Introduction:Commissioning of a new planning system involves extensive data acquisition which can be onerous involving significant clinic downtime. This could be circumvented by extracting data from existing treatment planning system (TPS) to speed up the process.Material and methods:In this study, commissioning beam data was obtained from a clinically commissioned TPS (Pinnacle™) using Matlab™ generated Pinnacle™ executable scripts to commission an independent 3D dose verification TPS (Eclipse™). Profiles and output factors for commissioning as required by Eclipse™ were computed on a 50 × 50 × 50 cm3 water phantom at a dose grid resolution of 2 mm3. Verification doses were computed and compared to clinical TPS dose profiles based on TG-106 guidelines. Standard patient plans from Pinnacle™ including intensity modulated radiation therapy and volumetric modulated arc therapy were re-computed on Eclipse™ TPS while maintaining the same monitor units. Computed dose was exported back to Pinnacle for comparison with the original plans. This methodology enabled us to alleviate all ambiguities that arise in such studies.Results:Profile analysis using in-house software showed that for all field sizes including small multi-leaf collimator-generated fields, >95% of infield and penumbra data points of Eclipse™ match Pinnacle™ generated and measured profiles with 2%/2 mm gamma criteria. Excellent agreement was observed in the penumbra regions, with >95% of the data points passing distance to agreement criteria for complex C-shaped and S-shaped profiles. Dose volume histograms and isodose lines of patient plans agreed well to within a 0·5% for target coverage.Findings:Migration of TPS is possible without compromising accuracy or enduring the cumbersome measurement of commissioning data. Economising time for commissioning such a verification system or for migration of TPS can add great QA value and minimise downtime.

Background:Prostate cancer is one of the most common solid malignancies and has a high morbidity rate. The uncertainty of the prostate location compromises the overall treatment plan optimisation. To account for the location uncertainty, the radiation oncologist needs to expand the margin of the planning target volume (PTV), which may increase the radiation toxicity to organs in proximity.Materials and methods:In this study, we investigated the quality of treatment plans for a patient with different ring sizes (2 and 3 cm). A small ring-shaped structure circumferentially around the PTV helps in defining the location of PTV. Prostate and pelvic node plans were analysed with dose prescription to 99% of PTV.Results:Additional ring-shaped structures led to more conformal dose coverage for target with reduced radiation side effects to nearby organ at risk (OAR). Expected treatment time was slightly higher for 2 cm ring compared to 3 cm ring. In case of prostate, expected duration was 4% higher, while for node plan, expected duration for 2 cm ring was 16% higher compared to 3 cm ring plan.Conclusions:It was observed that using a smaller size ring can lead to improved dose sparing to OAR with same target coverage as with larger dimension ring. The composite plans do not show any clinically significant difference in dose to OARs.

Purpose Daily magnetic resonance (MR)–guided radiation has the potential to improve stereotactic body radiation therapy (SBRT) for tumors of the liver. Magnetic resonance imaging (MRI) introduces unique variables that are untested clinically: electron return effect, MRI geometric distortion, MRI to radiation therapy isocenter uncertainty, multileaf collimator position error, and uncertainties with voxel size and tracking. All could lead to increased toxicity and/or local recurrences with SBRT. In this multi-institutional study, we hypothesized that direct visualization provided by MR guidance could allow the use of small treatment volumes to spare normal tissues while maintaining clinical outcomes despite the aforementioned uncertainties in MR-guided treatment. Methods and materials Patients with primary liver tumors or metastatic lesions treated with MR-guided liver SBRT were reviewed at 3 institutions. Toxicity was assessed using National Cancer Institute Common Terminology Criteria for Adverse Events Version 4. Freedom from local progression (FFLP) and overall survival were analyzed with the Kaplan-Meier method and χ2 test. Results The study population consisted of 26 patients: 6 hepatocellular carcinomas, 2 cholangiocarcinomas, and 18 metastatic liver lesions (44% colorectal metastasis). The median follow-up was 21.2 months. The median dose delivered was 50 Gy at 10 Gy/fraction. No grade 4 or greater gastrointestinal toxicities were observed after treatment. The 1-year and 2-year overall survival in this cohort is 69% and 60%, respectively. At the median follow-up, FFLP for this cohort was 80.4%. FFLP for patients with hepatocellular carcinomas, colorectal metastasis, and all other lesions were 100%, 75%, and 83%, respectively. Conclusions This study describes the first clinical outcomes of MR-guided liver SBRT. Treatment was well tolerated by patients with excellent local control. This study lays the foundation for future dose escalation and adaptive treatment for liver-based primary malignancies and/or metastatic disease.

Purpose With the increasing use of MR‐guided radiation therapy (MRgRT), it becomes important to understand and explore accuracy of medical dosimeters in the presence of magnetic field. The purpose of this work is to characterize metal‐oxide‐semiconductor field‐effect transistors (MOSFETs) in MRgRT systems at 0.345 T magnetic field strength. Methods A MOSFET dosimetry system, developed by Best Medical Canada for in‐vivo patient dosimetry, was used to study various commissioning tests performed on a MRgRT system, MRIdian® Linac. We characterized the MOSFET dosimeter with different cable lengths by determining its calibration factor, monitor unit linearity, angular dependence, field size dependence, percentage depth dose (PDD) variation, output factor change, and intensity modulated radiation therapy quality assurance (IMRT QA) verification for several plans. MOSFET results were analyzed and compared with commissioning data and Monte Carlo calculations. Results MOSFET measurements were not found to be affected by the presence of 0.345 T magnetic field. Calibration factors were similar for different cable length dosimeters either placed at the parallel or perpendicular direction to the magnetic field, with variations of less than 2%. The detector showed good linearity (R2 = 0.999) for 100–600 MUs range. Output factor measurements were consistent with ionization chamber data within 2.2%. MOSFET PDD measurements were found to be within 1% for 1–15 cm depth range in comparison to ionization chamber. MOSFET normalized angular response matched thermoluminescent detector (TLD) response within 5.5%. The IMRT QA verification data for the MRgRT linac showed that the percentage difference between ionization chamber and MOSFET was 0.91%, 2.05%, and 2.63%, respectively for liver, spine, and mediastinum. Conclusion MOSFET dosimeters are not affected by the 0.345 T magnetic field in MRgRT system. They showed physics parameters and performance comparable to TLD and ionization chamber; thus, they constitute an alternative to TLD for real‐time in‐vivo dosimetry in MRgRT procedures.

