according to the dimensions of the neurofibroma. In the actual literature this approach represents the gold standard in the treatment of small intrathoracic neoplasm., Conclusion: In the chest wall localization of the Von Recklinghausen disease the Authors recommend the minimal surgical treatment by using V.A.T.S.

The gastrointestinal stromal tumors (GISTs) represent 1% among bowel diseases. Their clinical history is unknown, and it seems to develop from muscolar or nervous tissue in the bowel wall. The grade of malignancy is directly correlate to the dimension of tumor and the number of mythosis for field. We report our experience in diagnosis and treatment.

- Abstract
Chemotherapy is an established approach for several malignancies, but its utility may be hampered by induced cardiac toxicity possibly leading to heart failure, with a negative impact on the patient's quality of life and ultimately survival. Prospective left ventricular systolic function monitoring has demonstrated that cardiotoxicity could be subclinically present for many months or years before its overt manifestation. Although considered irreversible, some reports suggested recovery or delayed progression of cardiac dysfunction by preventive cardioactive therapies. Thus, the identification of earlier instrumental or biochemical markers of cardiac injury able to predict heart failure remains a major task. Diastolic indexes as a primary expression of hemodynamic dysfunction after cardiac damage, analyzed by means of conventional or newer Doppler technologies (tissue Doppler, color M-mode, etc.) are discussed. Moreover, brain natriuretic peptides, troponins and endothelin-1, as possible sensitive/specific markers/predictors of subclinical cardiotoxicity are reviewed in order to update and possibly improve the strategy for the detection and clinical management of chemotherapy-related cardiotoxic effects.

Background: Tension free hernioplasty under local anaesthesia in day care basis is now the gold standard for inguinal hernia surgery, but a severe postoperative pain may affect convalescence and return to work, so minimizing economic benefits of day hospital., Methods: To evaluate the incidence and predisposing factors of severe postoperative pain the authors examined 100 patients (96 men and 4 women, min. age 16 yrs, max 91 yrs, median age 53 yrs) operated from January 1999 to January 2000; in all cases a tension free hernioplasty under local anesthesia was performed., Results: Good results were obtained in 83% of patients for intraoperative comfort, in 91% for postoperative pain, in 86% for immediate postoperative walking and in 93% for walking in the first week. Postoperative pain (first 24 hours) was strictly correlated to intraoperative pain, while a difficult postoperative walking was predictive for a difficult walking in the first week., Conclusions: Tension free hernioplasty allows a good functional recovery to daily activities since the first postoperative week; with an even more accurate patients selection and aggressive postoperative analgesia in pain risk patients better results will be achievable.

- Abstract
Thrombocytopenia can be a pathophysiological feature of pregnancy and spontaneous splenic rupture in pregnancy is a rare and potentially catastrophic event. A case of splenic rupture after cesarean delivery, performed for increasing thrombocytopenia is reported. The diagnosis was not suspected until laparotomy. Low transverse cesarean birth required a vertical extension of the incision into the upper uterine segment. An emergency splenectomy was undertaken without complications. The operation was followed by a massive increase in the thrombocyte count. Both mother and baby are in good health 10 months later. The newborn had a normal thrombocyte count at delivery and thereafter. The importance of early diagnosis and management techniques are discussed.

- Abstract
Background: Recurrent inguinal hernia is still frequently observed today., Methods: In order to evaluate factors causing failure of hernia repair 64 recurrent hernias are examined (63 men, 1 woman; age: min 32 years, max 88, median 60 years), 54 previously operated with "traditional" Italian surgical school techniques (Bassini and Postempskij) and 10 with "tension free" operations. For every patient previous surgical procedures, complications occurred, timing of recurrence and intraoperative findings at reoperation were recorded., Results: In the "traditional" hernioplasty group, 31 indirect and 23 direct hernias occurred; median relapse time was 11 years. In "tension free" techniques, 4 mesh suture failures (Lichtenstein), 2 mesh or plug dislocations (Trabucco) and 4 indirect hernias (mesh ring failures) were found at reoperation; median relapse time in this group was 2 years. The findings of a so long relapse time for traditional techniques was surprising, it is obvious now that every new "tension free" technique must have (at least) a such success rate. In the first group, inadequate technique of operation and physiologic weakening of the abdominal wall were responsible for the relapse, while in the second a technical failure was the main reason., Conclusions: "Traditional" hernia surgery is now progressively abandoned in favour of "tension free" techniques, in which recurrences are due to the "physiologic" training period of the surgeon; mesh itself doesn't warrant success, but anatomical knowledge and an adequate dissection of musculo-fascial planes are mandatory.
