Background: Bone bruises are commonly seen on magnetic resonance imaging (MRI) in acute anterior cruciate ligament (ACL) injuries and can provide insight into the underlying mechanism of injury. There are limited reports that have compared the bone bruise patterns between contact and noncontact mechanisms of ACL injury. Purpose: To examine and compare the number and location of bone bruises in contact and noncontact ACL injuries. Study Design: Cross-sectional study; Level of evidence, 3. Methods: Three hundred twenty patients who underwent ACL reconstruction surgery between 2015 and 2021 were identified. Inclusion criteria were clear documentation of the mechanism of injury and MRI within 30 days of the injury on a 3-T scanner. Patients with concomitant fractures, injuries to the posterolateral corner or posterior cruciate ligament, and/or previous ipsilateral knee injury were excluded. Patients were stratified into 2 cohorts based on a contact or noncontact mechanism. Preoperative MRI scans were retrospectively reviewed by 2 musculoskeletal radiologists for bone bruises. The number and location of the bone bruises were recorded in the coronal and sagittal planes using fat-suppressed T2-weighted images and a standardized mapping technique. Lateral and medial meniscal tears were recorded from the operative notes, while medial collateral ligament (MCL) injuries were graded on MRI. Results: A total of 220 patients were included, with 142 (64.5%) noncontact injuries and 78 (35.5%) contact injuries. There was a significantly higher frequency of men in the contact cohort compared with the noncontact cohort (69.2% vs 54.2%, P = .030), while age and body mass index were comparable between the 2 cohorts. The bivariate analysis demonstrated a significantly higher rate of combined lateral tibiofemoral (lateral femoral condyle [LFC] + lateral tibial plateau [LTP]) bone bruises (82.1% vs 48.6%, P < .001) and a lower rate of combined medial tibiofemoral (medial femoral condyle [MFC] + medial tibial plateau [MTP]) bone bruises (39.7% vs 66.2%, P < .001) in knees with contact injuries. Similarly, noncontact injuries had a significantly higher rate of centrally located MFC bone bruises (80.3% vs 61.5%, P = .003) and posteriorly located MTP bruises (66.2% vs 52.6%, P = .047). When controlling for age and sex, the multivariate logistical regression model demonstrated that knees with contact injuries were more likely to have LTP bone bruises (OR, 4.721 [95% CI, 1.147-19.433], P = .032) and less likely to have combined medial tibiofemoral (MFC + MTP) bone bruises (OR, 0.331 [95% CI, 0.144-0.762], P = .009) compared with those with noncontact injuries. Conclusion: Significantly different bone bruise patterns were observed on MRI based on ACL injury mechanism, with contact and noncontact injuries demonstrating characteristic findings in the lateral tibiofemoral and medial tibiofemoral compartments, respectively.

Examine longitudinal changes in trunk, hip, and knee kinematics in maturing boys during an unanticipated cutting task.Prospective cohort study.Biomechanical laboratory.Forty-two high-school male basketball, volleyball, and soccer athletes.Trunk, hip, and knee range-of-motion (RoM), peak angles, and angles at initial contact during an unanticipated 45 degrees sidestep cutting task were estimated using laboratory-based three-dimensional optoelectronic motion capture. Maturation was classified using a modified Pubertal Maturational Observational Scale (PMOS) into prepubertal, midpubertal, or postpubertal stages.Trunk total RoM in frontal, sagittal, and transverse planes; peak trunk flexion, right lateral flexion and right rotation angles; hip total RoM in frontal, sagittal, and transverse planes; hip flexion angle at initial contact; peak hip flexion and adduction angles; knee total RoM in frontal, sagittal, and transverse planes; knee flexion angle at initial contact; peak knee flexion and abduction angles.As boys matured, there was a decrease in hip sagittal-plane RoM (49.02 degrees to 43.45 degrees, Benjamini-Hochberg adjusted P = 0.027), hip flexion at initial contact (29.33 degrees to 23.08 degrees, P = 0.018), and peak hip flexion (38.66 degrees to 32.71 degrees, P = 0.046), and an increase in trunk contralateral rotation (17.47 degrees to 25.05 degrees, P = 0.027).Maturing male athletes adopted a more erect cutting strategy that is associated with greater knee joint loading. Knee kinematic changes that increase knee joint loading were not observed in this cohort.

Background The incidence of second anterior cruciate ligament (ACL) injury after ACL reconstruction (ACLR) is high in young, active populations. Failure to successfully meet return-to-sport (RTS) criteria may identify adult athletes at risk of future injury; however, these studies have yet to assess skeletally mature adolescent athletes. Objective To determine if failure to meet RTS criteria would identify adolescent and young adult athletes at risk for future ACL injury after ACLR and RTS. The tested hypothesis was that the risk of a second ACL injury after RTS would be lower in participants who met all RTS criteria compared with those who failed to meet all criteria before RTS. Design Prospective case-cohort (prognosis) study. Setting Laboratory. Patients or Other Participants A total of 159 individuals (age = 17.2 ± 2.6 years, males = 47, females = 112). Main Outcome Measure(s) Participants completed an RTS assessment (quadriceps strength, functional hop tests) and the International Knee Documentation Committee patient survey (0 to 100 scale) after ACLR and were then tracked for occurrence of a second ACL tear. Athletes were classified into groups that passed all 6 RTS tests at a criterion level of 90% (or 90 of 100) limb symmetry and were compared with those who failed to meet all criteria. Crude odds ratios and 95% CIs were calculated to determine if passing all 6 RTS measures resulted in a reduced risk of second ACL injury in the first 24 months after RTS. Results Thirty-five (22%) of the participants sustained a second ACL injury. At the time of RTS, 26% achieved ≥90 on all tests, and the remaining athletes scored less than 90 on at least 1 of the 6 assessments. The second ACL injury incidence did not differ between those who passed all RTS criteria (28.6%) and those who failed at least 1 criterion (19.7%, P = .23). Subgroup analysis by graft type also indicated no differences between groups (P > .05). Conclusions Current RTS criteria at a 90% threshold did not identify active skeletally mature adolescent and young adult athletes at high risk for second ACL injury.

