Oesophageal stents have been widely used to prevent occlusion or stenosis in the treatment of oesophageal cancers. However, stent restenosis caused by tumour ingrowth occurs frequently after stent placement. Incorporating anti-cancer drugs into endoluminal stents is a promising strategy to provide a sustained release of drugs to oesophageal malignant tissues while prolonging the retention of the stent and relieving dysphagia. Recognizing the potential of 3D printing to produce personalised stents with patient specific geometries, we herein report the development of a drug-loaded 3D printed stent for the sustained local delivery of 5-fluorouracil (5-FU) to treat oesophageal cancer. The 3D printed drug-eluting stents (DESs) were fabricated via fused deposition modelling using 5-FU-loaded polyurethane filament. Determination of the 5-FU in the filament and stent (>97%) confirmed that minimal degradation of the drug occurred during the thermal extrusion and 3D printing processes. The physicochemical properties of the stents were investigated using photoacoustic Fourier-transform infrared (PA-FTIR) spectrophotometry, X-ray diffraction (XRD), differential scanning calorimetry (DSC), thermal gravimetric analysis (TGA) and mechanical testing. In vitro release studies revealed that the drug-loaded stents provided a sustained release of 5-FU over a period of 110 days and allowed the constant diffusion of 5-FU when in contact with oesophageal mucosa. Furthermore, the 3D printed stents exhibited good stability following sterilization with gamma or UV irradiation, and during accelerated storage. This study demonstrates that 3D printing is a powerful tool for manufacturing DESs which could easily be customized to provide personalized, patient specific geometries and drug doses. Background: Rheumatoid arthritis (RA) is an inflammatory autoimmune condition associated with an increased risk of developing depression and anxiety. Depression and anxiety are associated with worse outcomes in RA, but the magnitude of the effect of each condition on RA outcomes is unclear. It is also unknown how pharmacological treatment of depression affects RA outcomes. Objective: The primary aim of this study was to investigate the association of comorbid depression and anxiety with remission in patients with RA. Secondary aims were to determine the association between comorbid depression and anxiety on patient-reported outcomes and the relationship between concomitant use of antidepressants and remission in patients with depression. Design: Data from patients with moderate to severe RA were pooled from five randomised controlled trials investigating tocilizumab and conventional synthetic disease-modifying agents. Methods: Remission was defined as a clinical disease activity index (CDAI) of ⩽2.8 and simple disease activity index (SDAI) of ⩽3.3. The association between the time to reach remission and depression and anxiety was analysed using Cox proportional hazard analysis. Results: Individual patient data were available from 5502 subjects, of whom 511 had depression, 236 had anxiety and 387 were using antidepressants. Depression was significantly associated with reduced remission [adjusted HR (95% CI): 0.62 (0.48–0.80), p Conclusion: Comorbid depression, but not anxiety, was associated with less frequent remission. Concomitant antidepressant use was not associated with improvements in RA outcomes in patients with depression.

Purpose Sorafenib is an effective therapy for advanced hepatocellular carcinoma (HCC). Hand–foot syndrome (HFS) is a serious adverse effect associated with sorafenib therapy. This study aimed to develop an updated clinical prediction tool that allows personalized prediction of HFS following sorafenib initiation. Methods Individual participant data from Phase III clinical trial NCT00699374 were used in Cox proportional hazard analysis of the association between pre-treatment clinicopathological data and grade ≥ 3 HFS occurring within the first 365 days of sorafenib treatment for advanced HCC. Multivariable prediction models were developed using stepwise forward inclusion and backward deletion and internally validated using a random forest machine learning approach. Results Of 542 patients, 116 (21%) experienced grades ≥ 3 HFS. The prediction tool was optimally defined by sex (male vs female), haemoglobin (P ≤ 0.001). The high (score = 3 +)-, intermediate (score = 2)- and low (score = 0–1)-risk subgroups had 40%, 27% and 14% probability of developing grade ≥ 3 HFS within the first 365 days of sorafenib treatment, respectively. Conclusion A clinical prediction tool defined by female sex, high haemoglobin and low bilirubin had high discrimination for predicting HFS risk. The tool may enable improved evaluation of personalized risks of HFS for patients with advanced HCC initiating sorafenib.

