- Abstract
Neonatal immune cells have long been considered as immature versions of their adult counterparts, but overwhelming evidence suggests that they are as potent as adult cells to mount a protective response under specific circumstances. Rather, they are biased towards immunosuppressive mechanisms which correspond to the physiological demands of this time window. Indeed, neonates require to protect against infection but in the meantime, avoid alloimmune reaction, prevent Th1-mediated rejection and allow the commensal microbial colonization. This unique neonatal immune system, defined by a bias against Th1 cells polarizing cytokines, makes newborns more vulnerable to pathogenic infection with impaired immune responses to most vaccines and increased susceptibility to develop Th2-type inflammatory disorders such as allergies. Given the antigen inexperienced adaptive immunity, neonates rely on their innate immune system to confer protection but due to cell intrinsic hyperresponsiveness, frustrating efforts have been made to protect this vulnerable population. Recently, a lot of attention has focused on the first exposure to commensals that can activate innate immunity and potentially accelerate the neonatal maturation process. However, the mechanistic basis of the crosstalk between gut microbiota and immune system in newborns is poorly defined.To better understand how exogenous factors such as the microbiota prime the neonatal innate cells, we first assess the importance of commensal exposure on splenic and pulmonary conventional dendritic cells, which represent the bridge between innate and adaptive responses. In the neonatal spleen, the main subset is type 1 conventional dendritic cell precursors that have the particularity to produce the anti inflammatory cytokine IL-10, which limits Th1 responses as well as cytotoxic responses and thus maintain a tolerogenic environment. We report that the first exposure to microbiota induces a myeloid TNF secretion that drives the diffe, Le système immunitaire néonatale est associé au maintien d’un équilibre entre tolérance et protection afin de protéger l’hôte contre les infections, éviter les réactions allo immunes et permettre la colonisation microbienne commensale. Le début de vie est donc caractérisé par une susceptibilité accrue aux infections et aux réactions allergiques ainsi qu’une faible réponse vaccinale. Récemment, il a été mis en évidence que la colonisation du microbiote permet le développement du système immunitaire. Cependant, le mécanisme d'interaction entre le microbiote intestinal et le système immunitaire des nouveau-nés est mal défini.Dans un premier temps, nous avons démontré que la première exposition aux bactéries commensales induit une sécrétion de TNF par les cellules myéloïdes qui permet la différenciation et la maturation fonctionnelle des cellules dendritiques conventionnelles spléniques, en inhibant leur production d’IL-10 et en augmentant leur production d'IL-12p40. En outre, la colonisation du microbiote intestinal au début de la vie exerce également un effet à distance sur l'immunité pulmonaire en régulant positivement la fréquence des cellules dendritiques conventionnelles pulmonaires ainsi que des macrophages interstitiels en début de vie.Dans un second temps, nous avons mis en évidence que la supplémentation maternelle en probiotique Lactobacillus rhamnosus prévient l'asthme de type Th2 dans la progéniture en inhibant la maturation des cellules dendritiques conventionnelles pulmonaires et la polarisation ultérieure des cellules T contre les allergènes inhalés. Plus qu’une histoire de l’allaitement, il dresse un bilan des savoirs les plus actuels autour de ces pratiques, depuis les premières minutes de vie jusqu’à l’abandon définitif du lait maternel. Un moment unique, un regard croisé sur les modes d’alimentation des tout-petits et leurs déterminants, cette thématique, qui implique tant les sciences humaines et sociales que les sciences médicales, est abordée dans une perspective diachronique, pluridisciplinaire et internationale. Les contributions réunies dans cet ouvrage vont au-delà des connaissances déjà produites par les historiens. S’appuyant sur d’autres sources documentaires, comme celles contenues dans les milliers de squelettes humains issus des fouilles archéologiques, et les techniques d’analyse du contenu chimique des ossements, il est désormais possible d’explorer le temps long, et de nuancer parfois les informations fournies par les textes anciens. Le large panorama ici retracé permet de conclure qu’au-delà des normes imposées par le discours médical ou militant, l’allaitement maternel n’a jamais été le mode d’alimentation exclusif des nourrissons. International audience; Background: The high incidence of food allergy in childhood points to the need of elucidating early life factors dictating allergy susceptibility. Here, we aim to address in a mouse model how the exposure to a major cow's milk allergen through breastmilk of mothers with different immune status influences food allergy outcome in offspring. Methods: BALB/cJ future dams were