Abstract

A prospective study was conducted on the incidence of intrauterine pathology after abortions diagnosed by post-abortion hysteroscopy. In 80 patients outpatient hysteroscopy was performed 8-12 weeks after a dilatation and curettage for incomplete or missed abortions. Intrauterine pathological changes were found in 40 patients. There were 10 cases of uterus subseptus/ bicornis and 7 of uterus arcuatus, 2 submucous myoma and 1 corpus polyp. Intrauterine adhesions were diagnosed in 20 patients. The incidence of intrauterine adhesions was about the same after incomplete abortions and after missed abortions, but in patients with recurrent abortions the incidence was significantly higher than in patients after the first abortion (27.8% versus 14.9%). Post-abortion hysteroscopy is a simple and useful method for early diagnosis of congenital and acquired intrauterine pathology after abortions.

Abstract

The article presents the case study of a young patient suffering from desquamative gingivitis. Early examination could not any indication of the disease, which later spread to entire mandibular gingivae. Gynecological examination was conducted which revealed ovarian insufficiency. After failure of several therapies cycle-adjusted hormone therapy was used results of which have not yet appeared.

Abstract

We performed gasless lymph node dissection in a woman with cervical carcinoma FIGO stage Ib. The dissection of the parametrium was done vaginally by hysterectomy including the Schuchardt incision and the removal of 2 cm of vagina. We removed 45 lymph nodes. For the laparoscopic procedures we used a Laparofan and Laparolift (Origin Medsystems). The estimated blood loss was 250 ml and operative time 330 minutes. An advantage of the gasless method is that one can use endoscopic and conventional instruments. We combined advantages of laparoscopy (good visibility, enlargement of blood and lymph vessels) and of the gasless method (use of the same instruments as in laparotomy, quick change of endoscopic and conventional instruments without gas loss). This technique achieves the same surgical results as radical laparotomy in selected women with cervical carcinoma.

34. New and Conventional Evaporative Systems in Concentrating Nitrogen Samples Prior to Isotope‐ratio Analysis

Abstract

Studies were conducted to quantify and compare the efficiencies of various evaporative systems used in evaporating 15N samples prior to mass spectrometric analysis. Two new forced‐air systems were designed and compared with a conventional forced‐air system and with an open‐air drybath technique for effectiveness in preventing atmospheric contamination of evaporating samples. One of the new system designs was compared with the conventional forced‐air evaporative system and with the open‐air drybath technique for levels of 15N cross‐contamination among evaporating samples. The forced‐air evaporative systems significantly reduced the time needed to evaporate samples as compared to the open‐air drybath technique; samples were evaporated to dryness in 2.5 h with the forced‐air systems as compared to 8 to 10 h on the open‐air drybath. The effectiveness of a given forced‐air system to prevent atmospheric contamination of evaporating samples was significantly affected by the flow rate of the airstream flowing over the samples; the optimum flow rate was different for each of three forced‐air systems tested. When operated at optimum air flow rate, the forced‐air evaporative systems reduced atmospheric contaminants to <0.2µg N per sample, even when a contaminant of ∼77 µg NH3‐N per sample vial was added upstream of the scrubbing systems. The average atmospheric contaminant N found in samples evaporated on the open‐air drybath was 0.3 µ N, indicating very low concentrations of atmospheric NH3during this study. Both these contaminant levels are sufficiently low to be of little or no significance in most tracer N studies. However, in previous studies we have experienced significant contamination of 15N samples evaporated on an open‐air drybath because the level of contaminant N in the laboratory atmosphere varied and could not be adequately controlled, even when NH3‐containing chemicals, cleansers, and paints were not stored or used in or around the working area. Average cross‐contaminant levels of 0.28, 0.20, and 1.01 µ of N were measured between samples evaporated on the open‐air dry‐bath, the newly‐designed forced‐air system, and the conventional forced‐air system, respectively. The cross‐contamination level is significantly higher on the conventional forced‐air system than on the other two systems, and could significantly alter the atom % 15N of high‐enriched, low [N] evaporating samples. The data from this study demonstrate that effectiveness in preventing atmospheric contamination and potential for cross‐contamination vary from system to system; thus, a given evaporative system should be carefully tested before being used in routine 15N sample processing.

