Abstract

Introduction: Dialysis patients have a 10–25 times higher risk of reactivation of tuberculosis (TB). In this study, we investigated the diagnostic ability of QuantiFERON (QFT)‐plus for TB in hemodialysis patients. QFT‐plus, an interferon gamma release assay, is characterized by its use of CD4 and CD8 T cell signals. Methods: Hemodialysis patients aged 20 years or older who underwent QFT‐plus measurement in our hospital were included, inclusion criteria being fever above 37°C, high inflammatory response, and infiltrative pulmonary shadows. Results: Forty‐six patients were enrolled. Of these, 15% were QFT positive, 4% were diagnosed with active TB, 76% were QFT negative, 8% had inconclusive results. Sensitivity, specificity, positive predictive value, and negative predictive value were 100%, 87.5%, 28%, and 100%, respectively. Conclusions: QFT‐plus may be useful for the diagnosis of active TB in dialysis patients. Further studies in cohorts with larger sample sizes are expected. [ABSTRACT FROM AUTHOR]

Abstract

Objective: To evaluate the efficacy and safety of direct hemoperfusion using a polymyxin B‐immobilized polystyrene column (PMX‐DHP) in severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2)‐positive pneumonia patients. Methods: This study was a case series conducted at a designated infectious diseases hospital. Twelve SARS‐CoV‐2‐positive patients with partial pressure of arterial oxygen/percentage of inspired oxygen (P/F) ratio < 300 were treated with PMX‐DHP on two consecutive days each during hospitalization. We defined day 1 as the first day when PMX‐DHP was performed. PMX‐DHP efficacy was assessed on days 7 and 14 after the first treatment based on eight categories. Subsequently, improvement in P/F ratio and urinary biomarkers on days 4 and 8, malfunctions, and ventilator and extracorporeal membrane oxygenation avoidance rates were also evaluated. Results: On day 14 after the first treatment, disease severity decreased in 58.3% of the patients. P/F ratio increased while urine β2‐microglobulin decreased on days 4 and 8. Cytokine measurement pre‐ and post‐PMX‐DHP revealed decreased levels of interleukin‐6 and the factors involved in vascular endothelial injury, including vascular endothelial growth factor. Twenty‐two PMX‐DHPs were performed, of which seven and five PMX‐DHPs led to increased inlet pressure and membrane coagulation, respectively. When the membranes coagulated, the circuitry needed to be reconfigured. Circuit problems were usually observed when D‐dimer and fibrin degradation product levels were high before PMX‐DHP. Conclusions: Future studies are expected to determine the therapeutic effect of PMX‐DHP on COVID‐19. Because of the relatively high risk of circuit coagulation, coagulation capacity should be assessed beforehand. [ABSTRACT FROM AUTHOR]

Abstract

当院は 1971 年血液透析を開始し 2005 年末で 35 年経過した． この間の総患者数 1,169 例を対象に， 導入原因疾患， 導入年齢， 生存率， 死亡原因の推移を日本透析医学会の行っている全国統計調査と比較し， さらに慢性糸球体腎炎 （CGN）， 糖尿病性腎症 （DN） の 2 群に分けて比較検討した． その結果， 導入原因疾患において DN は全国平均以上の増加で， 2005 年末の透析患者における原疾患は DN が CGN とほぼ同率となった． 導入時平均年齢は 35 年間で 25 歳高齢化し， 当初 CGN と DN で年齢差がみられたが， CGN 患者の高齢化により近年では両群とも 65 歳前後と差がなくなっている． 透析年数は 2005 年末の透析患者でみると， 5 年未満が 55.5%を占めている． CGN の平均透析年数は 10.9 年であるのに対し， DN は 3.8 年であった． 近年， 長期生存率の低下がみられ， 10 年生存率は CGN 54.6%であるのに対し DN は 22.4%と予後不良であった． 透析患者の主な死亡原因では， CGN 及び DN ともに心不全死の減少がみられたが， 最近の傾向として DN の感染症死， 及び CGN の尿毒症死が著明に増加していた． 今後生命予後の不良な糖尿病患者の感染症対策， 及び高齢者の透析管理が課題と考えられた．

Abstract

A three-dimensional Tyranno fibre reinforced SiC composite was fabricated by infiltration of molten polycarbosilane into three-dimensional textile preform (volume fraction; 39%) in an autoclave followed by pyrolysis in argon up to 1200 °C. An attempt to improve the density of the composite was done by repeated infiltration and pyrolysis. As a result of 10 times repetition, the density of the composite was increased to 2.4 g cm−3 (relative density; 83%, open porosity; 5%). Bending strength and fracture toughness were 420 MPa and 11.5 MPa m12, respectively. The composite showed semistable fracture behaviour because of bridging of fibre parallel with the tensile direction.

