Background: The difference between the pathogenic isolates and commensals of Escherichia coli is based on their phylogenetic antecedents. In Venezuela there are few studies that describe the pathogenic potential of phylogenetic groups in E. coli., Aims: Relate antimicrobial susceptibility, distribution of phylogenetic groups and virulence genes in strains of uropathogenic E. coli (ECUP) isolated from patients with UTI., Methods: We studied 17 ECUP strains, isolated from adult patients hospitalized in two health institutions. The susceptibility to 8 antibiotics was determined by the broth microdilution (MDC) method. Extended spectrum β-lactamases (ESBL) and carbapenemases were phenotypically detected. The phylogenetic groups and the detection of the virulence genes were determined by PCR., Results: All strains synthesized ESBL and of these, 41% were associated with the production of a carbapenemases (KPC or MBL). The phylogroup B2 (41%) was the most predominant. The most frequent virulence genes were fimH and fyuA with 82% each. Only one strain from group F was positive to the 6 genes studied., Discussion: The diversity of associations between virulence genes and resistance profiles in the ECUP are evolving continuously, their distribution in the different phylogenetic groups depends to a large extent on the clinical epidemiological characteristics of the study groups.

Background: Malaria is expanding rapidly across Venezuela, spreading outwards from traditional high transmission regions in the southeast of the country, but the lack of official data make it impossible to understand the reasons for this expansion and to estimate its real magnitude. This study aims to evaluate the epidemiological characteristics driving the re-emergence of malaria in Mérida, a state in the west of Venezuela, where no cases have been reported since 2003, and also to study the clinical presentation of the disease in patients presenting with malaria., Methods: Thirty-three patients who presented with anemia and fever and with a microscopic diagnosis of malaria were examined and interviewed. Data were collected in standardized forms and analyzed. One-way analysis of variance was used to study differences among patients infected with different parasites., Results: Twenty-two patients were from the Zulia state and eleven were from the Mérida state, mainly from the lowlands south of Lake Maracaibo. Six of these patients traveled to the Bolívar state between 2017 and 2019. Thirteen patients presented with the WHO criteria for severe malaria.Conclusions:Domestic migration to the southeast of Venezuela may have played an important role in the expansion of malaria in previously existing endemic areas of transmission and also in the increase in the number of cases of severe malaria., (© The Author(s) 2019. In this study, the distribution of phylogenetic groups and the genetic detection of virulence factors in CTX-M-15 β-lactamase-producing uropathogenic Escherichia coli (UPEC) strains were analyzed. Twenty eight strains were isolated between January 2009 and July 2011 from patients with urinary tract infection (UTI) who attended the Public Health Laboratory at Mérida, Venezuela. Determination of phylogenetic groups and detection of six virulence genes, fimH, fyuA, kpsMTII, usp, PAI and papAH, were performed by PCR amplification. Fifteen of the 28 isolates were mainly located in the phylogenetic group A, followed by B2 (12/28) and D (1/28). No direct relationship between the severity or recurrence of UTI and the distribution of phylogroups was observed. All studied virulence factors were found in group B2 strains with the highest frequency. The prevalent virulence profile included the combination of three main genes: fimH, kpsMTII and fyuA and, to a lesser extent, the presence of other determinants such as usp, PAI and/or papAH. These results indicate that virulent UPEC incorporated three important properties: adhesion, iron uptake and evasion of phagocytosis, which favored the production of recurrent UTI. This is the first report describing the association of phylogenetic groups with the potential virulence of CTX-M-15 β-lactamase producing UPEC strains in Venezuela., (Copyright © 2014 Asociación Argentina de Microbiología. Klebsiella pneumoniae as a producer of extended spectrum beta-lactamase (ESBL) has played an important role as a cause of infection in the neonatal high risk unit (NHRU) of the Autonomous Hospital Institute of the Universidad de Los Andes (AHIULA). In this paper an outbreak caused by this bacterial specie that affected neonates hospitalized in this unit during February 2007 is described. Besides, the environment and the personnel were studied as possible sources of this organism. The strains of K. pneumonia isolated from the outbreak had the same resistance phenotype, produced ESBL type TEM and SHV and belonged to the same genotype as the isolated strains from the hands and the soapy solutions, possible sources of infection. This indicates that it was the same clone. The outbreak was resolved using two important measurements: reinforcing hand washing and with the opportune treatment of neonates with imipenem.

