2. Monitoring ammonia concentrations in more than 10 stations in the Po Valley for the period 2007–2022 in relation to the evolution of different sources

Abstract: Regarding secondary aerosols, in addition to the significant contribution of anthropogenic gases such as NOx and SO2, atmospheric ammonia (NH3) plays a crucial role as the primary basic gaseous species capable of neutralizing acidic compounds. This acid–base reaction is responsible for the formation of ammonium nitrate (NH4NO3), constituting up to 60% of PM10 within the Po River basin in Italy. Ion chromatographic analyses performed on offline samples indicate that this secondary inorganic species exhibits minimal concentration variability over the Po Valley because of limited air circulation due to orography and mesoscale air circulation. Therefore, investigating gaseous precursors becomes crucial. From the northern to the southern part of Lombardy—the region at the center of the basin—NH3 emission amounts account for 2.5, 11.1, and 27.7 t/y/km2, mainly due to agriculture and livestock activities (∼97%). To study NH3 temporal and spatial variability, the Environmental Protection Agency of Lombardy Region has been monitoring NH3 concentrations across its territory since 2007, with 10 active monitoring sites. Annual and seasonal cycles are presented, along with a focus on different stations, including urban, low-mountain background, high-impact livestock, and rural background, highlighting the impact of various sources. Measurements indicate that within the Po basin, NH3 concentrations can reach up to 700 µg/m3 (as an hourly average) in proximity to the main gaseous NH3 source. Instrument intercomparisons among online monitors and passive vials, as well as different online monitors, are presented. Therefore, this paper provides crucial data to understand the formation of secondary inorganic aerosols in one of the most important hotspot sites for air pollution.

Abstract: In Big Cypress National Preserve, the federally threatened Everglades bully (Sideroxylon reclinatum subsp. austrofloridense) is sympatric with its conspecific, more widespread relative, the Florida bully (Sideroxylon reclinatum subsp. reclinatum). In this area of overlap, the only reliable characters to distinguish the two are cryptic, micromorphological traits of the abaxial laminar surface. In order to better understand the distribution of the federally threatened taxon, we used a combination of habitat suitability modeling (HSM), field surveys, and microscopy. Using models to inform initial surveys, we collected leaf material of 96 individuals in the field, 86 of which we were able to identify to subspecies. Of these, 73 (85%) were identified as the threatened taxon, expanding both the known range and population size within Big Cypress. We used these 73 new occurrences to rerun HSMs to create a more accurate picture of where the taxon is likely to occur. A total of 15,015 hectares were predicted to be suitable habitat within Big Cypress, with 34,069 hectares across the entire study area. These model results could be used to inform the critical habitat designation for this taxon. For at-risk, cryptic taxa, such as the Everglades bully, multiple approaches are needed to inform management and conservation priorities, including the consideration of a hybridization zone.

Abstract: The ASTRI project is the INAF (Italian National Institute for Astrophysics) flagship project developed in the context of the Cherenkov Telescope Array (CTA) international project. ASTRI is dedicated to the realization of the prototype of a Cherenkov small-size dual-mirror telescope (SST-2M) and then to the realization of a mini-array composed of a few of these units. The prototype and all the necessary hardware devices are foreseen to be installed at the Serra La Nave Observing Station (Catania, Italy) in 2014. The upcoming data flow will be properly reduced by dedicated (online and offline) analysis pipelines aimed at providing robust and reliable scientific results (signal detection, sky maps, spectra and light curves) from the ASTRI silicon photo-multipliers camera raw data. Furthermore, a flexible archiving system has being conceived for the storage of all the acquired ASTRI (scientific, calibration, housekeeping) data at different steps of the data reduction up to the final scientific products. In this contribution we present the data acquisition, the analysis pipeline and the archive architecture that will be in use for the ASTRI SST prototype. Abstract: The only supernovae (SNe) to have shown early gamma-ray or X-ray emission thus far are overenergetic, broad-lined Type Ic SNe (Hypernovae - HNe). Recently, SN 2008D shows several novel features: (i) weak XRF, (ii) an early, narrow optical peak, (iii) disappearance of the broad lines typical of SNIc HNe, (iv) development of He lines as in SNeIb. Detailed analysis shows that SN 2008D was not a normal SN: its explosion energy (KE ~ 6*10^{51} erg) and ejected mass (~7 Msun) are intermediate between normal SNeIbc and HNe. We derive that SN 2008D was originally a ~30Msun star. When it collapsed a black hole formed and a weak, mildly relativistic jet was produced, which caused the XRF. SN 2008D is probably among the weakest explosions that produce relativistic jets. 9. From MAGIC to CTA: the INAF participation to Cherenkov Telescopes experiments for Very High Energy Astrophysics