Background Investigators have proposed that various physical head and neck characteristics, such as neck strength and head and neck size, are associated with protection from mild traumatic brain injury (mTBI/concussion). Objectives To systematically review the literature and investigate potential relationships between physical head and neck characteristics and mTBI risk in athletic and military populations. Methods A comprehensive search of seven databases was conducted: MEDLINE, EMBASE, CINAHL, Scopus, SPORTDiscus, Cochrane Library, and Web of Science. Potential studies were systematically screened and reviewed. Studies on military and athletic cohorts were included if they assessed the relationship between physical head-neck characteristics and mTBI risk or proxy risk measures such as head impact kinematics. Results The systematic search yielded a total of 11,723 original records. From these, 22 studies met our inclusion criteria (10 longitudinal, 12 cross-sectional). Relevant to our PECO (Population, Exposure, Comparator, and Outcomes) question, exposures included mTBI incidence and head impact kinematics (acceleration, velocity, displacement) for impacts during sport play and training and in controlled laboratory conditions. Outcome characteristics included head and neck size (circumference, mass, length, ratios between these measures), neck strength and endurance, and rate of force development of neck muscles. Discussion We found mixed evidence for head and neck characteristics acting as risk factors for and protective factors against mTBI and increased susceptibility to head impacts. Head-neck strength and size variables were at times associated with protection against mTBI incidence and reduced impact kinematics (14/22 studies found one or more head-neck variable to be associated with protection); however, some studies did not find these relationships (8/22 studies found no significant associations or relationships). Interestingly, two studies found stronger and larger athletes were more at risk of sustaining high impacts during sport. Strength and size metrics may have some predictive power, but impact mitigation seems to be influenced by many other variables, such as behaviour, sex, and impact anticipation. A meta-analysis could not be performed due to heterogeneity in study design and reporting. Conclusion There is mixed evidence in the literature for the protective capacity of head and neck characteristics. We suggest field-based mTBI research in the future should include more dynamic anthropometric metrics, such as neck stiffness and response to perturbation. In addition, laboratory-based mTBI studies should aim to standardise design and reporting to help further uncover these complicated relationships.

Existing return to play (RTP) assessments have not demonstrated the ability to decrease risk of subsequent anterior cruciate ligament (ACL) injury after reconstruction (ACLR). RTP criteria are standardized and do not simulate the physical and cognitive activity required by the practice of sport. Most RTP criteria do not include an ecological approach. There are scientific algorithms as the “5 factor maximum model” that can identify risk profiles and help reduce the risk of a second anterior cruciate ligament injury. Nevertheless, these algorithms remain too standardized and do not include the situations experienced in games by soccer players. This is why it is important to integrate ecological situations specific to the environment of soccer players in order to evaluate players under conditions closest to their sporting activity, especially with high cognitive load. One should identify high risk players under two conditions: Clinical analyses commonly include assessments such as isokinetic testing, functional tests (hop tests, vertical force-velocity, profile), running, clinical assessments (range of motion and graft laxity), proprioception and balance (Star Excursion Balance Test modified, Y-Balance, stabilometry) and psychological parameters (kinesophobia, quality of life and fear of re-injury). Field testing usually includes game simulation, evaluation under dual-task conditions, fatigue and workload analysis, deceleration, timed-agility-test and horizontal force-velocity profiles. Although it seems important to evaluate strength, psychological variables and aerobic and anaerobic capacities, evaluation of neuromotor control in standard and ecological situations may be helpful for reducing the risk of injury after ACLR. This proposal for RTP testing after ACLR is supported by the scientific literature and attempts to approximate the physical and cognitive loads during a soccer match. Future scientific investigation will be required to demonstrate the validity of this approach. Level of Evidence 5

The physiological and psychological changes after anterior cruciate ligament reconstruction (ACLR) do not always allow a return to sport in the best condition and at the same level as before. Moreover, the number of significant re-injuries, especially in young athletes should be considered and physical therapists must develop rehabilitation strategies and increasingly specific and ecological test batteries to optimize safe return to play. The return to sport and return to play of athletes after ACLR must progress through the recovery of strength, neuromotor control, and include cardiovascular training while considering different psychological aspects. Because motor control seems to be the key to a safe return to sport, it should be associated with the progressive development of strength, and cognitive abilities should also be considered throughout rehabilitation. Periodization, the planned manipulation of training variables (load, sets, and repetitions) to maximize training adaptations while minimizing fatigue and injury, is relevant to the optimization of muscle strengthening, athletic qualities, and neurocognitive qualities of athletes during rehabilitation after ACLR. Periodized programming utilizes the principle of overload, whereby the neuromuscular system is required to adapt to unaccustomed loads. While progressive loading is a well-established and widely used concept for strengthening, the variance of volume and intensity makes periodization effective for improving athletic skills and attributes, such as muscular strength, endurance, and power, when compared with non-periodized training. The purpose of this clinical commentary is to broadly apply concepts of periodization to rehabilitation after ACLR.

Background: Despite knee extensor and flexor strength reportedly being associated with injury risk, including rupture of the anterior cruciate ligament in girls, there is limited evidence for the longitudinal changes in lower extremity strength. Purpose: To investigate the sex-specific relationship with longitudinal changes of knee extensor and flexor strength associated with maturation. Study design: Cohort study; Level of evidence, 2. Methods: Adolescent boys and girls (N = 257; 208 adolsecent girls) participating in high school basketball, volleyball, and soccer were assessed longitudinally in at least 2 different pubertal stages. Pubertal status (prepubertal, pubertal, and postpubertal) was determined with the modified Pubertal Maturation Observation Scale questionnaire. After a warm-up of 5 submaximal repetitions, participants were tested for concentric peak isokinetic strength for knee extension and flexion at 300 deg/s over 10 repetitions and normalized to body weight. Linear mixed models were used to test for the effect of pubertal stage, sex, and their interaction. Results: Significant interactions were identified that indicated different maturational trajectories for knee muscle strength for adolescent boys and girls, particularly between prepubertal and pubertal stages, in which boys demonstrated greater mass normalized knee extensor increases than girls (right, +12% vs +5%; left, +13% vs +7%; P < .001). For knee flexors, boys demonstrated increased strength, while girls demonstrated decreased relative knee flexor strength (right, +4% vs −1%, P = .03; left, +3 vs −3%, P = .009). Conclusion: The findings of this study support a differential effect of sex and maturation on important knee strength outcomes that may have implications for knee injury reduction, particularly in adolescent girls as they mature.

The stretch-shortening cycle (SSC) assists in effective force attenuation upon landing and augments force generation at take-off during a drop vertical jump (DVJ). General performance outcomes such as jump height or peak measures have been used to assess SSC function in youth populations; however, these discrete metrics fail to provide insight into temporal jump-landing characteristics. This study assessed DVJ force-time profiles in 1013 middle and high-school female athletes (n = 279 prepubertal, n = 401 pubertal, and n = 333 postpubertal). Maturity status was determined using the Pubertal Maturation Observation Scale. Ground reaction force data were analyzed to extract a range of variables to characterize force-time profiles. SSC function was categorized as poor, moderate, or good dependent on the presence of an impact peak and spring-like behavior. No differences in jump height or ground contact time were observed between maturity groups (p 0.05). Significant differences in absolute peak landing and take-off force were evident between all maturational statuses (p 0.05). Relative to bodyweight normalized forces, only peak take-off force was significantly different between prepubertal and postpubertal groups (p 0.05; d = 0.22). Spring-like behavior showed small improvements from pubertal to postpubertal (p 0.05; d = 0.25). Most females displayed poor SSC function at prepubertal (79.6%), pubertal (77.3%), and postpubertal (65.5%) stages of maturity. Large increases in absolute forces occur throughout maturation in female athletes; however, only small maturational differences were found in relative force or spring-like behavior. Consequently, most girls display poor SSC function irrespective of maturity.

