Background: In 2010, the first comprehensive National HIV/AIDS Strategy for the United States was released and included three goals: (1) reducing the number of people who become infected with HIV, (2) increasing access to care and improving health outcomes for people living with HIV, and (3) reducing HIV-related health disparities and health inequities. In 2013, as part of its effort to help address the National HIV/AIDS Strategy, the National Institute on Drug Abuse (NIDA) funded a type 2 effectiveness-implementation hybrid trial titled the Substance Abuse Treatment to HIV Care (SAT2HIV) Project. Aim 1 of the SAT2HIV Project tests the effectiveness of a motivational interviewing-based brief intervention (MIBI) for substance use as an adjunct to usual care within AIDS Service Organizations (ASOs) as part of its MIBI Experiment. Aim 2 of the SAT2HIV Project tests the effectiveness of implementation and sustainment facilitation (ISF) as an adjunct to the Addiction Technology Transfer Center (ATTC) model for training staff in motivational interviewing as part of its ISF Experiment. The current paper describes the study protocol for the ISF Experiment. Methods: As part of a multisite randomized controlled trial, individuals with comorbid HIV/AIDS and problematic substance use are randomized to receive either the ASOs'; usual care (control condition) or usual care plus a MIBI for substance use (experimental condition) delivered by trained ASO case-management staff. Primary outcome measures are reductions in days of primary substance use, number of substance-related problems, times engaging in risky behaviors, days of non-adherence to HIV medications, and increases in substance use treatment. As part of this paper, we describe the trial protocol in accordance with the Standard Protocol Items: Recommendations for Interventional Trials guidelines. Discussion: If successfully able to implement MIBI as an effective adjunct to usual care, the current trial may have a significant impact on increasing the capacity of ASOs to address problematic substance use among individuals living with HIV/AIDS. Reducing the prevalence of problematic substance use among individuals living with HIV/AIDS within the United States may lead to significant improvements on key performance measures (i.e., the HIV Care Continuum and the 90-90-90 target). Aims To examine how institutional constraints, comprising federal actions and states' substance abuse policy environments, influence states' decisions to activate Medicaid reimbursement codes for screening and brief intervention for risky substance use in the United States. Methods A discrete-time duration model was used to estimate the effect of institutional constraints on the likelihood of activating the Medicaid reimbursement codes. Primary constraints included federal Screening, Brief Intervention and Referral to Treatment (SBIRT) grant funding, substance abuse priority, economic climate, political climate and interstate diffusion. Study data came from publicly available secondary data sources. Results Federal SBIRT grant funding did not affect significantly the likelihood of activation ( P = 0.628). A $1 increase in per-capita block grant funding was associated with a 10-percentage point reduction in the likelihood of activation ( P = 0.003) and a $1 increase in per-capita state substance use disorder expenditures was associated with a 2-percentage point increase in the likelihood of activation ( P = 0.004). States with enacted parity laws ( P = 0.016) and a Democratic-controlled state government were also more likely to activate the codes. Conclusion In the United States, the determinants of state activation of Medicaid Screening, Brief Intervention and Referral to Treatment (SBIRT) reimbursement codes are complex, and include more than financial considerations. Federal block grant funding is a strong disincentive to activating the SBIRT reimbursement codes, while more direct federal SBIRT grant funding has no detectable effects. [ABSTRACT FROM AUTHOR]

The article comments on articles concerning the development of nationalism in European countries and in the U.S. The discussion of Russian nationalism and Russian and Soviet behavior strikes me as the most convincing, and Greenfeld may have developed a model that students will find useful in integrating the Soviet period into Russian history as a whole. Anti-Semitism may have characterized German nationalism from the start, but Germany in 1914 was not significantly more anti-Semitic than France. In addition, the founders of modern, scientific anti-Semitism, Chamberlain and Gobineau, were British and French, not German. Charles Tilly covers a great deal of ground that illuminates some of the subtleties of nationalism and the difficulty of making binding generalizations. State-led nationalism has undoubtedly existed in various European countries at various times during the nineteenth and twentieth centuries. Even in the nineteenth century, states showed quite ambivalent relationships to nationalism. Nationalism obviously played a critical role in the early stages of the French Revolution, but Napoleon neither understood nor encouraged it, and the Prussian monarchy, which also paid lip-service to it in 1813-14, helped keep it under control at the Congress of Vienna. Roger Brubaker's article has a much narrow focus, yet makes a valuable contribution to the question of nationalism in the former Soviet Union, the last great multinational empire. Each of these articles makes a worthy effort to relate the idea of nationalism to a relatively wide variety of specific situations. The authors have not always managed to do so without doing violence to history, yet I heartily applaud their efforts. Nationalism, like humankind, has spread throughout the globe, but not without assuming very different forms, with different consequences, at different times and in different places.

In this article the author discusses aspects of research funding among U.S. universities particularly the Massachusetts Institute of Technology (MIT).