Introduction : The discrimination performance of Bellmunt risk score for immune checkpoint inhibitor (ICI) therapy is largely unknown. This study aimed to validate and enhance discrimination of the Bellmunt score in patients with urothelial carcinoma treated with ICIs. Patients and methods : Cox proportional hazard analysis was used to validate overall survival (OS) discrimination performance of Bellmunt score in patients with urothelial carcinoma treated with atezolizumab in IMvigor210. The c-statistic (c) was used to evaluate the ability of C-reactive protein (CRP), neutrophil-to-lymphocyte ratio (NLR), lactate dehydrogenase (LDH), PD-L1 gene expression level on immune cells (PD-L1 ICs), albumin, time from prior chemotherapy, and tumor site count to enhance Bellmunt score. External validation of an enhanced Bellmunt score utilized independent atezolizumab arm of IMvigor211. Results : In IMvigor210, Bellmunt score displayed moderate OS discrimination (c=0.66). Addition of CRP (one point for CRP>30mg/L) to Bellmunt score resulted in greatest improvement in performance (c=0.70), followed by NLR (c=0.69). On external validation, CRP-Bellmunt score had superior performance (OS c=0.67, PFS c=0.60) than original Bellmunt score (OS c=0.64, PFS c=0.59) with 30% of patients reclassified into a higher risk group. Patients with CRP-Bellmunt score of 0, 1, 2, or 3-plus had 1-year OS probabilities of 63%, 44%, 21%, and 15%, respectively. Conclusions : CRP inclusion within Bellmunt score enhanced the ability to discriminate high risk patients misclassified using the original Bellmunt model. We propose that the CRP-Bellmunt score may enable improved patient stratification in ICI clinical trials and provide more accurate prognostic information for patients with urothelial carcinoma initiating ICIs. Micro Abstract The Bellmunt risk score is a well-recognized prognostic model in second-line urothelial carcinoma. However, its discrimination performance for immune checkpoint inhibitor (ICI) therapy is unclear. The present analysis indicates addition of one point for C-reactive protein (CRP>30mg/L) to Bellmunt score resulted in superior survival discrimination. The CRP-Bellmunt score will enable more accurate patient stratification in clinical trials and enhanced prognostic information for patients with urothelial carcinoma initiating ICIs.