35. A Comparison of Cross‐Contamination in Distillation Units Used in Total Nitrogen and Nitrogen‐15 Analyses

Abstract

A laboratory study was conducted to investigate 15N cross‐contamination errors between distilled standards [1 mg N as (NH4)2SO4] with various 15N excess contents (0, 3, or 10% excess 15N). Several stills differing in rate of distillation and construction were tested. We also compared the effectiveness of the ethanol (ETOH) cleansing technique with the (NH4)2SO4cleansing technique in reducing cross‐contamination errors. The ETOH wash was less effective than the (NH4)2SO4wash in reducing cross‐contamination errors for samples with normal abundance 15N but was equally effective in reducing cross‐contamination errors for samples with high‐enrichment 15N. The (NH4)2SO4wash sometimes significantly diluted high‐enrichment 15N samples. Ammonia seemed to be absorbed and sporadically released from the nonglass components of the partial‐glass still. Replacing the rubber and teflon components with glass and decreasing the glass surface area of the distillation unit decreased cross‐contamination errors and increased the percent recovery of atom % 15N from the distillation unit.

Abstract

This report summarizes experimental and theoretical studies carried out during the period of November 1985 to October 1988 on laser surface modification of Nickel, Niobium, and Magnesium alloys for improved environmental resistance at high temperature. Major emphasis has been on Ni-Cr-Al-Hf system. Microstructural evolution and oxidation properties of Ni and Nb alloys were examined. For Mg alloys microstructural characterization and potentiodynamic corrosion testing were carried out. One-dimensional diffusion model for finite domain to examine the extended solid solubility in laser cladding was also developed.

Abstract

Heterotopic pregnancies are a rare phenomenon defined by the simultaneous occurrence of intrauterine and extrauterine pregnancy. The incidence of heterotopic pregnancy occurring through natural fertilization is low but is increased by risk factors such as assisted reproductive techniques or pelvic inflammatory disease increase. We report the case of a 36-year-old female patient in the 6th week of pregnancy who presented to the emergency unit with severe acute lower abdominal pain. Laboratory chemistry and sonography revealed a suspected heterotopic pregnancy. The patient was admitted for observation. The sonographic reevaluation on the next day confirmed the suspicion of extrauterine gravidity with intact intrauterine gravidity with additional decreasing hemoglobin and hematoperitoneum, so that laparoscopy was indicated. Intraoperatively, the mass on the left ovary was removed without complications. The patient could be discharged quickly postoperatively after a complication-free course and gave birth to a healthy child by spontaneous partus in the 38th week of gestation. Due to their rarity, there is limited research related to heterotopic pregnancies, and most scientific articles are case studies. Diagnostically, the most important thing in clinical practice is to think about the possibility of EUG even if there is evidence of an intact IUG. Transvaginal sonography is of particular importance in diagnostics. Abstract

Resistance of high grade tumors to treatment involves cancer stem cell features, deregulated cell division, acceleration of genomic errors, and emergence of cellular variants that rely upon diverse signaling pathways. This heterogeneous tumor landscape limits the utility of the focal sampling provided by invasive biopsy when designing strategies for targeted therapies. In this roadmap review paper, we propose and develop methods for enabling mapping of cellular and molecular features in vivo to inform and optimize cancer treatment strategies in the brain. This approach leverages (1) the spatial and temporal advantages of in vivo imaging compared with surgical biopsy, (2) the rapid expansion of meaningful anatomical and functional magnetic resonance signals, (3) widespread access to cellular and molecular information enabled by next-generation sequencing, and (4) the enhanced accuracy and computational efficiency of deep learning techniques. As multiple cellular variants may be present within volumes below the resolution of imaging, we describe a mapping process to decode micro- and even nano-scale properties from the macro-scale data by simultaneously utilizing complimentary multiparametric image signals acquired in routine clinical practice. We outline design protocols for future research efforts that marry revolutionary bioinformation technologies, growing access to increased computational capability, and powerful statistical classification techniques to guide rational treatment selection., (© 2022 The Authors. Abstract

