4. Cutibacterium acnes strains associated with bone prosthesis infections cannot evade the host immune system.

Abstract

Introduction: Cutibacterium acnes is a commensal skin bacterium that is involved in bone prosthesis infections (BPIs) and presents low-grade clinical symptoms. C. acnes has been thought to escape the immune system at bone sites. Material and methods: Our study was carried out on a laboratory strain and two BPI-related clinical strains, one of which surprisingly induced clinical symptoms of inflammation in the patient. We investigated the ability of these C. acnes strains to trigger in vitro human primary neutrophils (PMN) response through inflammatory mediators measurements (antibody arrays, ELISA, RT-qPCR, zymography) and activation status assessment (flow cytometry), and to induce in vivo PMN recruitment from the bloodstream in mice air-pouch model. PMN-mediated inflammation was also studied in an original in vitro model mimetic of an infected bone site that combine titanium alloy, human primary osteoblasts, human primary neutrophils and C. acnes strains. Results: We demonstrated for the first time that both C. acnes planktonic and biofilm cultures, triggered an effective immune response by neutrophils in vitro and their recruitment in vivo. This host response was enhanced when using a strain from a patient with inflammatory signs. In an original infected prosthesis mimetic model, osteoblasts and neutrophils were able to detect C. acnes , but their response to the clinical C. acnes inflammatory strain decreased. Conclusion: This work provides the first evidence showing that the immune cell response to pathogenic C. acnes may be tuned by nonimmune cells at the infected site, such as osteoblasts, which may promote bacterial persistence. Abstract

Featured Application: Development of a new non-thermal O2 plasma process for future dentures sterilization related to denture stomatitis. Denture stomatitis is a disease involving C. albicans, which can affect elderly and immuno-compromised people. To avoid any recurrence of this pathology, it is necessary to treat patients regularly and disinfect dentures. However, the denture cleansers' efficacy is not optimal and often leads to adverse color effects on the denture base resins. The aim of this study was to investigate the efficacy of a low-pressure non-thermal O2 plasma (NTP) treatment on C. albicans seeded on ProBase®Hot resin (Ivoclar Vivadent). The viability reduction of C. albicans was assessed by colony forming units (CFU) analysis and by scanning electron microscopy (SEM). The effect of repeated treatments on the resin color was evaluated by spectrophotometry. The resin samples were placed in a sealed bag in which O2 plasma was generated in low-pressure conditions. The results showed that a 120-min O2 NTP treatment led to a 6-log reduction of C. albicans viability (p < 0.05) and to yeasts' major alterations observed by SEM. Furthermore, significant slight color changes of the resin (∆E00 = 1.33) were noted only after six plasma treatments (p < 0.05). However, the denture aesthetic was preserved, as the color changes were not perceptible and remained below the acceptability threshold (∆E00 < 4). [ABSTRACT FROM AUTHOR]

Abstract

Osteoclasts (OCs), the bone-resorbing cells, play a key role in skeletal development and adult bone remodeling. They also participate in the pathogenesis of various bone disorders. One of the major technical difficulties in the generation of OCs, when working on human material, is the ability to achieve large differentiation of mature OCs from human peripheral blood mononuclear cells (PBMCs). Access to a standardized source of active OCs is needed to better analyze the roles of human OCs. The aim of this study was to develop a procedure yielding active and mature OCs from fresh human PBMCs. We therefore examined the differentiation of PBMCs to OCs in different cell culture media, using non-stripped and charcoal-stripped sera in the presence of macrophage colony-stimulating factor (M-CSF) and receptor activator of nuclear factor kappa-B ligand (RANKL). We also studied the effects of vitamin D3 in the differentiation level of PBMCs to OCs. Phalloidin-AlexaFluor®488/DAPI fluorescent stainings and dentin resorption analyses by scanning electron microscopy were used to identify the number and size of differentiated OCs, number of nuclei per cell and resorption activities of OCs for a 7–14–21-day culture period. This study reports an optimized method for an efficient production of human active OCs from a low seeding density of PBMCs, after a 14-day culture period by using a medium containing fetal bovine charcoal-stripped serum in the presence of M-CSF and RANKL, and in the absence of vitamin D3.

International audience; The objective of this computational study was to propose a rapid procedure in obtaining an estimation of elastic moduli of solid phases of porous natural-polymeric biomaterials used for bone tissue engineering. This procedure was based on the comparison of experimental results to finite element (FE) responses of parallelepiped so-called representative volume elements (rev) of the material at hand. To address this issue a series of quasi-static unconfined compression tests were designed and performed on three prepared cylindrical biopolymer samples. Subsequently, a computed tomography scan was performed on fabricated specimens and two 3D images were reconstructed. Various parallelepiped revs of different sizes and located at distinct places within both constructs were isolated and then analyzed under unconfined compressive loads using FE modelling. In this preliminary study, for the sake of simplicity, the dried biopolymer solid is assumed to be linear elastic.

