Abstract

Background/objectivesPatients with epidermolysis bullosa (EB) require care of wounds that are colonized or infected with bacteria. A subset of EB patients are at risk for squamous cell carcinoma, and bacterial-host interactions have been considered in this risk. The EB Clinical Characterization and Outcomes Database serves as a repository of information from EB patients at multiple centers in the United States and Canada. Access to this resource enabled broad-scale analysis of wound cultures.MethodsA retrospective analysis of 739 wound cultures from 158 patients from 13 centers between 2001 and 2018.ResultsOf 152 patients with a positive culture, Staphylococcus aureus (SA) was recovered from 131 patients (86%), Pseudomonas aeruginosa (PA) from 56 (37%), and Streptococcus pyogenes (GAS) from 34 (22%). Sixty-eight percent of patients had cultures positive for methicillin-sensitive SA, and 47%, methicillin-resistant SA (18 patients had cultures that grew both methicillin-susceptible and methicillin-resistant SA at different points in time). Of 15 patients with SA-positive cultures with recorded mupirocin susceptibility testing, 11 had mupirocin-susceptible SA and 6 patients mupirocin-resistant SA (2 patients grew both mupirocin-susceptible and mupirocin-resistant SA). SCC was reported in 23 patients in the entire database, of whom 10 had documented wound cultures positive for SA, PA, and Proteus species in 90%, 50%, and 20% of cases, respectively.ConclusionsSA and PA were the most commonly isolated bacteria from wounds. Methicillin resistance and mupirocin resistance were reported in 47% and 40% of patients tested, respectively, highlighting the importance of ongoing antimicrobial strategies to limit antibiotic resistance.

3. Systematic review of methods used to measure training load in dance

Abstract

Dance is a popular physical activity. Increased dance training has been associated with an increased risk of injury. Given the established association between training load (TL) and injury in sport, knowledge of how TL is currently being measured in dance is critical. The objective of this study is to summarise published literature examining TL monitoring in dance settings. Six prominent databases (CINAHL, EMBASE, Medline, ProQuest, Scopus, SportDiscus) were searched and nine dance-specific journals were handsearched up to May 2022. Selected studies met inclusion criteria, where original TL data were collected from at least one dancer in a class, rehearsal and/or performance. Studies were excluded if TL was not captured in a dance class, rehearsal or performance. Two reviewers independently assessed each record for inclusion at title, abstract and full-text screening stages. Study quality was assessed using Joanna Briggs Institute Critical Appraisal Tool checklists for each study design. The 199 included studies reported on female dancers (61%), ballet genre (55%) and the professional level (31%). Dance hours were the most common tool used to measure TL (90%), followed by heart rate (20%), and portable metabolic systems (9%). The most common metric for each tool was mean weekly hours (n=381; median=9.5 hours, range=0.2–48.7 hours), mean heart rate (n=143) and mean oxygen consumption (n=93). Further research on TL is needed in dance, including a consensus on what tools and metrics are best suited for TL monitoring in dance.

Abstract

The COVID-19 pandemic has caused significant shifts in patient care including a steep decline in ambulatory visits and a marked increase in the use of telemedicine. Infantile hemangiomas (IH) can require urgent evaluation and risk stratification to determine which infants need treatment and which can be managed with continued observation. For those requiring treatment, prompt initiation decreases morbidity and improves long-term outcomes. The Hemangioma Investigator Group has created consensus recommendations for management of IH via telemedicine. FDA/EMA-approved monitoring guidelines, clinical practice guidelines, and relevant, up-to-date publications regarding initiation and monitoring of beta-blocker therapy were used to inform the recommendations. Clinical decision-making guidelines about when telehealth is an appropriate alternative to in-office visits, including medication initiation, dosage changes, and ongoing evaluation, are included. The importance of communication with caregivers in the context of telemedicine is discussed, and online resources for both hemangioma education and propranolol therapy are provided.