Background and Purpose: Pediatric supratentorial tumors such as embryonal tumors, high-grade gliomas, and ependymomas are difficult to distinguish by histopathology and imaging because of overlapping features. We applied machine learning to uncover MR imaging-based radiomics phenotypes that can differentiate these tumor types., Materials and Methods: Our retrospective cohort of 231 patients from 7 participating institutions had 50 embryonal tumors, 127 high-grade gliomas, and 54 ependymomas. For each tumor volume, we extracted 900 Image Biomarker Standardization Initiative-based PyRadiomics features from T2-weighted and gadolinium-enhanced T1-weighted images. A reduced feature set was obtained by sparse regression analysis and was used as input for 6 candidate classifier models. Training and test sets were randomly allocated from the total cohort in a 75:25 ratio., Results: The final classifier model for embryonal tumor-versus-high-grade gliomas identified 23 features with an area under the curve of 0.98; the sensitivity, specificity, positive predictive value, negative predictive value, and accuracy were 0.85, 0.91, 0.79, 0.94, and 0.89, respectively. The classifier for embryonal tumor-versus-ependymomas identified 4 features with an area under the curve of 0.82; the sensitivity, specificity, positive predictive value, negative predictive value, and accuracy were 0.93, 0.69, 0.76, 0.90, and 0.81, respectively. The classifier for high-grade gliomas-versus-ependymomas identified 35 features with an area under the curve of 0.96; the sensitivity, specificity, positive predictive value, negative predictive value, and accuracy were 0.82, 0.94, 0.82, 0.94, and 0.91, respectively., Conclusions: In this multi-institutional study, we identified distinct radiomic phenotypes that distinguish pediatric supratentorial tumors, high-grade gliomas, and ependymomas with high accuracy. 47. Maternal and neonatal complications in women with congenital heart disease: a nationwide analysis.

Abstract

Aims: The aim of this study was to provide population-based data on maternal and neonatal complications and outcome in the pregnancies of women with congenital heart disease (CHD)., Methods and Results: Based on administrative data from one of the largest German Health Insurance Companies (BARMER GEK, ∼9 million members representative for Germany), all pregnancies in women with CHD between 2005 and 2018 were analysed. In addition, an age-matched non-CHD control group was included for comparison and the association between adult CHD (ACHD) and maternal or neonatal outcomes investigated. Overall, 7512 pregnancies occurred in 4015 women with CHD. The matched non-CHD control group included 6502 women with 11 225 pregnancies. Caesarean deliveries were more common in CHD patients (40.5% vs. 31.5% in the control group; P < 0.001). There was no excess mortality. Although the maternal complication rate was low in absolute terms, women with CHD had a significantly higher rate of stroke, heart failure and cardiac arrhythmias during pregnancy (P < 0.001 for all). Neonatal mortality was low but also significantly higher in the ACHD group (0.83% vs. 0.22%; P = 0.001) and neonates to CHD mothers had low/extremely low birth weight or extreme immaturity (<0.001) or required resuscitation and mechanical ventilation more often compared to non-CHD offspring (P < 0.001 for both). On multivariate logistic regression maternal defect complexity, arterial hypertension, heart failure, prior fertility treatment, and anticoagulation with vitamin K antagonists emerged as significant predictors of adverse neonatal outcome (P < 0.05 for all). Recurrence of CHD was 6.1 times higher in infants to ACHD mothers compared to controls (P < 0.0001)., Conclusions: This population-based study illustrates a reassuringly low maternal mortality rate in a highly developed healthcare system. Nevertheless, maternal morbidity and neonatal morbidity/mortality were significantly increased in women with ACHD and their offspring compared to non-ACHD controls highlighting the need of specialized care and pre-pregnancy counselling., (Published on behalf of the European Society of Cardiology. Abstract