Purpose: Magnetic resonance imaging–guided radiation therapy has entered clinical practice at several major treatment centers. Treatment of early-stage non-small cell lung cancer with stereotactic body radiation therapy is one potential application of this modality, as some form of respiratory motion management is important to address. We hypothesize that magnetic resonance imaging–guided tri-cobalt-60 radiation therapy can be used to generate clinically acceptable stereotactic body radiation therapy treatment plans. Here, we report on a dosimetric comparison between magnetic resonance imaging–guided radiation therapy plans and internal target volume–based plans utilizing volumetric-modulated arc therapy. Materials and Methods: Ten patients with early-stage non-small cell lung cancer who underwent radiation therapy planning and treatment were studied. Following 4-dimensional computed tomography, patient images were used to generate clinically deliverable plans. For volumetric-modulated arc therapy plans, the planning tumor volume was defined as an internal target volume + 0.5 cm. For magnetic resonance imaging–guided plans, a single mid-inspiratory cycle was used to define a gross tumor volume, then expanded 0.3 cm to the planning tumor volume. Treatment plan parameters were compared. Results: Planning tumor volumes trended larger for volumetric-modulated arc therapy–based plans, with a mean planning tumor volume of 47.4 mL versus 24.8 mL for magnetic resonance imaging–guided plans ( P = .08). Clinically acceptable plans were achievable via both methods, with bilateral lung V20, 3.9% versus 4.8% ( P = .62). The volume of chest wall receiving greater than 30 Gy was also similar, 22.1 versus 19.8 mL ( P = .78), as were all other parameters commonly used for lung stereotactic body radiation therapy. The ratio of the 50% isodose volume to planning tumor volume was lower in volumetric-modulated arc therapy plans, 4.19 versus 10.0 ( P < .001). Heterogeneity index was comparable between plans, 1.25 versus 1.25 ( P = .98). Conclusion: Magnetic resonance imaging–guided tri-cobalt-60 radiation therapy is capable of delivering lung high-quality stereotactic body radiation therapy plans that are clinically acceptable as compared to volumetric-modulated arc therapy–based plans. Real-time magnetic resonance imaging provides the unique capacity to directly observe tumor motion during treatment for purposes of motion management.

Purpose With advance magnetic resonance (MR)-guided online adaptive radiotherapy (MRgoART) relying on calculation-based intensity-modulated radiation therapy (IMRT) quality assurance (QA), accurate and sensitive QA of the multileaf collimator (MLC) becomes an increasingly essential component for routine machine QA. As such, it is important to assure compliance with the AAPM TG142 guidelines to supplement calculation-based QA methods for an online adaptive radiotherapy program. We have developed and implemented an efficient and highly sensitive QA procedure using an ionization chamber profiler (ICP) array to enable real-time characterization of the positional accuracy of a double-focused and double-stacked MLC on a clinical MR-guided radiotherapy (MRgRT) system and to supplement calculation-based QA for an MRgoART program. Methods An in-house MR-compatible jig was used to position the ICP (detector resolution 5 mm on X/Y axis) at an extended SDD of 108.4 cm to enable each MLC leaf (8.3 mm leaf width at isocenter) to be uniquely determined by two neighboring ion chambers. The MRgRT linac system utilizes a novel jawless, double-focused, and double-stacked MLC design such that the upper bank (MLC1) and lower bank (MLC2) are offset by half a leaf width. Positional accuracy was characterized by three methods: single bank half-beam block (HBB) at central axis, forward slash diagonal (FSD), and backslash diagonal (BSD) at off-axis. Measurements were performed for each bank in which each leaf occludes half of a detector. A corresponding reference field with the MLC retracted from occlusion was measured. The sensitivities of HBB, FSD, and BSD were evaluated by introducing 0.5-2.5 mm of known errors in 0.5 mm increments, in both positive and negative directions. The relationship between detector response and MLC error was established. Over a 6-month longitudinal assessment, we have evaluated MLC performance with weekly QA of HBB among cardinal angles, and monthly QA of FSD and BSD. Results A strong correlation was found between detector response of percentage dose difference and MLC positional error introduced (N = 350 introduced errors) for both HBB and FSD/BSD with coefficient of determination of 0.999 and 0.977, respectively. The relationship between detector response to MLC positional change was found to be 20.65%/mm for HBB and 11.14%/mm for FSD and BSD. At baseline, the mean MLC positional accuracy averaged across all leaves was 0.06 ± 0.27 mm (HBB), 0.04 ± 0.52 mm (FSD), -0.06 ± 0.51 mm (BSD). The mean MLC positional accuracy relative to baseline over the 6-month assessment was found to be highly reproducible at 0.00 ± 0.12 mm (HBB; N = 28 weeks), -0.02 ± 0.19 mm (FSD; N = 6 months), -0.03 ± 0.19 mm (BSD; N = 6 months). Conclusions Positional accuracy of a novel jawless, double-focused, double-stacked MLC has been characterized and monitored over 6 months with an efficient, highly sensitive, and robust method using an ICP array. This routine QA method supplements calculation-based IMRT QA for an online adaptive radiotherapy program. Longitudinal assessment demonstrated no-drift in the MLC calibration. A highly reproducible jig setup allowed the validation of MLC positional accuracy to be within TG142 criteria of ±1 mm for 99% of measurements (i.e., 100% HBB, 95% BSD, 95% FSD) over the 6-month assessment.

Magnetic resonance-guided radiation therapy (MRgRT) offers advantages for image guidance for radiotherapy treatments as compared to conventional computed tomography (CT)-based modalities. The superior soft tissue contrast of magnetic resonance (MR) enables an improved visualization of the gross tumor and adjacent normal tissues in the treatment of abdominal and thoracic malignancies. Online adaptive capabilities, coupled with advanced motion management of real-time tracking of the tumor, directly allow for high-precision inter-/intrafraction localization. The primary aim of this case series is to describe MR-based interventions for localizing targets not well-visualized with conventional image-guided technologies. The abdominal and thoracic sites of the lung, kidney, liver, and gastric targets are described to illustrate the technological advancement of MR-guidance in radiotherapy.