Klebsiella pneumoniae is the principal opportunist pathogen associated with nosocomial infections. The morbidity and mortality produced by this microorganism is related mostly to antimicrobial resistance and its capacity to form biofilms. In this study, the phenotypic variations of susceptibility in 50 Klebsiella pneumoniae strains (26 betaLEE-producer and 24 no betaLEE-producer) isolated from pediatric patients with nosocomial septicemia at the High-risk Neonatal Unit of the Instituto Aut6nomo Hospital Universitario de Los Andes, Mérida, Venezuela and their associations with biofilm formation were analyzed. These strains were biochemically identified by standard microbiology methods. Antimicrobial susceptibility was determined by minimal inhibitory concentration and biofilm formation was detected by spectrophotometric techniques. Most of betaLEE strains were frequently associated with resistance markers such as tobramycin, kanamycin, tetracycline, chloramphenicol (38% each), streptomycin (36%) and netilmicin (34%). The predominant multiresistent phenotype was composed of betaLEE+ -AMG-TET-CLF (30%). 18% of K. pneumoniae strains were sensible to the antibiotics tested. Formation of biofilm was observed in 77.8% and 96.2% of sensible and resistent strains, respectively. There was no statistic relation between phenotipic variations of susceptibility and biofilm formation (p > = 0.05). However, both characteristics of K. pneumoniae should be evaluated to determine the therapeutic conduct to treat patients with nosocomial infection.

Background: Nontyphoid Salmonella (NTS) infections are a frequent cause of self-limited diarrhoeal illness in healthy children that do not usually require antibiotic treatment. This study was conducted by analyzing the phenotypic and genotypic traits of NTS strains from pediatric patients with acute gastroenteritis living in urban areas in the city of Mérida, Venezuela., Methodology: Thirty-seven Salmonella strains (18 S. Enteritidis; 14 S. Typhimurium; 2 S. Java; 2 S. Saintpaul; 1 S. Infantis) were isolated from 243 stool specimens. These strains were biochemically identified and serotyped. Antimicrobial susceptibility was determined by the disc-diffusion assay. Genetic characterization included plasmid profiling, PCR detection of the spv region and inv genes, and IS200 typing., Results: Thirty (81.0%) of the Salmonella isolates were resistant to the antimicrobial tested. Of these strains, 17 (56.7%) were resistant to at least one antibiotic. Five resistance patterns were observed, of which the most frequently found was the single type (tetracycline, streptomycin or ampicillin). All the S. Typhimurium harbored plasmids, but only three large plasmids (60, 72 and 84 kb) yielded amplicons with a spvR specific primers. All the Salmonella serotypes showed the presence of an inv region. Eight distinct IS200 profiles could be detected among the 37 Salmonella strains studied., Conclusions: Predominant Enteritidis and Typhimurium serotypes, as well as serotypes Java, Saintpaul and Infantis, are circulating in the city of Mérida, Venezuela. Most of these strains are susceptible to first-line antibiotics but active monitoring of isolates for antimicrobial resistance is necessary. IS200 typing, applied in association with conventional methods, allowed the characterization of all isolates and suggested the presence of different infection sources.