either kept naive, or sensitized through the oral route using cholera toxin ("orally sensitized") or through the i.p. route using alum ("i.p. sensitized"), or rendered fully tolerant (oral gavage without any adjuvant) to bovine beta-lactoglobulin (BLG). After mating with naive males and delivery, mothers were orally exposed or not to BLG during the whole lactation. Then, eight groups of lactating mothers were considered: naive, i.p. sensitized, orally sensitized, or tolerant, each exposed or not during lactation. In order to specifically address breastmilk effects on their allergy susceptibility, pups from naive-synchronized mothers were cross-fostered by the different groups of treated dams and lactating mothers at delivery. In some experiments, mothers kept their own pups to address a possiblein uteroeffect. BLG antigen, BLG-specific antibodies, and BLG-immune complexes were measured in breastmilk from the different lactating mother groups. Allergic sensitization was monitored in 5-weeks old female offspring (n= 7-8/group of lactating mothers) by determining BLG-specific antibodies in plasma and splenocytes cytokine secretion after i.p. injections of BLG/alum. Allergic reaction to oral BLG challenge was evaluated by measuring mMCP1 in plasma.Results: Offspring was protected from one allergic i.p. sensitization when nursed by i.p. sensitized mothers, independently of BLG exposure during lactation. Orally sensitized dams conferred protection in offspring solely when exposed to BLG during lactation, while naive mothers did not provide any protection upon BLG exposure. The levels of protection correlated with the levels of BLG-specific antibodies and BLG-immune complex in breastmilk. There was a trend for decreased sensitization in offspring breastfed by tolerant and exposed mothers, which was not associated with transfer of specific antibodies through breastmilk. Protection provided by nursing by treated/exposed mothers was not persistent after a boost i.p. injection of the progeny and then did not protect them from an allergic reaction induced at this time point. No additionalin uteroeffects were evidenced. Conclusion: Our study demonstrates the strong potential of breastmilk to modulate immune response to a major cow's milk allergen in the progeny. It highlights the importance of maternal immune status and of her consumption of the allergen during lactation in dictating the outcomes in offspring. This opens perspectives where modulating maternal immune status might increase the chance of cow's milk allergy prevention in breastfed children.

Background: More than 17 million people across Europe have allergies to food and the burden of food allergies is increasing. In 2014, the European Academy of Allergy and Clinical Immunology (EAACI) published guidelines for preventing food allergy. Important research has been published since then and it is essential to ensure the guidelines reflect the latest evidence. A systematic review will be undertaken to help prepare new guidelines due to be published in 2020. Methods: Eleven bibliographic databases will be searched from inception to 31 October 2019 for randomized controlled trials about any intervention designed to prevent the development of new cases of immediate‐type/IgE‐mediated food allergy in infants, children and adults. There are few randomized controlled trials about the impact of breastfeeding on food allergy so prospective cohort studies about breastfeeding with at least 1000 participants at general risk or 200 at high risk of food allergy will also be eligible. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach will be used to assess the certainty of the evidence and tabulate summary data. The risk of bias in individual trials will be assessed using the Cochrane risk of bias tool. All data extraction and quality appraisal will be undertaken independently by two reviewers in partnership with a taskforce of EAACI members. Conclusions: Preventing food allergy has the potential to improve personal well‐being and reduce societal healthcare costs. It is important that forthcoming European guidelines take the latest research into account. Past reviews have tended to focus on single interventions or combined food allergy with other outcomes, making it difficult to draw robust conclusions about potential impacts for policy and practice. - Abstract