ViewRay is a novel MR-guided radiotherapy system capable of imaging in near real-time at four frames per second during treatment using 0.35T field strength. It allows for improved gating techniques and adaptive radiotherapy. Three cobalt-60 sources (~ 15,000 Curies) permit multiple-beam, intensity-modulated radiation therapy. The primary aim of this study is to assess the imaging stability, accuracy, and automatic segmentation algorithm capability to track motion in simulated and in vivo targets. Magnetic resonance imaging (MRI) characteristics of the system were assessed using the American College of Radiology (ACR)-recommended phantom and accreditation protocol. Images of the ACR phantom were acquired using a head coil following the ACR scanning instructions. ACR recommended T1- and T2-weighted sequences were evaluated. Nine measurements were performed over a period of seven months, on just over a monthly basis, to establish consistency. A silicon dielectric gel target was attached to the motor via a rod. 40 mm total amplitude was used with cycles of 3 to 9 s in length in a sinusoidal trajectory. Trajectories of six moving clinical targets in four canine patients were quantified and tracked. ACR phantom images were analyzed, and the results were compared with the ACR acceptance levels. Measured slice thickness accuracies were within the acceptance limits. In the 0.35 T system, the image intensity uniformity was also within the ACR acceptance limit. Over the range of cycle lengths, representing a wide range of breathing rates in patients imaged at four frames/s, excellent agreement was observed between the expected and measured target trajectories. In vivo canine targets, including the gross target volume (GTV), as well as other abdominal soft tissue structures, were visualized with inherent MR contrast, allowing for preliminary results of target tracking.

Chondrosarcomas are relatively radiotherapy resistant, and also delivering high radiation doses is not feasible owing to anatomic constraints. In this study, the feasibility of helical tomotherapy for treatment of chondrosarcoma of thoracic spine is explored and compared with other available photon and proton radiotherapy techniques in the clinical setting. A patient was treated for high-grade chondrosarcoma of the thoracic spine using tomotherapy. Retrospectively, the tomotherapy plan was compared with intensity-modulated radiation therapy, dynamic arc photon therapy, and proton therapy. Two primary comparisons were made: (1) comparison of normal tissue sparing with comparable target volume coverage (plan-1), and (2) comparison of target volume coverage with a constrained maximum dose to the cord center (plan-2). With constrained target volume coverage, proton plans were found to yield lower mean doses for all organs at risk (spinal cord, esophagus, heart, and both lungs). Tomotherapy planning resulted in the lowest mean dose to all organs at risk amongst photon-based methods. For cord dose constrained plans, the static-field intensity-modulated radiation therapy and dynamic arc plans resulted target underdosing in 20% and 12% of planning target volume2 volumes, respectively, whereas both proton and tomotherapy plans provided clinically acceptable target volume coverage with no portion of planning target volume2 receiving less than 90% of the prescribed dose. Tomotherapy plans are comparable to proton plans and produce superior results compared with other photon modalities. This feasibility study suggests that tomotherapy is an attractive alternative to proton radiotherapy for delivering high doses to lesions in the thoracic spine.

Characterization of textural features (spatial distributions of image intensity levels) has been considered as a tool for automatic tumor segmentation. The purpose of this work is to study the variability of the textural features in PET images due to different acquisition modes and reconstruction parameters.Twenty patients with solid tumors underwent PET/CT scans on a GE Discovery VCT scanner, 45-60 minutes post-injection of 10 mCi of [(18)F]FDG. Scans were acquired in both 2D and 3D modes. For each acquisition the raw PET data was reconstructed using five different reconstruction parameters. Lesions were segmented on a default image using the threshold of 40% of maximum SUV. Fifty different texture features were calculated inside the tumors. The range of variations of the features were calculated with respect to the average value.Fifty textural features were classified based on the range of variation in three categories: small, intermediate and large variability. Features with small variability (range ≤ 5%) were entropy-first order, energy, maximal correlation coefficient (second order feature) and low-gray level run emphasis (high-order feature). The features with intermediate variability (10% ≤ range ≤ 25%) were entropy-GLCM, sum entropy, high gray level run emphsis, gray level non-uniformity, small number emphasis, and entropy-NGL. Forty remaining features presented large variations (range30%).Textural features such as entropy-first order, energy, maximal correlation coefficient, and low-gray level run emphasis exhibited small variations due to different acquisition modes and reconstruction parameters. Features with low level of variations are better candidates for reproducible tumor segmentation. Even though features such as contrast-NGTD, coarseness, homogeneity, and busyness have been previously used, our data indicated that these features presented large variations, therefore they could not be considered as a good candidates for tumor segmentation.

Precise calibration of Hounsfield units (HU) to electron density (HU-density) is essential to dose calculation. On-board kV cone beam computed tomography (CBCT) imaging is used predominantly for patients' positioning, but will potentially be used for dose calculation. The impacts of varying 3 imaging parameters (mAs, source-imager distance [SID], and cone angle) and phantom size on the HU number accuracy and HU-density calibrations for CBCT imaging were studied. We proposed a site-specific calibration method to achieve higher accuracy in CBCT image-based dose calculation. Three configurations of the Computerized Imaging Reference Systems (CIRS) water equivalent electron density phantom were used to simulate sites including head, lungs, and lower body (abdomen/pelvis). The planning computed tomography (CT) scan was used as the baseline for comparisons. CBCT scans of these phantom configurations were performed using Varian Trilogy system in a precalibrated mode with fixed tube voltage (125 kVp), but varied mAs, SID, and cone angle. An HU-density curve was generated and evaluated for each set of scan parameters. Three HU-density tables generated using different phantom configurations with the same imaging parameter settings were selected for dose calculation on CBCT images for an accuracy comparison. Changing mAs or SID had small impact on HU numbers. For adipose tissue, the HU discrepancy from the baseline was 20 HU in a small phantom, but 5 times lager in a large phantom. Yet, reducing the cone angle significantly decreases the HU discrepancy. The HU-density table was also affected accordingly. By performing dose comparison between CT and CBCT image-based plans, results showed that using the site-specific HU-density tables to calibrate CBCT images of different sites improves the dose accuracy to approximately 2%. Our phantom study showed that CBCT imaging can be a feasible option for dose computation in adaptive radiotherapy approach if the site-specific calibration is applied.

Purpose To assess patient setup corrections based on daily megavoltage CT (MVCT) imaging for four anatomic treatment sites treated on tomotherapy. Method and Materials Translational and rotational setup corrections, based on registration of daily MVCT to planning CT images, were analyzed for 1,179 brain and head and neck (H&N), 1,414 lung, and 1,274 prostate treatment fractions. Frequencies of three-dimensional vector lengths, overall distributions of setup corrections, and patient-specific distributions of random and systematic setup errors were analyzed. Results Brain and H&N had lower magnitude positioning corrections and smaller variations in translational setup errors but were comparable in roll rotations. Three-dimensional vector translational shifts of larger magnitudes occurred more frequently for lung and prostate than for brain and H&N treatments, yet this was not observed for roll rotations. The global systematic error for prostate was 4.7 mm in the vertical direction, most likely due to couch sag caused by large couch extension distances. Variations in systematic errors and magnitudes of random translational errors ranged from 1.6 to 2.6 mm for brain and H&N and 3.2 to 7.2 mm for lung and prostate, whereas roll rotational errors ranged from 0.8° to 1.2° for brain and H&N and 0.5° to 1.0° for lung and prostate. Conclusions Differences in setup were observed between brain, H&N, lung, and prostate treatments. Patient setup can be improved if daily imaging is performed. This analysis can assess the utilization of daily image guidance and allows for further investigation into improved anatomic site-specific and patient-specific treatments.