Introduction Second anterior cruciate ligament (SACL) injuries are a devastating complication following return to sport (RTS). Appropriate and safe RTS criteria that reduce the risk of SACL injuries are needed. The goal of this study was to investigate the relationship between functional and isokinetic testing at 6 months following primary ACL reconstruction (ACLR) as risk factors for SACL injuries. Methods Patients with primary ACLR from 1990 to 2010 were identified. Those with 6-month postoperative functional and isokinetic testing and a minimum of 2-year follow-up were included. Functional testing included vertical jump, single-leg hop, and single-leg triple hop. Isokinetic testing included concentric quadriceps and hamstrings (HSs) strength at 60 and 180 degree/s speeds. Statistical analysis evaluated the significance of the various tests between those with and without a secondary tear. Results In total, 344 patients with a mean age of 26.0 ± 9.8 years at an average time of 9.1 ± 3.5 years of follow-up were analyzed. Fifty-nine patients (17%) experienced SACL injuries at an average time of 4.8 years following ACLR with 34 (58%) ipsilateral graft ruptures and 25 (43%) contralateral tears. Several isokinetic measures were significantly different between the NO SACL and the SACL groups: quadriceps 60 degree/s limb symmetry index (LSI) (75 vs. 82% p = 0.01), HS 60 degree/s LSI (92 vs. 97%, p = 0.04), quadriceps 180 degree/s involved/body weight (BW) (41 vs. 47%, p = 0.04), and HS 180 degree/s involved/BW (30 vs. 34%, p = 0.04). Patients with involved limb peak quadriceps torque value greater than 65 or 50% of BW (60 and 180 degree/s) had a 2.2 and 3.1 times higher risk of an SACL injury. Conclusions Patients with certain elevated isokinetic scores in the injured limb at 6 months after ACLR experienced a higher rate of subsequent ACL injuries than those who had lower peak torque. Although a quantitative only analysis may not be sufficient to determine RTS criteria, clinicians should caution high-performing patients about the risk of subsequent ACL injury. The purpose of this study was to determine the ability to utilize clinical measures of hop performance and thigh muscle strength to predict total limb work, a measure of cumulative single leg power, in young athletes at the time of return to sport after ACLR. We hypothesized that hop test performance and thigh muscle strength would predict total limb work during a maximal effort repeated vertical single-leg jump and that hop test performance would better predict total limb work during a maximal effort repeated vertical single-leg jump than measures of thigh strength.Cross-sectional study.Analysis on data from 50 individuals after unilateral ACLR (74% women, age 14-23 years) MAIN OUTCOME MEASURES: Performed 10-s repeated vertical single-leg jump test on a force platform where single limb power and resultant total work were calculated. At the same session, participants completed a clinical single leg hop test battery and thigh strength testing.All clinical measures were associated with total work during the repeated vertical single-leg jump test on the involved and uninvolved limbs, respectfully. After controlling for height and weight, quadriceps femoris peak torque at 180°/s was the strongest predictor of total work for both limbs. On the involved limb, performance on the triple hop test for distance also uniquely contributed to the prediction of total work.Quadriceps femoris muscle strength and distance hop test performance predict total limb work capacity of the involved limb during a single-leg dynamic task. Optimizing both measures after ACLR may positively impact vital components of sports performance.

The purpose of this study was to analyze the biomechanical outcomes of patients with Anterior Cruciate Ligament (ACL) deficiency or insufficiency and ACL reconstruction and to determine if they follow a biomechanical "Rule of Thirds."The Cincinnati Group reported nearly four decades ago that approximately one-third of patients do not experience a decline in biomechanical function in the absence of an ACL, one-third adapt their biomechanics to avoid knee symptoms, and one-third of patients do not adapt biomechanically to the loss of their ACL in order to function during activities of daily living without pain, swelling and giving way episodes. Subsequently, three decades ago the San Diego Group developed the Surgical Risk Factor (SURF) algorithm, which was designed to prospectively classify the biomechanics of patients who are ACL deficient. These classification systems have also delineated patient function into three categories. Currently, especially over the last decade, a growing body of work has documented that the incidence of second ACL injuries is consistent with the division of patient function by thirds. Approximately one-third of young, active individuals who return to high intensity sports sustain a second injury to either the ipsi- or contralateral knee.In this Biomechanics focused article in the Journal of Orthopedics, the authors describe differential patient outcomes with a Rule of Thirds concept, including the original study performed by our former group in Cincinnati and the SURF algorithm out of San Diego, the authors also present second ACL injury rates and how they are consistent with the Rule of Thirds, as well as the biomechanical implications for patient care.

Objective To conduct a systematic review of reviews to summarize the (1) risk for development and (2) prevalence of knee osteoarthritis (OA) after anterior cruciate ligament (ACL) injury and surgical treatment and (3) compare prevalence rates between surgical and nonsurgical treatment of ACL injury. Date sources Five electronic databases were searched using medical subject heading and text words up to February 2020 to identify English language reviews. Study selection Studies were included if they were a systematic review or meta-analysis. Data extraction Thirteen eligible reviews were included, and the main outcome extracted was knee OA prevalence or risk data and any meta-analysis results. Data synthesis Results from reviews were combined with a summary meta-analysis based on odds ratios (ORs) or proportions. There was a near 7-fold and 8-fold increase in the odds for the development of knee OA post ACL injury [OR = 6.81 (5.70-8.13)] and ACL reconstruction [OR = 7.7, (6.05-9.79)]. Data were too heterogenous to specify a point estimate prevalence for OA after ACL injury, but OA prevalence was estimated at 36% (19.70-53.01) at near 10 years after reconstruction surgery. A significantly higher prevalence of OA was found for those who received surgical treatment at a minimum 10-year follow-up [OR = 1.40 (1.17-1.68)]. Conclusions This study combines all data from previous systematic reviews into a single source to show that ACL injury markedly increases the risk for development of knee OA, which is likely to be present in the long term in approximately a third of patients who have reconstruction surgery. Surgical treatment does not reduce OA prevalence in the longer term compared with nonsurgical treatment.