Glioblastoma (GB) is a primary malignancy of the central nervous system that is classified by the WHO as a grade IV astrocytoma. Despite decades of research, several aspects about the biology of GB are still unclear. Its pathogenesis and resistance mechanisms are poorly understood, and methods to optimize patient diagnosis and prognosis remain a bottle neck owing to the heterogeneity of the malignancy. The field of omics has recently gained traction, as it can aid in understanding the dynamic spatiotemporal regulatory network of enzymes and metabolites that allows cancer cells to adjust to their surroundings to promote tumor development. In combination with other omics techniques, proteomic and metabolomic investigations, which are a potent means for examining a variety of metabolic enzymes as well as intermediate metabolites, might offer crucial information in this area. Therefore, this review intends to stress the major contribution these tools have made in GB clinical and preclinical research and highlights the crucial impacts made by the integrative “omics” approach in reducing some of the therapeutic challenges associated with GB research and treatment. Thus, our study can purvey the use of these powerful tools in research by serving as a hub that particularly summarizes studies employing metabolomics and proteomics in the realm of GB diagnosis, treatment, and prognosis. Refereed/Peer-reviewed Background: Atezolizumab is an immune checkpoint inhibitor (ICI) and a frontline treatment of patients with cisplatin-ineligible advanced urothelial carcinoma (UC). There is limited evidence on the prognostic value of patient reported outcomes (PROs) in advanced UC treatment, particularly in the context of ICI therapy. Objective: To investigate the prognostic association of PROs with survival in patients with advanced UC treated with atezolizumab. Methods: This study used data from 467 patients with advanced UC initiating atezolizumab in the IMvigor211 trial. Pre-treatment PROs association with overall survival (OS) and progression free survival (PFS) was assessed using Cox proportional hazard analysis. PROs were recorded via the European Organisation for Research and Treatment of Cancer QLQ-C30. Discrimination performance was assessed via the C-statistic (c). Objectives: Acquiring the ability to identify deviations in dental aesthetic norms has been considered as an essential skill for the development of a dental professional. The timing and quality of this development of aesthetic perception has not been explicitly discussed in the dental education literature. This study aimed to evaluate the impact of dental training on the development of smile aesthetic perception, and to compare this perception between dental students and medical students in a similar cohort. Methods: One-hundred dental and medical students were invited to complete a self-administered questionnaire, designed to evaluate eight images; using a visual analogue scale (1 least attractive; 100 most attractive). The students rated the smile aesthetics of one ‘model’ smile image, and seven digitally modified smiles representing major variations in smile features. Data were analyzed using the independent student’s t-test. Results: forty-four male and fifty-five female participants were successfully recruited, with a response rate of 99%. Dental students’ ratings of smile aesthetics were significantly lower than medical students for seven of the eight images (p < 0.05). This difference was greatest for the model smile image, and least for the diminutive lateral incisors image. There were no statistically significant gender-related differences. Conclusions: Within the limitation of this study, the dental curriculum has a significant impact on developing students’ perception of dental aesthetics. Longitudinal studies with larger samples sizes, are needed to examine if progress through the dental curriculum affects students’ perception of dental aesthetics.

