- Abstract
Overview of infectious and non-infectious diseases in French Guiana in 2022 Source of many myths, French Guiana represents an exceptional territory due to the richness of its biodiversity and the variety of its communities. The only European territory in Amazonia, surrounded by the Brazilian giant and the little-known Suriname, Ariane 6 rockets are launched from Kourou while 50% of the population lives below the poverty line. This paradoxical situation is a source of health problems specific to this territory, whether they be infectious diseases with unknown germs, intoxications or chronic pathologies. Some infectious diseases such as Q fever, toxoplasmosis, cryptococcosis or HIV infection are in common with temperate countries, but present specificities leading to sometimes different management and medical reasoning. In addition to these pathologies, many tropical diseases are present in an endemic and / or epidemic mode such as malaria, leishmaniasis, Chagas disease, histoplasmosis or dengue. Besides, Amazonian dermatology is extremely varied, ranging from rare but serious pathologies (Buruli ulcer, leprosy) to others which are frequent and benign such as agouti lice (mites of the family Trombiculidae) or papillonitis. Envenomations by wild fauna are not rare, and deserve an appropriate management of the incriminated taxon. Obstetrical, cardiovascular and metabolic cosmopolitan pathologies sometimes take on a particular dimension in French Guiana that must be taken into account in the management of patients. Finally, different types of intoxication are to be known by practitioners, especially due to heavy metals. European-level resources offer diagnostic and therapeutic possibilities that do not exist in the surrounding countries and regions, thus allowing the management of diseases that are not well known elsewhere. Thanks to these same European-level resources, research in Guyana occupies a key place within the Amazon region, despite a smaller population than in the surrounding countries. Thus, certain pathologies such as histoplasmosis of the immunocompromised patient, Amazonian toxoplasmosis or Q fever are hardly described in neighboring countries, probably due to under-diagnosis linked to more limited resources. French Guiana plays a leading role in the study of these diseases. The objective of this overview is to guide health care providers coming to or practicing in French Guiana in their daily practice, but also practitioners taking care of people returning from French Guiana. Seul territoire européen en Amazonie, entouré du géant brésilien et du méconnu Suriname, on y lance des fusées Ariane 6 depuis Kourou tandis que 50 % de la population vit en dessous du seuil de pauvreté. Cette situation paradoxale est source de problématiques de santé spécifiques à ce territoire, qu’il s’agisse de maladies infectieuses à germes méconnus, d’intoxications, ou de pathologies chroniques. Certaines maladies infectieuses telles que la fièvre Q, la toxoplasmose, la cryptococcose ou l’infection à VIH sont communes aux pays tempérés, mais présentent en Guyane des spécificités entraînant une prise en charge et un raisonnement médical parfois différents. Parallèlement à ces pathologies, de nombreuses maladies tropicales sont par ailleurs présentes sur un mode endémique et / ou épidémique telles que le paludisme, la leishmaniose, la maladie de Chagas, l’histoplasmose ou la dengue. De plus, la dermatologie amazonienne est extrêmement variée, allant de pathologies rares, mais graves (ulcère de Buruli, lèpre), à d’autres fréquentes et bénignes telles que les poux d’agouti (acariens de la famille des Trombiculidae) ou la papillonite. Les envenimations par la faune sauvage ne sont pas rares, et méritent une prise en charge appropriée au taxon incriminé. Les pathologies obstétricale, cardiovasculaire et métabolique cosmopolites prennent parfois en Guyane une dimension particulière à prendre en compte dans la prise en charge des patients. Enfin, différents types d’intoxication sont à connaître par les praticiens, notamment aux métaux lourds. Les ressources de niveau européen offrent des possibilités diagnostiques et thérapeutiques inexistantes dans les pays et régions des environs, permettant ainsi la prise en charge de maladies peu connues ailleurs. Du fait de ces mêmes ressources de niveau européen, la recherche en Guyane occupe une place clé au sein de la région amazonienne, malgré une population moins nombreuse que dans les pays alentour. Ainsi, certaines pathologies telles que l’histoplasmose du patient immunodéprimé, la toxoplasmose amazonienne ou la fièvre Q ne sont pratiquement pas décrites dans les pays voisins, probablement du fait d’un sous-diagnostic lié à des ressources plus limitées. La Guyane joue ainsi un rôle moteur dans l’étude de ces pathologies. During the Chikungunya epidemic in the Caribbean and Latin America, pregnant women were affected by the virus in French Guiana. The question of the impact of the virus on pregnancy was raised because of the lack of scientific consensus and published data in the region. Thus, during the Chikungunya outbreak in French Guiana, a comparative study was set up using a cohort of pregnant women. The objective was to compare pregnancy and neonatal outcomes between pregnant women with Chikungunya virus (CHIKV) infection and pregnant women without CHIKV. Of 653 mothers included in the cohort, 246 mothers were included in the case-control study: 73 had CHIKV fever during pregnancy and 173 had neither fever nor CHIKV during pregnancy. The study did not observe any severe clinical presentation of CHIKV in the participating women. There were no intensive care unit admissions. In addition, the study showed no significant difference between the two groups with regard to pregnancy complications. However, the results showed a potential excess risk of neonatal ICU admission of the newborn when the maternal infection occurred within 7 days before delivery. These results suggest that special attention should be paid to neonates whose mothers were infected with CHIKV shortly before delivery. [ABSTRACT FROM AUTHOR]

Members of the Noir-Marron ethnic group in French Guiana appear to have a high rate of infection with the human T-cell lymphotropic virus type I (HTLV-I). HTLV-I infection is associated with a number of diseases including adult T-cell leukemia/lymphoma. It may be transmitted from mother to child through breastfeeding. Blood samples were taken from 1,873 mothers delivering between July 1991 and June 1993. Seventy-five of the women (4.4%) tested positive for HTLV-I infection. Ninety-three percent of those testing positive were members of the Noir-Marron ethnic group. Noir-Marrons account for 5.4% of the population in French Guiana. Of Noir-Marrons, 5.7% tested positive. Among Haitians, who account for 20% of the population, 6.3% tested positive. No Creole, Amerindian or Hmong women tested positive for infection. Among Noir-Marron women, being Rh negative, having given birth more than four times, having been pregnant more than six times and having had prior miscarriages and cesarean sections significantly increased the risk of being HTLV-I positive.