Abstract: The next decade can be considered the "golden age" of the Gamma Ray Astronomy with the two satellites for Gamma Ray Astronomy (AGILE and GLAST) in orbit. Therefore, thanks to many other X-ray experiments already in orbit (e.g. Swift, Chandra, NewtonXMM, etc.) it will be possible to image the Universe for the first time all over the electromagnetic spectrum almost contemporarily. The new generations of ground-based very high gamma-ray instruments are ready to extend the observed band also to the very high frequencies. Scientists from the Italian National Institute for Astrophysics (INAF) are involved in many, both space- and ground- based gamma ray experiments, and recently such an involvement has been largely improved in the field of the Imaging Atmospheric Cherenkov Telescopes (IACT). INAF is now member of the MAGIC collaboration and is participating to the realization of the second MAGIC telescope. MAGIC, as well other IACT experiments, is not operated as an observatory so a proper guest observer program does not exist. A consortium of European scientists (including INAF scientists) is thus now thinking to the design of a new research infrastructure: the Cherenkov Telescope Array (CTA). CTA is conceived to provide 10 times the sensitivity of current instruments, combined with increased flexibility and increased coverage from some 10 GeV to some 100 TeV. Abstract: The Swift Gamma-Ray Burst Explorer, launched on 2004 November 20, is a multiwavelength, autonomous, rapid-slewing observatory for gamma-ray burst (GRB) astronomy. On 2004 December 23, during the activation phase of the mission, the Swift X-Ray Telescope (XRT) was pointed at a burst discovered earlier that day by the Swift Burst Alert Telescope. A fading, uncataloged X-ray source was discovered by the XRT and was observed over a period of about 3 hours, beginning 4.6 hours after the burst. The X-ray detection triggered a VLT observation of the optical/NIR counterpart, located about 1.1 arcseconds from the XRT position. The X-ray counterpart faded rapidly, with a power law index of -1.72 +/- 0.20. The average unabsorbed X-ray flux 4.6-7.9 hours after the burst was 6.5 x 10^{-12} erg cm^{-2} s^{-1} in the 0.5-10 keV band, for a power-law spectrum of photon index 2.02 +/- 0.13 with Galactic absorption. The NIR counterpart was observed at three epochs between 16 and 87 hours after the burst, and faded with a power-law index of -1.14 +/- 0.08 with a reddening-corrected SED power-law slope of -0.40 +/- 0.03. We find that the X-ray and NIR data are consistent with a two-component jet in a wind medium, with an early jet break in the narrow component and an underlying electron index of 1.8-2.0., Comment: 16 pages, including 4 figures. 11. The role of disgust propensity and sensitivity on sexual excitation and inhibition in obsessive-compulsive disorder

Abstract: Sexuality is understudied in Obsessive-Compulsive Disorder (OCD). According to the Dual Control Model, low sexual excitation (SE) and high sexual inhibition (SI) are indicative of a higher probability of experiencing a sexual dysfunction. The present study investigated SE and SI in OCD patients compared with controls. It was hypothesized that OCD patients report lower SE and higher SI than controls. Given their potential role as inhibitors of sexual response, it was hypothesized that in the OCD group higher disgust propensity/sensitivity, contamination/washing symptoms, unacceptable thoughts, and obsessive beliefs predicted lower SE, higher SI due to Threat of Performance Failure, and higher SI due to Threat of Performance Consequences. Seventy-two OCD patients and 72 controls matched on gender/age completed the Disgust Propensity and Sensitivity Scale-Revised, Obsessive Beliefs Questionnaire-46, Obsessive Compulsive Inventory-Revised, and Sexual Inhibition/Sexual Excitation Scales. OCD patients had higher SE, SI due to Threat of Performance Failure, and SI due to Threat of Performance Consequences than controls. In the OCD group, higher disgust sensitivity, SI due to Threat of Performance Consequences, and perfectionism predicted higher SI due to Threat of Performance Failure. Higher SI due to Threat of Performance Failure and contamination/washing symptoms predicted higher SI due to Threat of Performance Consequences. These findings highlight the presence of sexual difficulties in OCD patients, particularly a higher SE and SI. The latter is especially relevant in those patients with higher disgust sensitivity, contamination/washing symptoms and perfectionism. Psychotherapists should assess and target sexuality during clinicalpractice with OCD patients.

