Mortgage debt is financially burdensome for many older homeowners in the United States. As people age, declining health can bring about increased healthcare costs. Focusing on homeowners aged 50 and older in the U.S., we investigate two research questions: (1) To what extent does a heath shock affect the likelihood of paying off a mortgage voluntarily or involuntarily? and (2) how long does a health shock delay the time it takes to pay off a mortgage? We used eight biannual waves (2004–2018) of the Health and Retirement Study containing 11,772 borrowers to build survival regression models. Results showed that a health shock reduced the likelihood of voluntary payoff by 12%, while it increased the probability of involuntary payoff by 18%. A health shock delayed voluntary and involuntary payoffs for 30 and 21 months, respectively. We discuss tax deduction and HELOCs as strategies to reduce older homeowners' mortgage strain. Background. Portal vein tumor thrombosis (PVTT) is one of the major predictive factors for patients with hepatocellular carcinoma (HCC). The objective of this study was to establish a prognostic nomogram for identifying individual survival outcomes in patients with HCC and PVTT on conservative treatment based on specific factors. Methods. Two hundred and ten patients with HCC and PVTT on conservative treatment in Beijing Ditan Hospital between June 2008 and May 2017 were studied retrospectively as a derivation cohort. We built a nomogram based on independent risk factors for survival prediction. The concordance index (c-index) and a calibration curve were used to evaluate the predictive accuracy. During the study, 102 patients were included at the Putuo Hospital and Third People's Hospital of Changzhou as a validation cohort. Results. In the derivation cohort, the independent factors for overall survival were identified by multivariate analysis, namely, aspartate aminotransferase ≥119 IU/L, gamma-glutamyl transferase ≥115 IU/L, Child–Pugh class C liver function, creatinine ≥91 μmoI/L, α-fetoprotein ≥400 ng/ml, and largest tumor diameter ≥5 cm. The nomogram had a c-index of 0.737 (95% confidence interval, 0.692–0.782) and the calibration curves fitted well. The median survival time was 4.2 months in the derivation cohort, with an MST of 5 months for BCLC C stage and 1.8 months for BCLC D stage patients. Kaplan–Meier analysis showed significant statistical differences in the 6-month overall survival rates of the primary and validation cohorts after the total scores were divided into three quartiles (low risk: 0–85; intermediate risk: 86–210; high risk: ≥211; p < 0.0001 in both cohorts). Conclusions. The nomogram can be a more accurate and individualized prediction for 6-month overall survival of patients with HCC and PVTT on conservative treatment, and it is possible to consider further active interventions for patients in the low-risk group (0–85 scores) to achieve the aim of prolonging survival. Objectives: To investigate whether intravenous thrombolysis (IVT) with alteplase (a recombinant tissue plasminogen activator, rt-PA) before endovascular treatment (EVT) is beneficial for acute ischemic stroke (AIS) patients in different periods., Methods: This study enrolled a total of 140 patients hospitalized between 2019 and 2022 with AIS from large vessel occlusion (LVO) in the anterior circulation. Those patients were divided into the EVT alone group and IVT + EVT group, in which EVT was preceded by intravenous rt-PA. According to the time from onset to femoral artery puncture, the above two groups were divided into the following subgroups: < 4.5 h, between 4.5 and 6 h, between 6 and 8 h, and between 8 and 10 h. There were 78 patients in the EVT alone group and 62 patients in the IVT + EVT group., Results: There was no statistically significant difference in functional independence, recanalization rate, favorable outcome rate, or mortality between the EVT and IVT + EVT groups (P > 0.05). After adjusting for confounding factors, a lower incidence of intracerebral hemorrhage was observed in the EVT group (P < 0.05). A comparison of time-dependent efficacy between the two groups showed that within 6-8 h, there were statistically significant differences between admission and postoperation in the National Institutes of Health Stroke Scale scores at 24 h (P = 0.01) or 7 days (P = 0.02)., Conclusions: Although there was no difference in clinical efficacy and safety between the abovementioned two groups, treatment with IVT + EVT could increase the risk of bleeding compared to EVT. Palpation is one of the major diagnostic methods in Chinese medicine. The article has summed up the current status, features and origins of various palpations in different acupuncture schools in the United States, including palpation on meridian, collaterals and acupoints in traditional Chinese acupuncture, abdominal palpation of Japanese acupuncture, pulse checking for couples in five-element acupuncture, palpation on tendon straps and muscle response testing in acupuncture physical medicine. Since great importance has always been attached on social psychological factors in all acupuncture schools of America, it is thus concluded that the interactive mechanism of diagnosis and treatment represented by palpation feedback examination is not only the manifestation of people oriented individualized treatment both physically and psychologically in America, but also a kind of inheritance and development of diagnosis and treatment in the modernization of TCM.