Abstract

International audience; The hallmark pathological features of bone disease in patients with cystic fibrosis (CF) include bone micro architectural changes and chronic inflammation which are associated with an irreversible loss in bone strength and density that tracks from childhood to adulthood. Given the equal importance of inflammation and CFTR-related remodeling in bone pathogenesis, there is a significant disparity in studies undertaken to investigate the contribution of each. Up to now, the majority focus on the role of systemic and lung inflammation, and although novel therapeutics such as improved pulmonary function and inflammation have arisen, it is apparent that targeting inflammation alone has not allowed amelioration of cystic fibrosis-related bone disease (CFBD). Therefore, unless bone remodeling is addressed for future therapeutic strategies, it is unlikely that we will progress towards a cure for bone disease. Having acknowledged these limitations, the focus of this review is to highlight the gaps in our current knowledge about the mechanisms underlying CFTR-related bone remodeling, the relationships between inflammation, bone remodeling and clinical phenotypes, and the importance of utilizing innovative pre-clinical models to uncover effective, diseasemodifying therapeutic strategies.

Abstract

Bone fragility and loss are a significant cause of morbidity in patients with cystic fibrosis (CF), and the lack of effective therapeutic options means that treatment is more often palliative rather than curative. A deeper understanding of the pathogenesis of CF-related bone disease (CFBD) is necessary to develop new therapies. Defective CF transmembrane conductance regulator (CFTR) protein and chronic inflammation in bone are important components of the CFBD development. The receptor activator of nuclear factor kappa-B ligand (RANKL) and osteoprotegerin (OPG) drive the regulation of bone turnover. To investigate their roles in CFBD, we evaluated the involvement of defective CFTR in their production level in CF primary human osteoblasts with and without inflammatory stimulation, in the presence or not of pharmacological correctors of the CFTR. No major difference in cell ultrastructure was noted between cultured CF and non-CF osteoblasts, but a delayed bone matrix mineralization was observed in CF osteoblasts. Strikingly, resting CF osteoblasts exhibited strong production of RANKL protein, which was highly localized at the cell membrane and was enhanced in TNF (TNF-α) or IL-17-stimulated conditions. Under TNF stimulation, a defective response in OPG production was observed in CF osteoblasts in contrast to the elevated OPG production of non-CF osteoblasts, leading to an elevated RANKL-to-OPG protein ratio in CF osteoblasts. Pharmacological inhibition of CFTR chloride channel conductance in non-CF osteoblasts replicated both the decreased OPG production and the enhanced RANKL-to-OPG ratio. Interestingly, using CFTR correctors such as C18, we significantly reduced the production of RANKL by CF osteoblasts, in both resting and TNF-stimulated conditions. In conclusion, the overexpression of RANKL and high membranous RANKL localization in osteoblasts are related to defective CFTR, and may worsen bone resorption, leading to bone loss in patients with CF. Abstract

Cyclic nucleotide phosphodiesterase type 4 (PDE4), which controls the intracellular level of cyclic adenosine monophosphate (cAMP), has aroused scientific attention as a suitable target for anti-inflammatory therapy of respiratory diseases. This work describes the development and characterization of pyridazinone derivatives bearing an indole moiety as potential PDE4 inhibitors and their evaluation as anti-inflammatory agents. Among these derivatives, 4-(5-methoxy-1H-indol-3-yl)-6-methylpyridazin-3(2H)-one possesses promising activity, and selectivity towards PDE4B isoenzymes and is able to regulate potent pro-inflammatory cytokine and chemokine production by human primary macrophages.

Abstract

Internationale.; International audience; Cette étude expérimentale et numérique vise à proposer une procédure permettant l'obtention rapide d'une estimation des modules d'élasticité des phases solides de biomatériaux poreux à base de polymères naturels destinés à l'ingénierie tissulaire de l'os. La procédure s'appuie sur la comparaison des résultats expérimentaux fournis par des essais de compression simple réalisés sur trois éprouvettes cylindriques et des résultats de calculs aux éléments finis de divers éléments parallélépipèdiques réputés être représentatifs du matériau considéré. Ces derniers sont de tailles différentes et arbitrairement répartis au sein de deux reconstructions à très haute résolution par tomographie à rayons X des éprouvettes testés. Pour cette première \étude, le matériau constitutif de la phase solide est supposé élastique linéaire, par souci de simplification.