Objective: Given the high rate of premature birth in French Guiana (13.5%), and its stability in time, the aim of the present study was to define a predictive score for preterm birth in women with a unique pregnancy in order to help prioritize health resources in the local context.Methods: A retrospective study was conducted on all deliveries of unique pregnancies in French Guiana collected between 1 January 2013 and 31 December 2014 in the Registre d'Issue de Grossesse Informatisé (RIGI), a registry that collects data on live births over 22 weeks of amenorrhea on the territory. Statistically significant predictors (p < .05) of preterm delivery were included in a logistic regression model. The selected variables were chosen to be available during the first trimester. Coefficients were used to establish a score which was categorized and prospectively validated using data from 2015.Results: Seven explanatory variables, all measurable during the first trimester of pregnancy, were significantly associated with preterm birth. The predictive score divided in deciles allowed to establish sensitivity and specificity thresholds. Overall, depending on the chosen threshold the score sensitivity was low and the specificity was high. Lowering the threshold identified half of women as "at risk" for preterm birth.Conclusion: This first trimester score was insufficiently sensitive to identify individual women at risk for preterm delivery. National audience; Context – Cervical cancer is the second most frequent cancer of women in French Guiana. Studies have shown that populations living in remote villages in the interior of the territory have early sexual relations and that multiple sexual partnerships are frequent. Hence, the main objective of the study was to determine the prevalence of Human Papilloma Virus (HPV) in these areas.Methods – This multicentric cross sectional study was conducted in women aged 20 to 65 years, living in theremote villages along the Maroni and Oyapock rivers. Women were included on a voluntary basis after beinginformed of the study team schedule through radio address and key opinion leaders.The search for HPV and cytologic examinations were performed on each sample. HPV infection was defined bythe detection of viral DNA using the Greiner Bio One® kit. Cytologic analyses were performed using the 2001Bethesda classification.Results – Overall, 643 women were included in the study. Age-standardized HPV prevalence was 35%. Theprevalence curve by age group showed a U-shaped pattern: women over 50 years had the highest risk of being infected by HPV, followed by women aged 20-29 years. In addition, 26.1% of women with a normal smear test had HPV infection. The most frequent genotypes were HPV52, HPV58, and HPV16. On interpretable smears 1.16% of women had high grade superficial intra-epithelial (HSIL) lesions.Conclusions – The standardized prevalence of HPV in French Guiana is among the highest in the world. HSILcytological lesions were nearly 4 times more frequent than in the Île-de-France region. Given the importance of the incidence of cervical cancer in French Guiana, these results suggest it may be preferable to use HPV testing than cytology. Vaccination against HPV, preferably with the new nonavalent vaccine seems an appropriate prevention measure. Aguas residuales - Depuración - Abstract
Las aguas residuales procedentes de una fábrica de levaduras se generan, principalmente en tres operaciones: el sobrenadante resultante de la centrifugación, el agua procedente de la deshidratación y el agua de lavado de la levadura. La fábrica considerada en este proyecto genera un caudal medio de aguas residuales de 2100 m3/día. Se trata de unas aguas con un alto contenido en contaminación biológica (DBO = 5000 mg/L) y con una relación DBO/DQO entorno a 0,7. El proceso consta de un pretratamiento, consistente en la homogeneización y el ajuste de pH, concentración de nutrientes y temperatura; además de un tratamiento biológico, tanto anarobio (reactor UASB) como aerobio (fangos activos y decantador final). El tratamiento de fangos consiste en un secado de fangos mediante un filtro de bandas con polielectrolito como agente coagulante. Los costes de explotación y mantenimiento son de aproximadamente 187.881,92 €/año, que equivale a 0,27 €/m3 de agua depurada, Departamento de Química Analítica, Grado en Ingeniería Química
