Background: Physical activity (PA) is linked to many health benefits. However, many children and adults do not meet PA recommendations. Specifically, Mexican-heritage (MH) families are less physically active and at greater risk for obesity and related co-morbidities than the general population. Culturally-tailored programs are needed to support MH families in healthy PA behaviors. Purpose: This article describes creation and pilot testing of ¡Haz Espacio para Papi! (Make Room for Daddy!), a PA curriculum tailored for MH families. Methods: Academic-based and promotora-researchers developed a six-week curriculum including weekly interactive lessons provided on weekends and take-home challenges focused on father-child co-participation in light-to-moderate PA. Participating families consisted of child (aged 9–11), mother, and father. Weekly telephone calls were placed to adult participants to secure qualitative evaluation data. Results: Most families reported trying program activities at home between sessions. Families reported spending time together as the most valuable part of the program, specifically engaging in father/child activities. Very few families reported suggestions to change program. Discussion: Positive responses by families speak to the success of culturally and contextually tailoring this curriculum to MH families. Context Food insecurity matters for women's nutrition and health. Objective This review sought to comprehensively evaluate how food insecurity relates to a full range of dietary outcomes (food groups, total energy, macronutrients, micronutrients, and overall dietary quality) among adult women living in Canada and the United States. Data sources Peer-reviewed databases (PubMed/MEDLINE, CINAHL, Scopus, Web of Science) and gray literature sources from 1995 to 2016 were searched. Data extraction Observational studies were used to calculate a percentage difference in dietary intake for food-insecure and food-secure groups. Results Of the 24 included studies, the majority found food-insecure women had lower food group frequencies (dairy, total fruits and vegetables, total grains, and meats/meat alternatives) and intakes of macro- and micronutrients relative to food-secure women. Methodological quality varied. Among high-quality studies, food insecurity was negatively associated with dairy, fruits and vegetables, grains, meats/meats alternatives, protein, total fat, calcium, iron, magnesium, vitamins A and C, and folate. Conclusions Results hold practical relevance for selecting nutritional targets in programs, particularly for nutrient-rich foods with iron and folate, which are more important for women's health. Background: Food insecurity is a critical problem in the United States and throughout the world. There is little published data that provides insights regarding the extent and severity of food insecurity among the hard-to-reach Mexican-origin families who reside in the growing colonias along the Texas border with Mexico. Considering that culture, economics, and elements of the environment may increase the risk for food insecurity and adverse health outcomes, the purpose of this study was to examine the relation between household and community characteristics and food insecurity. Methods: The study used data from the 2009 Colonia Household and Community Food Resource Assessment (CHCFRA). The data included 610 face-to-face interviews conducted in Spanish by promotoras (indigenous community health workers) in forty-four randomly-identified colonias near the towns of Progreso and La Feria in Hidalgo and Cameron counties along the Texas border with Mexico. C-HCFRA included demographic characteristics, health characteristics, food access and mobility, food cost, federal and community food and nutrition assistance programs, perceived quality of the food environment, food security, eating behaviors, and alternative food sources. Results: 78% of participants experienced food insecurity at the level of household, adult, or child. The most severe - child food insecurity was reported by 49% of all households and 61.8% of households with children. Increasing levels of food insecurity was associated with being born in Mexico, increasing household composition, decreasing household income, and employment. Participation in federal food assistance programs was associated with reduced severity of food insecurity. Greater distance to their food store and perceived quality of the community food environment increased the odds for food insecurity. Conclusions: The Mexican-origin population is rapidly expanding; record numbers of individuals and families are experiencing food insecurity; and for those living in rural or underserved areas such as the colonias, the worst forms of food insecurity are an ongoing reality. The rates of households with adult and child food insecurity in this border area are alarming and among the highest reported. Clearly, systematic and sustained action on federal, state, and community levels is needed to reduce household, adult, and child food insecurity that integrates cultural tailoring of interventions and programs to address food and management skills, multi-sector partnerships and networks, expansion of food and nutrition assistance programs, and enhanced research efforts. We surveyed nonindustrial private (family) forestland owners in five states in the southern United States to identify their perception, awareness, and adoption of wildfire prevention and mitigation programs. Wildfire is perceived as an imminent threat by the majority of the survey respondents, and over two-thirds of them have taken some preventive measure. Program awareness, wildfire experience and risk perception, information sources, wildfire preventive activities, and preferences for government interventions differ across racial groups; experience with wildfire, knowledge and activities of fire protection, information sources, and desire for government intervention and technical assistance are also significantly different between male and female landowners. Additionally, program awareness by landowners does not necessarily translate into action in preventing and mitigating wildfire, suggesting that additional assistance and stimuli would be needed to encourage private landowners to be more proactive against wildfire. We used a national household survey to examine knowledge, attitudes, and preferences pertaining to wildland fire. First, we present nationwide results and trends. Then, we examine opinions across region and race. Despite some regional variation, respondents are fairly consistent in their beliefs about assuming personal responsibility for living in fire-prone areas and believing that residents of such areas should follow government guidelines for managing fire risk. However, we find divergence of opinion on "trusting forest professionals" between African-American and Caucasian people. Across all survey questions related to fire management and public confidence, African-Americans appear to be relatively more concerned than Caucasian or Hispanic people. [ABSTRACT FROM AUTHOR]

