Unfortunately, foot ulcers and amputations due to diabetes are very common in developing countries. The purpose of this study was to characterize the clinical presentation of diabetic foot ulcer (DFU) infections, isolate the causative agent, and analyze the biofilm formation and distribution of biofilm-related genes among isolated Staphylococci.Material and Methods: The study included 100 diabetic patients suffering from DFUs attending Assiut University Hospital. Swabs were collected and antimicrobial susceptibility testing of the isolates was performed. Biofilm formation was tested phenotypically among staphylococcal isolates and the frequency of different biofilm genes was analyzed by PCR. Clinical presentations of diabetic foot ulcers were correlated with bacterial genetic characteristics. Spa types were determined using DNA Gear-a software.Results: Microbiological analysis showed that 94/100 of the DFUs were positive for bacterial growth. The majority of infections were polymicrobial (54%, n=54/100). Staphylococci were the most commonly detected organisms, of which S. aureus represented 37.5% (n=24/64), S. haemolyticus 23.4% (n=15/64), S. epidermidis 34.3% (n=22/64) and other CNS 4.7% (n=3/64). Interestingly, co-infection with more than one species of Staphylococci was observed in 17.1% (n=11/64) of samples. A high level of antibiotic resistance was observed, where 78.1% (n=50/64) of Staphylococci spp were multidrug-resistant (MDR). Phenotypic detection showed that all isolated Staphylococci were biofilm-formers with different grades. Analysis of biofilm-forming genes among Staphylococci showed that the most predominant genes were icaD, spa, and bap. Isolates with a higher number of biofilm-related genes were associated with strong biofilm formation. Sequencing of the spa gene in S. aureus showed that our isolates represent a collection of 17 different spa types.Conclusion: The majority of DFUs in our hospital are polymicrobial. Staphylococci other than S.aureus are major contributors to infected DFUs. MDR and biofilm formation are marked among isolates, which is paralleled by the presence of different categories of virulence-related genes. All severely infected wounds were associated with either strong or intermediate biofilm formers. Abstract

The yellow polyphenolic pigment known as curcumin, originating from the rhizome of the turmeric plant Curcuma longa L., has been utilized for ages in ancient medicine, as well as in cooking and food coloring. Recently, the biological activities of turmeric and curcumin have been thoroughly investigated. The studies mainly focused on their antioxidant, antitumor, anti-inflammatory, neuroprotective, hepatoprotective, and cardioprotective impacts. This review seeks to provide an in-depth, detailed discussion of curcumin usage within the food processing industries and its effect on health support and disease prevention. Curcumin’s bioavailability, bio-efficacy, and bio-safety characteristics, as well as its side effects and quality standards, are also discussed. Finally, curcumin’s multifaceted uses, food appeal enhancement, agro-industrial techniques counteracting its instability and low bioavailability, nanotechnology and focused drug delivery systems to increase its bioavailability, and prospective clinical use tactics are all discussed.

The objective of this study was to assess the safety and esthetic outcome of medial pedicle TM for breast tumors at different locations. Methods: The study was conducted from February 2012 to July 2014. Consecutive patients with early breast carcinoma with medium- and large-sized breasts, with or without ptosis, who were offered medial pedicle TM were included in the study. Patients who were not candidates for breast-conserving surgery or those with tumors located along the medial pedicle were excluded. All patients received immediate postoperative adjuvant chemoradiotherapy. Results: Thirty patients with a mean age of 48.5 years received medial pedicle TM in the breast harboring the tumor or, additionally, the other breast (N=14). The tumors were in the upper (60.0%), lower (26.7%), and lateral (13.3%) quadrants. Minor complications occurred in five cases (5/30, 16.7%) in the ipsilateral and in two (2/14, 14.3%) contralateral breasts. No wound dehiscence or areolar necrosis was recorded. A total of 22 (73.3%) patients were scored as excellent cosmesis. After a median follow-up of 20 months, no locoregional recurrence or distant metastases were observed. Conclusion: TM using a medial pedicle is a safe and appealing technique among women with tumors at different locations. Keywords: therapeutic mammoplasty, medial pedicle, breast cancer, mastectomy