The global prevalence of COVID-19 has necessitated the use of personal protective equipment, particularly face masks, amongst healthcare workers (HCWs). However, extended face mask usage has led to adverse health effects. This cross-sectional study investigates the relationship between prolonged surgical mask-wearing and the development of adverse effects among 500 HCWs in Jazan, Saudi Arabia. The study utilized an anonymous questionnaire distributed across multiple departments in Jazan and Sabya Hospitals. The results indicated that most participants believed that wearing face masks for extended periods led to adverse skin and respiratory effects. The primary respiratory complaints were difficulty breathing and nasal itching, while skin-related issues included sweating, itching, and acne. Therefore, this study concludes that extended surgical mask usage during the COVID-19 pandemic could potentially lead to adverse health implications among HCWs in Jazan. - Abstract
COVID-19 (Coronavirus Disease 2019) vaccinations are a critical control measure for the coronavirus pandemic that began in 2019. Several COVID-19 vaccines have been developed, and their effectiveness will almost certainly vary. This study aimed to assess how effective two doses of the Pfizer and Oxford-AstraZeneca vaccines were in preventing SARS‐CoV‐2 infection six months after administration. This is a retrospective cohort study of adult individuals from the Jazan Region of Saudi Arabia who received their second dose of the COVID-19 vaccine [Pfizer and Oxford-AstraZeneca (ASZ)] between April and June 2021. The monitoring and follow-up period continued until the end of January 2022. Data were retrieved from the Health Electronic Surveillance Network and National Vaccination Records. Logistic regression was performed to assess the risk of COVID-19 infection among the vaccinated subjects. This study included randomly enrolled 4458 participants in Jazan who received two doses of the COVID-19 vaccine during the research period. The majority of them received the Pfizer vaccine (3136/4458; 70.3%), while the remaining received the ASZ vaccine (1322/4458; 29.7%). The study participants' mean age was 59.7 years, with a male-to-female ratio of 1.9:1.0 (2920:1538). The results showed that the Pfizer and ASZ vaccines' protection against infection decreased from 93.2% and 90.2%, respectively, during the first three months, to 68.5% and 68.1% after a six-month interval. In the current study population, being Saudi Arabian, younger as well as having longer intervals between vaccines or crossing a 6-month period after the second vaccine dose were factors linked to higher rates of breakthrough infections. Our findings revealed variations in the efficacy of different COVID-19 vaccine types against COVID-19 breakthrough infections. Salmonella enterica is a significant foodborne disease, originating from contaminated eggs and meat from poultry being the predominant cause of illness and a serious global public health problem. The aim of this study to determine of antimicrobial resistance, Salmonella enterica virulence (invA) gene and extended spectrum bata-lactamase (ESBL) production in food and hospitalized patients. In the present study, a total (of 630) samples were collected from different specimens. Conventional protocols used to isolate Salmonella enterica. Typical black colonies were verified using the VITEK®2 automated system., subsequently antimicrobial susceptibility test (AST) for isolated strains was conducted. In addition, all Salmonella enterica isolates were examined for the presence of the invA gene, as well as phenotype and genetic ESBL production genes (targeting three different resistance genes., bla
TEM , blaSHV and blaCTX-M ) using polymerase chain reaction (PCR). Identification of 60 samples among (630) via traditional method were black colony, in which 52 positive samples were obtained using VITEK®2 system, while 55 positive samples were reidentified by applying the PCR technique. The highest resistant rate via AST that was recorded against Gentamycin antibiotic, and the result was (94.6%). The disc placement method revealed that all 55 Salmonella enterica isolates showed (ESBL) production. PCR assay revealed that the most frequently identified genes were blaTEM, which was present in (100%) of strains. Genotype analysis of isolates for the presence of the invA gene, in which 37 positive PCR products were sequenced and all sequences (n = 37) were uploaded to GenBank. In a recent paper, a new method was proposed to find the common invariant subspaces of a set of matrices. This paper investigates the more general problem of putting a set of matrices into block triangular or block-diagonal form simultaneously. Based on common invariant subspaces, two algorithms for simultaneous block triangularization and block diagonalization of sets of matrices are presented. As an alternate approach for simultaneous block diagonalization of sets of matrices by an invertible matrix, a new algorithm is developed based on the generalized eigenvectors of a commuting matrix. Moreover, a new characterization for the simultaneous block diagonalization by an invertible matrix is provided. Crystal structure prediction (CSP) has been historically used to complement experimental solid form screening and applied to individual molecules in drug development. The fast development of algorithms and computing resources offers the opportunity to use CSP earlier and for a broader range of applications in the drug design cycle. This study presents a novel paradigm of CSP specifically designed for structurally related molecules, referred to as Quick-CSP. The approach prioritizes more accurate physics through robust and transferable tailor-made force fields (TMFFs), such that significant efficiency gains are achieved through the reduction of expensive ab initio calculations. The accuracy of the TMFF is increased by the introduction of electrostatic multipoles and the fragment-based force field parameterization scheme is demonstrated to be transferable for a family of chemically related molecules. The protocol is benchmarked with structurally related compounds from the Bromodomain and Extraterminal (BET) domain inhibitors series. A new convergence criterion is introduced that aims at performing only as many ab initio optimizations of crystal structures as required to locate the bottom of the crystal energy landscape within a user-defined accuracy. The overall approach provides significant cost savings ranging from three to eight-fold less than the Full-CSP workflow. The reported advancements expand the scope and utility of the underlying CSP building blocks as well as their novel reassembly to other applications earlier in the drug design cycle to guide molecule design and selection.

- Abstract
A mechanical system, in general, undergoes vibrational motion when the system is subjected to a tension or an external force. One of the examples of such a system is a cantilever beam when it is exposed to a bending action. When the tension is released, the cantilever beam suffers from the oscillations until the strain energy is totally released through the damping characteristics of the cantilever beam. Depending on the stiffness and damping factors of the beam, the vibrational motion can be non-linear; in which case, the analytical solution becomes challenging formulating the flexural characteristics of the beam. Although numerical solution for the non-linear problem is possible, the analytical solution provides useful information between the mechanical response and the cantilever beam characteristics. In the present study, the analytical solution of the non-linear equations governing the motion of the cantilever beam is presented. The governing equation is linearized incorporating the Lie-Tresse linearization method. The closed form solution for the displacement of the cantilever beam is reduced to a linear solution after introducing the appropriate beam characteristics. The dynamic behavior of the flexural motion due to non-linear and linear cantilever beams are compared.

