- Abstract
Disorders of consciousness are neurological conditions characterized by impaired arousal and awareness of self and environment. Behavioural responses are absent or are present but fluctuate. Disorders of consciousness are commonly encountered as a consequence of both acute and chronic brain injuries, yet reliable epidemiological estimates would require inclusive, operational definitions of the concept, as well as wider knowledge dissemination among involved professionals. Whereas several manifestations have been described, including coma, vegetative state/unresponsive wakefulness syndrome and minimally conscious state, a comprehensive neurobiological definition for disorders of consciousness is still lacking. The scientific literature is primarily observational, and studies-specific aetiologies lead to disorders of consciousness. Despite advances in these disease-related forms, there remains uncertainty about whether disorders of consciousness are a disease-agnostic unitary entity with a common mechanism, prognosis or treatment response paradigm. Our knowledge of disorders of consciousness has also been hampered by heterogeneity of study designs, variables, and outcomes, leading to results that are not comparable for evidence synthesis. The different backgrounds of professionals caring for patients with disorders of consciousness and the different goals at different stages of care could partly explain this variability. The Prospective Studies working group of the Neurocritical Care Society Curing Coma Campaign was established to create a platform for observational studies and future clinical trials on disorders of consciousness and coma across the continuum of care. In this narrative review, the author panel presents limitations of prior observational clinical research and outlines practical considerations for future investigations. - Abstract
The convergence of an interdisciplinary team of neurocritical care specialists to organize the Curing Coma Campaign is the first effort of its kind to coordinate national and international research efforts aimed at a deeper understanding of disorders of consciousness (DoC). This process of understanding includes translational research from bench to bedside, descriptions of systems of care delivery, diagnosis, treatment, rehabilitation, and ethical frameworks. The description and measurement of varying confounding factors related to hospital care was thought to be critical in furthering meaningful research in patients with DoC. Interdisciplinary hospital care is inherently varied across geographical areas as well as community and academic medical centers. Access to monitoring technologies, specialist consultation (medical, nursing, pharmacy, respiratory, and rehabilitation), staffing resources, specialty intensive and acute care units, specialty medications and specific surgical, diagnostic and interventional procedures, and imaging is variable, and the impact on patient outcome in terms of DoC is largely unknown. The heterogeneity of causes in DoC is the source of some expected variability in care and treatment of patients, which necessitated the development of a common nomenclature and set of data elements for meaningful measurement across studies. Guideline adherence in hemorrhagic stroke and severe traumatic brain injury may also be variable due to moderate or low levels of evidence for many recommendations. This article outlines the process of the development of common data elements for hospital course, confounders, and medications to streamline definitions and variables to collect for clinical studies of DoC.

To develop consensus-based recommendations for the management of adult and paediatric patients with NORSE/FIRES based on best evidence and experience.The Delphi methodology was followed. A facilitator group of 9 experts was established, who defined the scope, users and suggestions for recommendations. Following a review of the current literature, recommendation statements concerning diagnosis, treatment and research directions were generated which were then voted on a scale of 1 (strongly disagree) to 9 (strongly agree) by a panel of 48 experts in the field. Consensus that a statement was appropriate was reached if the median score was greater or equal to 7, and inappropriate if the median score was less than or equal to 3. The analysis of evidence was mapped to the results of each statement included in the Delphi survey.Overall, 85 recommendation statements achieved consensus. The recommendations are divided into five sections: 1) disease characteristics, 2) diagnostic testing and sampling, 3) acute treatment, 4) treatment in the post-acute phase, and 5) research, registries and future directions in NORSE/FIRES. The detailed results and discussion of all 85 statements are outlined herein. A corresponding summary of findings and practical flowsheets are presented in a companion article.This detailed analysis offers insight into the supporting evidence and the current gaps in the literature that are associated with expert consensus statements related to NORSE/FIRES. The recommendations generated by this consensus can be used as a guide for the diagnosis, evaluation, and management of patients with NORSE/FIRES, and for planning of future research.

