The purpose of the present study was to investigate the clinical results of extensor indicis proprius tendon transfer for extensor pollicis longus tendon rupture, with a focus on objective measurement of extensor strength. We studied 40 patients who underwent extensor indicis proprius transfer with a minimum follow-up of 12 months. Outcome measurements included the Disabilities of the Arm, Shoulder and Hand (DASH) score, range of motion and grip power of the thumb and index finger. The thumb and index finger of the operated hand showed approximately 20% lower extensor power (p < 0.001) compared to the non-operated hand at final follow-up. However, the DASH score significantly improved. The range of motion in the index finger was near normal, while there was an extension deficit in the thumb of 1.2 cm and a flexion deficit of 1.7 cm compared to the normal side. It was reported that substance P had beneficial effects in the healing of acute tendon injury. However, the relationship between substance P and degenerative tendinopathy development remains unclear. The purpose of this study was to determine the role of substance P in the pathogenesis of tendinopathy. Healthy and tendinopathy tendon were harvested from human and tenocytes were cultured individually. The expression levels of genes associated with tendinopathy were compared. Next, substance P was exogenously administered to the healthy tenocyte and the effect was evaluated. The results showed that tendinopathy tenocytes had higher levels of COL3A1, MMP1, COX2, SCX, ACTA2, and substance P gene expression compared to healthy tenocytes. Next, substance P treatment on the healthy tenocyte displayed similar changes to that of the tendinopathy tenocytes. These differences between the two groups were also determined by Western blot. Additionally, cells with substance P had the tendinopathy change morphologically although cellular proliferation was significantly higher compared to that of the control group. In conclusion, substance P enhanced cellular proliferation, but concomitantly increased immature collagen (type 3 collagen). Substance P plays a crucial role in tendinopathy development and could be a future therapeutic target for treatment.

Purpose: The distal radioulnar joint (DRUJ) at the wrist facilitates pronation and supination, and both bone structure and soft tissues contribute to its stability. This study analyzed the characteristics of patients with traumatic posterior DRUJ injuries and examined the clinical outcomes of open or arthroscopic treatment methods for these patients. Methods: A retrospective cohort study was conducted on 14 patients with traumatic posterior DRUJ instability, excluding those with associated radius fractures. The study evaluated patient demographics, injury mechanisms, radiologic findings (DRUJ relationship in the coronal plane, sigmoid notch in the axial plane, the presence and location of an accompanying distal ulnar fracture, and ulnar variance in the opposite wrist), and clinical outcomes (visual analog scale, Disability of Arm, Shoulder, and Hand [DASH] score, and range of motion [ROM]). Patients were treated with either open repair or arthroscopic methods, and postoperative results were monitored over an average of 8.8 months. Results: Ten patients had ulnar styloid fractures, with most occurring at the base or more proximally. The sigmoid notch was classified as the flat-face type in nine cases (64.3%) and the ski-slope type in five cases (35.7%). The clinical outcomes were favorable, with no significant differences between the open and arthroscopic groups regarding pain levels, DASH scores, and ROM. Purpose: This study compared clinical outcomes depending on the fixation modality in patients who underwent arthrodesis surgery for Eaton-Littler stage III trapeziometacarpal (TM) joint arthritis between 2009 and 2022. Methods: In total, 39 patients with 44 hands (five patients had surgery on both hands) were selected. Depending on the operative fixation instruments, patients were divided into two groups: group 1, surgery with 25-gauge interosseous wiring and Kirschner wire or headless screw fixation; group 2, surgery with only multiple headless screws for fixation. Bone union was achieved at 62 days after surgery in group 1 and 75 days in group 2 (p=0.165). Results: The preoperative mean visual analogue scores improved during the postoperative outpatient follow-up period in both groups (group 1, from 4.0 preoperatively to 1.0 at the final follow-up; group 2, from 5.0 preoperatively to 2.0 at the final follow-up). Mean grip strength and pinch strength increased after surgery. One patient (4.8%) experienced nonunion in group 1, whereas nonunion occurred in four patients (17.4%) in group 2. This difference was not statistically significant. Four patients in group 1 and no patients in group 2 underwent fixation instrument removal after sound union. Conclusion: Adequate stability can be achieved during arthrodesis by combining interosseous wiring and other implants or multiple headless compression screws, resulting in successful outcomes for patients with Eaton-Littler stage III degenerative TM arthritis.