Abstract

BACKGROUND: Most studies of Campylobacter infection triggering Guillain-Barré Syndrome (GBS) are conducted in western nations were Campylobacter infection and immunity is relatively rare. In this study, we explored Campylobacter infections, Campylobacter serotypes, autoantibodies to gangliosides, and GBS in Egypt, a country where Campylobacter exposure is common. METHODS: GBS cases (n = 133) were compared to age- and hospital-matched patient controls (n = 374). A nerve conduction study was performed on cases and a clinical history, serum sample, and stool specimen obtained for all subjects. RESULTS: Most (63.3%) cases were demyelinating type; median age four years. 8. A proteomics approach to investigate uropathogenic Escherichia coli

Abstract

Uropathogenic Escherichia coli (UPEC) is the most commonly known cause of urinary tract infection (UTI). The possession of many virulence factors and global dissemination of multi-drug resistant UPEC strains is posing a critical risk to treatment worldwide. The most virulent UPEC strains have descended from major lineages such as sequence type (ST) 131, ST38 and ST127. Strains of ST127 exhibit the highest virulence potential of most UPEC lineages, but little is known about their pathogenicity. Previous studies have reported the important role of metabolic adaptations in UPEC virulence. This study investigated the metabolic adaptations of UPEC isolates, with a focus on ST127, using a quantitative proteomics approach. We employed a classical shotgun proteomics approach to analyse proteins extracted from multiple strains of UPEC following growth in various environments (Lysogeny broth, artificial urine medium and co-culture with uroepithelial cells). The digested proteins and peptides from all fractions were separated on a Dionex Ultimate 3000 RSLC nano flow system and analysed in an Orbitrap Velos Pro FTMS. Data were processed using Perseus software. Expression and Gene Ontology Enrichment analysis revealed different proteomic profiles of UPEC ST127 strains cultured in LB and AUM. This study showed that environmental changes have an effect on the metabolic pathways expressed by a specific strain. These conditions can be engineered to simulate the variety of environments UPEC isolates may need to survive in. This methodology was then employed to enhance our understanding of the pathogenesis of UPEC. Building on previous work with UPEC paired isolates (urine and blood), proteomics analysis revealed that members of some STs have a higher metabolic potential that could provide virulence advantages. The study also revealed minimal proteomic variations among the paired isolates for some STs. The methods were then applied during co-culture experiments with UPEC and HT1197 bladder epithelial cells. UPEC strain SA189 caused exfoliation effects in HT1197 not seen with other UPEC strains. Analysis of the UPEC SA189 secretome revealed highly abundant bacterial proteins, including hemolysin A. Proteomic analysis is helping to understand the pathogenic potential of UPEC and may facilitate identification of novel diagnostic or therapeutic targets to reduce UTI and potential subsequent bacteraemia.

Abstract

The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135–15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359–5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138–5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184–5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598–9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090–6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286–5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p < 0.001; 95% CI 1.912–7.442; aOR 3.772), whereas endoscopic drainage of pancreatic necrosis (p = 0.018; 95% CI 0.138–0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143–0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible.

21. UHP-SWSSC FILLED GFRP TUBES UNDER COMBINED SUSTAINED LOAD AND SEAWATER EXPOSURE

Abstract

This experimental investigation evaluates the durability of GFRP tubular composites filled with marine resource-based ultra-high-performance concrete under combined sustained loading and seawater exposure at an elevated temperature. Material properties of the FRPs with different diameter to thickness ratio were determined in both longitudinal and hoop directions. Creep strains were monitored in both directions for 2000 hours, and a comprehensive analysis was conducted on the axial compressive stress-strain response, stiffness, and failure modes of unconditioned and conditioned composite specimens. GFRP composites exhibited larger creep strains in hoop direction, which were exacerbated due to swelling in long-term seawater exposure. The GFRP confining tube, with a lower diameter-to-thickness ratio and higher confining pressure, exhibited improved confinement, increased strength and stiffness, and comparable rupture strain after the combined conditioning process., Paper 30