14. P475 RECURRENT CORONARY ARTERY VASOSPASM COMPLICATED BY CARDIAC ARRESTS IN HEART TRANSPLANT RECIPIENT: AN UNUSUAL ENEMY

Abstract: Even if coronary vasospasm (CV) is considered a rare phenomenon in heart transplant (HT) recipients, during routine coronary angiography asymptomatic CV may be observed. We report the case of a 30–year–old male who underwent HT in 2007 for dilatative cardiomyopathy. He was maintained on tacrolimus and mycophenolate mofetil and had no significant rejection or coronary allograft vasculopathy (CAV) demonstration. In December 2020 patient presented with cardiac arrest preceded by angina. Coronary angiography demonstrated diffuse spasm of the right coronary artery (RCA) with resolution after intracoronary nitrate administration. An endomyocardial biopsy (EBM) revealed no signs of cellular rejection whilst cardiac magnetic resonance showed signs of acute rejection (Fig.1). Moreover, two major histocompatibility complex (MHC) Class 2 donor–specific antibodies (DSAs) were positive with a high title. Treatment for antibody–mediated rejection (AMR) was initiated with steroids and plasmapheresis, followed by immunoglobulin infusion. Cardiac defibrillator was implanted before discharge. In August 2021, patient experienced a second cardiac arrest. Left heart catheterization found diffuse CV and after nitrates administration, proximal critical stenosis of RCA was pointed out and treated with stent insertion (Fig. 2–3). EMB and DSAs assessment were both negative. The patient was discharged after medical therapy optimization for CV prevention including the switch from mycophenolate to everolimus. After two more episodes of ACS with angiography finding of CV we decided to empirically treat the patient with weekly photopheresis sessions. Despite aggressive AMR management, a third ACC occurred in March 2022. The patient was finally reintroduced to the HT waiting list. Most CV episodes run without any symptoms due to the denervated heart post–transplant so probably re–innervation occurred in our patient. Calcium channel blockers, nitrates, and beta–blockers failed in CV prevention, suggesting ineffectiveness in HT recipients because of a different pathogenetic mechanism when compared to healthy subjects. Furthermore, documentation of coronary disease progression after the second ACC supports the hypothesis that CV could be the first manifestation of CAV. An intriguing hypothesis would be the presence of pathological vasoreactivity in the donor‘s heart, ascribable to an “inherited” endothelial dysfunction that could last be exacerbated by multiple factors, including AMR.

Abstract: Background Current surgical treatment options for partial-thickness tears (eg, takedown and repair, in situ repair) are limited by the degenerative nature of the underlying tendon and may require extensive intervention that can alter the anatomic footprint. The complexity of available techniques to address these issues led to the development of a resorbable collagen implant, which can be used to create a bioinductive repair of partial-thickness tears. Methods We prospectively enrolled 33 patients with chronic, degenerative, intermediate-grade (n = 12), or high-grade (n = 21) partial-thickness tears (11 articular, 10 bursal, 4 intrasubstance, and 8 hybrid) of the supraspinatus tendon in a multicenter study. After arthroscopic subacromial decompression without a traditional rotator cuff repair, a bioinductive implant was secured over the bursal surface of the tendon. Clinical outcomes were assessed using American Shoulder and Elbow Surgeons (ASES) and Constant-Murley scores (CMS) preoperatively and at 3 months, 1 year, and 2 years postoperatively. Magnetic resonance imaging was performed to assess postoperative tendon healing and thickness at the original tear site. Results At 2-year follow-up, mean ASES and CMS scores improved both clinically and statistically at 1 and 2 years, compared with baseline, for intermediate- and high-grade tears. There was magnetic resonance imaging evidence of new tissue fill-in within the original baseline tear in 100% of the intermediate-grade tears and 95% of the high-grade tears. In 90.9% of the intermediate-grade tears and 84.2% of the high-grade tears, this new tissue fill-in represented at least an additional 50% of the volume of the initial lesion. From baseline to 2-year follow-up, the mean tendon thickness increased by 1.2 mm (standard deviation, 1.3; P = .012) and 1.8 mm (standard deviation, 2.2; P = .003) in the intermediate- and high-grade tears, respectively. The analysis of tear grade and location revealed no statistically significant difference in the change in mean tendon thickness at any time point. One patient with a high-grade articular lesion demonstrated progression to a full-thickness tear; however, the patient was noncompliant and the injury occurred while shoveling snow 1 month after surgery. Neither tear location nor treatment of bicep pathology affected the ASES or CMS scores at any follow-up point. No serious adverse events related to the implant were reported. Conclusion Final results from this 2-year prospective study indicate that the use of this resorbable bovine collagen implant for isolated bioinductive repair of intermediate- and high-grade partial-thickness rotator cuff tears of the supraspinatus is safe and effective, regardless of tear grade and location.