The prevalence of hematozoan infections (Hepatozoon canis and Babesia sp., particularly Babesia canis vogeli) in canids from Venezuela, Thailand and Spain was studied by amplification and sequencing of the 18S rRNA gene. H. canis infections caused simultaneously by two different isolates were confirmed by RFLP analysis in samples from all the geographic regions studied. In Venezuela, blood samples from 134 dogs were surveyed. Babesia infections were found in 2.24% of the dogs. Comparison of sequences of the 18S rRNA gene indicated that protozoan isolates were genetically identical to B. canis vogeli from Japan and Brazil. H. canis infected 44.77 per cent of the dogs. A representative sample of Venezuelan H. canis isolates (21.6% of PCR-positives) was sequenced. Many of them showed 18S rRNA gene sequences identical to H. canis Spain 2, albeit two less frequent genotypes were found in the sample studied. In Thailand, 20 dogs were analyzed. No infections caused by Babesia were diagnosed, whereas 30 per cent of the dogs were positive to hematozoan infection. Two protozoa isolates showing 99.7-100% identity to H. canis Spain 2 were found. In Spain, 250 dogs were studied. B. canis vogeli infected 0.01% of the animals. The sequence of the 18S rRNA gene in Spanish isolates of this protozoa was closely related to those previously deposited in GenBank (> 99% identity). Finally, 20 red foxes were screened for hematozoans employing semi-nested PCR and primers designed to detect Babesia/Theileria. Fifty percent of the foxes were positive to Theileria annae. In addition, it was found that the PCR assay was able as well to detect Hepatozoon infections. Thirty five percent of the foxes were infected with two different H. canis isolates showing 99.8-100% identity to Curupira 1 from Brazil.

Trypanosoma cruzi causes Chagas' disease, a systemic infection that affects cells of meso-, endo-, and ectodermic origin. However, as far as we know, the presence of T. cruzi stages in bone has not been reported previously, and it has scarcely been investigated in cartilage. We inoculated 7- and 20-day-old (8 and 15 g) NMRI albino mice i.p. with metacyclic trypomastigotes from Rhodnius prolixus used for xenodiagnosis of mice previously infected with mammalian, human, and triatomines isolates, characterized by randomly amplified polymorphic DNA as zymodeme 1 (equivalent to T. cruzi I). Tissular parasitism (quantified according to the number of pseudocysts/50 fields 400x) showed amastigotes, intermediate forms, or trypomastigotes in sternum chondroblasts, osteoblasts, macrophages, and fibroblasts; chondrocyte and osteocyte invasion was rare. All isolates parasitized bone marrow macrophages, with few amastigotes. We observed marked associated myotropism, with or without inflammatory infiltration; there were small numbers of intensely parasitized mononuclear cells in perichondrium and periosteum. We discuss the results in relation to the marked differences of the T. cruzi tropism toward the different types of sternum cells, and, additionally, we outline the possibility of transmitting parasitized bone marrow through transplants. The fact of finding parasite stages in sternum bone and cartilage may be considered important due to the studies on Chagas' disease paleoparasitology that are based on histological and molecular analysis.

Helicobacter pylori infection is common in people. However, the existence of extra gastric reservoirs and transmission routes remain controversial in the field. Because the oral cavity has been proposed as a reservoir for H. pylori, a study was carried out to determine the presence of H. pylori in dental plaque and saliva. The results were asociated with those obtained in the gastric biopsy. Ninety-seven dyspeptic and fifty asymptomatic patients were studied and samples taken for biopsy, dental plaque and saliva. The gastric biopsies were evaluated using microbiology and histology methods. Cultures and urease tests were carried out on the oral cavity samples and included pretreatment methods using urea and HCl. The frequency of H. pylori for all the patients evaluated was 75.5%. H. pylori was not isolated in saliva or dental plaque in any of the two groups studied with or without sample pretreatment. The urease test in dental plaque was positive in 99.3% of the patients and 89.8% in saliva. There was no statistically significant difference between the infection prevalence by H. pylori in dyspeptic or not dyspeptic patients. The obtained results suggest that the methodology used for the detection of H. pylori is not sufficiently sensitive for the determination of the microorganism in the oral cavity.