During the last decade, there has been an explosion of new radiation therapy planning and delivery tools. We went through a rapid transition from conventional three-dimensional (3D) conformal radiation therapy to intensity-modulated radiation therapy (IMRT) treatments, and additional new techniques for motion-adaptive radiation therapy are being introduced. These advances push the frontiers in our effort to provide better patient care; and with the addition of IMRT, temporal dimensions are major challenges for the radiotherapy patient dosimetry and delivery verification. Advanced techniques are less tolerant to poor implementation than are standard techniques. Mis-administrations are more difficult to detect and can possibly lead to poor outcomes for some patients. Instead of presenting a manual on quality assurance for radiation therapy, this manuscript provides an overview of dosimetry verification tools and a focused discussion on breath holding, respiratory gating and the applications of four-dimensional computed tomography in motion management. Some of the major challenges in the above areas are discussed.

Cone-beam computed tomography (CBCT) using an “on-board” x-ray imaging device integrated into a radiation therapy system has recently been made available for patient positioning, target localization, and adaptive treatment planning. One of the challenges for gantry mounted image-guided radiation therapy (IGRT) systems is the slow acquisition of projections for cone-beam CT (CBCT), which makes them sensitive to any patient motion during the scans. Aiming at motion artifact reduction, four-dimensional CBCT (4D CBCT) techniques have been introduced, where a surrogate for the target’s motion profile is utilized to sort the cone-beam data by respiratory phase. However, due to the limited gantry rotation speed and limited readout speed of the on-board imager, fewer than 100 projections are available for the image reconstruction at each respiratory phase. Thus, severe undersampling streaking artifacts plague 4D CBCT images. In this paper, the authors propose a simple scheme to significantly reduce the streaking artifacts. In this method, a prior image is first reconstructed using all available projections without gating, in which static structures are well reconstructed while moving objects are blurred. The undersampling streaking artifacts from static structures are estimated from this prior image volume and then can be removed from the phase images using gated reconstruction. The proposed method was validated using numerical simulations, experimental phantom data, and patient data. The fidelity of stationary and moving objects is maintained, while large gains in streak artifact reduction are observed. Using this technique one can reconstruct 4D CBCT datasets using no more projections than are acquired in a 60 s scan. At the same time, a temporal gating window as narrow as 100 ms was utilized. Compared to the conventional 4D CBCT reconstruction, streaking artifacts were reduced by 60% to 70%.

Recent initiatives of the American Board of Medical Specialties (ABMS) in the area of maintenance of certification (MOC) have been reflective of the response of the medical community to address public concerns regarding quality of care, medical error reduction, and patient safety. In March 2000, the 24 member boards of the ABMS representing all medical subspecialties in the USA agreed to initiate specialty-specific maintenance of certification (MOC) programs. The American Board of Radiology (ABR) MOC program for diagnostic radiology, radiation oncology, and radiologic physics has been developed, approved by the ABMS, and initiated with full implementation for all three disciplines beginning in 2007. The overriding objective of MOC is to improve the quality of health care through diplomate-initiated learning and quality improvement. The four component parts to the MOC process are: Part I: Professional standing, Part II: Evidence of life long learning and periodic self-assessment, Part III: Cognitive expertise, and Part IV: Evaluation of performance in practice (with the latter being the focus of this paper). The key components of Part IV require a physicist-based response to demonstrate commitment to practice quality improvement (PQI) and progress in continuing individual competence in practice. Diplomates of radiologic physics must select a project to be completed over the ten-year cycle that potentially can improve the quality of the diplomate's individual or systems practice and enhance the quality of care. Five categories have been created from which an individual radiologic physics diplomate can select one required PQI project: (1) Safety for patients, employees, and the public, (2) accuracy of analyses and calculations, (3) report turnaround time and communication issues, (4) practice guidelines and technical standards, and (5) surveys (including peer review of self-assessment reports). Each diplomate may select a project appropriate for an individual, participate in a project within a clinical department, participate in a peer review of a self-assessment report, or choose a qualified national project sponsored by a society. Once a project has been selected, the steps are: (1) Collect baseline data relevant to the chosen project, (2) review and analyze the data, (3) create and implement an improvement plan, (4) remeasure and track, and (5) report participation to the ABR, using the template provided by the ABR. These steps begin in Year 2, following training in Year 1. Specific examples of individual PQI projects for each of the three disciplines of radiologic physics are provided. Now, through the MOC programs, the relationship between the radiologic physicist and the ABR will be continuous through the diplomate's professional career. The ABR is committed to providing an effective infrastructure that will promote and assist the process of continuing professional development including the enhancement of practice quality improvement for radiologic physicists.

SBRT is increasingly utilized in liver tumor treatment. MRI-guided RT allows for real-time MRI tracking during therapy. Liver tumors are often poorly visualized and most contrast agents are transient. Gadoxetate may allow for sustained tumor visualization. Here, we report on the first use of gadoxetate during real-time MRI-guided SBRT.

Target-tracking techniques can be categorized based on the mechanism of the feedback loop. In real time tracking, breathing-delivery phase correlation is provided to the treatment delivery hardware. Clinical implementation of target tracking in real time requires major hardware modifications. In breathing synchronized delivery (BSD), the patient is guided to breathe in accordance with target motion derived from four-dimensional computed tomography (4D-CT). Violations of mechanical limitations of hardware are to be avoided at the treatment planning stage. Hardware modifications are not required. In this article, using sliding window IMRT delivery as an example, we have described step-by-step the implementation of target tracking by the BSD technique: (1) A breathing guide is developed from patient's normal breathing pattern. The patient tries to reproduce this guiding cycle by following the display in the goggles; (2) 4D-CT scans are acquired at all the phases of the breathing cycle; (3) The average tumor trajectory is obtained by deformable image registration of 4D-CT datasets and is smoothed by Fourier filtering; (4) Conventional IMRT planning is performed using the images at reference phase (full exhalation phase) and a leaf sequence based on optimized fluence map is generated; (5) Assuming the patient breathes with a reproducible breathing pattern and the machine maintains a constant dose rate, the treatment process is correlated with the breathing phase; (6) The instantaneous average tumor displacement is overlaid on the dMLC position at corresponding phase; and (7) DMLC leaf speed and acceleration are evaluated to ensure treatment delivery. A custom-built mobile phantom driven by a computer-controlled stepper motor was used in the dosimetry verification. A stepper motor was programmed such that the phantom moved according to the linear component of tumor motion used in BSD treatment planning. A conventional plan was delivered on the phantom with and without motion. The BSD plan was also delivered on the phantom that moved with the prescheduled pattern and synchronized with the delivery of each beam. Film dosimetry showed underdose and overdose in the superior and inferior regions of the target, respectively, if the tumor motion is not compensated during the delivery. BSD delivery resulted in a dose distribution very similar to the planned treatments.