Abstract: The Imaging X-ray Polarimetry Explorer (IXPE) will expand the information space for study of cosmic sources, by adding linear polarization to the properties (time, energy, and position) observed in x-ray astronomy. Selected in 2017 January as a NASA Astrophysics Small Explorer (SMEX) mission, IXPE will be launched into an equatorial orbit in 2021. The IXPE mission will provide scientifically meaningful measurements of the x-ray polarization of a few dozen sources in the 2-8 keV band, including polarization maps of several x-ray-bright extended sources and phase-resolved polarimetry of many bright pulsating x-ray sources.

19. Impact of black carbon aerosol over Italian basin valleys: high-resolution measurements along vertical profiles, radiative forcing and heating rate

Abstract: A systematic study of black carbon (BC) vertical profiles measured at high-resolution over three Italian basin valleys (Terni Valley, Po Valley and Passiria Valley) is presented. BC vertical profiles are scarcely available in literature. The campaign lasted 45 days and resulted in 120 measured vertical profiles. Besides the BC mass concentration, measurements along the vertical profiles also included aerosol size distributions in the optical particle counter range, chemical analysis of filter samples and a full set of meteorological parameters. Using the collected experimental data, we performed calculations of aerosol optical properties along the vertical profiles. The results, validated with AERONET data, were used as inputs to a radiative transfer model (libRadtran). The latter allowed an estimation of vertical profiles of the aerosol direct radiative effect, the atmospheric absorption and the heating rate in the lower troposphere. The present measurements revealed some common behaviors over the studied basin valleys. Specifically, at the mixing height, marked concentration drops of both BC (range: from −48.4 ± 5.3 to −69.1 ± 5.5%) and aerosols (range: from −23.9 ± 4.3 to −46.5 ± 7.3%) were found. The measured percentage decrease of BC was higher than that of aerosols: therefore, the BC aerosol fraction decreased upwards. Correspondingly, both the absorption and scattering coefficients decreased strongly across the mixing layer (range: from −47.6 ± 2.5 to −71.3 ± 3.0% and from −23.5 ± 0.8 to −61.2 ± 3.1%, respectively) resulting in a single-scattering albedo increase along height (range: from +4.9 ± 2.2 to +7.4 ± 1.0%). This behavior influenced the vertical distribution of the aerosol direct radiative effect and of the heating rate. In this respect, the highest atmospheric absorption of radiation was predicted below the mixing height (~ 2–3 times larger than above it) resulting in a heating rate characterized by a vertical negative gradient (range: from −2.6 ± 0.2 to −8.3 ± 1.2 K day−1 km−1). In conclusion, the present results suggest that the BC below the mixing height has the potential to promote a negative feedback on the atmospheric stability over basin valleys, weakening the ground-based thermal inversions and increasing the dispersal conditions.

Abstract: Aims The angiotensin–II receptor neprilysin inhibitor (ARNI), sacubitril/valsartan has been shown to be effective in treatment of patients with heart failure (HF), but limited data is available in patients with advanced disease. This retrospective observational study assessed the effects of ARNI treatment in patients with advanced HF. Methods We reviewed medical records of all advanced HF patients evaluated at our center for unconventional therapies from September 2016 to January 2019. We studied 44 patients who started ARNI therapy and who had a hemodynamic assessment before beginning ARNI and after 6 ± 2 months. The primary endpoint was variation in pulmonary pressures and filling pressures at 6 months after starting ARNI therapy. Results Mean patient age was 51.6±7.4 years; 84% were male. At 6±2 months after starting ARNI, there was significant reduction of systolic pulmonary artery pressure (PAP) (32 mmHg, IQR 27–45 vs 25 mmHg, IQR 22.3–36.5; p Conclusions Sacubitril/valsartan is effective in reducing filling pressures and pulmonary pressures in patients with advanced HF. The absence of adverse events during follow–up suggests that sacubitril/valsartan is safe and well–tolerated in this cohort of patients.

