The United States plays a crucial role in shaping global health through its policy decisions and engagements. Historically, bipartisan support underpinned U.S. involvement in multilateral and bilateral global health initiatives in advancing its national health, security interests, and foreign policy. However, recent decades have witnessed increased politicization and polarization of global health and fluctuating stances between Republican and Democratic administrations. This commentary speculates the potential implications of the 2024 presidential election on global health, focusing on how ideological differences between parties and previous administrative actions might affect U.S.'s strategies in addressing key global health issues, including governance, funding allocation, sexual and reproductive health policies, responses to humanitarian crises, and efforts to combat climate change. The election may be a critical juncture that could determine whether the U.S. global health strategies will continue to reflect the globalist and liberal policies typically associated with recent Democratic administrations or shift back to the isolationist tendencies observed during Trump's presidency. The outcome will significantly determine the direction of U.S. global health policy and its broader implications for global health equity and security. Purpose: Recruit and sequence breast cancer subjects in Guatemalan and US Hispanic populations. Identify optimum strategies to recruit Latin American and Hispanic women into genetic studies of breast cancer., Methods: We used targeted gene sequencing to identify pathogenic variants in 19 familial breast cancer susceptibility genes in DNA from unselected Hispanic breast cancer cases in the US and Guatemala. Recruitment across the US was achieved through community-based strategies. In addition, we obtained patients receiving cancer treatment at major hospitals in Texas and Guatemala., Results: We recruited 287 Hispanic US women, 38 (13%) from community-based and 249 (87%) from hospital-based strategies. In addition, we ascertained 801 Guatemalan women using hospital-based recruitment. In our experience, a hospital-based approach was more efficient than community-based recruitment. In this study, we sequenced 103 US and 137 Guatemalan women and found 11 and 10 pathogenic variants, respectively. The most frequently mutated genes were BRCA1, BRCA2, CHEK2, and ATM. In addition, an analysis of 287 US Hispanic patients with pathology reports showed a significantly higher percentage of triple-negative disease in patients with pathogenic variants (41% vs. 15%). Finally, an analysis of mammography usage in 801 Guatemalan patients found reduced screening in women with a lower socioeconomic status (p < 0.001)., Conclusion: Guatemalan and US Hispanic women have rates of hereditary breast cancer pathogenic variants similar to other populations and are more likely to have early age at diagnosis, a family history, and a more aggressive disease. Patient recruitment was higher using hospital-based versus community enrollment. The global obesity epidemic shows no signs of slowing down. Endoscopic bariatric and metabolic therapies (EBMTs) are being increasingly adopted as treatment options for obesity and obesity-related comorbidities, due to their minimally invasive nature and ease of delivery. According to the site of action along the gastrointestinal tract, EBMTs can be divided into two categories: gastric EBMTs, and small intestine-targeted EBMTs. Unlike gastric EBMTs, which work through a volume-restricting mechanism leading to early satiety and reduced caloric intake, small intestine-targeted EBMTs can be metabolically beneficial through foregut and/or hindgut pathways independent of weight loss, and therefore have great potential for the treatment of obesity-related metabolic comorbidities such as type 2 diabetes. Although none of the small intestine-targeted EBMTs have been approved by the US Food and Drug Administration to date, their clinical efficacy and safety have been extensively explored in investigational trials. Background: Artificial intelligence (AI) has become widely used in a variety of fields, including disease prediction, environmental monitoring, and pollutant prediction. In recent years, there has also been an increase in the volume of research into the application of AI to air pollution. This study aims to explore the latest trends in the application of AI in the field of air pollution., Methods: All literature on the application of AI to air pollution was searched from the Web of Science database. CiteSpace 5.8.R1 was used to analyze countries/regions, institutions, authors, keywords and references cited, and to reveal hot spots and frontiers of AI in atmospheric pollution., Results: Beginning in 1994, publications on AI in air pollution have increased in number, with a surge in research since 2017. The leading country and institution were China ( N = 524) and the Chinese