Abstract

This article presents a case study of a 32-month-old girl who developed blistering dermatitis on her buttocks after using a senna laxative. The diagnosis was senna laxative-induced dermatitis, which is a rare complication in infants. The article discusses the use of senna as a common treatment for childhood constipation and highlights the importance of recognizing the clinical features of senna-induced diaper dermatitis. It also provides recommendations for managing and preventing this condition, such as discontinuing the laxative, using barrier cream, and prompt diaper changes. 12. Psychosocial and Neurobiological Correlates of Past and Prospective Adolescent Suicidal Thoughts and Behaviors

Abstract

Suicide is the second leading cause of death for youth in the United States and the rates are continuing to rise. Unfortunately, national policy efforts to combat this increase in youth deaths by suicide have made little difference. Suicidal thoughts and behaviors (STB) disproportionately affect female adolescents, and youth of color, including Latino youth. To date, there have been few longitudinal studies of adolescent STB in Latino adolescents, thus, many of the prospective risk factors for suicidality in these populations are not well understood. Additionally, there is limited research which has examined the neurobiological predictors of STB in community or clinical adolescent samples across all genders and ethnicities. Given this imminent and intensifying public health crisis, additional investigations are needed to examine and understand both proximal and distal factors related to the increase in adolescent STB. Moreover, as the US becomes more ethnically diverse, risk and protective factors which influence both the general population as well as specific ethnic minorities should be examined, including environmental, social, and biological factors which may act to either ameliorate or exacerbate rates of youth STB. This dissertation investigated: (1) the associations between psychosocial and culturally specific factors and the growth of STB from early to late adolescence; (2) the role of the autonomic nervous system (ANS) in stress activation during social exclusion and its relation to concurrent and prospective STB in late adolescence; and (3) whether pubertal timing moderates the link between parasympathetic stress activity and concurrent and subsequent STB in clinically high-risk adolescent females. In Paper 1, female sex and later generation status were associated with increasing prevalence in STB across adolescence, and family conflict and peer conflict predicted increased STB whereas greater familism predicted less STB, in Mexican-origin youth. In Paper 2, prior STB was associated with lower sympathetic activity at baseline and during social exclusion, and prospective STB was predicted by lower basal ANS activity paired with greater ANS activation during social exclusion, in Mexican-origin youth. In paper 3, lower baseline parasympathetic activity and earlier pubertal timing were associated with prior STB, whereas girls with later pubertal timing and higher parasympathetic activity during a speech task were buffered from endorsing STB one year later. In summary, this body of research suggests that interpersonal processes, especially within the family, are critical for understanding suicidality risk in Mexican-origin adolescents. Additionally, basal autonomic activity may be a biomarker or a physiological “scar” of prior suicidality whereas certain patterns of autonomic stress activation may confer greater risk for prospective STB years in advance. Lastly, the female pubertal transition confers risk for STB and later pubertal timing may provide a protective buffer against STB for those youth who display dysregulated physiological responses to stress.

14. Homozygous deletion of the DSG3 terminal exon associated with acantholytic blistering of the oral and laryngeal mucosa.

Abstract

We report a novel homozygous 49.6 kb deletion of chromosome 18q12.1 involving the last exon of DSG3 in dizygotic twins with phenotype consistent with acantholytic blistering of the oral and laryngeal mucosa (ABOLM). The twin siblings presented predominantly with friability of the laryngeal and respiratory mucosa. This is only the second report in the literature of this unusual autosomal recessive blistering disorder. The diagnosis explains the mucosal phenotype of a pemphigus‐like disorder without evidence of autoimmune dysfunction. The exclusion of an autoimmune basis has management implications. The deletion also involved the DSG2 gene, which is associated with arrhythmogenic right ventricular dysplasia (ARVD). The affected siblings and heterozygous parents do not show any cardiac phenotype at this time. Functional studies would further clarify how deletions resulting in loss of function of DSG3 may cause the reported phenotypes of DSG3‐related ABOLM. [ABSTRACT FROM AUTHOR]