Abstract: Summary Background Among asymptomatic patients with severe carotid artery stenosis but no recent stroke or transient cerebral ischaemia, either carotid artery stenting (CAS) or carotid endarterectomy (CEA) can restore patency and reduce long-term stroke risks. However, from recent national registry data, each option causes about 1% procedural risk of disabling stroke or death. Comparison of their long-term protective effects requires large-scale randomised evidence. Methods ACST-2 is an international multicentre randomised trial of CAS versus CEA among asymptomatic patients with severe stenosis thought to require intervention, interpreted with all other relevant trials. Patients were eligible if they had severe unilateral or bilateral carotid artery stenosis and both doctor and patient agreed that a carotid procedure should be undertaken, but they were substantially uncertain which one to choose. Patients were randomly allocated to CAS or CEA and followed up at 1 month and then annually, for a mean 5 years. Procedural events were those within 30 days of the intervention. Intention-to-treat analyses are provided. Analyses including procedural hazards use tabular methods. Analyses and meta-analyses of non-procedural strokes use Kaplan-Meier and log-rank methods. The trial is registered with the ISRCTN registry, ISRCTN21144362. Findings Between Jan 15, 2008, and Dec 31, 2020, 3625 patients in 130 centres were randomly allocated, 1811 to CAS and 1814 to CEA, with good compliance, good medical therapy and a mean 5 years of follow-up. Overall, 1% had disabling stroke or death procedurally (15 allocated to CAS and 18 to CEA) and 2% had non-disabling procedural stroke (48 allocated to CAS and 29 to CEA). Kaplan-Meier estimates of 5-year non-procedural stroke were 2·5% in each group for fatal or disabling stroke, and 5·3% with CAS versus 4·5% with CEA for any stroke (rate ratio [RR] 1·16, 95% CI 0·86–1·57; p=0·33). Combining RRs for any non-procedural stroke in all CAS versus CEA trials, the RR was similar in symptomatic and asymptomatic patients (overall RR 1·11, 95% CI 0·91–1·32; p=0·21). Interpretation Serious complications are similarly uncommon after competent CAS and CEA, and the long-term effects of these two carotid artery procedures on fatal or disabling stroke are comparable. 26. Reduction in Prescribed Opioids after General Surgery Procedures at a Public Hospital

Abstract: Overdose of opioids is the number one cause of accidental death in the United States, and surgeons are overprescribing these medications. The aim of this study was to assess the feasibility of implementing postoperative opioid prescribing guidelines for general surgery procedures at a public hospital, where patients have lower socioeconomic status, public insurance, and limited access to care. We implemented a quality improvement project, which included in-service training for surgical staff and distribution of standardized guidelines. An infographic for patients was created to facilitate education on postoperative pain management. Pre- and postintervention opioid prescriptions and emergency room visits were compared for patients undergoing common general surgery procedures (inguinal hernia repair, appendectomy, and laparoscopic cholecystectomy). The median number of narcotic pills prescribed significantly decreased from 30 (n = 64) to 15 (n = 63) after the intervention ( P < 0.0001). Morphine milligram equivalents decreased from a median [range] of 150 [20,600] to 90 [5,300] ( P < 0.0001). The percentage of patients with postoperative pain-related emergency department visits remained low (1.6%). Standardization of postoperative opioid prescription practices was successfully implemented at a public hospital without an increase in the number of emergency room visits for pain.

Abstract: Background The compression of peritumoral healthy tissue in brain tumor patients is considered a major cause of life-threatening neurologic symptoms. This condition is associated with an increase in intracranial pressure. The literature reports that GBM has a mass effect in 72% of autoptic brains of patients who died of brain tumor, with herniation in 50% of all cases. Material and methods We present a preliminary clinical series on 5 cases. The patients are chosen based on criteria of recurrent glioblastoma without the possibility of a gross total resection with a recurrence after STUPP protocol. The surgeries include cytoreduction and augmentative craniotomy. The aim is to increase the intracranial volume and reduce the intracranial pressure in evolving glioblastoma. Clinical and radiological follow-up considers different objective scales and contrast enhancement MRI performed every 3 months. We evaluate KPS, survival rate, complication rate, quality of life using QOL-BN20 and neuropsychological assessment. Results The 12 months of follow-up shows a mean survival rate of 9 months after the surgery. This procedure has no negative impact on the survival rate, complication rate, and quality of life reduction. The neuropsychological evaluation does not show an impact on self-perception of aesthetic appearance. The surgery does not show surgical wound complications. The mean operative time was 225 minutes and was comparable with standard recurrence glioblastoma procedure. Conclusion This work has the main limitation of being preliminary results on small population. However, these results show encouraging outcomes and could be an innovative and salvage therapy for recurrent and incurable glioblastoma. Further follow-up and study must be performed.