- Abstract
The physicochemical properties of molecular crystals, such as solubility, stability, compactability, melting behaviour and bioavailability, depend on their crystal form1. In silico crystal form selection has recently come much closer to realization because of the development of accurate and affordable free-energy calculations2–4. Here we redefine the state of the art, primarily by improving the accuracy of free-energy calculations, constructing a reliable experimental benchmark for solid–solid free-energy differences, quantifying statistical errors for the computed free energies and placing both hydrate crystal structures of different stoichiometries and anhydrate crystal structures on the same energy landscape, with defined error bars, as a function of temperature and relative humidity. The calculated free energies have standard errors of 1–2 kJ mol−1for industrially relevant compounds, and the method to place crystal structures with different hydrate stoichiometries on the same energy landscape can be extended to other multi-component systems, including solvates. These contributions reduce the gap between the needs of the experimentalist and the capabilities of modern computational tools, transforming crystal structure prediction into a more reliable and actionable procedure that can be used in combination with experimental evidence to direct crystal form selection and establish control5.

This study aimed at exploring the relationship of goal orientations with self-regulated learning among Yarmouk University students in light of faculty and gender. The sample of the study consisted of 684 students. Two scales were used: a goal orientations scale, and aself-regulated learning scale. The results showed statisticallysignificant differences in the means of participants' scores on the goal orientation scales due to the student gender in mastery goals, and performance-avoidance goals; females scored higher on these subscales than males. No significant gender difference was found in performance-approach goals. The results also showed significant differences in the means scores on the self-regulated learning scale due to gender; females once again scored higher than males on all skills, except for the planning and goal specification skill. No significant difference, however, was found in self-regulated learning due tostudent faculty or the interaction between faculty and gender.

Objective Saudi Arabia has succeeded in having one of the lowest rates of COVID-19 worldwide due to the government’s initiatives in taking swift action to control both the spread and severity of the virus. However, Covid-19 can serve as a test case of the expected response of the new healthcare system under Vision 2030. This study used data from the thirteen present administrative regions of KSA to simulate the variations in ICU admission as a quality indicator in the five business units proposed by a new Model of Care. Methods We determined the rates of ICU admission for patients with confirmed SARS-CoV-2 (COVID-19) from March to mid-July 2020. The final sample included 1743 inpatients with moderate to severe COVID-19. Patient characteristics, including demographics, pre-existing chronic conditions, and COVID-19 complications, were collected. Business units (BUs) were compared with respect to the relative odds of ICU admission by using multiple logistic regression. Results After keeping patient and clinical characteristics constant, clear BU differences were observed in the relative odds of ICU admission of COVID-19 patients. Inpatient admission to ICU in our total sample was almost 50%. Compared to the Central BU, the Northern and Western BUs showed significantly higher odds of ICU admission while the Eastern & Southern BUs had significantly lower odds. Conclusion ICU use for COVID-19 patients differed significantly in KSA healthcare BUs, consistent with variations in care for other non-COVID-19-related conditions. These differences cannot be explained by patient or clinical characteristics, suggesting quality-of-care differences. We believe that privatization and the shift to fewer administrative BUs will help lessen or eliminate altogether the present variations in healthcare service provision.