BACKGROUND: Innovations in radiation therapy (RT) technology could have the potential to allow for radiation dose escalation by evaluating tumor motion, minimizing and compensating for motion, and evaluating delivery technologies such as 3-dimensonal (3D) conformal radiation therapy (CRT) and intensity-modulated RT (IMRT) using tomotherapy. MATERIALS AND METHODS: Ninety different RT plans were generated using 3 different treatment techniques for 10 patients. These were evaluated using dosimetric tools such as dose-volume histogram (DVH) analysis, tumor equivalent uniform dose (EUD), and dosimetric parameters predictive for lung toxicity, such as the volume of lung receiving > 20 Gy of radiation (V20) and the normalized mean total radiation dose to the lung (NTD mean ). The 3 techniques studied included free breathing using 3D CRT, 3D CRT with maximum-inspiration breath-hold (MIBH) to minimize tumor motion, and IMRT delivery with MIBH; the combination of 3 separate planning treatment-volume sets resulted in the generation of 90 different treatment plans. To plan these, patients underwent treatment-planning computed tomography in MIBH and free breathing followed by simulation with measurement of tumor motion and generation/evaluation of DVHs, EUDs, V20, and NTD mean . RESULTS: Average tumor motion was 1.54 cm in the cephalocaudad directions, 1.26 cm in the anteroposterior directions, and 0.56 cm in the lateral directions between maximum inspiration and expiration. Maximum-inspiration breath-hold produced superior lung sparing evidenced by lower V20 and NTD mean values, and these parameters predicted lower modeled pneumonitis rates. Tomotherapy-based IMRT provided further lung sparing. CONCLUSION: Treatment in MIBH results in lower V20 and NTD mean values and lower modeled pneumonitis rates. This effect is enhanced by the use of IMRT. The use of MIBH with IMRT may therefore aid in escalating the dose in RT.

In image-guided radiation therapy, megavoltage computerized tomography (MVCT) delivers higher dose to the patient for lower image quality than diagnostic kilovoltage CT (kVCT). One way to reduce the mean imaging dose is to reduce the imaging volume, which is often sufficient for registration and dosimetry purposes. The filtered back projection using truncated data causes artefacts that degrade the image quality. Those artefacts can be effectively reduced by wavelet-based multi-resolution analysis (WMRA), in which the detail and approximate information are reconstructed separately to bypass the non-locality of filtered back projection. In this study, WMRA was used to reconstruct local images from both very low-dose kVCT scans from a bench-top tomotherapy unit and MVCT scans from helical tomotherapy. Results show that mean imaging dose can be significantly reduced by imaging a small region of interest. In simulation, the root-mean-square error brought by the truncation is smaller than 1-2% and depends on the level of dose reduction. On the other hand, the same mean dose that would have been delivered by a low-quality global CT can be conformed to a smaller volume to improve the visibility of low-contrast organs and fine structures using WMRA. Organs at risk can be avoided during repeated daily CT imaging when irregular-shaped reconstruction areas are used. WMRA does not involve computationally expensive iterations and is suitable for image-guided radiation therapy where imaging speed is essential. Compared with extrapolation methods, errors are further reduced to improve the detection of low contrast and fine structures.

Maintenance of Certification (MOC) recognizes that in addition to medical knowledge, several essential elements involved in delivering quality care must be developed and maintained throughout one's career. The MOC process is designed to facilitate and document the professional development of each diplomate of The American Board of Radiology (ABR) through its focus on the essential elements of quality care in Diagnostic Radiology and its subspecialties, and in the specialties of Radiation Oncology and Radiologic Physics. The initial elements of the ABR-MOC have been developed in accord with guidelines of The American Board of Medical Specialties. All diplomates with a ten-year, time-limited primary certificate in Diagnostic Radiologic Physics, Therapeutic Radiologic Physics, or Medical Nuclear Physics who wish to maintain certification must successfully complete the requirements of the appropriate ABR-MOC program for their specialty. Holders of multiple certificates must meet ABR-MOC requirements specific to the certificates held. Diplomates with lifelong certificates are not required to participate in the MOC, but are strongly encouraged to do so. MOC is based on documentation of individual participation in the four components of MOC: (1) professional standing, (2) lifelong learning and self-assessment, (3) cognitive expertise, and (4) performance in practice. Within these components, MOC addresses six competencies: medical knowledge, patient care, interpersonal and communication skills, professionalism, practice-based learning and improvement, and systems-based practice.

A novel treatment approach utilizing helical tomotherapy for partial breast irradiation for patients with early-stage breast cancer is described. This technique may serve as an alternative to high dose-rate (HDR) interstitial brachytherapy and standard linac-based approaches. Through helical tomotherapy, highly conformal irradiation of target volumes and avoidance of normal sensitive structures can be achieved. Unlike HDR brachytherapy, it is noninvasive. Unlike other linac-based techniques, it provides image-guided adaptive radiotherapy along with intensity modulation. A treatment planning CT scan was obtained as usual on a post-lumpectomy patient undergoing HDR interstitial breast brachytherapy. The patient underwent catheter placement for HDR treatment and was positioned prone on a specially designed position-supporting mattress during C T. The planning target volume (PTV) was defined as the lumpectomy bed plus a 20 mm margin. The prescription dose was 34 Gy (10 fx of 3.4 Gy) in both the CT based HDR and on the tomotherapy plan. Cumulative dose-volume histograms (DVHs) were generated and analyzed for the target, lung, heart, skin, pectoralis muscle, and chest wall for both HDR brachytherapy and helical tomotherapy. Dosimetric coverage of the target with helical tomotherapy was conformal and homogeneous. “Hot spots” (≥150% isodose line) were present around implanted dwell positions in brachytherapy plan whereas no isodose lines higher than 109% were present in the helical tomotherapy plan. Similar dose coverage was achieved for lung, pectoralis muscle, heart, chest wall and breast skin with the two methods. We also compared our results to that obtained using conventional linac-based three dimensional (3D) conformal accelerated partial breast irradiation. Dose homogeneity is excellent with 3D conformal irradiation, and lung, heart and chest wall dose is less than for either HDR brachytherapy or helical tomotherapy but skin and pectoral muscle doses were higher than with the other techniques. Our results suggest that helical tomotherapy can serve as an effective means of delivering accelerated partial breast irradiation and may offer superior dose homogeneity compared to HDR brachytherapy.

