Abstract

In 3GPP the architecture of a New Radio Access Network (New RAN) has been defined where the evolved NodeB (eNB) functions can be split between a Distributed Unit (DU) and Central Unit (CU). Furthermore, in the virtual RAN (VRAN) approach, such functions can be virtualized (e.g., in simple terms, deployed in virtual machines). Based on the split type, different performance in terms of capacity and latency are requested to the network (i.e., fronthaul) connecting DU and CU. This study experimentally evaluates, in the 5G segment of the Advanced Research on NetwOrking (ARNO) testbed (ARNO-5G), the fronthaul latency requirements specified by Standard Developing Organizations (SDO) (3GPP in this specific case). Moreover it evaluates how much virtualization impacts the fronthaul latency budget for the the Option 7-1 functional split. The obtained results show that, in the considered Option 7-1 functional split, the fronthaul latency requirements are about 250us but they depend on the radio channel bandwidth and the number of the connected UEs. Abstract

Objective: Stereotyped beliefs about schizophrenia are well-established in the society and relatively common among healthcare professionals and students. The aim of this study was to investigate the opinions about the causes, treatment, and outcome of schizophrenia among healthcare students. Method: Undergraduate nursing and psychology students completed selected items of the Opinion on Mental Illness Questionnaire after reading a clinical vignette of undiagnosed schizophrenia. Results: Students who labelled the description as schizophrenia were more pessimistic regarding full recovery from the disorder. Those who acknowledged greater relevance to biogenetic risk factors were more convinced of the efficacy of medications. Respondents’ opinions on the efficacy of psychological interventions were more positive among psychology students than among nursing students. Conclusions: The study confirmed the associations of schizophrenia labelling with prognostic pessimism and beliefs about the efficacy of pharmacological treatment among future healthcare professionals. Students’ opinions were less influenced by differences between academic degree programs. Information about current recovery rate and comprehensive care for schizophrenia may support future healthcare professionals in the relationship and the clinical management of PWS.

Abstract

Background & Aims: The ANSWER study reported that long-term albumin administration in patients with cirrhosis and uncomplicated ascites improves survival. During treatment, serum albumin increased within a month and remained stable thereafter. In this post hoc analysis, we aimed to determine whether on-treatment serum albumin levels could guide therapy. Methods: Logistic regression was used to assess the association between baseline serum albumin and mortality, as well as to determine on-treatment factors associated with mortality and to predict the achievement of a given on-treatment serum albumin level. Survival was assessed by Kaplan-Meier estimates and second-order polynomial regression. Patients whose on-treatment serum albumin remained below normal were compared with a subset of patients from the control arm matched by principal score. Results: Baseline serum albumin was closely associated with 18-month mortality in untreated patients; albumin treatment almost effaced this relationship. On-treatment serum albumin and MELD-Na at month 1 were the sole independent variables associated with mortality. Second-order polynomial regression revealed that survival improved in parallel with increased 1-month on-treatment serum albumin. Kaplan-Meier estimations showed that any value of 1-month on-treatment serum albumin (0.1 g/dl intervals) in the range 2.5–4.5 g/dl discriminated patient survival. In the normal range of serum albumin, the best discriminant value was 4.0 g/dl. Compared to untreated patients, survival even improved in patients whose on-treatment serum albumin remained below normal. Conclusion: Baseline serum albumin per se should not guide the decision to start albumin therapy. Conversely, 1-month on-treatment serum albumin levels are strongly associated with outcomes and could guide the use of albumin – 4.0 g/dl being the target threshold. Abstract

Objectives: To determine the reproducibility of the National Comprehensive Cancer Network (NCCN) resectability status classification for pancreatic cancer. Background: The NCCN classification defines 3 resectability classes (resectable, borderline resectable, locally advanced), according to vascular invasion. It is used to recommend different approaches and stratify patients during clinical trials. Methods: Prospective, multicenter, observational study (trial ID: NCT03673423). Main outcome measure was the interobserver agreement of tumor assignment to different resectability classes and quantification of vascular invasion degrees. Agreement was measured by Fleiss' k (k = 1 perfect agreement; k = 0 agreement by chance). Sixty-nine computed tomography (CT) scans of pathologically confirmed pancreatic adenocarcinoma were independently reviewed in a blinded fashion by 22 observers from 11 hospitals (11 surgeons and 11 radiologists). Rating differences between surgeons or radiologists and between hospitals with different volumes (≥60 or <60 resections/year) were assessed. Results: Complete agreement among 22 observers was recorded in 5 CT scans (7.2%), whereas 25 CT scans (36.2%) were variously assigned to all 3 resectability classes. Interobserver agreement varied from fair to moderate (Fleiss' k range: 0.282-0.555), with the lowest agreement for borderline resectable tumors. Assessing vascular contact ≤180° had the lowest agreement for all vessels (k range: 0.196-0.362). The highest concordance was recorded for venous invasion >180° (k range: 0.619-0.756). Neither reviewers' specialty nor hospital volume influenced the agreement. Conclusions: There is high variability in the assignment to resectability categories, which may compromise the reliability of treatments recommendations and the evidence of trials stratifying patients in resectability classes. Criteria should be revised to allow a reproducible classification.