22. Use of a mHealth Approach for the Training of Health-care Providers on Nutrition Counseling in a Malnutrition Clinic

Abstract

BACKGROUND: The use of mobile phones as job aides is one of the most innovative mHealth applications for community health professionals. However, few studies indicated an influence of mHealth on clinical outcomes in low-income countries. AIM: Therefore, the researchers conducted the present study to assess the impact of providing updated training packages using mobile technology to service providers on the knowledge of children’s caregivers and the quality of performance of service providers. METHODS: A quasi-experimental pre-posttest of separate samples was conducted in a malnutrition clinic. The total sample size was 400 cases (200 caregivers as a pre-intervention group [pre-IG] and 200 caregivers as a post-intervention group [post-IG]), with inclusion criteria of having their children 6–24 months old. The study interventions composed of five activities: Orientation sessions on nutrition counseling for physicians and nurses, a software (e-health) program loaded in e-tablet to be used during nutrition counseling by physicians, a booklet on proper infant and child feeding, conducting on-the-job training for the nurses, and establishing counseling cycle in the clinic. RESULTS: After the study intervention, the total knowledge score for all items of children’s nutrition increased from 39% among pre-IG to be 80% among post-IG. The majority (more than 90%) of post-IG received four services packages. CONCLUSION: Participation of service providers in five articulating interventions for nutrition counseling contributes to improving the knowledge of children’s caregivers. Practice Implications: Using mobile technology improved the quality of nutrition care services delivered in the malnutrition clinic.

Abstract

The yellow polyphenolic pigment known as curcumin, originating from the rhizome of the turmeric plant Curcuma longa L., has been utilized for ages in ancient medicine, as well as in cooking and food coloring. Recently, the biological activities of turmeric and curcumin have been thoroughly investigated. The studies mainly focused on their antioxidant, antitumor, anti-inflammatory, neuroprotective, hepatoprotective, and cardioprotective impacts. This review seeks to provide an in-depth, detailed discussion of curcumin usage within the food processing industries and its effect on health support and disease prevention. Curcumin’s bioavailability, bio-efficacy, and bio-safety characteristics, as well as its side effects and quality standards, are also discussed. Finally, curcumin’s multifaceted uses, food appeal enhancement, agro-industrial techniques counteracting its instability and low bioavailability, nanotechnology and focused drug delivery systems to increase its bioavailability, and prospective clinical use tactics are all discussed.

24. Workability, strength, and shrinkage of ultra-high-performance seawater, sea sand concrete with different OPC replacement ratios

Abstract

An experimental investigation on the chemical, physical, mechanical, and shrinkage of seawater and sea sand-based ultra-high-performance concrete (UHP-SWSSC) with supplementary cementitious materials (SCM) (i.e. slag and silica fume) is reported. Several mixes were designed with varying proportions of SCMs (25%, 37.5%, 50%, and 62.5% of binder), aggregate source, and water-to-binder ratio. Heat evolution, density, workability, compressive strength development, and long-term autogenous and drying shrinkage of UHP-SWSSC were monitored. Seawater accelerates cement hydration as reflected in the heat evolution, and consequently, dictates the early-age strength, and autogenous shrinkage. SCM addition although limits the early-age strength development offers a comparable 90 days strength. The chloride content increases from marine resources and may limit the application to nonstructural components. Nonetheless, a UHP-SWSSC mix with 50% OPC replaced by 37.5% slag and 12.5% silica fume is recommended in this study, which can achieve satisfactory workability, long-term strength, and shrinkage properties.

25. The mechanical, transport and chloride binding characteristics of ultra-high-performance concrete utilising seawater, sea sand and SCMs

Abstract

While concrete production accounts for significant CO2 emissions and natural resources (e.g., river sand and freshwater) depletion, the incorporation of sea sand, seawater, and industrial by-products in ultra-high-performance concrete (UHPC) manufacture could offset such impacts. However, the transport and durability performances of marine resource-based UHPC are not well documented. This paper presents an experimental investigation into the mechanical, transport, and chloride binding characteristics of seawater and sea sand-based ultra-high-performance concrete (SWSS-UHPC) with supplementary cementitious materials (SCM) (i.e., slag and silica fume). Mixes were developed with varying proportions of SCMs (up to 62.5%) and sources of aggregates. Early-age and long-term mechanical (i.e., modulus of elasticity, splitting tensile strength) and transport properties (i.e., water absorption, porosity, and ingress of chloride) were evaluated. Non-destructive tests (i.e., ultrasonic pulse velocity, and electrical resistivity) were correlated to the mechanical and durability properties. The chloride binding capacity was determined. Results reveal that although SCMs in SWSS-UHPC marginally improve its mechanical performance, it offers a significant enhancement in transport properties and durability, particularly at 50% cement replacement. The incorporation of marine resources can be beneficial against chloride diffusion and does not adversely affect the binding of external chloride. Electrical resistivity can successfully predict chloride penetration resistance. SCM-based SWSS-UHPC can be extremely durable for long-term exposure in spray and tidal/splash zones of maritime structures.