36. Adding to the Pool of Methods for Program Evaluation: A Comparison of Latent Class Analysis and Propensity Score Analysis

Abstract

Considering the amount of funding that is distributed to educational research each year, leaders and policymakers have a vested interest in finding scientifically based evidence that answers causal questions regarding program effectiveness. The importance of program evaluation has long been recognized in many fields of research; however, the most appropriate methods for evaluating programs and estimating the causal effect is less clear. In an effort to gain insight into some of the methods used in program evaluation, this dissertation highlights the advantages and disadvantages of a commonly used statistical method called propensity score analysis. Additionally, this dissertation introduces latent class analysis as a possible new method in program evaluation. The dataset chosen for this dissertation comes from a study done by Shadish, Clark, and Steiner (2008), which consists of both a randomized and a quasi-experimental component. In having the unique feature of both a randomized group and a nonrandomized group, I hope to show that latent class analysis can produce similar outcome results as propensity score analysis and a randomized design, while also presenting how the unique features of latent class analysis would be beneficial to program evaluation. The current study consisted of four main analyses. Analysis 1 was a latent class analysis that evaluated the method's ability to work with missing values. Analysis 2 consisted of both a latent class analysis and a propensity score analysis using the Shadish et al. (2008) imputed dataset and all 25 predictor variables. Analysis 3 used the theory-reduced set of variables to conduct multiple latent class analyses and propensity score analyses on four varying sample sizes (i.e., 100%, 75%, 50%, and 25%). Analysis 4 conducted multiple latent class analyses and propensity score analyses on the reduced set of variables, created from theory and latent class analysis standards applied to the same four sample sizes from Analysis 3. All propensity score analyses were completed in Stata 12.0, using the pscore command, while, all latent class analyses began in Mplus 6.0, and then were exported to SPSS 20.0 Results of the independent sample t-test, conducted on the randomized dataset, placed the target average treatment value for the vocabulary posttest at 8.11 and the mathematics posttest at 4.19. When looking at each of the analyses, the closest estimate to the randomized design for the vocabulary posttest was produced by the propensity score analysis; while for the mathematics posttest, the closest estimate to the randomized design was calculated by the latent class analysis. In general, the results do not indicate which method is better at producing more accurate average treatment effects. However, the latent class analysis identified four unique response patterns where the individual mean difference varied for each of the classes. Abstract

Background/objectives: We sought to describe the experience among members of the Hemangioma Investigator Group with pulsed dye laser (PDL) in the treatment of nonulcerated infantile hemangioma (IH) in pediatric patients in the pre‐ and post‐beta‐blocker era. Methods: A multicenter retrospective cohort study was conducted in patients with nonulcerated IH treated with laser therapy. Patient demographics, IH characteristics, indications for/timing of laser therapy, as well as laser parameters were collected. Responses to laser therapy were evaluated using a visual analog scale (VAS). Results: One hundred and seventeen patients with IH were treated with PDL. 18/117 (15.4%) had early intervention (defined as <12 months of life), and 99/117 (84.6%) had late intervention (≥12 months of life). In the late intervention group, 73.7% (73/99) had additional medical management of their IH. The mean age at PDL initiation for the late intervention group was 46.7 ± 35.3 months of life (range 12–172 months) with total number of treatments to maximal clearing of 4.2 ± 2.8 (range 1–17). Those who received propranolol prior to PDL received fewer sessions (1.1 fewer sessions, approaching significance [p =.056]). On the VAS, there was a mean 85% overall improvement compared to baseline (range 18%–100%), with most improvement noted in erythema and/or telangiectasias. The incidence of adverse effects was 6/99 (6.1%). Conclusions: PDL is a useful tool in the treatment of IH, with notable improvement of telangiectasia and erythema and low risk of complications.
