2. Do attachment-related differences in reflective functioning explain associations between expressed emotion and youth self-harm?

Abstract

Youth self-harm is associated with poor health outcomes and attempted and completed suicide. Associations exist between self-harm and expressed emotion (EE), attachment insecurity, and reflective functioning (RF), but these associations are poorly understood. This study evaluates a mediation model in which perceived caregiver EE (pEE) exerts an indirect effect on youth self-harm through attachment insecurity and RF uncertainty. 461 participants aged 16-24 years completed an online survey. Statistical analyses revealed significant direct effects of pEE on attachment insecurity, and of RF uncertainty on self-harm; however, some direct effects were specific to pEE from female caregivers, and attachment insecurity in youth relationships with female caregivers. A significant direct effect of pEE on self-harm was found for pEE from male caregivers only. Significant indirect effects of pEE on self-harm through attachment anxiety and RF uncertainty were found only in relation to female caregivers. Abstract

IntroductionIncidents of myocardial infarction and sudden cardiac arrest vary with time of the day, but the mechanism for this effect is not clear. We hypothesized that diurnal changes in the ability of cardiac mitochondria to control calcium homeostasis dictate vulnerability to cardiovascular events.ObjectivesHere we investigate mitochondrial calcium dynamics, respiratory function, and reactive oxygen species (ROS) production in mouse heart during different phases of wake versus sleep periods.MethodsWe assessed time-of-the-day dependence of calcium retention capacity of isolated heart mitochondria from young male C57BL6 mice. Rhythmicity of mitochondrial-dependent oxygen consumption, ROS production and transmembrane potential in homogenates were explored using the Oroboros O2k Station equipped with a fluorescence detection module. Changes in expression of essential clock and calcium dynamics genes/proteins were also determined at sleep versus wake time points.ResultsOur results demonstrate that cardiac mitochondria exhibit higher calcium retention capacity and higher rates of calcium uptake during sleep period. This was associated with higher expression of clock gene Bmal1, lower expression of per2, greater expression of MICU1 gene (mitochondrial calcium uptake 1), and lower expression of the mitochondrial transition pore regulator gene cyclophilin D. Protein levels of mitochondrial calcium uniporter (MCU), MICU2, and sodium/calcium exchanger (NCLX) were also higher at sleep onset relative to wake period. While complex I and II-dependent oxygen utilization and transmembrane potential of cardiac mitochondria were lower during sleep, ROS production was increased presumably due to mitochondrial calcium sequestration.ConclusionsTaken together, our results indicate that retaining mitochondrial calcium in the heart during sleep dissipates membrane potential, slows respiratory activities, and increases ROS levels, which may contribute to increased vulnerability to cardiac stress during sleep-wake transition. This pronounced daily oscillations in mitochondrial functions pertaining to stress vulnerability may at least in part explain diurnal prevalence of cardiac pathologies.

12. A feminist theory of state responsibility for violence against women

Abstract

Over the past forty years the transnational women’s movement has worked to conceptualise and institutionalise the understanding of violence against women as a gendered human rights violation. Their efforts have led the response at the international level, and have shaped the (markedly feminist) international human rights law approach to violence against women. Despite successes in conceptualising and institutionalising norms concerning the elimination of violence against women, there remain problems in implementing these norms and holding states accountable. As such, violence against women is still pandemic, affecting at least one in three women. An area worthy of additional research and attention is state responsibility for prevention – particularly primary prevention, aimed at the ‘upstream’ or ‘root’ causes of violence against women – and the use of the due diligence standard as a tool for actualising this obligation. The due diligence standard is a significant development of state responsibility, which, within the context of human rights protection, broadens notions of state responsibility to include instances where there is a failure to exercise due care to prevent or respond to violative acts or omissions of private or non-state actors. In the context of violence against women, this provides a ‘juridical bridge’ for addressing private violence, particularly domestic violence, as a human rights violation, for which the state can – and should – be held accountable. The evolution of ‘systemic due diligence’ – aimed at the broader level of human rights protection – has furthered this feminist theory of state responsibility. The aim of this thesis is to deepen the understanding of violence against women as a gendered human rights violation and to discover how the due diligence standard can be better used as a tool to bring about its elimination; if, indeed, it is fit for this purpose.

18. Effects of carotenoids on mitochondrial dysfunction

Abstract

Oxidative stress, an imbalance between pro-oxidant and antioxidant status, favouring the pro-oxidant state is a result of increased production of reactive oxygen species (ROS) or inadequate antioxidant protection. ROS are produced through several mechanisms in cells including during mitochondrial oxidative phosphorylation. Increased mitochondrial-derived ROS are associated with mitochondrial dysfunction, an early event in age-related diseases such as Alzheimer's diseases (ADs) and in metabolic disorders including diabetes. AD post-mortem investigations of affected brain regions have shown the accumulation of oxidative damage to macromolecules, and oxidative stress has been considered an important contributor to disease pathology. An increase in oxidative stress, which leads to increased levels of superoxide, hydrogen peroxide and other ROS in a potentially vicious cycle is both causative and a consequence of mitochondrial dysfunction. Mitochondrial dysfunction may be ameliorated by molecules with antioxidant capacities that accumulate in mitochondria such as carotenoids. However, the role of carotenoids in mitigating mitochondrial dysfunction is not fully understood. A better understanding of the role of antioxidants in mitochondrial function is a promising lead towards the development of novel and effective treatment strategies for age-related diseases. This review evaluates and summarises some of the latest developments and insights into the effects of carotenoids on mitochondrial dysfunction with a focus on the antioxidant properties of carotenoids. The mitochondria-protective role of carotenoids may be key in therapeutic strategies and targeting the mitochondria ROS is emerging in drug development for age-related diseases.