Abstract: Autoimmune diseases (AIDs) share similar serological, clinical, and radiological findings, but, behind these common features, there are different pathogenic mechanisms, immune cells dysfunctions, and targeted organs. In this context, multiple lines of evidence suggest the application of precision medicine principles to AIDs to reduce the treatment failure. Precision medicine refers to the tailoring of therapeutic strategies to the individual characteristics of each patient, thus it could be a new approach for management of AIDS which considers individual variability in genes, environmental exposure, and lifestyle. Precision medicine would also assist physicians in choosing the right treatment, the best timing of administration, consequently trying to maximize drug efficacy, and, possibly, reducing adverse events. In this work, the growing body of evidence is summarized regarding the predictive factors for drug response in patients with AIDs, applying the precision medicine principles to provide high-quality evidence for therapeutic opportunities in improving the management of these patients. © 2020

Abstract: Background In patients with systolic chronic heart failure (CHF) clinical signs of congestion cannot always be evident at clinical examination. Right atrial pressure (RAP) measured by right heart catheterization (RHC) is an accurate and reproducible marker of blood volume. A non-invasive accurate tool to identify CHF patients with normal RAP would be desirable to tailor therapy. Purpose To validate an ultrasound (US)-assessed internal jugular vein distensibility (JVD) ratio to identify patients with normal mean RAP (defined as 7 mmHg or less) measured by RHC. Methods We first identify the JVD ratio that allows the most accurate identification of patients with normal RAP in a prospective calibration cohort of 100 patients with systolic CHF. Then, we tested the JVD ratio threshold to identify patients with normal RAP in a validation cohort of 101 consecutive patients with systolic CHF. All patients had a left ventricular ejection fraction (LVEF) Results In the calibration cohort (mean age 53 years, 13% female; median LVEF 25%, 81% in NYHA class III/IV) we define the best threshold of the JVD ratio to identify patients with normal RAP that has 1.6 with an area under the curve (AUC of 0.74; p1.6, n=42; median RAP 4 mmHg). High JVD ratio and low JVD ratio groups had similar clinical and laboratory characteristics. In the validation cohort (mean age 55 years, 13% female; median LVEF 25%; 56% in NYHA class III/IV) using the previously defined 1.6 JVD ratio threshold, we identified 51 patients with low JVD ratio (median RAP 8 mmHg) and 50 patients with high JVD ratio (median RAP 3 mmHg; p Conclusions We found that US-assessed JVD ratio is a convenient and accurate diagnostic tool to identify patients with advanced systolic CHF with normal vs. increased RAP. Abstract: In the early 1990s, several studies reported the misuse of codeine and promethazine hydrochloride cough syrup. Since then, the combination of this pharmaceutical, together with sprite or alcohol, known on the streets as "purple drank" or "lean", has become a popular drug among rap singers who promote its tranquilizing and euphoric effects through their music and videos. This review examines the "purple drank" phenomenon, taking into consideration its clinical and social implications. The study was conducted using PubMed, Scopus, and Web of Science as search engines, applying several inclusion and exclusion criteria and the string "Purple AND drank", resulting in 138 records. Seven papers that met our criteria were found. The risk of bias assessment, when applicable, was also considered, resulting in a low level of risk. Epidemiological data highlighted a heterogeneous diffusion of the misuse of this mixture, which is not exclusively linked to a specific type of user (African-American teenagers, athletes, and rappers), as previously reported in American newspapers and in the social media. New digital tools should be taken into consideration for further social and medical evaluations of this phenomenon.

Abstract: The role of the elemental carbon (EC), in synergy with hygroscopic ionic species, was investigated to study the formation of electrical bridging phenomena once the aerosol deliquescence is achieved. Ambient aerosol samples were collected on hydrophobic surfaces in urban and rural sites in Northern Italy, their conductance was measured in an Aerosol Exposure Chamber (AEC) while varying the relative humidity. An electric signal was detected on 64% of the collected samples with conductance values (11.20 &plusmn, 7.43 &mu, S) above the failure threshold (1 &mu, S) of printed circuit boards. The ionic content was higher for non-electrically conductive samples (43.7 &plusmn, 5.6%) than for electrically conductive ones (37.1 &plusmn, 5.6%). Conversely, EC was two times higher for electrically conductive samples (26.4 &plusmn, 4.1 &mu, g cm&minus, 2, 8.4 &plusmn, 1.7%) than for non-electrical ones (12.0 &plusmn, 5.2 &plusmn, 1.9%) suggesting that the synergy between the ionic and carbonaceous fractions is necessary to promote a bridging phenomenon. Synthetic aerosols (EC only, saline only, mixed saline and EC) were generated in laboratory and their conductance was measured in the AEC to verify the ambient results. Only in case of a contemporary presence of both EC and ionic components the bridging phenomenon occurred in keeping with the theoretical deliquescence values of each salt (R2 = 0.996).