Nosocomial infections are a significant cause of morbidity and mortality throughout the world. In developing countries it is difficult to carry out effective surveillance and control programs for this type of infection because of the cost in both human and material resources. These considerations prompted us to perform a prospective study to determine the epidemiologic and microbiologic characteristics of nosocomial infections due to Staphylococcus aureus in the High-risk Neonatal Unit (HRNU) of the Instituto Autónomo Hospital Universitario de Los Andes (IAHULA), during the period of November 1997 to October 1998. Among a total of 120 microorganisms, 24 (20%) strains of Staphylococcus aureus were isolated; 47% were recovered from blood and 33% from conjunctive samples. Among the cases of conjunctivitis, S. aureus was the only pathogen isolated in 42%. Twenty of the 24 Staphylococcus aureus strains (83%) were methicillin-resistant (MRSA). According to their resistance profiles, we established 12 groups of strains from neonates with nosocomial infections and 1 group of strains from the two carriers among the healthcare personnel detected by microbiological screening. The MeRGmR pattern was the most frequent. Plasmid analysis disclosed two profiles, each having a plasmid molecular weight over 23.130 bp. The MRSA strains isolated from the neonates and those isolated from the carriers showed the same plasmid profile. This suggests that the healthcare personnel may have acted as reservoirs of the MRSA strains found in neonates with nosocomial infection.

The aim of this study was to evaluate foods microbiological quality in food-service establishments. A total of 620 food samples were obtained from four establishments and analyzed for aerobic mesophilic (AM), yeasts, moulds, Staphylococcus aureus and Escherichia coli counts and the presence of Salmonella spp. Drinking water, equipment, surfaces, environment and food handlers were also sampled. E. coli was found in raw vegetables (76.2%), cooked vegetables (15.2%), beef and pork (15.9%), poultry (16.7%), fish (11.8%), desserts (27.3%), equipments (57.9%), surfaces and environment (53.6%) and in 21.9% of food handlers. Survey results were compared with the recommended maximum microbial levels. Our results demonstrate the need for the adoption of more effective hygienic measures in this kind of establishment in order to avoid any risk to consumers.

Twelve Klebsiella pneumoniae isolates resistant to expanded-spectrum cephalosporins and aztreonam, from patients with nosocomial septicaemia at the intensive care unit of the Andes University Hospital, Mérida, Venezuela, were studied for production of extended-spectrum beta-lactamase (ESbetaL) activity. All were also resistant to gentamicin, kanamycin, tetracycline and chloramphenicol but sensitive to cefoxitin, imipenem, amikacin and tobramycin. Production of ESbetaL activity was confirmed by restoring susceptibility to ceftazidime in the presence of clavulanic acid. All isolates carried an identical plasmid of approximately 87 kb. Resistance to beta-lactams, aminoglycosides, tetracycline and chloramphenicol was lost en bloc after plasmid curing by treatment with acridine orange and was transferable en bloc to Escherichia coli by conjugation. Transconjugants always showed the same plasmid profile as that of Klebsiella donors. Isoelectric focusing analysis of the crude extracts of transconjugants showed in all cases, the presence of two beta-lactamases of pI 5.4 and 8.2. Analysis of the plasmid carried by one of the transconjugants by means of hybridization assays, revealed the presence of both bla(TEM) and bla(SHV) determinants. Cloning and sequencing of each determinant identified them as bla(TEM-1) and bla(SHV-5), respectively, the latter being responsible for the ESbetaL activity. Results of this study indicate that ESbetaL determinants of the SHV-type carried by transferable elements, are spreading among nosocomial isolates of K. pneumoniae in Mérida, Venezuela.

A total of 83 adult patients between 25 and 60 years of age who acquired pulmonary infectious disease within 48 hours after being admitted to the medical, surgical and/or intensive care units at the university hospital of the "Universidad de Los Andes" (Mérida, Venezuela) were studied during a period of 18 months. The most frequent clinical entity observed was pneumonia (49.4%). In all types of pulmonary samples obtained by secretion or puncture, more than half gave positive results to the microbiological analysis. Gram-negative bacilli prevailed as the etiological agents, with a preponderance of the species from Klebsiella (21.7%) and Pseudomonas (15.1%) genus. Staphylococcus aureus was also isolated in 13.3% of all cases. Satisfactory sensitivity to third generation cephalosporins and aminoglucosides was observed on these strains. Quinolones showed the highest antimicrobial activity. The application of an adequate antimicrobial therapy, according to the antibiotics sensitivity test results, allowed a successful clinical efficacy in 67.4% of all cases.