Abstract

Citrus aurantium L. hydrosol extracted by steam distillation from its flowers is a highly consumed herbal product in the Iranian traditional market, widely used as a food flavor and therapeutic food and drinks. This study investigated ten commercial hydrosol samples of C. aurantium flowers produced by conventional extraction methods and industrial processes, as well as a laboratory-prepared control sample. A liquid-liquid extraction method and sonication were used to extract essential oils from commercial hydrosols. Samples were then subjected to GC/MS analysis. ATR-IR spectroscopy was another efficient tool used to analyze hydrosol samples. All HPTLC chromatograms exhibited a close resemblance between samples and controls. The cluster analysis was used to compare the results of GC/MS and IR screening. In the HCA dendrograms derived from the GC/MS data, most of the oil sample profiles were found to be very similar to those of the control. Linalool was the most abundant compound in eight samples and the control. α-Terpineol in all hydrosol samples and geraniol in five samples and control were other marker compounds. Ethyl disulfide, dillapiol, and hotrienol were detected in two samples that have not been reported in previous studies and might have resulted from the addition of other herbal hydrosols. Microbial content and pH values were within permissible limits in all samples, making them safe for oral consumption.

Abstract

Khosro Abad iron ore deposit is located 130 km of Kermanshah, 30 km northeast of Sonqor city close to Khosro Abad village, in the Sanandaj-Sirjan zone. Based on the field observation, exposed units in the area include; basaltic-andesite volcanic rocks and limestone of the Sonqor Volcanic-Sedimentary Complex. The quartz-monzonite body intruded the sequence and caused a contact metamorphism of up to greenschist facies, and development of hydrothermal alteration halos. Discharged iron bearing hydrothermal fluid destabilized due to physicochemical changes and deposited the iron ore in the contact of limestone, basaltic-andesite and quartz-monzonite intrusion. Control of iron ore mineralization by the contact of quartz-monzonite and volcano-sedimentary sequence and presence of skarn mineralogy is evidence of skarn mineralization in Khosrow Abad. Geophysical survey was carried out using magnetometry technique (1305 stations), covering an area of 3.6 Km2 for defining the high potential areas of iron mineralization and depth estimation. In this research, total magnetic, reduction to pole, analytical signal, and upward continuation maps were prepared, and based on the obtained results four anomaly blocks of; An(A)-An(A´)-An(B)-An(C) were identified. An(C) anomaly is shallower than the others and extend approximately down to 50 meters, while other anomalies are much deeper. The maximum magnetic anomaly in total field intensity and RTP maps is 46426.1 and 49474.6 nanotesla, respectively, along a northeast-southwest trend, aligned with the contact of meta-andesite-basalt and crystallized limestone units. Introduction The magnetometric exploration technique is one of the low-cost, widely used, and indirect methods for mineral exploration and tracking of oxidizing intrusive bodies containing Fe-Ti oxides, such as magnetite, titanomagnetite, and titanohematite, as well as some iron sulfides such as pyrrhotite (Donohue et al., 2012). The Khosrow Abad iron ore deposit is located in west Iran’s iron metallogenic zone of the Sonqor series. It is located northwest of the Sanandaj-Sirjan zone; northeast of the Sonqor town (Kermanshah province) (Ghorbani, 2007) in Sonqor-Baneh volcanic belt (Azizi and Moinevaziri, 2009). In the northeast of Sonqor, hypabyssal acidic intrusive bodies penetrated the Upper Jurassic rock units, including basaltic meta-andesite volcanic rocks and limy slates (calc-shale), causing skarn development and iron ore mineralization. Mineralization is mainly hosted in the meta basaltic andesite unit's skarn zone, differentiating it from other common carbonate-hosted skarn deposits (e.g., Ebrahimi Fard et al., 2022a; Ebrahimi Fard et al., 2022b). The main purpose of current research is to examine the magnetometric geophysical explorations' accuracy in detecting possible ore zones as well as the depth of oxidized (magnetic) intrusive bodies and current emplacement depth and possible associated mineralization. The main challenges were locating the magnetite ore zone covered by recent alluvium and ore zone has limited exposure, and skarn ore facies without classic carbonates as a host rock. Calc-silicate minerals in association with oxide and sulfide minerals formed the skarn zones in the Khosrow Abad deposit. Magnetite, pyrite, chalcopyrite, garnet, clinopyroxene, sodic plagioclase (albite), actinolite, epidote, chlorite, calcite, quartz, and iron hydroxides such as hematite, goethite, and limonite are the main constitute of the skarn zones. The main fracture system in the Khosrow Abad is a large strike-slip fault with an approximately north-south (NNW-SSE) trend and more than 80 degrees dip towards the east. Iron ore mineralization is mainly located in the eastern edge of this fault which formed a shear zone in the metamorphosed volcanic unit. Material and methods Based on the geological map, field observations, geological structures, mineralization limited exposure, and geophysical exploration were carried out perpendicular to apparent structures (probable veins and faults trend) along the northwest-southeast direction. The exploration pattern of the survey was, 25 10 m (profile distance of 25 m and 10 m reading intervals) in the ore zone and a 20 25 m pattern in surrounding rocks (1350 stations). Measurements were carried out by a Proton magnetometer model G856 manufactured by Geometrics. Magnetic surveys were mainly focused on meta basaltic andesite and quartz monzonite units. Qualitative and quantitative data processing and interpretations of data were carried out by using Geosoft Oasis Montaj and Magmap software. The highest field intensity recorded in the area is 52049 nT and the lowest recorded is 43619 nT, which according to the IGRF of 47515nT, is possibly due to the changes caused by the presence of medium-grade magnetite mineralization. Discussion In the total magnetic intensity map, magnetic anomalies were detected with the northwest-southeast trend which is related to discontinuous lenses along the main fault. In the IGRF-REM map, the maximum intensity value of anomalies is equal to 4534 nanotesla. All 4 detected anomalies; (A), (A´), (B), and (C) are high-intensity anomalies in this range. Anomaly (C) is marked on the map with moderate to relatively weak intensity. The upward continuation map indicates that the ore zone continues at least to a depth of more than 70 meters. The Euler's deconvolution method shows an estimate of the anomaly depth, An(C) anomaly is shallower than the others and has a depth of about 50 meters. Based on Spector and Grant's method, the estimated depth of Khosrow Abad iron ore mineralization is about 0.03 km, which is a reasonable estimate. The minimum depth of anomalies in most areas is about 30 meters. The model prepared by Encom Quickmag software shows that linear anomalies are located in line with the regional faults and parallel to each other with medium depth. For validation of the modeling results, based on the data interpretation (analytical signal map, pole-to-pole map, and transversal continuation), several drilling points were proposed for borehole drilling. Conclusion Based on the geophysical data processing and interpretation (analytical signal map, reduction to pole map, and upward continuation) as well as modeling results, several locations were proposed for borehole drilling. The BH-1 borehole is located in the An (A) block anomaly central point, and according to the core logging data, this anomaly is introduced as an anomaly with good intensity, and according to the continuation map with a depth of more than 30 meters is almost match the geophysical interpretation and model. After the drilling campaign, it was discovered that geophysical prediction shows a good agreement with the drilling data, which confirms the effectiveness of the proper design and implemented geophysical survey for iron ore exploration in the Khosrow Abad deposit, which could be used for other skarn iron ore deposits of the region.