Background: While medial meniscocapsular tears (ramp lesions) are commonly associated with isolated anterior cruciate ligament injuries, there are limited descriptions of these meniscal injuries in multiligament knee injuries (MLKIs). Purpose: To (1) retrospectively evaluate preoperative magnetic resonance imaging (MRI) scans for the presence of ramp lesions in patients surgically treated for acute grade 3 combined posterolateral corner (PLC) knee injuries and (2) determine if a preoperative posteromedial tibial plateau (PMTP) bone bruise is associated with the presence of preoperative ramp lesions on MRI in these same patients. Study Design: Cross-sectional study; Level of evidence, 3. Methods: Data on consecutive patients at a level 1 trauma center with MLKIs between 2001 and 2021 were retrospectively reviewed. Only patients with acute grade 3 combined PLC injuries who received an MRI scan within 30 days of injury were assessed. Two musculoskeletal radiologists retrospectively reviewed each patient’s preoperative MRI for evidence of ramp lesions and bone bruises. Intraclass correlation coefficients (ICCs) were used to calculate reliability among the reviewers. Multivariate analysis was used to evaluate the relationship between PMTP bruising and the presence of a ramp lesion on MRI. Results: A total of 68 patients (79.4% male; mean age, 33.8 ± 13.7 years) with an acute grade 3 combined PLC injury were included in the study. On MRI, the ICCs for detection of ramp lesions and PMTP bone bruising were 0.921 and 0.938, respectively. Medial meniscal ramp lesions were diagnosed in 18 of 68 (26.5%) patients. Eleven of 18 (61.1%) patients with ramp lesions also showed evidence of PMTP bruising, while 13 of 50 (26.0%) patients without ramp lesions had PMTP bruising ( P = .008). When controlling for age and sex, PTMP bruising was significantly associated with the presence of a ramp lesion in combined PLC injuries (odds ratio, 4.62; P = .012). Conclusion: Preoperative medial meniscal ramp lesions were diagnosed on MRI in 26.5% of patients with acute grade 3 combined PLC injuries. PMTP bone bruising was significantly associated with the presence of a ramp lesion on MRI. These findings reinforce the need to assess for potential ramp lesions at the time of multiligament reconstruction.

BACKGROUND: Quadriceps weakness is a predictor of long-term knee function and strength recovery can vary from months to years after anterior cruciate ligament reconstruction (ACLR). However, few studies evaluate quadriceps strength and self-reported function within the first several weeks after ACLR. HYPOTHESIS/PURPOSE: To examine changes over time in quadriceps strength symmetry, quadriceps peak torque, and self-reported knee function prior to and at six, 12, and 24 weeks post-ACLR. The hypotheses were 1) quadriceps strength symmetry, bilateral quadriceps peak torque, and patient-reported function would improve over time from pre-ACLR to 24 weeks post-ACLR and 2) significant improvements in patient-reported function, but not strength symmetry, would occur between time points. STUDY DESIGN: Prospective, cohort study METHODS: Thirty participants completed four testing sessions: pre-surgery and six, 12, and 24 weeks post-ACLR. Isometric quadriceps strength testing was performed at six weeks and isokinetic quadriceps strength was measured at all other testing points. Quadriceps index was calculated to evaluate between limb quadriceps strength symmetry. The Knee injury and Osteoarthritis Outcome Score (KOOS) and International Knee Documentation Committee Subjective Knee Evaluation Form (IKDC) were administered at each time point. A repeated-measures analysis of variance evaluated changes over time, with post-hoc comparisons to determine at which time-point significant changes occurred. Background: Tibiofemoral bone bruise patterns seen on magnetic resonance imaging (MRI) are associated with ligamentous injuries in the acutely injured knee. Bone bruise patterns in multiligament knee injuries (MLKIs) and particularly their association with common peroneal nerve (CPN) injuries are not well described. Purpose: To analyze the tibiofemoral bone bruise patterns in MLKIs with and without peroneal nerve injury. Study Design: Case series; Level of evidence, 4. Methods: We retrospectively identified 123 patients treated for an acute MLKI at a level 1 trauma center between January 2001 and March 2021. Patients were grouped into injury subtypes using the Schenck classification. Within this cohort, patients with clinically documented complete (motor and sensory loss) and/or partial CPN palsies on physical examination were identified. Imaging criteria required an MRI scan on a 1.5 or 3 Tesla scanner within 30 days of the initial MLKI. Images were retrospectively interpreted for bone bruising patterns by 2 board-certified musculoskeletal radiologists. The location of the bone bruises was mapped on fat-suppressed T2-weighted coronal and sagittal images. Bruise patterns were compared among patients with and without CPN injury. Results: Of the 108 patients with a MLKI who met the a priori inclusion criteria, 26 (24.1%) were found to have a CPN injury (N = 20 complete; N = 6 partial) on physical examination. For CPN-injured patients, the most common mechanism of injury was high-energy trauma (N = 19 [73%]). The presence of a grade 3 posterolateral corner (PLC) injury (N = 25; odds ratio [OR], 23.81 [95% CI, 3.08-184.1]; P = .0024), anteromedial femoral condyle bone bruising (N = 24; OR, 21.9 [95% CI, 3.40-202.9]; P < .001), or a documented knee dislocation (N = 16; OR, 3.45 [95% CI, 1.38-8.62]; P = .007) was significantly associated with the presence of a CPN injury. Of the 26 patients with CPN injury, 24 (92.3%) had at least 1 anteromedial femoral condyle bone bruise. All 20 (100%) patients with complete CPN injury also had at least 1 anteromedial femoral condyle bone bruise on MRI. In our MLKI cohort, the presence of anteromedial femoral condyle bone bruising had a sensitivity of 92.3% and a specificity of 64.6% for the presence of CPN injury on physical examination. Conclusion: In our MLKI cohort, the presence of a grade 3 PLC injury had the greatest association with CPN injury. Additionally, anteromedial femoral condyle bone bruising on MRI was a highly sensitive finding that was significantly correlated with CPN injury on physical examination. The high prevalence of grade 3 PLC injuries and anteromedial tibiofemoral bone bruising suggests that these MLKIs with CPN injuries most commonly occurred from a hyperextension-varus mechanism caused by a high-energy blow to the anteromedial knee.