Abstract

Objectives This study aimed at investigating the relationship between perceived public stigma towards people with schizophrenia (PWS) and their family members in a large sample of medical and psychology students. We hypothesised that: a) schizophrenia labelling would be related to greater perceived public stigma; b) contact with PWS would be related with lower perceived stigma; c) perceived public stigma would be similar between medical and psychology students and would be higher among students attending the clinical stage compared to their pre-clinical colleagues. Methods Participants were 592 students attending either the pre-clinical or clinical stage of coursework in Medicine and Psychology, at the University of Palermo (Italy) (Tab. I). Study measures included a short socio-demographic questionnaire, the Devaluation of Consumers Scale (DCS), and the Devaluation Consumers Families Scale (DCFS). Results Students who identified schizophrenia in an unlabelled clinical description expressed greater perceived public stigma towards PWS (t = -2.895, p = 0.004) and their family members (t = -2.389, p = 0.017). A trend-level association was found between previous contact with PWS and lower perceived public stigma (t = 1.903, p = 0.058), which became significant for those students who had a more extensive contact (Mann-Whitney z = 2.063, p = 0.039). Compared to medical students, psychology students perceived greater public stigma towards PWS. No difference was observed between students at different stages of their academic coursework (Tab. II). In a multivariate linear regression model, schizophrenia labelling and degree course predicted perceived public stigma towards severe mental disorders. Conclusions This study replicated previous findings on the relationship between public stigma towards PWS, schizophrenia labelling, and contact with PWS. Perception of public stigma was similar among pre-clinical and clinical students and greater among psychology students. The findings suggest the importance of promoting a critical awareness of negative stereotypes towards schizophrenia among healthcare students, since the beginning of their coursework. In addition to correct information about schizophrenia, anti-stigma intervention should include contact with PWS who live in the community.

Abstract

Risk factors related to the family environment and mental distress have been identified as risk factors for deviant behaviour and delinquency during adolescence. This paper, in particular, wants to highlight the specificity of adverse childhood experiences in a sample of young prisoners at the Minor Court of Palermo. In addition to the high presence of emotional neglect and psychological and physical abuse, in this sample there are high percentages of psychopathological symptoms, significantly related to an increased risk of recidivism. The data collected will be improve the studies on the subject but also offer interesting insights for the prevention and the management of juvenile delinquency in the psychoeducational, social and legal fields.

Abstract

Background: Evidence is scarce on the efficacy of long-term human albumin (HA) administration in patients with decompensated cirrhosis. The human Albumin for the treatmeNt of aScites in patients With hEpatic ciRrhosis (ANSWER) study was designed to clarify this issue. Methods: We did an investigator-initiated multicentre randomised, parallel, open-label, pragmatic trial in 33 academic and non-academic Italian hospitals. We randomly assigned patients with cirrhosis and uncomplicated ascites who were treated with anti-aldosteronic drugs (≥200 mg/day) and furosemide (≥25 mg/day) to receive either standard medical treatment (SMT) or SMT plus HA (40 g twice weekly for 2 weeks, and then 40 g weekly) for up to 18 months. The primary endpoint was 18-month mortality, evaluated as difference of events and analysis of survival time in patients included in the modified intention-to-treat and per-protocol populations. This study is registered with EudraCT, number 2008–000625–19, and ClinicalTrials.gov, number NCT01288794. Findings: From April 2, 2011, to May 27, 2015, 440 patients were randomly assigned and 431 were included in the modified intention-to-treat analysis. 38 of 218 patients died in the SMT plus HA group and 46 of 213 in the SMT group. Overall 18-month survival was significantly higher in the SMT plus HA than in the SMT group (Kaplan-Meier estimates 77% vs 66%; p=0·028), resulting in a 38% reduction in the mortality hazard ratio (0·62 [95% CI 0·40–0·95]). 46 (22%) patients in the SMT group and 49 (22%) in the SMT plus HA group had grade 3–4 non-liver related adverse events. Interpretation: In this trial, long-term HA administration prolongs overall survival and might act as a disease modifying treatment in patients with decompensated cirrhosis. Abstract

This paper has been presented at : 5th IEEE INFOCOM Workshop on Computer and Networking Experimental Research using Testbeds 2019 (IEEE CNERT 2019) Packet loss in the fronthaul adversely affects radio services, especially the low-layer functional split options between Distributed Unit (DU) and Central Unit (CU). A 1+1 protection scheme is implemented in a Software Defined Networking (SDN) packet-over-optical transport test-bed, achieving seamless recov-ery of the 7-1 split option from an optical link failure in the fronthaul. 37. Impact of Virtualisation Technologies on Virtualised RAN Midhaul Latency Budget: A Quantitative Experimental Evaluation

Abstract

In the Next Generation Radio Access Network (NGRAN) defined by 3GPP for the fifth generation of mobile communications (5G), the next generation NodeB (gNB) is split into a Radio Unit (RU), a Distributed Unit (DU), and a Central Unit (CU). RU, DU, and CU are connected through the fronthaul (RU-DU) and midhaul (DU-CU) segments. If the RAN is also virtualised RAN (VRAN), DU and CU are deployed in virtual machines or containers. Different latency and jitter requirements are demanded on the midhaul according to the distribution of the protocol functions between DU and CU. This study shows that, in VRAN, the virtualisation technologies, the functional split option, and the number of elements deployed in the same computational resource affect the latency budget available for the midhaul. Moreover, it provides an expression for the midhaul allowable latency as a function of the aforementioned parameters. Finally, it shows that, the virtualised DUs featuring a lower layer split option shall be deployed not in the same computational resources where other vDUs are deployed.

Abstract

International audience; This article proposes a reflection on the impact that some postmodern features can have on the sense of identity and on mental functioning to propose possible explanations for identity changes and new forms of psychopathology. After revisiting the most recent literature on the theme, this study proposes a clinical consideration for a possible link between new forms of psychopathology and mental suffering. It particularly suggests that the changes observed in affectivity (including increase in dysphoria, irritability, restlessness, boredom, feeling of emptiness) and behaviour (reduction in impulse control ability, in interpersonal skills and reflective function, etc.) are important indices of a bigger change that involves the entire personality and indicates a shifting towards a borderline level of personality functioning. Finally, the article offers a possible explanation for this shift and advances therapeutic questions.