Academy of Sciences ( N = 58), followed by the United States ( N = 455) and Tsinghua University ( N = 33), respectively. In addition, the United States (0.24) and the England (0.27) showed a high degree of centrality. Most of the identified articles were published in journals related to environmental science; the most cited journal was Atmospheric Environment , which reached nearly 1,000 citations. There were few collaborations among authors, institutions and countries. The hot topics were machine learning, air pollution and deep learning. The majority of the researchers concentrated on air pollutant concentration prediction, particularly the combined use of AI and environmental science methods, low-cost air quality sensors, indoor air quality, and thermal comfort., Conclusion: Researches in the field of AI and air pollution are expanding rapidly in recent years. The majority of scholars are from China and the United States, and the Chinese Academy of Sciences is the dominant research institution. The United States and the England contribute greatly to the development of the cooperation network. Cooperation among research institutions appears to be suboptimal, and strengthening cooperation could greatly benefit this field of research. Background and Objective: Endoscopic bariatric and metabolic therapies (EBMTs) are emerging minimally invasive therapeutic options for obesity and its related complications, including non-alcoholic fatty liver disease (NAFLD). This study aimed to evaluate the effects of EBMTs on NALFD in patients with obesity., Methods: Four databases were searched until Nov 2021. Randomized controlled trials (RCTs) and observational studies reporting liver-related outcomes following Food and Drug Administration (FDA)-approved and non-FDA-approved EBMTs were included. Liver parameters, metabolic parameters, and weight loss were evaluated. Risk of bias was assessed using the "risk of bias" tool in the Cochrane Collaboration for RCTs and the Methodological Index for Non-Randomized Studies criteria for observational studies., Results: Thirty-three studies with 1710 individuals were included. Regarding the effects of EBMTs on liver fibrosis, a significant decline of NAFLD Fibrosis Score, but not transient elastography-detected liver stiffness or Fibrosis-4 Index, was observed. EBMTs significantly improved liver steatosis (control attenuation parameter and Hepatic Steatosis Index), NAFLD Activity Score, and Homeostasis Model Assessment of Insulin Resistance. EBMTs reduced serum levels of alanine transaminase, aspartate aminotransferase, and gamma-glutamyl transpeptidase considerably. Moreover, EBMTs had reducing effects on the serum levels of triglycerides and total cholesterol as well as body weight., Conclusions: Our meta-analysis suggested that EBMTs could ameliorate NAFLD based on the evidence of improved liver steatosis, liver function, and insulin resistance. Smoking remains the leading cause of preventable death in the United States. Although combustible cigarettes are largely being replaced by tobacco-free products, nicotine use continues to increase in vulnerable populations, including youth, adolescents, and pregnant women. Nicotine exerts unique effects on specific brain regions during distinct developmental periods due to the dynamic expression of nicotinic acetylcholine receptors (nAChRs) throughout the lifespan. Nicotine exposure is a health concern not only for adults but also has neurotoxic effects on the fetus, newborn, child, and adolescent. In this review, we aim to highlight the dynamic roles of nAChRs throughout gestation, adolescence, and adulthood. We also provide clinical and preclinical evidence of the neurodevelopmental, cognitive, and behavioral consequences of nicotine exposure at different developmental periods. This comprehensive review highlights unique effects of nicotine throughout the lifespan to help elucidate interventions and public health measures to protect sensitive populations from nicotine exposure., (Copyright © 2022 The Authors. Objective: To determine the prevalence of Barth syndrome in the pediatric population., Study Design: Data were collected from the Barth Syndrome Foundation Registry and relevant literature. With the advent of genetic testing and whole-exome sequencing, a multipronged Bayesian analysis was used to estimate the prevalence of Barth syndrome based on published data on the incidence and prevalence of cardiomyopathy and neutropenia, and the respective subpopulations of patients with Barth syndrome indicated in these publications., Results: Based on 7 published studies of cardiomyopathy and 2 published studies of neutropenia, the estimated prevalence of Barth syndrome is approximately 1 case per million male population. This contrasts with 99 cases in the Barth Syndrome Foundation Registry, 58 of which indicate a US location, and only 230-250 cases known worldwide., Conclusions: It appears that Barth syndrome is greatly underdiagnosed.