BACKGROUND: Non-contact injuries are common in sports as abnormal lower extremity joint mechanics can place athletes at risk for injury. It is important to have reliable, feasible, cost-effective assessment tools to determine lower limb control and injury risk. HYPOTHESIS/PURPOSE: The purpose of the study was to assess the intra- and inter-rater reliability of a three-tiered anterior cruciate ligament (ACL) injury risk rating assessment of the drop vertical jump using frontal plane, two-dimensional (2-D) motion capture. STUDY DESIGN: Repeated measures. METHODS: Twenty male elite basketball athletes performed the drop vertical jump during a 2-D video assessment at Mayo Clinic Sports Medicine Center in Minneapolis, Minnesota. DVJ scores indicated the following: 1 no visible knee valgus, 2 slight wobble, inward motion of the knees, and 3 knee collision or large frontal plane knee excursion. Score assessment from video of the drop vertical jump was obtained by four independent investigators. The four raters then re-examined the same videos 1 month later, blinded to their original scores. PURPOSE: The sensorimotor system is a subcomponent of the comprehensive motor control system of the body. However, the complex nature of the sensorimotor system, makes it difficult to interpret findings for clinical application. The purpose of this study was to utilize principal component analysis to identify sex differences and relationships between sensorimotor variables during a dynamic perturbation. MATERIALS AND METHODS: Thirty physically-active individuals (15 males and 15 females) were blindfolded and positioned on an isokinetic dynamometer with their knee flexed to 70°. At random, the dynamometer moved rapidly toward knee extension. Subjects were asked to resist the dynamometer as it would randomly and rapidly move toward knee extension. Torque and position values were used to calculate stiffness values. RESULTS: Principal component analysis revealed sex differences in two principal components (PCs): PC2 in female was comprised from higher position, torque and time values (P = 0.038), PC4 in females was comprised from higher active stiffness and lower short-range stiffness values (P = 0.032) compared to males. Torque at the resting position was correlated to the short-range passive stiffness (ρ = 0.539, P = 0.002), time to peak torque (ρ = −0.375, P = 0.003) and reactive stiffness (ρ = 0.526, P = 0.041). CONCLUSIONS: Females had later reaction time and lower short-range passive stiffness and they resisted the dynamometer by their voluntary activation compared to the males thus requiring muscle activation for meaningful response. In addition, the higher resting muscle activities may correlate to short-range passive stiffness and quicker active stiffness.

Background:The most commonly damaged structures of the knee are the anterior cruciate ligament (ACL), medial collateral ligament (MCL), and menisci. Given that these injuries present as either isolated or concomitant, it follows that these events are driven by specific mechanics versus coincidence. This study was designed to investigate the multiplanar mechanisms and determine the important biomechanical and demographic factors that contribute to classification of the injury outcome.Hypothesis:Linear discriminant analysis (LDA) would accurately classify each injury type generated by the mechanical impact simulator based on biomechanical input variables (ie, ligament strain and knee kinetics).Study Design:Controlled laboratory study.Methods:In vivo kinetics and kinematics of 42 healthy, athletic participants were measured to determine stratification of injury risk (ie, low, medium, and high) in 3 degrees of knee forces/moments (knee abduction moment, anterior tibial shear, and internal tibial rotation). These stratified kinetic values were input into a cadaveric impact simulator to assess ligamentous strain and knee kinetics during a simulated landing task. Uniaxial and multiaxial load cells and implanted strain sensors were used to collect mechanical data for analysis. LDA was used to determine the ability to classify injury outcome by demographic and biomechanical input variables.Results:From LDA, a 5-factor model (Entropy R2= 0.26) demonstrated an area under the receiver operating characteristic curve (AUC) for all 5 injury outcomes (ACL, MCL, ACL+MCL, ACL+MCL+meniscus, ACL+meniscus) of 0.74 or higher, with “good” prediction for 4 of 5 injury classifications. A 10-factor model (Entropy R2= 0.66) improved the AUC to 0.86 or higher, with “excellent” prediction for 5 injury classifications. The 15-factor model (Entropy R2= 0.85), produced 94.1% accuracy with the AUC 0.98 or higher for all 5 injury classifications.Conclusion:Use of LDA accurately predicted the outcome of knee injury from kinetic data from cadaveric simulations with the use of a mechanical impact simulator at 25° of knee flexion. Thus, with clinically relevant kinetics, it is possible to determine clinical risk of injury and also the likely presentation of singular or concomitant knee injury.Clinical Relevance:LDA demonstrates that injury outcomes are largely characterized by specific mechanics that can distinguish ACL, MCL, and medial meniscal injury. Furthermore, as the mechanics of injury are better understood, improved interventional prehabilitation can be designed to reduce these injuries.

A large variety of data filtration techniques exist in biomechanics literature. Data filtration is both an 'art' and a 'science' to eliminate noise and retain true signal to draw conclusions that will direct future hypotheses, experimentation, and technology development. Thus, data consilience is paramount, but is dependent on filtration methodologies. In this study, we utilized ligament strain, vertical ground reaction force, and kinetic data from cadaveric impact simulations to assess data from four different filters (12 vs. 50 Hz low-pass; forward vs. zero lag). We hypothesized that 50 Hz filtered data would demonstrate larger peak magnitudes, but exhibit consilience of waveforms and statistical significance as compared to 12 Hz filtered data. Results demonstrated high data consilience for matched pair t test correlations of peak ACL strain (≥ 0.97), MCL strain (≥ 0.93) and vertical ground reaction force (≥ 0.98). Kinetics had a larger range of correlation (0.06-0.96) that was dependent on both external load application and direction of motion monitored. Coefficients of multiple correlation demonstrated high data consilience for zero lag filtered data. With respect to in vitro mechanical data, selection of low-pass filter cutoff frequency will influence both the magnitudes of discrete and waveform data. Dependent on the data type (i.e., strain and ground reaction forces), this will not likely significantly alter conclusions of statistical significance previously reported in the literature with high consilience of matched pair t-test correlations and coefficients of multiple correlation demonstrated. However, rotational kinetics are more sensitive to filtration selection and could be suspect to errors, especially at lower magnitudes.

Background The incidence of anterior cruciate ligament (ACL) injuries has been estimated at 1 in 3500 individuals in the literature. Second ACL tears represent 7-14% of all ACL tears. The location of ACL tears has been noted to be primarily proximal. Purpose The purpose of this study was to corroborate previous data as well as to add novel data to the literature regarding the location of ACL tears, risk factors associated with second ACL tears, and correlation between hearing a "pop" at the time of injury and concomitant injuries. Study Design Epidemiologic Review. Methods A geographic database containing the medical records of individuals in a rural county in Midwest, USA was utilized to identify ACL injuries that occurred in the county from 2011 to 2016. A total of 743 ACL injuries were identified, which were reviewed and stratified by primary and second tears, sex, race, age, activity level, number of "pops" heard at time of injury, side of injury, location of tear, graft type used in reconstruction, location of fixation, and concomitant injuries. Findings ACL tear location was noted to be primarily midsubstance (44.1%) or proximal (34.1%). The majority of individuals (56.3%) who reported hearing "multiple pops" at the time of injury sustained multiple types of concomitant injuries. Background Anterior cruciate ligament (ACL) injuries are a common cause of time loss in sports. Approximately one-third of ACL reconstructed athletes who return to sport suffer secondary injury. The presence of fatigue during athletic performance has been hypothesized to increase susceptibility to ACL injury. However, the relative role of fatigue in secondary ACL failures remains unexplored. Purpose To assess how time elapsed within a game and within a season associate with secondary ACL injury occurrence in international professional athletes and American collegiate athletes. Study design Retrospective cohort analysis. Methods The public domain was searched for secondary ACL injuries that occurred during competitive matches between 2000-2018. Demographics (age, height, weight), side of injury, type of injury (contact, noncontact), and timing of injury within competition and within season were determined for each case. Results Sixty-seven secondary ACL injuries were identified. Within-game, there were no differences in the distribution of ACL injures across each quarter of game time (p = 0.284). This was consistent between sport (p = 0.120-0.448). Within-season, there were no differences in the distribution of secondary ACL injures across each quarter of the season (p = 0.491). This was again consistent between sport (p = 0.151-0.872). Relative risk was not found to be significantly greater for any combination of season and game. Conclusion The results of the current study indicate that the occurrences of secondary ACL injuries were equally distributed with respect to in-game and in-season timing. Both in-game and in-season timing were not significantly different across each individual sport examined. These results implicate that overall there is not an association between fatigue and secondary ACL injury occurrence in professional athletes. Level of evidence 3.