- Abstract
New-onset refractory status epilepticus (NORSE) is a clinical presentation, not a specific diagnosis, in a patient without active epilepsy or other preexisting relevant neurologic disorder, with new onset of refractory status epilepticus (RSE) that does not resolve after 2 or more rescue medications, without a clear acute or active structural, toxic, or metabolic cause. Febrile infection-related epilepsy syndrome is a subset of NORSE in which fever began at least 24 hours prior to the RSE. Both terms apply to all age groups. Until recently, NORSE was a poorly recognized entity without a consistent definition or approach to care. We review the current state of knowledge in NORSE and propose a roadmap for future collaborative research. Research investigating NORSE should prioritize the following 4 domains: (1) clinical features, etiology, and pathophysiology; (2) treatment; (3) adult and pediatric evaluation and management approaches; and (4) public advocacy, professional education, and family support. - Abstract
A systematic study of the atomic and electronic structure of the γ-TiAl(111)/α-Al
2 O3 (0001) interface with intermediate metal (Nb, Mo, Ni, Re) and oxide (Nb2 O5 , MoO3 ) layers has been performed by the projector augmented-wave method within density functional theory. The work of separation at the interfaces in dependence on the cleavage plane has been calculated. It is shown that a high adhesion energy obtained at the interface with the O-terminated α-Al2 O3 is decreased at the γ-TiAl/Me interface but it remains enough high at the Me/α-Al2 O3 (0001)O interface due to a large ionic contribution to the chemical bonding. The influence of formation of intermediate impurity oxide layers on the adhesive properties of the alloy/oxide interface is discussed as well. The obtained results indicate that the fracture will occur inside the impurity oxide or its interface with the alloy. [ABSTRACT FROM AUTHOR]- Published

To describe the demographics, etiologies, types of status epilepticus (SE), and outcomes in people with refractory and super-refractory SE from around the world, we prospectively collected cases of refractory SE (RSE) treated with continuous intravenous anesthetic drugs in an intensive care unit setting through online questionnaires using "active surveillance." We collected information about 776 cases of RSE in 50 countries over 4 years. Control of SE was achieved in 74% of the cases. Neurologic outcomes were poor in 41% of patients, and 24% died. Good outcome was associated with younger age and a history of epilepsy. Etiology strongly influenced the outcome. Patients from Asia were younger, more frequently presented with convulsive SE, and were more frequently affected by infectious etiologies when compared with patients from Europe and the Americas. Despite these differences, outcomes were similar in all countries. Demographics of patients with RSE in a global audit are similar to those in prior single center series, providing evidence of generalizability of those studies. Important differences exist among patients with RSE from different regions of the world, but these do not seem to significantly influence patient outcomes.

- Abstract
Objective: To determine the clinical correlates bilateral independent periodic discharges (BIPDs) and their association with electrographic seizures and outcome. Methods: Retrospective case-control study of patients with BIPDs compared to patients without periodic discharges (“No PDs”) and patients with lateralized periodic discharges (“LPDs”), matched for age, etiology and level of alertness. Results: We included 85 cases and 85 controls in each group. The most frequent etiologies of BIPDs were stroke, CNS infections, and anoxic brain injury. Acute bilateral cerebral injury was more common in the BIPDs group than in the No PDs and LPDs groups (70% vs. 37% vs. 35%). Electrographic seizures were more common with BIPDs than in the absence of PDs (45% vs. 8%), but not than with LPDs (52%). Mortality was higher in the BIPDs group (36%) than in the No PDs group (18%), with fewer patients with BIPDs achieving good outcome (moderate disability or better; 18% vs. 36%), but not than in the LPDs group (24% mortality, 26% good outcome). In multivariate analyses, BIPDs remained associated with mortality (OR: 3.0 [1.4–6.4]) and poor outcome (OR: 2.9 [1.4–6.2]). Conclusion: BIPDs are caused by bilateral acute brain injury and are associated with a high risk of electrographic seizures and of poor outcome. - Abstract
We convened an international group of experts to standardize definitions of New-Onset Refractory Status Epilepticus (NORSE), Febrile Infection-Related Epilepsy Syndrome (FIRES), and related conditions. This was done to enable improved communication for investigators, physicians, families, patients, and other caregivers. Consensus definitions were achieved via email messages, phone calls, an in-person consensus conference, and collaborative manuscript preparation. Panel members were from 8 countries and included adult and pediatric experts in epilepsy, electroencephalography (EEG), and neurocritical care. The proposed consensus definitions are as follows: NORSE is a clinical presentation, not a specific diagnosis, in a patient without active epilepsy or other preexisting relevant neurological disorder, with new onset of refractory status epilepticus without a clear acute or active structural, toxic or metabolic cause. FIRES is a subcategory of NORSE, applicable for all ages, that requires a prior febrile infection starting between 2 weeks and 24 hours prior to onset of refractory status epilepticus, with or without fever at onset of status epilepticus. Proposed consensus definitions are also provided for Infantile Hemiconvulsion-Hemiplegia and Epilepsy syndrome (IHHE) and for prolonged, refractory and super-refractory status epilepticus. This document has been endorsed by the Critical Care EEG Monitoring Research Consortium. The Fifth Neurocritical Care Research Network (NCRN) Conference held in Boca Raton, Florida, in September of 2018 was devoted to challenging the current status quo and examining the role of the Neurocritical Care Society (NCS) in driving the science and research of neurocritical care. The aim of this in-person meeting was to set the agenda for the NCS's Neurocritical Care Research Central, which is the overall research arm of the society. Prior to the meeting, all 103 participants received educational content (book and seminar) on the 'Blue Ocean Strategy®,' a concept from the business world which aims to identify undiscovered and uncontested market space, and to brainstorm innovative ideas and methods with which to address current challenges in neurocritical care research. Three five-member working groups met at least four times by teleconference prior to the in-person meeting to prepare answers to a set of questions using the Blue Ocean Strategy concept as a platform. At the Fifth NCRN Conference, these groups presented to a five-member jury and all attendees for open discussion. The jury then developed a set of recommendations for NCS to consider in order to move neurocritical care research forward. We have summarized the topics discussed at the conference and put forward recommendations for the future direction of the NCRN and neurocritical care research in general. [ABSTRACT FROM AUTHOR]