Abstract

Background: Almost a quarter of children and young people (CYP) in England have a long-term health condition (LTC), which increases the risk of developing mental health difficulties. There is a lack of understanding regarding the routine provision and efficacy of mental health interventions for CYP with LTCs within Children and Young People's Mental Health Services (CYPMHS). Methods: This study analysed national service-reported data in England from two secondary datasets. Data were submitted by services between 2011 and 2019. We evaluated data on the presence or absence of a serious physical health or neurological issue, and which interventions were offered. Results: A total of 789 CYP had serious physical health issues and 635 had neurological issues. The most common interventions delivered to CYP in either group have some evidence in the literature. Most CYP showed improvements across a range of outcomes. Conclusions: This study found that prevalence rates and psychological intervention and outcome data were widely under-reported across both datasets, posing questions about their utility for this population. Such data would benefit from triangulation with data from other sources to understand pathways of care for these young people and the extent to which clinical datasets underreport the number of CYP with LTCs. It is important these young people are able to access safe and effective treatments for their mental health. Therefore, they are sometimes referred to Children and Young People's Mental Health Services (CYPMHS) for appropriate treatment. However, at the moment, not much is known about the types of mental health support these services offer to children with co-existing physical health needs, or if this support is effective. The aim of this study was to try and find this out. We used data that had already been collected from mental health services across England. We looked at specific parts of this data that gave us information about the type of mental health treatments delivered to children with a long-term health condition. We separated long-term health conditions into two categories: physical health, such as diabetes or asthma; and neurological, such as epilepsy. In the sample we looked at, a range of mental health treatments were delivered to young people in both groups. Encouragingly, many of the young people's mental health improved. However, a lot of information we would hope to find was not available in the datasets. Also, the number of children with a long-term health condition was much lower than we expected. This might have been for a number of reasons, which we recommend other future research tries to find out. Going forward, it is important to think about how to make sure that accurate information about these children is collected from mental health services. Abstract

Background: Existing literature implies there may be gaps in post-treatment support for young people with cancer. This service evaluation explored the needs and experiences of young people when ending cancer treatment in a UK children's hospital to inform service provisions. Methods: Semi-structured interviews were conducted with nine young people, aged 13–18 years, who had finished active cancer treatment and were receiving follow-up care. The data was analysed using thematic analysis. Results: Four main themes were developed: being in the dark (i.e. limited awareness of what happens when treatment ends); separation from the hospital (i.e. the loss of valued support from staff); consequences of cancer (i.e. managing ongoing psychological and physical effects); and getting back to normal life (i.e. shifting from hospital to everyday life). Conclusions: Recommendations for improving clinical practice were made. Greater preparedness for ending treatment could be achieved by clearly setting out ongoing care arrangements, providing resource packs, having opportunities to mark the end of treatment, and offering peer support. To identify specific post-treatment needs, there should be an end of treatment multidisciplinary review and space for young people to share how they are feeling in follow-up medical appointments. Abstract

Objective: The aim of this study was to explore the relationship between affective disturbances and aberrant salience in the context of childhood trauma, attachment, and mentalization in an analogue study. Methods: Using a cross‐sectional design, an online community sample completed self‐report measures of key variables. Structural equation modelling was used to test childhood trauma's influence on aberrant salience via a set of intermediate risk factors (depression, negative schizotypy, and insecure attachment). These intermediate risk factors were assumed to lead to the proximal risk factors of aberrant salience (i.e., disorganized schizotypy and disorganized attachment) depending on the vulnerability of mentalizing capacity to elevated stress. Results: The sample (N = 1263) was 78% female and aged between 18 and 35 years. The tested models closely fitted the observed data, revealing significant pathways from childhood trauma to aberrant salience via the hypothesized pathways. The direct effect of childhood trauma on aberrant salience was significant. Conclusion: Findings suggest that the pathway to aberrant salience may be characterized by disorganization of self‐state and intersubjectivity as a function of diminishment in mentalizing ability. This may relate to changes in attachment organization and socio‐cognitive capacity, which could constitute possible risk factors signalling development of aberrant salience. [ABSTRACT FROM AUTHOR]