Abstract

This paper presents an error-tolerant video retrieval method based on the shot composition sequence in a scene. Conventional video players in the home can not access interesting scenes directly because they offer only play, fast-forward, and rewind. What is needed is an easy-to-use video scene player that can directly access scenes important to the user. This paper presents an error-tolerant video retrieval method that assesses the shot composition sequence in each scene. Our method has three characteristics: (1) easy query generation since the regular interface is used for query generation; (2) similar video scene retrieval based on the sequence of symbols created by shot composition clustering to effectively identify scenes similar to the query; (3) fast similarity measurement based on sequences of symbols. An experiment shows that the proposed system can retrieve video scenes similar to the query quickly as well as allowing semantic retrieval to be constructed in a non-supervised way.

再処理プロセスやスクラップ燃料湿式回収プロセスの溶媒抽出に使用している硝酸廃液処理法として,室温以下の低温技術(例えば,低温真空蒸留法,凍結真空乾燥法等)の適用性に関する研究を進めている。この低温真空蒸留法による硝酸廃液処理について検討する際,対象とする各種成分の物性データが不可欠である。今回,これらの物性データのうち,-10$\sim$50$^{\circ}C$の温度領域での気液平衡データについて,既報の硝酸/水系気液平衡データを基に,NRTL式及びWilson式で解析した後,定温及び定圧時のデータを温度をパラメータに計算し図表化した。なお,計算に使用したNRTL式のパラメータについては温度依存性を考慮し,気相の非理想性は無視した。これらの低温領域でのデータは,低温真空蒸留法による硝酸廃液処理について設計検討する際に役立つものと期待される。, None

Abstract

再処理プロセスやスクラップ燃料湿式回収プロセスに用いているTBP-nドデカン系混合溶媒のソルトフリー再生処理を目指して、凍結真空乾燥法、低温真空蒸留法、低温晶析法等の低温技術による技術開発を行っている。これらの低温技術の中から低温晶析法を取り上げ、模擬TBP-nドデカン系混合溶媒を用いて、TBPとnドデカンの粗分離試験を行った結果、次に示す結論を得た。(1)TBPの希釈剤である高融点物質のnドデカンが最初に結晶化し、その結晶成長速度は冷媒の温度に依存すると共に、TBPの取り込み率に影響する。最終的にはnドデカン中にTBPを取り込みシャーベット状になる。(2)TBPはnドデカンの結晶成長の過程で未結晶部に押し出されるため、nドデカンの結晶とTBPが粗分離できる。(3)nドデカン結晶中に取り込まれたTBPは、冷却エタノールと接触することにより洗浄できる。(4)溶媒再生プロセスの前処理に本低温晶析法を採用することにより、従来考えていた低温真空蒸留法で処理する対象物を削減する可能性があると共に、主再生処理プロセスとしても適用性がある。, None

Abstract

LOTUSプロセスの一環として,低温処理技術による処理の可能性について技術開発を進める。低温下での硝酸ウラニル・プルトニウム溶液の挙動観察,凍結真空乾燥法による除染効率及び低温晶析法による晶析分離試験を行う。硝酸ウラニル溶液は-44$^{\circ}C$付近で水 硝酸 硝酸ウラニルの3成分系の共晶点が確認され,-26$^{\circ}C$付近で硝酸ウラニルが晶析したが,硝酸プルトニムは-60$^{\circ}C$まで冷却しても晶析しなかった。このことから晶析法によるウラン・プルトニウムの相互分離の可能性を見い出した。また,硝酸プルトニウムの除染係数は回収液の$\alpha$放射能濃度がバックグランドレベルとなり,104$\sim$106の結果が得られた。核燃料サイクルにおいて取り扱う硝酸ウラニル溶液,硝酸プルトニウム溶液等を凍結真空乾燥法及び低温晶析法で処理する方法は,試薬を添加せずに高除染効果が得られ,相互分離が可能であり,クローズドシステム化が図られるホ, None