Deformable image registration is an important tool for image-guided radiotherapy. Physics-model-based deformable image registration using finite element analysis is one of the methods currently being investigated. The calculation accuracy of finite element analysis is dependent on given boundary conditions, which are usually based on the surface matching of the organ in two images. Such a surface matching, however, is hard to obtain from medical images. In this study, we developed a new boundary condition to circumvent the traditional difficulties. Finite element contact-impact analysis was employed to simulate the interaction between the organ of interest and the surrounding body. The displacement loading is not necessarily specified. The algorithm automatically deforms the organ model into the minimum internal energy state. The analysis was performed on CT images of the lung at two different breathing phases (exhalation and full inhalation). The result gave the displacement vector map inside the lung. Validation of the result showed satisfactory agreement in most parts of the lung. This approach is simple, operator independent and may provide improved accuracy of the prediction of organ deformation.

Similar to conventional conformal radiotherapy, during lung tomotherapy, a motion margin has to be set for respiratory motion. Consequently, large volume of normal tissue is irradiated by intensive radiation. To solve this problem, we have developed a new motion mitigation method by incorporating target motion into treatment optimization. In this method, the delivery-breathing correlation is determined prior to treatment plan optimization. Beamlets are calculated by using the CT images at the corresponding breathing phases from a dynamic (four-dimensional) image sequence. With the displacement vector fields at different breathing phases, a set of deformed beamlets is obtained by mapping the dose to the primary phase. Optimization incorporating motion is then performed by using the deformed beamlets obtained by dose mapping. During treatment delivery, the same breathing-delivery correlation can be reproduced by instructing the patient to breathe following a visually displayed guiding cycle. This method was tested using a computer-simulated deformable phantom and a real lung case. Results show that treatment optimization incorporating motion achieved similar high dose conformality on a mobile target compared with static delivery. The residual motion effects due to imperfect breathing tracking were also analyzed.

The signal from a spirometer is directly correlated with respiratory motion and is ideal for target respiratory motion tracking. However, its susceptibility to signal drift deters its application in radiotherapy. In this work, a few approaches are investigated to control spirometer signal drift for a Bernoulli-type spirometer. A method is presented for rapid daily calibration of the spirometer to obtain a flow sensitivity function. Daily calibration assures accurate airflow measurement and also reduces signal drift. Dynamic baseline adjustment further controls the signal drift. The accuracy of these techniques was studied and it was found that the spirometer is able to provide a long-term drift-free breathing signal. The tracking error is comprised of two components: calibration error and stochastic signal baseline variation error. The calibration error is very small (1% of 3 l) and therefore negligible. The stochastic baseline variation error can be as large as 20% of the normal breathing amplitude. In view of these uncertainties, the applications of spirometers in treatment techniques that rely on breathing monitoring are discussed. Spirometer-based monitoring is noted most suitable for deep inspiration breath-hold but less important for free breathing gating techniques.

In this paper we report on the testing and verification of a system for IMRT delivery quality assurance that uses a cylindrical solid water phantom with a spiral trajectory for radiographic film placement. This spiral film technique provides more complete dosimetric verification of the entire IMRT treatment than perpendicular film methods, since it samples a three-dimensional dose subspace rather than using measurements at only one or two depths. As an example, the complete analysis of the predicted and measured spiral films is described for an intracranial IMRT treatment case. The results of this analysis are compared to those of a single field perpendicular film technique that is typically used for IMRT QA. The comparison demonstrates that both methods result in a dosimetric error within a clinical tolerance of 5%, however the spiral phantom QA technique provides a more complete dosimetric verification while being less time consuming. To independently verify the dosimetry obtained with the spiral film, the same IMRT treatment was delivered to a similar phantom in which LiF thermoluminescent dosimeters were arranged along the spiral trajectory. The maximum difference between the predicted and measured TLD data for the 1.8 Gy fraction was 0.06 Gy for a TLD located in a high dose gradient region. This further validates the ability of the spiral phantom QA process to accurately verify delivery of an IMRT plan.

A clinical helical tomotherapy treatment machine has been installed at the University of Wisconsin Comprehensive Cancer Center. Beam alignment has been finalized and accepted by UW staff. Helical tomotherapy will soon be clinically available to other sites. Clinical physicists who expect to work with this machine will need to be familiar with its unique dosimetric characteristics, and those related to the geometrical beam configuration and its verification are described here. A series of alignment tests and the results are presented. Helical tomotherapy utilizes an array of post-patient xenon-filled megavoltage radiation detectors. These detectors have proved capable of performing some alignment verification tests. That is particularly advantageous because those tests can then be automated and easily performed on an ongoing basis.

SUMMARY The purpose of our study was to examine the extent of patient-independent intrinsic error associated with multiple, repeat remounting of the Laitinen Stereoadapter. The Laitinen frame was repeatedly mounted on a solid water phantom and imaged using computed tomography (CT). The phantom contained five targets located in the center, anterior, right, left, and posterior orientations. The images were processed, fused, and analyzed on the Pinnacle™ 3-D treatment planning system. The coordinate values (in the x, y, and z directions) for each target were determined for each mounting, and an absolute mean deviation was calculated for 11 repetitions. The mean deviation in the x, y, and z direction for the central and right target, and in the ◊ and y direction for the posterior and anterior target was less than 2.0 mm. However, the mean error in the z direction of the anterior and posterior targets was 1.79 ± 1.02 mm and 2.20 ± 1.32 mm, respectively. Rotational misalignment during repeat frame fixation contributed to the observed deviations and in particular affected the antero-posterior plane. With the exception of two occasions where an obvious mounting error occurred, a significant portion of error from remounting the Laitinen Stereoadapter is associated with the operator and the imaging process. The observation of an angular displacement around the axis through the earplugs suggests that a certain degree of rotational misalignment in daily remounting is possible. Targets in the antero-posterior plane are most susceptible to localization error as a consequence of rotational misalignment. In summary, the overall error is within the limits of current imaging technology but not within submillimeter accuracy. Clinical application should take these errors into consideration when designing field margins. © 2001 Wiley-Liss, Inc.