A total of 76 relevant articles were selected based on strict inclusion and exclusion criteria, with 22 articles ultimately reviewed and analyzed regarding the effects of various corrective exercises and supportive tools on genu valgum. Findings from one study indicated that aerobic exercises had a significant impact on reducing body mass index (BMI) and the severity of genuvalgum. Other studies reported positive effects of resistance and corrective exercises on improving muscle electrical activity and knee stability. One study demonstrated that the use of orthotic insoles could significantly enhance knee joint support during stair descent. Six studies highlighted the positive effects of using orthoses, wedges, and braces in individuals with genu valgum. Conversely, one study pointed out the negative impact of these tools, citing the reduced limiting angle of the brace, increased applied forces, and, ultimately, greater injury risk. However, findings from three studies overwhelmingly supported the positive effects of using kinesiotape in individuals with genu valgum, providing optimism about the effectiveness of these interventions. In conclusion, our review underscores the promising potential of corrective exercises, particularly resistance exercises with TheraBand, and the combination of these with supportive tools such as insoles for arch support and kinesiotape. These interventions show significant promise in reducing the degree of knee valgus in individuals with genu valgum. This hopeful outlook can inspire healthcare professionals and individuals seeking evidence-based interventions for genu valgum, empowering them with the knowledge to make informed decisions about their care.

