Radioimmunotherapy (RIT) with 90Y-ibritumomab tiuxetan or 131I-tositumomab combines a radiation-emitting radionuclide with an antibody targeting CD20 to treat B-cell non-Hodgkin lymphoma. Multiple studies demonstrate favorable RIT efficacy and safety profiles in follicular lymphoma (FL). The primary toxicity is reversible myelosuppression. Various FL treatment options include single-agent immunotherapy, radiation, chemoimmunotherapy, and RIT. Examining RIT clinical effects and position within treatment algorithms is important to optimal patient benefit. Clinical studies support using single-agent RIT in relapsed/refractory FL, in selected patients with new, untreated FL, and as consolidation after induction chemotherapy or chemoimmunotherapy. RIT as consolidation enhances response rates (with conversion of partial to complete responses following induction therapy) and prolongs disease control versus observation. The overall response rate is 60--80%% in the relapsed setting. Time to progression is longer with low-bulk disease, fewer prior therapies, and retained rituximab sensitivity. RIT apparently does not preclude subsequent therapies or increase risk of secondary malignancies compared with chemotherapy's known risk. This article summarizes consensus recommendations for RIT and presents RIT treatment algorithms developed by hematologists/oncologists who regularly treat patients with FL. The article presents the clinical trial of several antineoplastic agents including rituximab, methotrexate, and cytarabine which treat patients with refractory mantle cell lymphoma (MCL) in the U.S. It notes that toxicity, neutropenic fever, and thrombocytopenia are the side effects of the anticancer agents. We use backstripping to quantify the roles of variations in global sea level (eustasy), subsidence, and sediment supply on the development of the Miocene stratigraphic record of the mid-Atlantic continental margin of the United States (New Jersey, Delaware, and Maryland). Eustasy is a primary influence on sequence patterns, determining the global template of sequences (i.e., times when sequences can be preserved) and explaining similarities in Miocene sequence architecture on margins throughout the world. Sequences canbecorrelatedthroughoutthemid-Atlantic region with Sr-isotopic chronology (±0.6 m.y. to ±1.2 m.y.). Eight Miocene sequences correlate regionally and can be correlated to global δ18O increases, indicating glacioeustatic control. This margin is dominated by passive subsidence with little evidence for active tectonic overprints, except possibly in Maryland during the early Miocene. However, early Miocene sequences in New Jersey and Delaware display a patchwork distribution that is attributable to minor (tens of meters) intervals of excess subsidence. Backstripping quantifies that excess subsidence began in Delaware at ca. 21 Ma and continued until 12 Ma, with maximum rates from ca. 21-16 Ma. We attribute this enhanced subsidence to local flexural response to the progradation of thick sequences offshore and adjacent to this area. Removing this excess subsidence in Delaware yields a record that is remarkably similar to New Jersey eustatic estimates. We conclude that sea-level rise and fall is a first-order control on accommodation providing similar timing on all margins to the sequence record. Tectonic changes due to movement of the crust can overprint the record, resulting in large gaps in the stratigraphic record. Smaller differences in sequences can be attributed to local flexural loading effects, particularly in regions experiencing large-scale progradation. [ABSTRACT FROM AUTHOR]

Purpose: Other malignancies have been reported to occur with increased frequency in chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL). The aim of this study was to determine the frequency, outcomes, and factors associated with other cancers in patients with CLL/SLL., Patients and Methods: We reviewed the records of consecutive patients with previously untreated CLL/SLL seen at The University of Texas M. D. Anderson Cancer Center from 1985 to 2005. The number of second cancers observed was compared with the number expected from the Surveillance, Epidemiology, and End Results database., Results: Among 2,028 patients, 324 (16%) had a history of other cancers and 227 (11.2%) developed other malignancies during the follow-up period. Overall, 625 cancers were observed in 551 patients, including skin (30%), prostate (13%), breast (9%), melanoma (8%), lymphoma (8%), gastrointestinal (9%), lung (6%), and other cancers (17%). The risk of a second cancer was 2.2 times higher than the expected risk. The response rates in patients with and without a history of other cancers were 86% and 92%, respectively (P = .04), and the 5-year survival rates were 70% and 82%, respectively (P < .001). In Cox analysis, independent factors predicting development of new cancers were older age, male sex, and elevated levels of beta2-microglobulin, lactate dehydrogenase, and creatinine. In patients who were treated for CLL/SLL, the treatment regimen did not affect the risk of subsequent cancer (P = .49)., Conclusion: Patients with CLL/SLL have more than twice the risk of developing a second cancer and an increased frequency of certain cancer types. Awareness of risk factors could permit early detection.

Cutaneous and systemic plasmacytosis is a rare disorder characterized by widely disseminated macular skin eruptions composed of polyclonal lymphoplasmacytic infiltrates associated with variable extracutaneous involvement. Previous reports have been largely restricted to the Japanese literature. We present the first documented case of cutaneous and systemic plasmacytosis in a patient residing in the United States. This 49-year-old man, who had immigrated from Korea 19 years earlier, developed innumerable persistent pink-to-brown macular lesions over his trunk and face. Initial and repeat skin biopsy specimens revealed dense perivascular and periadnexal infiltrates of mature plasma cells, and polyclonal plasmacytosis noted on two different biopsy specimens of mildly enlarged lymph nodes. Multiple tiny pulmonary nodules were found to be of the same histologic appearance. No evidence of clonal immunoglobulin gene rearrangements or human herpesvirus type 8 infection was noted in these biopsy specimens. Treatment with antibiotics, systemic chemotherapy, and anti-CD20 antibody therapy failed to eradicate these lesions, which have persisted for 6 years. This case demonstrates that cutaneous and systemic plasmacytosis can arise in a patient of Asian ancestry, even many years after emigration to the United States.
