Objective To compare the efficacy of dual-frequency dual-pulse laser lithotripsy under endoscopic retrograde cholangiopancreatography (ERCP) and traditional endoscopic mechanical lithotripsy in the treatment of patients with common bile duct stones, and summarize perioperative nursing measures. Methods From May 2018 to May 2022, the clinical data of 200 patients with common bile duct stones who underwent ERCP treatment and lithotripsy were retrospectively analyzed, of which 69 patients were treated with dual-frequency dual-pulse laser lithotripsy (Laser group) and 131 patients were treated with mechanical lithotripsy (Mechanical group), and the success rate of stone extraction, operation time, postoperative hospital stay time and complications of two groups of patients were compared. Results There was no significant difference in baseline data such as general conditions and preoperative clinical data between two groups (P＞0. 05). Neither neither Laser group nor Mechanical group had reported the perioperative death case. There were no significant difference in postoperative bleeding, postoperative pancreatitis and perforation complication rates in the two groups (P＞0. 05). The surgical time in the Laser group was significantly longer than that in the Mechanical group (P＜0. 05). The postoperative hospital stay was shorter in the Laser group than that in the Mechanical group(P＜0. 05), and the total complication rate and stone residue rate were lower in the Laser group than those in the Mechanical group (P＜0. 05). Conclusion The treatment of common bile duct stones by dual-frequency dual-pulse laser lithotripsy has better efficacy and lower complication rate than traditional surgical mechanical lithotripsy, but there is still room for improvement in the operation time. (目的 比较内镜逆行胰胆管造影术(ERCP)下双频双脉冲激光碎石与内镜下传统机械碎石治疗胆总管结石患者的效果, 总结护理经验。方法 回顾性分析2018年5月—2022年5月在北京中医药大学东方医院消化内镜中心行ERCP治疗并碎石的200例胆总管结石患者的临床资料, 其中69例患者采用双频双脉冲激光碎石术(激光组), 131例患者采用机械碎石(机械组)。比较两组患者的取石成功率、手术时间、术后住院时间、并发症等情况。结果 两组患者一般情况、术前临床资料等基线资料比较, 差异无统计学意义(P＞0. 05)。激光组和机械组均无围术期死亡患者。两组患者术后出血、术后胰腺炎、穿孔并发症率比较差异无统计学意义(P＞0. 05)。与机械组相比, 激光组的手术时间较长(P＜0. 05), 术后住院时间更短(P＜0. 05), 总并发症率和结石残留率更低(P＜0. 05)。结论 ERCP下双频双脉冲激光碎石治疗胆总管结石具有比传统手术机械碎石更好的效果及更低的并发症率, 但手术时间仍有改进的空间。)

MicroRNA (miRNA) dysregulation has been identified in several carcinomas, including non-small cell lung cancer (NSCLC), and is known to play a role in the development and progression of this disease. We initially conducted a miRNA microarray analysis, which revealed that the MNK inhibitor CGP57380 increased the expression of miR-150-3p. A similar analysis was performed using data from The Cancer Genome Atlas (TCGA). Cell proliferation, colony formation and migration assays were validated in A549 and H157 cells treated with miR-150-3p mimics. Quantitative polymerase chain reaction (qPCR) was then used to detect potential target genes. We observed significant downregulation of miR-150-3p in NSCLC samples compared with normal samples (P = 0.035). High miR-150-3p expression was associated with longer overall survival (P = 0.005), as determined via a tissue microarray (TMA). These results were validated in the TCGA and revealed that miR-150-3p was expressed at low levels in NSCLC tissues (P < 0.0001) and that patients with high miR-150-3p expression had a better prognosis (P = 0.042). Moreover, the combination of miR-150-3p and CGP57380 exerted a synergistic inhibitory effect on colony formation, growth, and migration and induced apoptosis in NSCLC cell lines. We investigated the potential targets of miR-150-3p and successfully validated six potential target genes through qPCR analysis. High miR-150-3p expression may enhance the response to immunotherapy, cisplatin and gemcitabine. In summary, this study underscores the promising therapeutic implications of combining miR-150-3p and CGP57380 for NSCLC treatment. This paper reported the nursing experience of a patient undergoing Chinese herbal medicine enema treatment for gastrointestinal dysfunction after laparoscopy for adrenal mass. The patient was diagnosed with intestinal Qi stagnation according to the Traditional Chinese Medicine syndrome differentiation. The Chinese herbal medicine enema technique and coordinated integrated nursing measures were carried out after nursing evaluation. This paper summarized the clinical observation and nursing measures of a patient with acute mastitis during lactation treated by milk-ejection manipulation technique combined with external application of Rutong