A solid water/cylindrical phantom is machined to create a spiral cavity for placing radiographic or radiochromatic film in a spiral configuration. This spiral phantom is used to sample, predict, and measure data in three-dimension subspace. The predicted data are obtained by projecting the patient plan data on the spiral phantom in the treatment planning software. The measured data are obtained by irradiating the spiral phantom (with film in the spiral cavity) as per the treatment plan. The predicted and measured data are converted to a two-dimensional matrix and plotted as a spiralogram. Comparison of these predicted and measured spiralograms provides a quantitative comparison and thus validation of treatment delivered as planned. The spiral phantom is a simple, cost-effective approach to sample 3D data from complexly shaped, intensity modulated or compensated multiple beams. A software script is being written to automate the entire process of projection, data sampling, and comparison. Design aspects and some examples of dose verification are presented. The usefulness of the spiral phantom for intensity modulated radiation therapy and dynamic field shaping are discussed.

Two patients developed skin ulcers secondary to high doses of diagnostic-energy x rays received during localization procedures as part of brachytherapy treatments. Both were morbidly obese and diabetic. The obesity led to the delivery of estimated skin doses of 83 Gy in one case and 29 Gy in the other in attempts to produce readable images on localization radiographs. This report discusses the factors leading to the injuries, the progression of the injuries over time, and the variables involved in the localization procedures with the aim of preventing future mishaps. The greatest contribution to the large skin dose was the need, with the equipment available, to use multiple exposures to produce a single film, because of the effect of the resultant reciprocity failure.

Seventy-one patient-specific delivery quality assurance (DQA) plans for the Tomotherapy HI-ART II helical tomotherapy system (TomoTherapy, Inc., Madison, WI, USA) were measured using film and ion chamber. The agreement in absolute point dose was 1.19 +/- 0.79%, 1.91 +/- 1.39%, 2.14 +/- 1.3%, 1.3 +/- 0.73% and 1.67 +/- 1.5% for head and neck, prostate, pelvis-abdomen sites, and for all other sites. The spatial agreement between the calculated and the measured film dose distributions was evaluated using the gamma metric distribution. The average frequency versus gamma interval was plotted as a bar graph to quantify the gamma index variation inside the region of interest for each body site.

Background and purpose : Anatomic contour irregularity and tissue inhomogeneity can lead to significant radiation dose variation across the complex treatment volumes found in the head and neck (H&N) region. This dose inhomogeneity can routinely create focal hot or cold spots of 10–20% despite beam shaping with blocks or beam modification with wedges. Since 1992, we have implemented the routine use of 3-D custom tissue compensators fabricated directly from CT scan contour data obtained in the treatment position in order to improve dose uniformity in patients with tumors of the H&N. Materials and methods : Between July 1992 and January 1997, 160 patients receiving comprehensive H&N radiotherapy had 3-D custom compensators fabricated for their treatment course. Detailed dosimetric records have been analyzed for 30 cases. Dose uniformity across the treatment volume and clinically relevant maximum doses to selected anatomic sub-sites were examined with custom-compensated, uncompensated and optimally-wedged plans. Results : The use of 3-D custom compensators resulted in an average reduction of dose variance across the treatment volume from 19±4% for the uncompensated plans to 5±2% with the use of 3-D compensators. Optimally-wedged plans were variable, but on average a 10±3% dose variance was noted. For comprehensive H&N treatment which encompassed the larynx within the primary field design, the peak doses delivered were reduced by 5–15% with 3-D custom compensation as compared to optimal wedging. Conclusions : The use of 3-D custom tissue compensation can improve dose homogeneity within the treatment volume for H&N cancer patients. Maximum doses to clinically important structures which often receive greater than 105–110% of the prescribed dose are routinely reduced with the use of 3-D custom compensators. Improved dose uniformity across the treatment volume can reduce normal tissue complication profiles and potentially allow for delivery of higher total doses in an attempt to enhance locoregional tumor control.

Purpose: With increased interest in 3-D conformal radiation therapy and dose escalation, it is necessary to provide advanced techniques to assure quality in treatment delivery. Multi-institutional trials for these newer treatment techniques require methods of verifying the consistency of treatments between the participating institutions. For this reason, a phantom was designed to address the quality and consistency of Radiation Therapy Oncology Group (RTOG) 3-D prostate treatment protocol. Methods and Materials: A solid water pelvic and prostate phantom for imaging, volume rendering, treatment planning, and dosimetry applications for performing comprehensive quality assurance has been designed and fabricated. Its configuration was based upon CT slices obtained from a patient study. Individual slices were machined with corresponding contours of the prostate, bladder, rectum, and the left and right femurs. Most of the phantom is made of solid water (Gammex/RMI, Middleton, WI), while the femurs are made of bone-equivalent material. The CT numbers from patient images were used to adjust the solid water composition within the organ volumes, providing image contrast from the remainder of the phantom. Cylindrical insertion grooves are machined in the phantom to allow placement of ionization chambers and thermal luminal dosimeters (TLDs) for dosimetry applications. During imaging, the cavities are filled with rods fabricated from solid water material. Results: The phantom is being used to evaluate the consistency of a range of processes in radiation therapy simulation, planning, and delivery of 3-D-based treatments for prostate cancer. Conclusion: The ultimate study objective is to use the phantom to evaluate the accuracy and consistency of treatments delivered by institutions participating in national collaborative clinical trials involving 3-D conformal dose escalation.

A new thermoplastic material with extremely desirable physical and radiation shielding properties is presented. The material softens between 108 degrees F and 132 degrees F and can then be easily molded to any desired shape. As it cools down it hardens at about 102 degrees F, retaining its molded shape. It is very light (rho = 1.66 g/cc), compared to most other compensating and shielding materials used in the clinic. Its photon and electron attenuation characteristics have been measured and are compared with other materials relevant to radiotherapy. Possible applications as a bolus material, compensator and partial or total shielding material in clinical radiation therapy are discussed.

Fluence of clinical electron beams enter into most dosimetry models, and it is an important quantity for evaluating the perturbation of beams by treatment accessories and collimation devices. Furthermore, electron beam dose calculation algorithms require incident fluence data as input. The magnitude of the electron fluence for patient treatments is very low and a specially designed fluence-meter is required. An ion-implanted semiconductor detector with an ultra-thin depletion layer is the most suitable instrument for this purpose. The energy deposited in a thin detector, with no window, is directly proportional to the incident number fluence of clinical electron beams. Such a detector was calibrated against a flat Faraday cup in vacuum and provided a measure of true electron fluence, with almost no energy and directional dependence. The precision of the calibrations is better than 2%. The accuracy of the results was verified against Monte Carlo calculations and clinical dose expectations. Fluence output and profiles, and angular fluence resulting from the use of a variety of cones and blocks were measured.

