The article examines economic developments that affect taxation in the U.S. Apparent inattention to globalization-driven tax problems so far may be largely attributed to their relative unimportance by comparison with other public and foreign policy challenges. Yet the failure to act with determination now allows current losses to grow and also permits abuse to become better entrenched. The stakes of reform opponents grow a bit larger with each passing day. Every modern country has recently addressed the size and extent of the welfare state, but very few citizens in any of them reject a substantial national role in providing social insurance. Most of that social insurance is redistributive. In addition, many would like to see a countervailing state role to address the increasing inequality of income and wealth generated by the global information economy. Even most of those who disagree with the second direction would rather see policy determined by citizen choice than imposed by the irresistible forces of globalization. For these reasons, a decline in the state's power to tax would signal a serious sovereign loss that should be resisted. Successful resistance demands determined policy responses to counter incipient threats.

The reports differ in their combinations of mortality data, process measures, access scores, and satisfaction surveys. The internet sites also differ by access charge, data sources, and presentation of information. Public disclosure of quality data has had little impact on the behavior of consumers, larger purchasers of health care, and physicians. However, health care provider organizations have responded to the public reports of quality. Analysis of the impact of public report cards is lagging as web-available reports rapidly grow and pay for performance programs emerge.
