In the current observational cohort study, we evaluated whether, in patients with metastatic colorectal cancer receiving first-line chemotherapy, cumulative toxicity comprising all grades of AEs is more predictive for QOL than cumulative toxicity due to only high-grade AEs.Methods: One hundred and five patients starting treatment completed the European Organization for the Research and Treatment of Cancer Quality of Life Questionnaire (EORTC-QLQ-C30) questionnaire at baseline and 10 weeks. AEs, clinical outcomes, and demographics were retrieved from patient records. Cumulative toxicity scores were calculated in three ways: total number of high-grade AEs, total number of all-grade AEs, and total number of AEs multiplied by their grade (the severity score). Relations between cumulative toxicity scores and QOL were studied using multivariable linear regression analyses.Results: The mean age of patients was 65 years, 68% were male, and 84% received oxaliplatin-based chemotherapy. A higher total number of AEs of all grades (B=−2.4, 95% CI=–3.9; –0.9) and the severity score (B=–1.4, 95% CI=–2.3; –0.5) were predictive for clinically relevant changes in physical QOL, whereas the total high-grade AEs was not. None of the cumulative toxicity scores were predictive for global QOL.Conclusion: Cumulative toxicity scores comprising all grades of AEs provide a better measure of treatment burden than a toxicity score comprising high-grade AEs only. Physical QOL seems to be more affected by AEs than global QOL. Our results emphasize that future clinical trials should present cumulative toxicity scores comprising all AE grades as well as physical QOL instead of global QOL. Comorbidity, which is highly prevalent in OA, may interfere with exercise therapy. To date, there is no evidence-based protocol for the treatment of patients with knee OA and comorbidity. Special protocols adapted to the comorbidity may facilitate the application of exercise therapy in patients with knee OA and one or more comorbidities.Purpose: The purpose of this study was to develop comorbidity-adapted exercise protocols for patients with knee OA and comorbidity.Method: Several steps were undertaken to develop comorbidity-adapted protocols: selection of highly prevalent comorbidities in OA, a literature search to identify restrictions and contraindications for exercise therapy for the various comorbid diseases, consultation of experts on each comorbid disease, and field testing of the protocol in eleven patients with knee OA and comorbidity.Results: Based on literature and expert opinion, comorbidity-adapted protocols were developed for highly prevalent comorbidities in OA. Field testing showed that the protocols provided guidance in clinical decision making in both the diagnostic and the treatment phase. Because of overlap, the number of exercise protocols could be reduced to three: one for physiological adaptations (coronary disease, heart failure, hypertension, diabetes type 2, chronic obstructive pulmonary diseases, obesity), one for behavioral adaptations (chronic a-specific pain, nonspecific low back pain, depression), and one for environmental adaptations (visual or hearing impairments). 19. Short‐term morbidity and mortality after right hemicolectomy: an update of national performance in the Netherlands.

Abstract

Aim: The purpose of this Dutch retrospective population‐based study was to evaluate how short‐term outcomes and inter‐hospital variability after right hemicolectomy for colon cancer have evolved between 2012 and 2020. Method: Patients who underwent right hemicolectomy for primary solitary colon cancer between 1 January 2012 and 31 December 2020 and were registered in the Dutch Colorectal Audit were included. Surgical characteristics and outcomes were assessed during three time periods (2012–2014, 2015–2017, 2018–2020). Complications and mortality were the primary outcomes, and reintervention, readmission and length of stay secondary outcomes. Results: In total, 29 274 patients were included. Significant increase in minimally invasive surgery (51.1% 2012–2014, 73.2% 2015–2017, 85.0% 2018–2020), increase in conversion (6.6%, 7.8%, 9.1%, P < 0.001) and decrease in acute/urgent resections (15.9%, 11.7%, 10.9%, P < 0.001) were found. The overall complication rate was slightly lower in the third period (30.9%, 30.6%, 28.8%, P = 0.004), primarily because of decreasing non‐surgical complications (19.7%, 20.6%, 17.6%, P < 0.001), while surgical complications remained unchanged (17.5%, 18.3%, 18.2%, P = 0.277). Postoperative mortality was 3.4%, 2.3% and 3.5%, respectively. Reintervention rate slightly decreased (9.4%, 8.3%, 8.6%, P < 0.001). The proportion of patients admitted for more than 6 days decreased over time (54.3%, 42.4%, 34.3%, P < 0.001), with an increase in readmission rate (7.4%, 6.8%, 9.3%, P < 0.001). Inter‐hospital variability decreased over time for complications, length of stay and conversion. Conclusion: This study shows a national decreasing inter‐hospital variability in clinical outcomes after right hemicolectomy and a decrease in postoperative complications. Abstract