Abstract

Abstract Temperature-responsive liquid chromatography (TRLC) offers a promising alternative to reversed-phase liquid chromatography (RPLC) for environmentally friendly analytical techniques by utilizing pure water as a mobile phase, eliminating the need for harmful organic solvents. TRLC columns, packed with temperature-responsive polymers coupled to silica particles, exhibit a unique retention mechanism influenced by temperature-induced polymer hydration. An investigation of the physicochemical parameters driving separation at high and low temperatures is crucial for better column manufacturing and selectivity control. Assessment of predictability using a dataset of 139 molecules analyzed at different temperatures elucidated the molecular descriptors (MDs) relevant to retention mechanisms. Linear regression, support vector regression (SVR), and tree-based ensemble models were evaluated, with no standout performer. The precision, accuracy, and robustness of models were validated through metrics, such as r and mean absolute error (MAE), and statistical analysis. At $$45\,^{\circ }\hbox {C}$$ 45 ∘ C , logP predominantly influenced retention, akin to reversed-phase columns, while at $$5^{\circ }\hbox {C}$$ 5 ∘ C , complex interactions with lipophilic and negative MDs, along with specific functional groups, dictated retention. Abstract

Background: Fosfomycin is a bactericidal drug that inhibits bacterial cell wall and also reduces the adherence ability of bacteria to uro-epithelial cells with a broad anti-bacterial activity against both Gram-negative and Gram-positive. Objective: To compare the efficacy and safety of fosfomycin compared to nitrofurantoin and cephalosporin in pregnant women presented with lower urinary tract infection. Patients and Methods: This was a double blinded, randomized controlled trial conducted at Shebin El-Kom Teaching Hospital from Aug 2019 to Aug 2020. Patients experienced uncomplicated lower urinary tract infection either asymptomatic bacteriuria or cystitis were recruited. Patients were randomly allocated via a random table number into three groups, each group comprised 35 patients. Group I received 3 g oral fosfomycin weekly, group II received 100 mg oral nitrofurantoin three times daily, and group III received 500 mg oral cephalosporin three times daily. After 7 days of treatment, all studied groups were subjected to urine sampling and culture analysis and were examined for occurrence of side effects. Results: All patients enrolled in our study had uncomplicated lower urinary tract infection either cystitis (n=85 cases) or asymptomatic bacteriuria (n=20 cases). There was clinical difference in urine pus cells and urine culture in fosfomycin after treatment compared to other groups. The side effects were reported in 7 cases with fosfomycin compared to 14 in other two groups with significant difference. Conclusion: Fosfomycin tromethamine was proved to be more effective, safe, and fewer side effects were reported, which makes fosfomycin the drug of choice for uncomplicated lower urinary tract infection during pregnancy.

Abstract

This study was conducted to isolate and identify potent pigment producers. From thirty-three fungal isolates, one isolate (FPO2) which isolated from yoghurt samples was able to produce an extracellular orange pigment. The fungal isolate was identified as M. ruber. Among the tested culture media, potato dextrose broth (PDB) was the most suitable medium for the maximum orange pigment secretion. The supplementation PDB with mannitol at concentration 1.5% the maximum orange pigment was produced by M. ruber. On the other hand, the addition of any nitrogen source to the medium led to decrease in orange pigment production. Pigment production gradually increased as the incubation period increased, reaching the maximum value (O.D460 2.856) after 9 days. Abstract

To assess the efficacy and safety of a new surgical approach for uterine preservation among patients with placenta accreta spectrum in a low-resource setting.The present prospective cohort included 63 women diagnosed with placenta accreta spectrum undergoing cesarean deliveries who desired future fertility at the obstetrics department of Menoufia University Hospital from January 2018 to November 2019. Surgical management involved direct bilateral uterine arteries clamping below placental bed after broad ligament opening by round ligaments division and ligation and gentle downward dissection of vesical from myometrial tissues from lateral aspect toward trigone of the bladder. Outcomes included intraoperative and postoperative adverse events, hysterectomy rate, and postoperative hospitalization.Mean operative blood loss was 1860 ± 537 mL (range, 1040-3111 mL) and the incidence of bladder and ureteric injuries were 6.3% (Our conservative surgical approach is a safe alternative to peripartum hysterectomy with high uterine preservation rate, less intraoperative and postoperative morbidity and less need for blood transfusion in low resource settings.ClinicalTrials.gov; NCT04161521.