OBJECTIVES: We recently observed that duct cells constitutively express CD40, a membrane molecule whose engagement results in duct cell activation and proinflammatory cytokine secretion. This observation suggests a potential role of this pathway in the pathogenesis of type 1 diabetes, islet graft rejection, or acute pancreatitis. In this article, we investigated whether a salt derivative of N-acetyl-L-cysteine, Nacystelyn, could modulate CD40 expression on duct cells and the response of activated duct cells to CD40 engagement. METHODS: We assessed the effects of Nacystelyn on CD40 expression and function in human caucasian pancreatic adenocarcinoma, ATCC n degrees THB-80 (CAPAN-2) cells, a human pancreatic duct cell line. CD40 expression was analyzed by flow cytometry. To assess CAPAN-2 cell responses to CD40 engagement, we looked at nuclear factor-kappaB transcription factor activation using enzyme-linked immunosorbent assay and electrophoretic mobility shift assay and cytokine mRNA levels by quantitative real-time reverse transcriptase polymerase chain reaction. RESULTS: We observed that Nacystelyn dose-dependently inhibited CD40 expression on CAPAN-2 cells as well as CD40-induced nuclear factor kappaB activation and proinflammatory cytokines up-regulation. AIMS/HYPOTHESIS: The early loss of beta cells after islet cell transplantation has been attributed in part to blood coagulation at the implant site. Tissue factor expressed by beta cells and contaminating duct cells is considered to activate this process. Here, we investigated the ability of N-acetyl-L-cysteine to suppress the in vitro procoagulant activity of duct cells and human islet cell preparations. MATERIALS AND METHODS: The effects of Nacystelyn, a salt derivative of N-acetyl-L-cysteine, were first assessed on procoagulant activity induced in human plasma by recombinant tissue factor, human primary duct cells or human islet cell preparations. The influence of Nacystelyn on clot formation, platelet counts and D-dimers were measured in a whole blood tubing loop model. Human beta cell viability and insulin synthesis after Nacystelyn treatment were assessed to exclude cytotoxicity of Nacystelyn. RESULTS: Nacystelyn efficiently inhibited the procoagulant activity of human recombinant tissue factor, primary duct cells and human islet cell preparations at clinically relevant concentrations without cellular toxicity. Background Abnormal gut barrier function is the basis of gut inflammatory disease. It is known that house dust mite (HDM) aero-allergens induce inflammation in respiratory mucosa. We have recently reported allergen from Dermatophagoides pteronyssinus (Der p1) to be present in rodent gut. Objective To examine whether Der p1 is present in human gut and to assess its effect on gut barrier function and inflammation. Design Colonic biopsies, gut fluid, serum and stool were collected from healthy adults during endoscopy. Der p1 was measured by ELISA. Effect of HDM was assessed on gut permeability, tight-junction and mucin expression, and cytokine production, in presence or absence of cysteine protease inhibitors or serine protease inhibitors. In vivo effect of HDM was examined in mice given oral HDM or protease-neutralised HDM. Role of HDM in low-grade inflammation was studied in patients with IBS. Results HDM Der p1 was detected in the human gut. In colonic biopsies from healthy patients, HDM increased epithelial permeability (p<0.001), reduced expression of tight-junction proteins and mucus barrier. These effects were associated with increased tumour necrosis factor (TNF)-α and interleukin (IL)-10 production and were abolished by cysteine-protease inhibitor ( p<0.01). HDM effects did not require Th2 immunity. Results were confirmed in vivo in mice. In patients with IBS, HDM further deteriorated gut barrier function, induced TNF-α but failed to induce IL-10 secretion (p<0.001). Conclusions HDM, a ubiquitous environmental factor, is present in the human gut where it directly affects gut function through its proteolytic activity. AIMS/HYPOTHESIS: Human pancreatic duct cells are closely associated with islet beta cells, and contaminate islet suspensions transplanted in Type 1 diabetes mellitus patients. Activated duct cells produce cytotoxic mediators and possibly contribute to the pathogenesis of Type 1 diabetes mellitus or islet graft rejection. As CD40 transduces activation signals involved in inflammatory and immune disorders, we investigated CD40 expression on duct cells and their response to CD40 engagement. METHODS: CD40 expression on human pancreatic duct cells was analysed by flow cytometry and immunohistochemical analyses. To assess the function of CD40 expression on duct cells, activation of the transcription factor nuclear factor-kappa B was determined using electrophoretic mobility shift assay and ELISA. Cytokine mRNA levels were quantified by real-time RT-PCR, and protein levels by Luminex technology. RESULTS: Isolated human pancreatic duct cells and Capan-2 cell lines were found to express constitutively CD40. The expression of CD40 on duct cells was confirmed in vivo on human normal and pathological pancreatic specimens. CD40 ligation on Capan-2 cells induced rapid nuclear factor-kappa B activation, and supershift assays demonstrated that p50/p65 heterodimers and p50/p50 homodimers were present in the activated complexes in the nucleus. This activation was accompanied by tumour necrosis factor-a and interleukin-1beta mRNA accumulation. Tumour necrosis factor-alpha protein secretion was confirmed in CD40-activated Capan-2 cells and in isolated human pancreatic duct cells. CONCLUSIONS/INTERPRETATION: Interaction between activated T lymphocytes expressing CD40 ligand and duct cells expressing CD40 may contribute to the immune responses involved in Type 1 diabetes mellitus and islet graft rejection.