Post-treatment dose verification refers to the process of reconstructing delivered dose distributions internal to a patient from information obtained during the treatment. The exit dose is commonly used to describe the dose beyond the exit surface of the patient from a megavoltage photon beam. Portal imaging provides a method of determining the dose in a plane distal to a patient from a megavoltage therapeutic beam. This exit dose enables reconstruction of the dose distribution from external beam radiation throughout the patient utilizing the convolution/superposition method and an extended phantom. An iterative convolution/superposition algorithm has been created to reconstruct dose distributions in patients from exit dose measurements during a radiotherapy treatment. The method is based on an extended phantom that includes the patient CT representation and an electronic portal imaging device (EPID). The convolution/superposition method computes the dose throughout the extended phantom, which allows the portal dose image to be predicted in the EPID. The process is then reversed to take the portal dose measurement and infer what the dose distribution must have been to produce the measured portal dose. The dose distribution is modeled without knowledge of the incident intensity distribution, and includes the effects of scatter in the computation. The iterative method begins by assuming that the primary energy fluence (PEF) at the portal image plane is equal to the portal dose image, the PEF is then back-projected through the extended phantom and convolved with the dose deposition kernel to determine a new prediction of the portal dose image. The image of the ratio of the computed PEF to the computed portal dose is then multiplied by the measured portal dose image to produce a better representation of the PEF. Successive iterations of this process then converge to the exiting PEF image that would produce the measured portal dose image. Once convergence is established, the dose distribution is determined by back-projecting the PEF and convolving with the dose deposition kernel. The method is accurate, provided the patient representation during treatment is known. The method was used on three phantoms with a photon energy of 6 MV to verify convergence and accuracy of the algorithm. The reconstructed dose volumes agree to within 3% of the forward computation dose volumes. Furthermore, this technique assumes no prior knowledge of the incident fluence and therefore may better represent the dose actually delivered.

The convolution/superposition method was used to predict the dose throughout an extended volume, which includes a phantom and a portal imaging device. From the calculated dose volume, the dose delivered in the portal image plane was extracted and compared to a portal dose image. This comparison aids in verifying the beam configuration or patient setup after delivery of the radiation. The phantoms used to test the accuracy of this method include a solid water cube, a Nuclear Associates CT phantom, and an Alderson Rando thorax phantom. The dose distribution in the image plane was measured with film and an electronic portal imaging device in each case. The calculated portal dose images were within 4% of the measured images for most voxels in the central portion of the field for all of the extended volumes. The convolution/superposition method also enables the determination of the scatter and primary dose contributions using the particular dose deposition kernels for each contribution. The ratio of primary dose to total dose was used to extract the primary dose from the detected portal image, which enhances the megavoltage portal images by removing scatter blurring. By also predicting the primary energy fluence, we can find the ratio of computed primary energy fluence to total dose. Multiplying this ratio by the measured dose image estimates the relative primary energy fluence at the portal imager. The image of primary energy fluence possesses higher contrast and may be used for further quantitative image processing and dose modeling.

Purpose: To compare concurrent vs. sequential ferromagnetic thermoradiotherapy in vivo . Methods and materials: Greene melanomas were implanted subretinally in rabbits and observed until they were 3–5 mm in diameter. Episcleral plaques were assembled with 125 I seeds for radiation therapy, or with ferromagnetic (FM) theremoseeds and nonradioactive I seeds for hyperthermia. Rabbits were implanted by centering a plaque over the intraocular melanoma. After a given dose of radiation had been delivered, the plaque was removed and a nonradioactive plaque containing FM thermoseeds was inserted into the same extrascleral space. One hour later, hyperthermia (46–47°C at the plaque-scleral interface) was initiated and continued for a period of 1 h by placing the rabbits in a magnetic induction coil powered to 1200 W. Tumor size was determined at 1-to 2-week intervals by indirect ophthalmoscopy and by ultrasound. Results: Dose-response analysis of 27 treated eye melanomas showed 50% local tumor control at 43 Gy for 125 I alone and 29.4 Gy for 125 I followed by FM hyperthermia. the thermal enhancement ratio was 1.4. Conclusion: Comparison with a previously published thermal enhancement ratio of 4.4 (for concurrent 125 I and FM hyperthermia) leads us to conclude that thermal enhancement of 125 I brachytherapy is more efficient in this tumor model system when hyperthermia is delivered during, rather than after, the irradiation process.

The accuracy of the point source approximation used in dose calculations for an implant comprised of multiple high dose rate (HDR) Ir-192 source dwell positions is investigated. First, a single dwell position implant is modeled. The exposure rate about the source is calculated using both the point source approximation and the more rigorous line source formalism. A comparison of these calculated exposure rates is made. It is found that for each HDR Ir-192 source dwell position, the point source approximation results in a dose overestimation of 1% at a distance of 1 cm on the source transverse axis, while dose underestimations of more than 2% can be found at a distance of 1 cm on the source longitudinal axis. Even larger errors occur closer to the source. The results of this academic study are then extended to two clinical cases--an endobronchial treatment and a tandem and ovoids setup, both involving multiple source dwell positions. Since clinical HDR Ir-192 implants are comprised of many individual source dwell positions, there will be inaccuracy in the calculated overall dose distribution leading to dose delivery errors. For example, the dose delivered to a prescription point located 0.5 cm from an endobronchial applicator will be 3% lower than prescribed. Similar errors are produced in gynecologic implants. To decrease below 0.5% the dose delivery error resulting from the point source approximation, prescription points should be at a distance of at least 1 cm from any applicator. Since the dosimetry error is a direct result of the choice of model used to describe the source, the use of anisotropy factors accounting for the variation of photon fluence around the HDR Ir-192 source will not completely correct the calculation.

Normalized head-scatter factors were measured with cylindrical beam coaxial miniphantoms and high purity graphite buildup caps for 4-, 6-, 10-, and 24-MV photon beams at field sizes from 4 x 4 to 40 x 40 cm2. The normalized head-scatter factors determined by the two methods matched well for 4- and 6-MV photon beams. The miniphantom technique produced normalized head-scatter factors 1.5% and 4.8% lower than the buildup caps for the 10- and 24-MV beams for large field sizes, respectively. At small field sizes, the miniphantom technique produced larger normalized head-scatter factors than the buildup caps. Measurements made with an electromagnet indicate that a significant portion of the ionization measured in the buildup cap at 24 MV arises from contamination electrons. Measurements made with the miniphantom and magnet found no contamination electron contribution. The miniphantom technique may exclude such contamination electrons, potentially leading to inaccuracies in tissue-maximum ratios and phantom scatter factors, as well as inaccuracies in monitor unit calculations.