With data comes uncertainty, which is a widespread and frequent phenomenon in data science and analysis. The amount of information available to us is growing exponentially, owing to never-ending technological advancements. Data visualization is one of the ways to convey that information effectively. Since the error is intrinsic to data, users cannot ignore it in visualization. Failing to observe it in visualization can lead to flawed decision-making by data analysts. Data scientists know that missing out on uncertainty in data visualization can lead to misleading conclusions about data accuracy. In most cases, visualization approaches assume that the information represented is free from any error or unreliability; however, this is rarely true. The goal of uncertainty visualization is to minimize the errors in judgment and represent the information as accurately as possible. This survey discusses state-of-the-art approaches to uncertainty visualization, along with the concept of uncertainty and its sources. From the study of uncertainty visualization literature, we identified popular techniques accompanied by their merits and shortcomings. We also briefly discuss several uncertainty visualization evaluation strategies. Finally, we present possible future research directions in uncertainty visualization, along with the conclusion.

Purpose Application of multi-scale modelling workflows to characterise polymorphism in ritonavir with regard to its stability, bioavailability and processing. Methods Molecular conformation, polarizability and stability are examined using quantum mechanics (QM). Intermolecular synthons, hydrogen bonding, crystal morphology and surface chemistry are modelled using empirical force fields. Results The form I conformation is more stable and polarized with more efficient intermolecular packing, lower void space and higher density, however its shielded hydroxyl is only a hydrogen bond donor. In contrast, the hydroxyl in the more open but less stable and polarized form II conformation is both a donor and acceptor resulting in stronger hydrogen bonding and a more stable crystal structure but one that is less dense. Both forms have strong 1D networks of hydrogen bonds and the differences in packing energies are partially offset in form II by its conformational deformation energy difference with respect to form I. The lattice energies converge at shorter distances for form I, consistent with its preferential crystallization at high supersaturation. Both forms exhibit a needle/lath-like crystal habit with slower growing hydrophobic side and faster growing hydrophilic capping habit faces with aspect ratios increasing from polar-protic, polar-aprotic and non-polar solvents, respectively. Surface energies are higher for form II than form I and increase with solvent polarity. The higher deformation, lattice and surface energies of form II are consistent with its lower solubility and hence bioavailability. Conclusion Inter-relationship between molecular, solid-state and surface structures of the polymorphic forms of ritonavir are quantified in relation to their physical-chemical properties.

ObjectivesStudies have demonstrated high rates of mortality in people with proximal femoral fracture and SARS-CoV-2, but there is limited published data on the factors that influence mortality for clinicians to make informed treatment decisions. This study aims to report the 30-day mortality associated with perioperative infection of patients undergoing surgery for proximal femoral fractures and to examine the factors that influence mortality in a multivariate analysis.SettingProspective, international, multicentre, observational cohort study.ParticipantsPatients undergoing any operation for a proximal femoral fracture from 1 February to 30 April 2020 and with perioperative SARS-CoV-2 infection (either 7 days prior or 30-day postoperative).Primary outcome30-day mortality. Multivariate modelling was performed to identify factors associated with 30-day mortality.ResultsThis study reports included 1063 patients from 174 hospitals in 19 countries. Overall 30-day mortality was 29.4% (313/1063). In an adjusted model, 30-day mortality was associated with male gender (OR 2.29, 95% CI 1.68 to 3.13, p80 years (OR 1.60, 95% CI 1.1 to 2.31, p=0.013), preoperative diagnosis of dementia (OR 1.57, 95% CI 1.15 to 2.16, p=0.005), kidney disease (OR 1.73, 95% CI 1.18 to 2.55, p=0.005) and congestive heart failure (OR 1.62, 95% CI 1.06 to 2.48, p=0.025). Mortality at 30 days was lower in patients with a preoperative diagnosis of SARS-CoV-2 (OR 0.6, 95% CI 0.6 (0.42 to 0.85), p=0.004). There was no difference in mortality in patients with an increase to delay in surgery (p=0.220) or type of anaesthetic given (p=0.787).ConclusionsPatients undergoing surgery for a proximal femoral fracture with a perioperative infection of SARS-CoV-2 have a high rate of mortality. This study would support the need for providing these patients with individualised medical and anaesthetic care, including medical optimisation before theatre. Careful preoperative counselling is needed for those with a proximal femoral fracture and SARS-CoV-2, especially those in the highest risk groups.Trial registration numberNCT04323644