The investigation of hydrogen, boron and carbon sorption properties at the Σ5(310) symmetrical tilt grain boundary (GB) and (310) free surface (FS) in B2 Ti-based alloys was carried out by the plane-wave pseudopotential method within density functional theory. The most preferential positions for interstitial impurities at GB were determined. It was shown that impurities sorption energies at GB depend strongly on their local environment. The analysis of electronic properties allows us to establish the microscopic na-ture of chemical bonding of all considered impurities at GB. It was shown that H decreases more signifi-cantly the surface energies than the GB energy in contrast to B and C. This results in decreasing the Grif-fith work that indicates also the decrease of the strength of grain boundary. The segregation of H at the GB makes intergranular fracture much easier because the bonding between metal atoms, which are neigh-bors of H, is weakened. The segregation behavior of hydrogen confirms it as an embrittler for B2 Ti-based alloys. At the same time boron and carbon segregation contrast to hydrogen increase the GB cohesion. A search for a narrow Higgs boson resonance in the diphoton mass spectrum is presented based on data corresponding to 10fb -1 of integrated luminosity collected by the CDF experiment from proton-antiproton collisions at s=1.96TeV. To increase the sensitivity of the search, we employ a multivariate discriminant technique for the first time in this channel at CDF. No evidence of signal is observed, and upper limits are set on the cross section times branching ratio of the resonant state as a function of the Higgs boson mass. The limits are interpreted in the context of the standard model with an expected (observed) limit on the cross section times branching ratio of 9.9 (17.0) times the standard model prediction at the 95% credibility level for a Higgs boson mass of 125GeV/c 2. Moreover, a Higgs boson with suppressed couplings to fermions is excluded for masses below 114GeV/c 2 at the 95% credibility level. © 2012 Elsevier B.V.