Abstract: BackgroundSexual response in obsessive–compulsive disorder (OCD) research and practice is overlooked. According to the Dual Control Model, satisfactory sexual response is based upon a balance of sexual excitation and inhibition. The assessment of sexual response in OCD may have clinical implications, such as the integration of sex therapy in psychotherapeutic intervention. The present study was aimed at comparing sexual excitation and inhibition levels between OCD patients and matched control subjects, and investigating whether obsessive beliefs might predict sexual excitation/inhibition.MethodsSeventy-two OCD patients (mean age ± standard deviation [SD]: 34.50 ± 10.39 years) and 72 matched control subjects (mean age ± SD: 34.25 ± 10.18) were included (62.50% men and 37.50% women in both groups). The Obsessive Compulsive Inventory-Revised (OCI-R), the Obsessive Beliefs Questionnaire-46 (OBQ-46), and the Sexual Inhibition/Sexual Excitation Scales (SIS/SES) were administered.ResultsPatients with OCD showed significantly higher levels of sexual excitation, inhibition due to threat of performance failure, and inhibition due to threat of performance consequences than the controls. In addition, the patients with more severe symptoms showed lower excitation than those with less severe symptoms, and those with higher perfectionism had stronger inhibition due to threat of performance failure than those with lower perfectionism.ConclusionsThis is the first study exploring sexual response in OCD according to the Dual Control Model. Sexual response is an impaired quality of life outcome in OCD that should be assessed in routine clinical practice. These findings support the importance of addressing specific obsessive beliefs to improve sexuality in OCD patients.

Abstract: To determine the inter-rater reliability (IRR) of a procedure-specific checklist scored in a binary fashion for the evaluation of surgical skill and whether it meets a minimum level of agreement (≥0.8 between 2 raters) required for high-stakes assessment.In a prospective randomized and blinded fashion, and after detailed assessment training, 10 Arthroscopy Association of North America Master/Associate Master faculty arthroscopic surgeons (in 5 pairs) with an average of 21 years of surgical experience assessed the video-recorded 3-anchor arthroscopic Bankart repair performance of 44 postgraduate year 4 or 5 residents from 21 Accreditation Council for Graduate Medical Education orthopaedic residency training programs from across the United States.No paired scores of resident surgeon performance evaluated by the 5 teams of faculty assessors dropped below the 0.8 IRR level (mean = 0.93; range 0.84-0.99; standard deviation = 0.035). A comparison between the 5 assessor groups with 1 factor analysis of variance showed that there was no significant difference between the groups (P = .205). Pearson's product-moment correlation coefficient revealed a strong and statistically significant negative correlation, that is, -0.856 (P.000), indicating that as intra-operative error rate scores increased, the IRR decreased.Arthroscopy Association of North America shoulder faculty raters from across the United States showed high levels of IRR in the assessment of an arthroscopic 3-anchor Bankart repair procedure. All paired assessments were above the 0.8 level and the mean IRR of all resident assessments was 0.93, indicating that they could be used for high-stakes decisions.With the move toward outcomes-based performance evaluation for graduate medical education, high-stakes assessments of surgical skill will require robust, reliable measurement tools that are able to withstand challenge. Surgical checklists employing metrics scored in a binary fashion meet the need and can show a high (80%) IRR.

Abstract: Sexuality is understudied in Obsessive-Compulsive Disorder (OCD). According to the Dual Control Model, low sexual excitation (SE) and high sexual inhibition (SI) are indicative of a higher probability of experiencing a sexual dysfunction. The present study investigated SE and SI in OCD patients compared with controls. It was hypothesized that OCD patients report lower SE and higher SI than controls. Given their potential role as inhibitors of sexual response, it was hypothesized that in the OCD group higher disgust propensity/sensitivity, contamination/washing symptoms, unacceptable thoughts, and obsessive beliefs predicted lower SE, higher SI due to Threat of Performance Failure, and higher SI due to Threat of Performance Consequences. Seventy-two OCD patients and 72 controls matched on gender/age completed the Disgust Propensity and Sensitivity Scale-Revised, Obsessive Beliefs Questionnaire-46, Obsessive Compulsive Inventory-Revised, and Sexual Inhibition/Sexual Excitation Scales. OCD patients had higher SE, SI due to Threat of Performance Failure, and SI due to Threat of Performance Consequences than controls. In the OCD group, higher disgust sensitivity, SI due to Threat of Performance Consequences, and perfectionism predicted higher SI due to Threat of Performance Failure. Higher SI due to Threat of Performance Failure and contamination/washing symptoms predicted higher SI due to Threat of Performance Consequences. These findings highlight the presence of sexual difficulties in OCD patients, particularly a higher SE and SI. The latter is especially relevant in those patients with higher disgust sensitivity, contamination/washing symptoms and perfectionism. Psychotherapists should assess and target sexuality during clinicalpractice with OCD patients.