Onderzoek - Door BIJ12 is aan het consortium Jasja Dekker, Bob van den Brink en Luuk Boerema gevraagd een onderzoek uit te voeren naar de populatieontwikkeling van de wolf in Nederland. Inclusief een beoordeling van de relatie met de binnen Europa te onderscheiden wolvenpopulaties. Meer specifiek richt de vraag zich om een analyse uit te voeren naar de mogelijke toekomstige ontwikkeling en (verdere) verspreiding van de wolf in Nederland. Met gebruikmaking van de door BIJ12 verzamelde gegevens. De wolven die in Nederland voorkomen zijn onderdeel van een grotere populatie (de CentraalEuropese populatie) die inmiddels een verspreidingsgebied hebben die zich uitstrekt over Polen, Tsjechië, Duitsland, Denemarken, België en zelfs Oostenrijk. Daarom blijft het van belang dat met deze analyse de relatie met de gehele populatie niet uit het oog wordt verloren. Het doel van het onderzoek is dat er inzicht kan worden verkregen in: 1. De huidige en toekomstige ontwikkeling van de wolvenpopulatie in Nederland en de factoren die hierop van invloed kunnen zijn (populatie en toekomstperspectief); 2. De huidige en toekomstige ontwikkeling van de verspreiding van de wolf in Nederland en de factoren die hierop van invloed kunnen zijn (verspreiding en toekomstperspectief); 3. De samenhang tussen de in Nederland voorkomende wolven(sub)populatie met de binnen Europa te onderscheiden wolvenpopulaties én de schaal waarop deze gedefinieerd kan worden (samenhang van de populatie).

Abstract

In this paper, we build on the work of Hughes and Sangalli (2007) dealing with the explicit computation of the Fine-Scale Greens’ function. The original approach chooses a set of functionals associated with a projector to compute the Fine-Scale Greens’ function. The construction of these functionals, however, does not generalise to arbitrary projections, higher dimensions, or Spectral Element methods. We propose to generalise the construction of the required functionals by using dual functions. These dual functions can be directly derived from the chosen projector and are explicitly computable. We show how to find the dual functions for both the L2 and the H01 projections. We then go on to demonstrate that the Fine-Scale Greens’ functions constructed with the dual basis functions consistently reproduce the unresolved scales removed by the projector. The methodology is tested using one-dimensional Poisson and advection–diffusion problems, as well as a two-dimensional Poisson problem. We present the computed components of the Fine-Scale Greens’ function, and the Fine-Scale Greens’ function itself. These results show that the method works for arbitrary projections, in arbitrary dimensions. De aanleiding hiervoor is de plaatsing van de haas op de Rode Lijst en het rapport van de WUR over de zeer ongunstige staat van instandhouding. De provincie Utrecht heeft daarop heroverwogen of het plaatsen van de haas op de vrijstellingslijst gerechtvaardigd is. De provincie is van oordeel dat dit niet gerechtvaardigd is. Concreet betekent dit dat bij alle ruimtelijke activiteiten vooraf onderzoek gedaan moet worden naar de effecten daarvan op hazen en dat bij overtreding van de verbodsbepalingen ook een vergunning moet worden aangevraagd. In dit toetsingskader wordt aangegeven wat de gevolgen zijn van deze beslissing op activiteiten zoals ruimtelijke ingrepen die vanaf 1 september 2024 plaatsvinden. Dit toetsingskader is opgesteld voor ecologische adviesbureaus die initiatiefnemers begeleiden bij hun werkzaamheden. Hiermee wordt inzichtelijk gemaakt hoe moet worden gehandeld bij activiteiten met negatieve effecten op hazen. Zo wordt een protocol gegeven voor het ecologische onderzoek naar de haas. Ook worden mitigerende en compenserende maatregelen voorgesteld waar rekening mee kan worden gehouden bij de vergunningaanvraag. Ook wordt uitgelegd hoe een vergunning van de vergunningplichtige flora- en fauna-activiteiten genoemd in de Omgevingswet kan worden aangevraagd.