INTRODUCTION: Coordination of multiple degrees of freedom in the performance of dynamic and complex motor tasks presents a challenging neuromuscular control problem. Experiments have inferred that humans exhibit self-organized, preferred coordination patterns, which emerge due to actor and task constraints on performance. The purpose of this study was to determine if the set of effective coordination strategies that exist for a task centers on a small number of robust, invariant patterns of behavior. METHODS: Kinetic movement patterns computed from a cohort of 780 primarily female adolescent athletes performing a drop vertical jump task (DVJ) were analyzed in order to discover distinct groups into which individuals could be classified based on the similarity of movement coordination solutions. RESULTS: Clustering of reduced-dimension joint moment of force time series revealed three very distinct, precisely delineated movement profiles that persisted across trials, and which exhibited different functional performance outcomes, despite no other apparent group differences. The same analysis was also performed on a different task—a single-leg drop landing (SLD)—which also produced distinct movement profiles; however, the three DVJ profiles did not translate to this task as group assignment was inconsistent between these two tasks. CONCLUSION: The task demands of the DVJ and SLD—successful landing, reversal of downward momentum, and, in the case of the DVJ, vertical propulsion toward a maximally positioned target—constrain movement performance such that only a few successful outcomes emerge. Discovery of the observed strategies in the context of associated task constraints may help our understanding of how injury risk movement patterns emerge during specific tasks, as well as how the natural dynamics of the system may be exploited to improve these patterns.

OBJECTIVE: Faulty neuromuscular and biomechanical deficits of the knee are nearly ubiquitous in athletes following anterior cruciate ligament (ACL) reconstruction (ACLR). Knee biomechanical deficits are directly associated with an increased risk of second ACL injury, which typically occurs during a sports-related movement on a single-limb. To date, the biomechanical effects of a neuromuscular training (NMT) program on knee biomechanics during a single-leg landing task has not been investigated. Anterior cruciate ligament (ACL) injuries typically occur during sports participation, and reconstruction surgery is commonly performed to enable the athlete to return to high-impact sports that involve landing, cutting, and pivoting movements. However, despite restoration of knee stability following surgery, many athletes do not return to their prior level of sport and of those that do the occurrence of second ACL injuries are of concern, particularly in younger athletes. Criteria-based return to sport rehabilitation has been shown to be of benefit to prepare athletes for a safe return to play and is discussed in this chapter. Such programs are conducted in stages, and criteria for progression from one phase to the next, which include return to sport, are proposed that target modifiable deficits. These typically include ligament, quadriceps, limb and trunk dominance patterns. Another approach to assess readiness to return to sport is the use of return to sport testing where a set of criteria or ‘test battery’ is given to the athlete for clearance to return to sport. This chapter also discusses the current knowledge of such return to sport testing.

The AI-generated conglomerate variable MSK_Health was on average 47.8 ± 9.7 in 2017, 47.4 ± 10.1 in 2018, 47.5 ± 10.1 in 2019, and 45.0 ± 11.2 in 2020 post-Covid lockdown. ANOVA showed that 2020 measures of MSK_Health were significantly decreased relative to the 3 prior seasons. Logistic regression analysis demonstrated a significant effect of the MSK_Health variable on MSK injury risk.ConclusionsAcross multiple seasons of force plate CMJ measures in PAFP, MSK_Health decreased following Covid-19 lockdown, which may be associated with higher risk for MSK injury. This greater understanding of the changes in longitudinal CMJ force plate measures in PAFP across seasons and after extended layoffs may assist in the development of effective MSK injury reduction measures.

‘Data science’ represents a set of mathematical and software development related techniques that are applied across a wide range of problems and industries. Practitioners of data science in human health-related domains typically see a world that differs substantially from practitioners in other domains such as advertising, finance, e-commerce, manufacturing, or social networking. This commentary discusses what those differences are (Project vs Product Focus, Independent vs Integrated Efforts, Causality vs Prediction Driven, Statistical vs Machine Learning Centricity) why they exist, and the future convergence that we believe is on the horizon. The concepts discussed can provide a starting point in which health and human performance-focused stakeholders can begin to align well-established data science applications from other domains to further enable innovative health and performance solutions.

Background Eccentric hamstring strength and hamstring/quadriceps strength ratios have been identified as modifiable risk factors of hamstring strains. Additionally, those strength and flexibility characteristics are commonly used as clinical tests to monitor progress of athletes with acute or chronic hamstring strains. Although hamstring strains are common among basketball athletes, normative values of knee strength and flexibility characteristics are scarce. Normative values for these athletes would be important in prevention and management of hamstring strains. Purpose To establish quadriceps and hamstring isokinetic strength and flexibility values among high school basketball athletes and examine the effects of sex and age. Study Design Cross-sectional research Methods Isokinetic knee muscular strength (concentric quadriceps [QuadC], concentric hamstring [HamC], eccentric hamstring [HamE], and strength ratios ([HamC/QuadC and HamE/Quad]), flexibility of hip flexors and quadriceps during a Modified Thomas test, and flexibility of hip extensors and hamstring during passive straight leg raise (SLR) and passive knee extension (PKE) tests were measured. Effects of sex and age were analyzed using t-tests and analysis of variance, respectively with Bonferroni corrected post hoc tests (p≤0.01). Results A total of 172 high school basketball athletes (64 males/108 females; mean age (range): 15.7 (14-18) years old) participated in the study. Male athletes were significantly stronger than female athletes (QuadC: ppp=0.759-0.816; HamE/QuadC: p=0.022-0.061). Among male athletes, a significant effect of age on quadriceps and hamstring strength was observed: older male athletes were stronger than younger male athletes. Contrarily, there were no effects of age on strength among female athletes. There were significant sex differences in quadriceps flexibility, SLR, and PKE (female athletes were more flexible; p=0.001-0.005) while no sex differences were found in hip flexor flexibility (p=0.105-0.164). There were no effects of age for any flexibility variables within male and female athletes (p=0.151-0.984). Conclusion The current results provide normative values for hamstring strength and flexibility in high school basketball athletes. These normative values may further assist sports medicine specialists to develop screening tests, interventions, and return-to-sport criteria in this population. Level of Evidence 3B