Abstract: Overdose of opioids is the number one cause of accidental death in the United States, and surgeons are overprescribing these medications. The aim of this study was to assess the feasibility of implementing postoperative opioid prescribing guidelines for general surgery procedures at a public hospital, where patients have lower socioeconomic status, public insurance, and limited access to care. We implemented a quality improvement project, which included in-service training for surgical staff and distribution of standardized guidelines. An infographic for patients was created to facilitate education on postoperative pain management. Pre- and postintervention opioid prescriptions and emergency room visits were compared for patients undergoing common general surgery procedures (inguinal hernia repair, appendectomy, and laparoscopic cholecystectomy). 48. Investigating Gender Differences in Faculty Evaluations by Trainees in a Gender-Balanced General Surgery Program

Abstract: Purpose Women account for 21% of faculty positions in general surgery. In fields with lower female representation, female faculty receive lower evaluation scores by trainees compared to male faculty. At 42%, the female faculty representation in our general surgery department doubles the national average. We sought to determine if variations in faculty evaluations would be observed in a more gender-balanced general surgery program. Methods Two years of faculty teaching evaluations by residents in a general surgery residency program were collected from the MedHub system. Total 3277 resident evaluations of 26 faculty members (11 female, 15 male) were analyzed. Seven areas (scored 1-7, with 1 = needs improvement and 7 = outstanding) were examined. Chi-square test was used to compare the percentage of male and female faculty members who scored a 6 or 7 in each category, and multivariate logistic regression analysis was used to determine the association of gender with the evaluation score, while adjusting for the number of encounters between the trainee and the faculty member. Results There were no significant differences between male and female faculty in the “overall” evaluation score, nor in the “practice-based learning” and the “interpersonal and communication skills” categories. Female faculty had statistically significantly higher scores in “patient care”, “professionalism,” and “systems-based care” categories, whereas male faculty had higher evaluations in the “medical knowledge” category. Conclusion In a general surgery residency program with a relatively gender-balanced faculty, there was no gender difference in the “overall” evaluation of faculty by residents. However, there were gender differences in specific domains. These findings suggest that gender balance in teaching faculty may help eliminate previously observed teaching evaluation bias in the traditionally male dominated fields.

Abstract: Background The objective of this multi-center study was to examine the follow-up trends after emergency department (ED) discharge in a large and socioeconomically diverse patient population. Methods We performed a 3-year retrospective analysis of adult patients with acutely symptomatic hernias who were discharged from the EDs of five geographically diverse hospitals. Results Of 674 patients, 288 (43%) were evaluated in the clinic after discharge from the ED and 253 (37%) underwent repair. Follow-up was highest among those with insurance. A total of 119 patients (18%) returned to the ED for hernia-related complaints, of which 25 (21%) underwent urgent intervention. Conclusion The plan of care for patients with acutely symptomatic hernias discharged from the ED depends on outpatient follow-up, but more than 50% of patients are lost to follow-up, and nearly 1 in 5 return to the ED. The uninsured are at particularly high risk.

Abstract: The purpose of this study was to perform an evidence-based, expert consensus survey using the Delphi panel methodology to develop recommendations for the treatment of degenerative meniscus tears.Twenty panel members were asked to respond to 10 open-ended questions in rounds 1 and 2. The results of the first 2 rounds served to develop a Likert-style questionnaire for round 3. In round 4, the panel members outside consensus were contacted and asked to either change their score in view of the group's response or argue their case. The level of agreement for round 4 was defined as 80%.There was 100% agreement on the following items: insidious onset, physiological part of aging, tears often multiplanar, not all tears cause symptoms, outcomes depend on degree of osteoarthritis, obesity is a predictor of poor outcome, and younger patients (50 years) have better outcomes. There was between 90% and 100% agreement on the following items: tears are nontraumatic, radiographs should be weightbearing, initial treatment should be conservative, platelet-rich plasma is not a good option, repairable and peripheral tears should be repaired, microfracture is not a good option for chondral defects, the majority of patients obtain significant improvement and decrease in pain with surgery but results are variable, short-term symptoms have better outcomes, and malalignment and root tears have poor outcomes.This consensus statement agreed that degenerative meniscus tears are a normal part of aging. Not all tears cause symptoms and, when symptomatic, they should initially be treated nonoperatively. Repairable tears should be repaired. The outcome of arthroscopic partial meniscectomy depends on the degree of osteoarthritis, the character of the meniscus lesion, the degree of loss of joint space, the amount of malalignment, and obesity. The majority of patients had significant improvement, but younger patients and patients with short-term symptoms have better outcomes.Level V - expert opinion.
