Abstract

Anaphylaxis is an acute and potentially fatal systemic reaction that sould be recognized and managed by all helthcare professionals. This is the first national guideline of Turkish National Society of Allergy and Clinical Immunology on diagnosis and management of anaphylaxis. The guideline is prepared by Anaphylaxis Interest Group with the incorporation of 20 allergy/immunology specialists and the epidemiology, etiology, pathophysiology, symptoms and signs, diagnosis/differentials, and acute and long-term management of anaphylaxis are reviewed in detail.

Abstract

In this research, Verbascum pyramidatum which is used for hemorrhoid and rheumatism treatment in Turkish folk medicine has been investigated. The air dried and powdered flowers and leaves of the plant were extracted with methanol separately. The methanolic exctracts were dissolved in water and then partitioned with chloroform in order to obtain the water and the chloroform extracts. These extracts were performed for the in vivo anti-inflammatory (carregeenan-induced hind paw edema model) and in vivo analgesic (p- benzoquinone-induced writhing reflex) activity assays. The water extract of the flowered parts were found to be the most active and the study was carried on it. By means of a serial of chromatographic studies on the active water extract, 3 iridoid glycosides (3′-O-β- glucopyranosylcatalpol: VP-1, Saccatoside: VP-2/VP-3a and 6-O-(3′′-O-trans-p- coumaroyl)-α-rhamnopyranosylcatalpol: VP-3b); 2 phenylethanoid glycosides (Verbascoside: VP-4, Echinacoside: VP-5) were isolated. The structures of the isolated compounds were elucidated by means of spectroscopic methods (1H NMR, 13C NMR, 2D NMR ). Türkiye’ de halk arasında hemoroid ve romatizma tedavisinde kullanılan Verbascum pyramidatum (Scrophulariacea) bitkisi üzerinde Farmakognozik araştırmalar yapılmıştır. Bitkinin açık havada ve gölgede kurutulan ve toz haline getirilen çiçek ve yaprak kısımları ayrı ayrı metanol ile ekstre edilmiştir. Yoğunlaştırılan metanol ekstrelerinin suda çözülüp, kloroform ile partisyona tabi tutulması sonucunda kloroform ve sulu ekstreleri elde edilmiştir. Bu ekstreler in vivo antienflamatuvar (karragen ile oluşturulan arka ayak ödemi testi) ve analjezik (p- benzokinon nedenli ağrı testi) aktivite tayin yöntemlerinde denenmiştir. Aktif bulunan çiçeklerden hazırlanan sulu ekstresine uygulanan çeşitli kromatografik yöntemlerle gerçekleştirilen izolasyon çalışmaları neticesinde, 3 iridoit glikoziti ( 3′- O-β-glukopiranozilkatalpol: VP-1, Sakkatozit: VP-2/VP-3a, 6-O-(3′′-O-trans-p- kumaroil)-α-ramnopiranozilkatalpol: VP-3b) ve 2 feniletanoit glikoziti (Verbaskozit: VP-4, Ekinakozit: VP-5) elde edilmiştir. Elde edilen bileşiklerin yapıları spektroskopik yöntemler (1H NMR, 13C NMR, 2D NMR) kullanılarak aydınlatılmıştır.

Abstract

Background: The number of Sp1–Egr1 binding tandem repeats at the ALOX5 promoter influences gene transcription and may modify the response to anti-leukotriene treatment. The relationship of ALOX5 variants to asthma severity and leukotriene production by eosinophils is unknown. Objective: To characterize ALOX5 mRNA expression and cysteinyl-leukotriene production by eosinophils from individuals bearing ALOX5 promoter deletional variants and their association with the severity of childhood asthma. Methods: Eosinophils from adult asthmatics bearing only variant alleles (with other than five tandem repeats on both chromosomes, non5/non5) or no variant alleles (5/5) were cultured in vitro and ALOX5 expression and leukotriene secretion were measured. A total of 621 children with mild or moderate-severe asthma were genotyped at the ALOX5 core promoter. Results: Asthmatics with non5/non5 genotype expressed less ALOX5 mRNA and produced less LTC4 into culture supernatants than 5/5 individuals (6.4 ± 2.0 and 20.0 ± 5.0 pg/ml, n = 5; P < 0.05). More asthmatic children bearing non5/non5 genotype had moderate-severe asthma than children with the 5/5 genotype (5.3% vs. 1.4%, P = 0.008). Multivariate logistic regression identified ALOX5 promoter genotype as a significant predictor of disease severity (OR = 3.647, 95% CI: 1.146–11.608, P = 0.03). Consistent with these findings, children bearing the non5/non5 genotype had greater bronchomotor response to exercise as measured by the maximum fall after exercise and the area under the exercise curve ( P < 0.05 for both). Conclusion: Our results suggest that children who express the asthma phenotype despite having a genetic variant that impairs their ability to express ALOX5 have more severe disease and thus are more likely to have asthma symptoms. 50. Physicians' perception of childhood asthma in Turkey: more appropriate practice among female physicians

Abstract

Abstract Background Low levels of asthma control worldwide point to the possibility of sub-optimal management; therefore, documentation of physicians' perception is critical for future interventions. Our aim was to examine self-reported management abilities of Turkish physicians dealing with children with asthma, document the factors affecting appropriate decisions and compare the results with those of a previous survey. Methods Physicians were surveyed via a questionnaire aimed to document self-perceived asthma knowledge and attitudes in asthma management. Results The majority of physicians were male (63%) and examined 234 ± 9 patients per week. Infrequent use of objective parameters in asthma diagnosis and attack severity assessment was reported and most preferred nebulized corticosteroids to the systemic form in acute asthma. Even though self-perceived overall asthma knowledge did not differ between genders (p = 0.098), male physicians scored higher than females for inhaled steroids for acute asthma (2.8 ± 0.12 vs 2.17 ± 0.2, respectively, p = 0.007), while female physicians recorded more frequent use of inhaled steroids for chronic asthma (3.72 ± 0.08 vs 3.43 ± 0.07, respectively, p = 0.006). Female physicians' scoring for "symptom control" as the main aim of asthma management was higher than that of their male counterparts (3.88 ± 0.04 vs 3.65 ± 0.06, respectively, p = 0.002). Conclusion Although there were some discrepancies between guidelines and clinical practice, most applications of Turkish physicians dealing with children with asthma were appropriate. Interestingly, when scores of female versus male physicians were compared, it can be suggested that female physicians have a more appropriate perception of asthma, indicating a significant contribution of gender-related factors in clinical attitudes and beliefs.