The Achilles tendon is the strongest tendon in the human body, but the basis of its high tensile strength has not been elucidated in detail. Here we have loaded healthy, human, Achilles tendons to failure in an anatomically authentic fashion while studying the local three-dimensional deformation and strains in real time, with very high precision, using digital image correlation (DIC). These studies identified a remarkable degree of anisotropic, medio-lateral auxetic behavior, with Poisson’s ratios not exceeding minus 1 in any part of the tendon at any time; under certain loads, discrete areas within the tendon had a Poisson’s ratio below minus 6. Early in the loading cycle, the proximal region of the tendon accumulated high lateral strains while longitudinal strains remained low. This behavior shielded the mid-substance of the tendon, its weakest part, from high longitudinal strains until immediately before rupture. These new insights are of great relevance to understanding the material basis of tendon injuries, designing improved prosthetic replacements, and developing regenerative strategies.

Weaker hamstrings muscular forces and lower ratio of the hamstrings/quadriceps muscular forces in female athletes have been identified as modifiable risk factors for anterior cruciate ligament (ACL) injuries. However, sex differences in athletes' ability to react to visual cues (Choice Visual-Motor Reaction Time: VMRT) and to generate knee muscular forces (rate of force development: RFD) immediately following the visual cues were largely unknown. Therefore, the purpose of the study was to examine sex differences in Choice VMRT and RFD. A total of 50 high school basketball athletes (26F/24 M) participated in the study. Subjects sat in the knee dynamometer chair with their knee secured at 70° of knee flexion and performed knee extension or flexion maximum voluntary isometric contractions immediately after they saw the visual cue: "UP" or "DOWN" arrows, respectively. Choice VMRT was defined as the time between the visual cue and the initiation of muscular force development (>5Newtons). RFD was calculated by dividing the changes in forces over the changes in time at four time points (0-50/100/150/200 ms). Peak muscular forces and RFD were normalized to their body mass. Average of three trials in each direction (flexion and extension) in each leg was used for statistical analyses. Females had significantly slower Choice VMRT (p < 0.001-0.027) and lower knee extension RFD at 100 ms (p = 0.005). In addition, females had significantly higher knee flexion/extension ratio than males in late RFD (150 ms and 200 ms) (p < 0.004). The current study has provided additional sensorimotor characteristics of athletes and sexes in addition to their knee muscular characteristics.

Background Athletes have traditionally been subdivided into risk classifications for ACL injury relative to the biomechanical traits they display during landing. This investigation aimed to discern whether these separate risk classifications elicit strain differences on the ACL and MCL during landing. It was hypothesized that the higher risk simulation profiles would exhibit greater ACL strain and that the ACL would exhibit greater strain than the MCL under all conditions. Method The mechanical impact simulator was used to simulate landing on a cohort of 46 cadaveric specimens. The simulator applied external joint loads to the knee prior to impulse delivery. These loads were organized into a series of profiles derived from in vivo motion capture previously performed on a cohort of 44 athletes and represented various risk classifications. Strain gauges were implanted on the ACL and MCL and simulations performed until a structural failure was elicited. Differences were assessed with Kruskal-Wallis tests. Findings The highest-risk profiles tended to exhibit greater peak ACL strain and change in ACL strain than the baseline- and moderate-risk profiles. Specimens that failed during lower-risk simulations expressed greater strain at these loads than specimens that completed higher-risk simulations. The ACL recorded greater strain than the MCL throughout all simulation profiles. Interpretation This behavior justifies why neuromuscular interventions have greater impact on higher-risk athletes and supports the continued screening and targeted training of those athletes that express greater injury risk. The loading disparity between ACL and MCL justifies their limited concomitant injury rate.

PURPOSE: The appearance of anterior cruciate ligament (ACL) grafts on magnetic resonance imaging (MRI) is related to graft maturity and mechanical strength after ACL reconstruction (ACLR). Accordingly, the purpose of this review was to quantitatively analyze reports of serial MRI of the ACL graft during the first year following ACLR; the hypothesis tested was that normalized MRI signal intensity would differ significantly by ACL graft type, graft source, and postoperative time. METHODS: PubMed, Scopus, and CINAHL were searched for all studies published prior to June 2018 reporting MRI signal intensity of the ACL graft at multiple time points during the first postoperative year after ACLR. Signal intensity values at 6 and 12 months post-ACLR were normalized to initial measurements and analyzed using a least-squares regression model to study the independent variables of postoperative time, graft type, and graft source on the normalized MRI signal intensity. RESULTS: An effect of graft type (P = 0.001) with interactions of graft type * time (P = 0.012) and graft source * time (P = 0.001) were observed. Post hoc analyses revealed greater predicted normalized MRI signal intensity of patellar tendon autografts than both hamstring (P = 0.008) and hamstring with remnant preservation (P = 0.001) autografts at postoperative month 12. CONCLUSION: MRI signal varies with graft type, graft source, and time after ACLR. Enhanced graft maturity during the first postoperative year was associated with hamstring autografts, with and without remnant preservation. Serial MRI imaging during the first postoperative year may be clinically useful to identify biologically or mechanically deficient ACL grafts at risk for failure.