‘Als een knoop in een knoopsgat, zo past het Nieuwe bij het Oude Testament.’ Deze uitspraak typeert Henk de Jong (1932-2023), eredoctor van de Theologische Universiteit Utrecht. Veel bijbellezers verbinden de komst van Christus vooral met bekende profetieën en psalmen die rond Kerst en Pasen worden gelezen. Maar hoe laten al die verhalen over Israël en andere, soms weerbarstige, teksten uit het Oude Testament zich met het christelijk geloof verbinden? Henk de Jong heeft er zijn levenswerk van gemaakt om te laten zien dat heel het Oude Testament zich naar Christus toe beweegt. Hij werkte zijn visie creatief uit en heeft daarmee velen geïnspireerd. Deze herdenkingsbundel is gewijd aan de bijbels-theologische erfenis van Henk de Jong. Vijf kenners van het Oude Testament wegen de waarde van zijn werk en vergelijken zijn bijbeluitleg met andere visies op Christus in het Oude Testament en met Joodse interpretaties. Ook bevat deze bundel een aantal teksten van De Jong zelf die deels niet eerder gepubliceerd zijn. Al met al een mooi palet aan bijdragen om stil te staan bij de betekenis van zijn werk of om daar nader kennis mee te maken.

The surface guided radiation therapy (SGRT) system from Varian Medical systems, IDENTIFY(TM) , consists of three pods, including cameras and a random pattern projector, mounted on the ceiling. The information captured by the cameras is used to make a reconstruction of the surface. The aim of the study was to assess the technical performance of this SGRT system on a closed-bore linac. METHODS: Phantom measurements were performed to assess the accuracy, precision, reproducibility and temporal stability of the system, both in align and in load position. Translations of the phantoms in lateral, longitudinal, and vertical direction, and rotations around three axes (pitch, roll and yaw) were performed with an accurate, in-house built, positioning stage. Different phantom geometries and different surface colors were used, and various ambient light intensities were tested. RESULTS: The accuracy of the IDENTIFY(TM) system at the closed-bore linac was 0.07 mm and 0.07 degrees at load position, and 0.06 mm and 0.01 degrees at align position for the white head phantom. The precision was 0.02 mm and 0.02 degrees in load position, and 0.01 mm and 0.02 degrees in align position. The accuracy for the Penta-Guide phantom was comparable to the white head phantom, with 0.06 mm and 0.01 degrees in align position. The system was slightly less accurate for translations of the CIRS lung phantom in align position (0.20 mm, 0.05 degrees). Reproducibility measurements showed a variation of 0.02 mm in load position. Regarding the temporal stability, the maximum drift over 30 min was 0.33 mm and 0.02 degrees in load position. No effect of ambient light level on the accuracy of the IDENTIFY(TM) system was observed. Regarding different surface colors, the accuracy of the system for a black phantom was slightly worse compared to a white surface, but not clinical relevant. CONCLUSION