powder. On the basis of nursing assessment and basic care, the milk-ejection manipulation technique combined with external application of Rutong powder showed a positive effect on relief of pain and anxiety of the patient during lactation. This method has the advantages of few side effects, high safety and high patient acceptance. This paper reviewed the clinical data of an acute mastitis patient undergoing paracentesis of breast abscess, and summarized the effect and measures of milk-ejection manipulation technique combined with external application of Rutong powder in postoperative pain relief. Nursing assessment, manipulation technique, component analysis of Rutong powder, external application methods and cautions and routine nursing intervention were summarized. Based on the Traditional Chinese Medicine nursing by syndrome differentiation, application of milk-ejection manipulation technique combined with external application of Rutong powder is effective to relief the pain of the patient after paracentesis of breast abscess. It is an effective Traditional Chinese Medicine nursing technology which helps mothers to build confidence in breastfeeding and ensure the maternal and infant safety. This paper summarized the nursing experience of a patient with Liver-Qi stagnation and phlegm stagnation type hyperplasia of mammary glands treated by Traditional Chinese Medicine (TCM) ointment rubbing combined with moxibustion. Nursing measures and efficacy observation of TCM ointment combined with moxibustion at Zusanli acupoint were performed. In addition to the targeted interventions by TCM syndrome differentiation, health education on dietary and emotion regulation was carried to achieve the effect of dredging milk collaterals, detumescence, relieving pain and improving gland hyperplasia. Objective To investigate the effect of external application of Rutong powder in the treatment of stagnation stage mastitis. Methods Totally 60 patients with stagnation stage mastitis were randomly divided into observation group and control group, with 30 cases in each group. Patients in both control group and observation group were treated with milk-ejection manipulation technique, and those in the observation group received the external application of Chinese herbal medicine Rutong powder. Results The overall effective rate was 96. 67%(29/30) in the observation group, which was higher than 83. 33%(25/30) in the control group (P＜0. 05). Conclusion Milk-ejection manipulation technique combined with external application of Rutong powder is effective in relief of symptoms and discomfort in patients with acute mastitis at stagnation stage. (目的 探讨乳通散外敷治疗郁滞期乳痈的临床疗效。方法 选取2021年5月—2022年1月收治的郁滞期乳痈患者60例, 随机分为观察组和对照组, 各30例。对照组采用单纯的手法排乳治疗, 观察组在对照组基础上实施乳通散外敷干预。对比2组临床疗效。结果 观察组有效率96. 67%(29/30), 高于对照组的83. 33%(25/30), 差异有统计学意义(P＜0. 05)。结论 手法排乳联合乳通散外敷治疗有助于缓解郁滞期乳痈患者不适症状。)

Objective To analyze the incidence and mortality of male breast cancer in Hubei Province from 2012 to 2015. Methods The data reported by the Hubei Provincial Cancer Registry from 2012 to 2015 were collected for analyzing the incidence and mortality trends of male breast cancer according to year, urban and rural areas and age. We also compared the difference of incidence and mortality between male and female breast cancer. Results A total of 106 new cases of male breast cancer were reported in Hubei Cancer Registry from 2012 to 2015 with an incidence rate of 0.43/105, and 10052 new cases of female breast cancer with an incidence rate of 42.76/105; the male-to-female incidence ratio was 1:99. Male breast cancer death cases was 37 with the death rate of 0.15/105, and female breast cancer death cases was 2201 with the death rate of 9.36/105; the male-to-female mortality ratio was 1:62. The incidence and mortality of breast cancer between male and female varied by year, urban and rural areas. Male breast cancer was less common in young men (< 35 years old), and the incidence increased with age after 35 years old; male breast cancer deaths were rare in men under 45 years old, and the mortality rate increased with age. The incidence and mortality of male breast cancer patients over 85 years old reached the peak. Conclusion Male breast cancer is rare in clinical practice, and the incidence rate is much lower than that of female breast cancer. Even so, it is necessary to study the epidemiological law of male breast cancers' incidence and mortality, which can be used for prevention and treatment of male breast cancer.