Background: Many studies have investigated factors that predict a return to sport participation after anterior cruciate ligament (ACL) reconstruction, but it is unclear whether the same factors are associated with a return to preinjury level of sport performance. Purpose: To identify factors that contribute to an athlete’s return to preinjury level of performance after ACL reconstruction. Study Design: Cohort study; Level of evidence, 2. Methods: A cohort of 222 patients (132 male, 90 female) who had ACL reconstruction surgery and completed a 12-month postoperative assessment were followed out to a mean 3 years (range, 2-4 years) to determine whether they had returned to their self-reported preinjury levels of sport performance. Rates of return to preinjury performance were calculated. Measures that had been recorded at the 12-month postoperative review—demographic (age, sex), sport activity level (Marx Activity Rating Scale, return to competition sport), knee laxity, limb symmetry (single and triple crossover hop), subjective function/symptoms (International Knee Documentation Committee subjective), and psychological readiness (Anterior Cruciate Ligament Return to Sport After Injury scale)—were compared between patients who returned to their preinjury levels of performance and those who did not. Univariate and multivariate logistic regression models were also used to prospectively determine the association between these measures and return to preinjury level of sport performance. Results: A total of 135 (61%) patients reported that they had returned to their preinjury levels of performance, with return rates similar between males (59%) and females (63%) and between those who had returned to competition at 12 months (62%) and those who had not (60%). There was no significant age difference between patients who returned to their preinjury levels of sport performance and those who did not; however, higher psychological readiness ( P < .0001), greater limb symmetry ( P < .05), higher subjective knee scores ( P = .01), and a higher activity level ( P < .04) were all associated with a return to performance. In the multivariate model, psychological readiness was the only variable that remained a significant predictor (odds ratio = 1.03; 95% CI, 1.01-1.04; P < .0001). Conclusion: A majority of athletes who returned to sport after ACL reconstruction reported that their performance was comparable with preinjury. Having a greater psychological readiness to return during rehabilitation was the most significant predictor of a subsequent return to comparable performance.

Context: Six months is the expected time frame to return to sport (RTS) after hip arthroscopy for femoroacetabular impingement syndrome (FAIS). Objective: The primary aims of this study were to (1) report the RTS rate of physically active individuals 6 months after arthroscopic surgery for FAIS and (2) compare the self-reported hip function between those who were able to RTS against those who had not. Design: Cohort study. Setting: The Ohio State University Wexner Medical Center. Patients: A total of 42 physically active individuals scheduled for hip arthroscopy for FAIS. Main Outcome Measures: Self-reported function, including preinjury and current activity levels and ability to participate in sport, were collected on 42 individuals with FAIS prior to surgery and at 6-month follow-up. Participants were allocated into 2 groups based on self-reported RTS status at 6-month follow-up. Separate 2-way analyses of variance were used to test the interaction of groups (those who did/did not RTS). Main effects were reported in the absence of statistically significant interactions (P ≤ .05). Results: Altogether, 28 of 42 participants (66%) returned for 6-month follow-up; 5 of the 14 participants (35.7%) lost to follow-up underwent contralateral hip surgery within the study time frame. At the 6-month follow-up, 16 of the remaining 28 participants (57.1%) reported that they had returned to sport, 5 of whom returned at their prior level of participation. There was no statistically significant interaction of group and time for either hip outcome score subscale (P ≥ .20). Self-reported hip function improved over time, regardless of group (P ≤ .001). Participants in the yRTS group demonstrated higher hip outcome score-sport scores than did the nRTS group, regardless of time (P = .04). Conclusions: Though just over half of participants returned to sport 6 months after hip arthroscopy for FAIS, only 18% returned to their previous level of sports participation. Participants who returned to sport reported better function than those who did not, but self-reported hip function improved over time regardless of group. The most commonly reported reasons for not returning to sport were weakness (69.6%), fear (65.2%), and pain (56.5%).

BACKGROUND: The anterior cruciate ligament is the primary structural restraint to tibial anterior shear force. The anterior force occurring at the knee during landing contributes to anterior cruciate ligament injury risk, but it cannot be directly measured experimentally. PURPOSE: The objective of this study was to develop electromyography-informed musculoskeletal simulations of the drop vertical jump motor task and assess the contribution of knee muscle forces to tibial anterior shear force. STUDY DESIGN: cross-sectional study. METHODS: Musculoskeletal simulations were used to estimate the muscle forces of thirteen female athletes performing a drop vertical jump using an electromyography-informed method. Muscle activation and knee loads that resulted from these simulations were compared to the results obtained with the more common approach of minimization of muscle effort (optimization-based method). Quadriceps-hamstrings and quadriceps-gastrocnemius co-contractions were progressively increased and their contribution to anterior shear force was quantified. RESULTS: The electromyography-informed method produced co-contraction indexes more consistent with electromyography data than the optimization-based method. The muscles that presented the largest contribution to peak anterior shear force were the gastrocnemii likely from their wrapping around the posterior aspect of the tibia. The quadriceps-hamstring co-contraction provided a protective effect on the ACL and reduced peak anterior shear force by 292 N with a co-contraction index increase of 25% from baseline (31%), whereas a quadriceps-gastrocnemius co-contraction index of 61% increased peak anterior shear force by 797 N compared to baseline (42%). CONCLUSION: An increase in gastrocnemius contraction, which might be required to protect the ankle from the impact with the ground, produced a large quadriceps-gastrocnemius co-activation, increasing peak anterior shear force. CLINICAL RELEVANCE: A better understanding of each muscle’s contribution to anterior shear force and, consequently, anterior cruciate ligament tension may inform subject-specific injury prevention programs and rehabilitation protocols.

Background: Both the anterior cruciate ligament (ACL) and the medial collateral ligament (MCL) bear load during athletic tasks of landing, cutting, pivoting, and twisting. As dynamic knee valgus is a purported mechanism for ACL injury, the MCL should bear significant strain load with valgus force. Hypothesis: The intact MCL will demonstrate a significant increase in strain upon failure of the ACL at 25° of knee flexion. Study Design: Controlled laboratory study. Methods: In vivo kinetics/kinematics of 44 healthy athletic participants were measured to determine stratification of injury risk (ie, low, medium, and high) in 3 degrees of knee forces/moments (knee abduction moment, anterior tibial shear, and internal tibial rotation). These stratified kinetic values were input into a cadaveric impact simulator to assess ligamentous strain during a simulated landing task. Uniaxial and multiaxial load cells and differential variable reluctance transducer strain sensors were utilized to collect mechanical data for analysis. Conditions of external loads applied to the cadaveric limbs were varied and randomized. Results: ACL strain increased with increased dynamic knee abduction moment (χ2[5] = 14.123, P = .0148). The most extreme dynamic knee abduction moment condition demonstrated significantly higher ACL strain compared with lower loaded trials ( P≤ .0203). Similarly, MCL strain increased with dynamic knee abduction moment (χ2[5] = 36.578, P < .0001). Matched-pairs analysis compared ACL strain with MCL strain (maximum ACL strain – maximum MCL strain) and demonstrated high strain for the ACL versus the MCL ( S177 = 6223.5, P < .0001). Conclusion: Although significant, MCL strain had minimal increase with increased dynamic knee abduction moment, and the event of ACL failure did not significantly increase MCL strain when compared with high dynamic knee abduction moment conditions in the cadaveric model. The ACL bears more strain than the MCL at increasing amounts of dynamic knee abduction moment at 25° of knee flexion, which may explain the limited concomitant MCL injury rate that can occur during a dynamic valgus collapse of the knee. Clinical Relevance: These characteristics of ACL and MCL strain are important to understand the mechanisms that drive these injuries at the knee and will improve rehabilitation and injury prevention techniques.
