Contains fulltext : 300177.pdf (Publisher’s version ) (Open Access), Adequate detection of the histopathological extraprostatic extension (EPE) of prostate cancer (PCa) remains a challenge using conventional radiomics on 3 Tesla multiparametric magnetic resonance imaging (3T mpMRI). This study focuses on the assessment of artificial intelligence (AI)-driven models with innovative MRI radiomics in predicting EPE of prostate cancer (PCa) at a lesion-specific level. With a dataset encompassing 994 lesions from 794 PCa patients who underwent robot-assisted radical prostatectomy (RARP) at two Dutch hospitals, the study establishes and validates three classification models. The models were validated on an internal validation cohort of 162 lesions and an external validation cohort of 189 lesions in terms of discrimination, calibration, net benefit, and comparison to radiology reporting. Notably, the achieved AUCs ranged from 0.86 to 0.91 at the lesion-specific level, demonstrating the superior accuracy of the random forest model over conventional radiological reporting. At the external test cohort, the random forest model was the best-calibrated model and demonstrated a significantly higher accuracy compared to radiological reporting (83% vs. 67%, p = 0.02). In conclusion, an AI-powered model that includes both existing and novel MRI radiomics improves the detection of lesion-specific EPE in prostate cancer.

Abstract

For millennia, ceramics have been densified via sintering in a furnace, a time-consuming and energy-intensive process. The need to minimize environmental impact calls for new physical concepts beyond large kilns relying on thermal radiation and insulation. Here, we realize ultrarapid heating with intense blue and UV-light. Thermal management is quantified in experiment and finite element modelling and features a balance between absorbed and radiated energy. With photon energy above the band gap to optimize absorption, bulk ceramics are sintered within seconds and with outstanding efficiency (~2 kWh/kg) independent of batch size. Sintering on-the-spot with blacklight as a versatile and widely applicable power source is demonstrated on ceramics needed for energy storage and conversion and in electronic and structural applications foreshadowing economic scalability.

Abstract

By providing green spaces for recreation and value formation, sustainable food production, social capital as well as education and skills training, urban community gardens have the potential to contribute to a just and sustainable food culture in cities. The community garden OxGrow in Oxford, UK is one such initiative striving for the creation of a food system that promotes environmental and social justice. However, like many other urban farming organisations worldwide, OxGrow struggles with social inclusion and participation of people living in low-income and food poor neighbourhoods. This reflects common issues around food justice and urban socio-economic disparities among Oxford’s communities, including inequalities in access to healthy, sustainable and affordable food. While previous studies have shown that urban community gardens have the potential to combat food poverty, little is known about the exclusionary dynamics of their establishment and how such initiatives can address structural challenges of participation. By applying Social Practice Theory to the case, the study aims to shed light on the material systems of provision as well as social and cultural conditions that shape participation at OxGrow. The theoretical framework explores how practices are recruited, reproduced and diffused based on the elements of materials, competences and meaning as well as the assessment of networks and spaces. During the case study conducted in Oxford, focus group discussions and interviews with community gardening practitioners at OxGrow, citizens living in low-income and food poor neighbourhoods as well as experts on urban farming and food poverty were conducted. The analysis showed that the practice of community gardening can only be moved by also focusing on the elements of related practice bundles. Abstract

Transcranial magnetic stimulation (TMS) in combination with neuroimaging techniques allows to measure the effects of a direct perturbation of the brain. When coupled with high-density electroencephalography (TMS/hd-EEG), TMS pulses revealed electrophysiological signatures of different cortical modules in health and disease. However, the neural underpinnings of these signatures remain unclear. Here, by applying multimodal analyses of cortical response to TMS recordings and diffusion magnetic resonance imaging (dMRI) tractography, we investigated the relationship between functional and structural features of different cortical modules in a cohort of awake healthy volunteers. For each subject, we computed directed functional connectivity interactions between cortical areas from the source-reconstructed TMS/hd-EEG recordings and correlated them with the correspondent structural connectivity matrix extracted from dMRI tractography, in three different frequency bands (α, β, γ) and two sites of stimulation (left precuneus and left premotor). Each stimulated area appeared to mainly respond to TMS by being functionally elicited in specific frequency bands, that is, β for precuneus and γ for premotor. We also observed a temporary decrease in the whole-brain correlation between directed functional connectivity and structural connectivity after TMS in all frequency bands. Notably, when focusing on the stimulated areas only, we found that the structure–function correlation significantly increases over time in the premotor area controlateral to TMS. Our study points out the importance of taking into account the major role played by different cortical oscillations when investigating the mechanisms for integration and segregation of information in the human brain.

Abstract

OBJECTIVE Presentation of a non-invasive method for the identification of a patent foramen ovale (PFO) in screening of diving-candidates or for the investigation of diving accidents. METHOD Simultaneous transthoracic echocardiography and transcranial Doppler sonography of the middle cerebral artery using ultrasound contrast media. RESULTS In an unselected collective of amateur and professional divers a PFO was identified according to statistical prediction. CONCLUSION This method can be used in diving medicine for PFO detection as an alternative procedure to transesophageal echocardiography.

Abstract

This study evaluated the subacute respiratory effects of diving, to try to separate the effects of ambient temperature from those of depth. In the first experiment 10 healthy men made a compressed-air dive to 50 m that exposed them to cold. They were compared with 10 matched control subjects who underwent the same dive profile but were exposed to a comfortable temperature. In the second experiment 16 healthy subjects made randomized cold dives to both 50 m and 10 m. Pulmonary function tests were made before, after 1 h, and 24 h after the dives. In the first experiment there was an increase in residual volume (P < 0.05) and a decrease in forced expiratory volume at 1 s (FEV1), in forced vital capacity (FVC) and in mid-expiratory flow at 75% of FVC (MEF75) 1 h after the cold dives (P < 0.05).