We report a measurement of the bottom-strange meson mixing phase βs using the time evolution of Bs0→J/ψ(→μ+μ-)ϕ(→K+K-) decays in which the quark-flavor content of the bottom-strange meson is identified at production. This measurement uses the full data set of proton-antiproton collisions at s=1.96 TeV collected by the Collider Detector experiment at the Fermilab Tevatron, corresponding to 9.6 fb-1 of integrated luminosity. We report confidence regions in the two-dimensional space of βs and the Bs0 decay-width difference ΔΓs and measure βs∈[-π/2,-1.51]∪[-0.06,0.30]∪[1.26,π/2] at the 68% confidence level, in agreement with the standard model expectation. Assuming the standard model value of βs, we also determine ΔΓs=0.068±0.026(stat)±0.009(syst) ps-1 and the mean Bs0 lifetime τs=1.528±0.019(stat)±0.009(syst) ps, which are consistent and competitive with determinations by other experiments., This work was supported by the U.S. Department of Energy and National Science Foundation; the Italian Istituto Nazionale di Fisica Nucleare; the Ministry of Education, Culture, Sports, Science and Technology of Japan; the Natural Sciences and Engineering Research Council of Canada; the National Science Council of the Republic of China; the Swiss National Science Foundation; the A. The atomic and electronic structure of the interfaces between metals with body-centered cubic (bcc) and face-centered cubic (fcc) structures and zirconium dioxide is studied systematically using the ab initio methods of the electron density functional theory (DFT). It is shown that high adhesion properties can be attained at the nonstoichiometric polar Me(001)/ZrO(001) interface with bcc metals from the middle of the 4d-5d periods (Mo, Ta, W, and Nb). Charge transfer from the metal to the oxide substrate ensures the strong ionic chemical bond on the metal-ceramic interfaces. The structural and electronic factors responsible for lowering of adhesion at differently oriented interfaces are analyzed. It is shown that a decrease of adhesion at the (110) nonpolar stoichiometric interface is due to an increase in the interfacial spacing as well as a decrease in the number of metal-oxygen bonds. The effect of doping with oxides (CaO, MgO, and YO) stabilizing zirconium dioxide at low temperatures on the adhesion energy at the Me(001)/ZrO(001) interface is analyzed. [ABSTRACT FROM AUTHOR]

49. Archean Synvolcanic Intrusions and Volcanogenic Massive Sulfide at the Genex Mine, Kamiskotia Area, Timmins, Ontario.

U-Pb geochronology and detailed field mapping has resulted in a new subdivision of the Kamiskotia Volcanic Complex into Lower and Upper Kamiskotia strata with a 3.5-m.y. volcanic hiatus between the two. The hiatus is interpreted as an unconformity, in common with newly recognized unconformities throughout the green-stone belt. The Genex, and other volcanogenic massive sulfide (VMS) deposits in the area, occur within the Upper Kamiskotia strata. Volcanic facies mapping and reconstruction indicate that the Genex VMS deposit formed within a volcanic graben where synvolcanic mafic and intermediate sills and dikes, with peperitic and locally pillowed contacts, define a proximal volcanic vent area. Sulfide mineralization at Genex occurs in three zones, the first hosted in pillow breccia and hyaloclastite, the second occurring at the contact between felsic tuff and an intermediate intrusion, and the third hosted within the intermediate intrusion. The mineralization represents subsea-floor replacement sulfides localized within zones of higher primary permeability. Although the synvolcanic mafic intrusions are not directly related to the Genex VMS mineralization they are indicative of a high heat-flow thermal regime. The synvolcanic faults that controlled the emplacement of synvolcanic dikes also provided conduits for the hydrothermal fluids responsible for the mineralization and alteration at Genex. This high-level mafic dike and sill complex was previously correlated with the upper zone of the Kamiskotia Gabbroic Complex but is now interpreted to be younger and correlative with the Upper Kamiskotia Volcanic Complex. New geochronological data also indicate that the Kamiskotia Gabbroic Complex is older than the VMS-hosting Upper Kamiskotia strata and was emplaced into the Lower Kamiskotia Volcanic Complex. This negates the possibility, of a genetic relationship between the Genex and other Kamiskotia VMS deposits within the Kamiskotia Gabbroic Complex. The small size of the synvolcanic intrusions indicates that they were also not the heat source for the Genex hydrothermal alteration system. However, their spatial coincidence suggests that this part of the Kamiskotia Volcanic Complex was the focus of long-lived intrusive activity and high heat flow that defined a thermal corridor encompassing the VMS deposits in the Kamiskotia Volcanic Complex. 50. Recurrent strokes in two patients with poems syndrome and castleman's disease.

- Abstract
POEMS syndrome is a disorder characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin changes. Castleman's disease is a rare lymphoproliferative disorder that can be present in patients with POEMS syndrome. Previous case reports have documented the association between POEMS syndrome and arterial and venous thromboses of the coronary and lower limb arteries. However, only 3 patients have ever been described with POEMS syndrome, Castleman's disease, and stroke. Two additional patients with this combination of disorders are reported herein.