Abstract

Health care workers (HCWs) may be at a variable risk of SARS-CoV2 infection. Regardless of their involvement in providing direct clinical treatment, most of the prior research had included all HCWs. Understanding infection rates, risk factors and outcomes among different subgroups of HCWs is crucial. From February 28, 2020 to January 1, 2022, we conducted a retrospective analysis encompassing all full-time non-clinical staff (NCS) subcontracted with Hamad Medical Corporation (HMC) facilities. To determine current or previous SARS-CoV2 infection, all personnel underwent RT-PCR and/or serology testing. To identify the demographic factors linked to the risk of infection, we utilized Cox-Hazard regression analysis. Herein 3158/6231 (50.7%) subcontracted NCS tested positive for SARS-CoV-2 by RT-PCR or serology during the research period. The median age was 30 years (IQR 25,35), 69.8% of the population were males, 82.4% were from South Asia, 86.6% did not have any concomitant conditions. 6032 (96.8%) of the population lived in shared housing, while 4749 (76.2%) had low to median levels of education. While infection (PCR positive with or without seropositive results) was independently predicted by male gender, working in the catering, laundry, and security sectors and being intermediate (7–12 years of education), lower (0–6 years of education), higher (exposure to confirmed case), and having symptoms. Male gender, working in the security sectors and being intermediate (7–12 years of education) were independently associated with accidently detected cases (PCR negative and seropositive). 299 (4.8%) required hospitalization, of them 3 cases were severe pneumonia and one required ICU admission without mechanical ventilation, with no deaths reported. In conclusion Infection rates among NCS are high. The majority are asymptomatic and may contribute to ongoing illness spread in the public or in healthcare facilities. During a pandemic, routine screening of this population is crucial and may aid in containing the spread of infection.

Abstract

A green-house experiment was conducted to investigate the efficiency of three heavy metal-tolerant plant growth promoting bacterial strains (HMT-PGPB) (Alcaligenes faecalis MG257493.1, Bacillus cereus MG257494.1 and Alcaligenes faecalis MG966440.1 to alleviate the heavy metal’s toxic effects on sorghum plant (Sorghum bicolor, L.) in addition to their ability to enhance plant growth. The oxidative enzymes, photosynthetic pigments, growth characteristics, heavy metals uptake and heavy metals translocation factor (TF) were estimated in sorghum cultivated in soil contaminated with heavy metals under green-house conditions. Results showed that the application of these HTM-PGPB strains as biofertilizer of sorghum help plant to ignore the toxic effects of heavy metals and enhance growth characteristics.

Abstract

Liver fibrosis is a chronic progressive disease, its resolution still unclear, and the current study explored the role of melatonin in modulation of interleukin-6 (IL-6), interleukin-4 (IL-4), transforming growth factor beta1 (TGF-ß1) and urokinase plasminogen activator receptor-associated protein/Endo180 (uPARAP/Endo180) pathway in thioacetamide (TAA)-induced hepatotoxicity. Thirty two adult Sprague-Dawley rats were divided into four groups: vehicle control group, TAA-induced liver fibrosis group that was left untreated, melatonin administration before and along with TAA and melatonin along with TAA group. TTA-induced massive liver necrosis, fibrosis around portal tract and increases serum levels of liver enzymes and total bilirubin when compared with control vehicle group. While both melatonin pretreatment and treatment retained liver parenchyma and liver enzymes quite similar to control group and reduced TAA-induced liver injury. Notably, melatonin pretreatment and treatment increased collagen degradation in TAA liver injury by19, 31.7-fold respectively evidence by collagen percentage area. Melatonin also decreased the amount of thiobarbituric acid reactive compounds and retained the reduced glutathione and superoxide dismutase to basal level quite similar to control group. Additionally, melatonin significantly (P value =0.05) decreased the levels of TGF-ß1, epidermal growth factor (EGF), hydroxyproline, tissues IL-6, caspase-3, and teceptor interacting serine/threonine kinase1 (RIPK1), fibrillin-1, and - smooth muscle actin in the liver tissues while significantly (P value =0.05) increasing the levels of IL-4 and uPARAP/Endo180. Due to its anti-inflammatory, anti-apoptotic, and antioxidant capabilities as well as its ability to decrease hepatic stellate cell activation and fibrogenesis, these data imply that melatonin has a powerful anti-fibrotic effect.