Abstract

Experiencing a physical health condition can have wide ranging physical and psychological outcomes for an individual, impacting on many aspects of daily living. The psychological outcomes can vary widely, from extreme psychological distress to psychological growth, and are complex and dynamic. Traditionally, research has focused on those experiencing psychological distress, despite evidence showing that many individuals are able to achieve positive psychological outcomes. The literature is unclear in defining different positive psychological outcomes, and indeed distinguishing different outcomes and processes from one another. Attention needs to be paid to the definition applied to individuals who are reporting positive psychological outcomes following physical health difficulties and further understanding the process through which individuals achieve these outcomes. This empirical study aimed to address some of the gaps in the literature, by exploring how six individuals reporting positive psychological outcomes following spinal cord injury described and explained these outcomes. Interpretative phenomenological analysis was used to investigate the experience of these individuals. Three superordinate themes were extracted: "Living a normal life, just doing things differently", "Overcoming challenges: Determination to succeed" and "Using the resources available to me". The research supported the idea that positive psychological outcomes arise through a complex interplay between personality, cognitive and environmental factors. Theoretically, this research has implications for defining positive psychological outcomes following spinal cord injury, in addition to contributing towards future theoretical frameworks that aim to provide a basis for understanding the process through which positive psychological outcomes following spinal cord injury are achieved. Clinically, this research provides a narrative that can be used with people following spinal cord injury and also provides evidence for the use of cognitive screening measures such as appraisal style, to identify individuals who may be showing less adaptive cognitions.

35. New school geographies : engaging young people?

Abstract

In 2003 school geography was in a state of crisis: enrolment in GCSE geography courses had fallen by a third over the previous eight years. In response, a radical new ‘pilot’ geography GCSE course was designed and implemented in England. The GCSE was an attempt to rejuvenate a school subject that had become out of date, with little change to its content since the inauguration of the National Curriculum in 1988. With student-centred learning at its heart the GCSE aimed to make the subject much more exciting and relevant to young people. The following thesis examines alternative pedagogical approaches to teaching school geography that draw on young people’s experiences as citizens and consumers to make geography more relevant and interesting to them. Written as an unfolding story this multi-sited ethnography began by exploring the networks behind the pilot. This involved not only several different actors/groups of actors (including geography educators, academic geographers, geography teachers and school pupils) but also several different spaces (including schools, classrooms, organisation headquarters, working group meetings and publications). It moves on to examine how the GCSE’s approaches to teaching, learning and assessing were being played out in practice and to what extent its aims, claims and intentions were being realised in the classroom. Through exploring the pilot’s approach to the pedagogy of school geography my research became action-oriented in approach, and I became involved in co-creating critical, connective curriculum materials for the GCSE. The development of these new materials and teaching and learning strategies are situated within debates in human geography about critical pedagogy, young people’s geographies and public geographies and the thesis forges links between these different theoretical strands. I conclude by asking what lessons can be learnt from the pilot GCSE and its implications for the role of geography within a wider educational context. Written autoethnographically to reflect the collaborative and iterative nature of my research my intention has been to critically engage with multiple publics who are involved in this area.

40. Saccades in the absence of binocular vision

Abstract

The mechanism of suppression in strabismus is unclear and contribution of the suppressing eye to the generation of eye movements has received little attention. A series of nine experiments tested how the strabismic eye contributes to saccade generation in the presence of suppression and also considered the effect of the strabismic eye in the presence of abnormal retinal correspondence (ARC). These data were compared with data from subjects with normal binocular single vision (BSV). Chapters 2 and 3 describe the equipment, laboratory set-up and testing of the equipment used in the thesis for measuring eye movements, Skalar IRIS 6500 infrared limbal tracker, and presenting stimuli to each eye separately. The design of a novel method for dissociation of the eyes using four liquid crystal polymer shutters is presented. Chapter 4 compares the characteristics of saccades made by subjects with normal BSV (n=5) and strabismus (n=8). The effect of distractors on saccades is explored in Chapter 5 in subjects with normal BSV (n=5). The experiment documents the distractor effect produced in the described laboratory set-up, and compares it with that previously reported by Walker et al (1997). This is investigated further by comparing the effect of distractor presentations to the dominant eye, non-dominant eye or both eyes. There was no difference in the effect on saccade latency or gain with distractors presented to the dominant or non-dominant eye. The effect of binocular distractors on saccade gain was greater than monocular presentations. Chapter 6 repeats the experiment of Chapter 5 in subjects with constant strabismus and suppression (n=6) and constant strabismus with ARC (n=2) and found that distractors in the strabismic eye did affect saccades however the response differed from normal BSV. This was true even though it was shown that the distractor was not perceived by the strabismic eye. Chapter 7 investigates the influence of the central fixation target in the strabismic eye on saccade generation by inducing disconjugate saccade adaptation in subjects with normal BSV (n=8) and constant strabismus and suppression (n=6). The findings were that in the presence of suppression, disconjugate adaptation similar to that in normal BSV was possible. The conclusion of this thesis is to suggest that information from the suppressed eye is available to the saccadic system by either a sub-cortical pathway or processed cortically without conscious awareness.