Background and Purpose: Atypical teratoid/rhabdoid tumors and medulloblastomas have similar imaging and histologic features but distinctly different outcomes. We hypothesized that they could be distinguished by MR imaging-based radiomic phenotypes., Materials and Methods: We retrospectively assembled T2-weighted and gadolinium-enhanced T1-weighted images of 48 posterior fossa atypical teratoid/rhabdoid tumors and 96 match-paired medulloblastomas from 7 institutions. Using a holdout test set, we measured the performance of 6 candidate classifier models using 6 imaging features derived by sparse regression of 900 T2WI and 900 T1WI Imaging Biomarker Standardization Initiative-based radiomics features., Results: From the originally extracted 1800 total Imaging Biomarker Standardization Initiative-based features, sparse regression consistently reduced the feature set to 1 from T1WI and 5 from T2WI. Among classifier models, logistic regression performed with the highest AUC of 0.86, with sensitivity, specificity, accuracy, and F1 scores of 0.80, 0.82, 0.81, and 0.85, respectively. Abstract

Background and Purpose: Posterior fossa tumors are the most common pediatric brain tumors. MR imaging is key to tumor detection, diagnosis, and therapy guidance. We sought to develop an MR imaging-based deep learning model for posterior fossa tumor detection and tumor pathology classification., Materials and Methods: The study cohort comprised 617 children (median age, 92 months; 56% males) from 5 pediatric institutions with posterior fossa tumors: diffuse midline glioma of the pons ( n = 122), medulloblastoma ( n = 272), pilocytic astrocytoma ( n = 135), and ependymoma ( n = 88). There were 199 controls. Tumor histology served as ground truth except for diffuse midline glioma of the pons, which was primarily diagnosed by MR imaging. A modified ResNeXt-50-32x4d architecture served as the backbone for a multitask classifier model, using T2-weighted MRIs as input to detect the presence of tumor and predict tumor class. Deep learning model performance was compared against that of 4 radiologists., Results: Model tumor detection accuracy exceeded an AUROC of 0.99 and was similar to that of 4 radiologists. Model tumor classification accuracy was 92% with an F 1 score of 0.80. The model was most accurate at predicting diffuse midline glioma of the pons, followed by pilocytic astrocytoma and medulloblastoma. Ependymoma prediction was the least accurate. Tumor type classification accuracy and F 1 score were higher than those of 2 of the 4 radiologists., Conclusions: We present a multi-institutional deep learning model for pediatric posterior fossa tumor detection and classification with the potential to augment and improve the accuracy of radiologic diagnosis., (© 2020 by American Journal of Neuroradiology.)

50. Task Feasibility Maximization Using Model-Free Policy Search and Model-Based Whole-Body Control.

Abstract

Producing feasible motions for highly redundant robots, such as humanoids, is a complicated and high-dimensional problem. Model-based whole-body control of such robots can generate complex dynamic behaviors through the simultaneous execution of multiple tasks. Unfortunately, tasks are generally planned without close consideration for the underlying controller being used, or the other tasks being executed, and are often infeasible when executed on the robot. Consequently, there is no guarantee that the motion will be accomplished. In this work, we develop a proof-of-concept optimization loop which automatically improves task feasibility using model-free policy search in conjunction with model-based whole-body control. This combination allows problems to be solved, which would be otherwise intractable using simply one or the other. Through experiments on both the simulated and real iCub humanoid robot, we show that by optimizing task feasibility, initially infeasible complex dynamic motions can be realized-specifically, a sit-to-stand transition.