People's connection to land is an important contributor to identity in traditional southern society. In small southern communities, to know where someone lives is to know who someone is because place assigns biography. Studies have investigated the physical and economic implications of landscape change in the South, but comparatively little research focuses on the impacts to culture of urban growth. We consider how sense of place (as an indicator of culture) may be impacted, over time, by physical and structural changes in a locale. This point of departure examines the temporal dimension of sense of place, or how place perceptions may vary as familiar places and practices are altered by landscape moderations. We review the literature on sense of place and changing Southern landscapes and also offer a conceptual framework for analyzing sense of place over the long-term. [ABSTRACT FROM AUTHOR]

The article reports on African American and Hispanic visits to federal recreation areas. According to the data on file with Donald English of the Visitor Use Monitoring Project based in Washington, D.C., African American visits account for very low percentages of visits to national forests across the U.S. On the other hand, Hispanic visits to national forests in the Southwest are high. It is posed that factors such as private landownership and social definitions of place may be integral in considering African American use of wildland public recreation regions in the Southern states in the U.S.

This research considers acculturation by Mexican and Chinese groups in the United States and how participation in five nature-based outdoor recreation activities may be an indicator of acculturation to American society. We argue that the greater incidence of professional human capital among Chinese immigrants helps this group acculturate more quickly than Mexicans, who are more likely to be labor immigrants and that as a result, Chinese immigrants will be more likely than Mexican immigrants to participate in nature-based outdoor recreation activities. We also posit that US-born Chinese have a greater likelihood of participation compared to US-born Mexicans. Results show Chinese immigrant participation is distinguished only slightly from Mexican immigrant participation; no differences were found between US-born Chinese and US-born Mexicans. Within-group comparisons show immigrant Chinese participation to be more aligned with US-born Chinese participation than immigrant Mexican participation to US-born Mexican participation. Results from this study are intended to help raise awareness among environmental professionals of the different ways nature may be perceived by various cultural groups and also to alert managers of the important role natural resources can play in acculturating immigrants to US society. Much of the research on environmental justice centers on environmental hazards. This article offers an overview of the emergence of environmental justice issues in outdoor recreation management and research. The authors argue that a major challenge to future research on environmental justice in outdoor recreation is clarifying definitions of environmental justice and generating awareness of the different dimensions of environmental justice. The authors also examine recent empirical studies of environmental justice issues related to outdoor recreation and other resource amenities. We assess whether traditionally marginalized groups in American society (African-Americans, women, rural dwellers) perceive more constraints to outdoor recreation participation than other groups. A series of logistic regressions are applied to a national recreation survey and used to model the probability that individuals perceive certain constraints to participating in outdoor recreation activities. Twelve constraints related to health, facilities, socioeconomic standing, and other personal factors are examined for both participants and nonparticipants of outdoor recreation. We model the probability that individuals report being constrained in participating in their favorite activities as a function ofrace, gender, and rural residence. In addition, we control for income, age, regional differences, and activity groupings. Of the three groups examined, women are most likely to feel constrained--for instance, by personal safety concerns, inadequate facilities and information, insufficient funds, and outdoor pests. Race is not a significant predictor of constraints for participants, but nonparticipating African-Americans are more likely than whites to feel personal safety concerns inhibit their outdoor recreation opportunities. Rural residence does not appear to be an important factor among either participants or nonparticipants in explaining the probability that an individual feels constrained in outdoor recreation participation. A portion of the 1995 National Survey on Recreation and the Environment examined public opinion toward user fees as a means of funding recreation services on public lands, including campgrounds, boat ramps, trails, picnic areas, historic sites, restrooms, parking areas, special exhibits & presentations, visitor centers, and other facilities. Respondents were offered five choices to fund these services including fees only, fees and taxes, taxes only, don't provide the service, and don't know. Sample frequencies indicated that over 95 percent of the respondents felt that fees or a combination of fees and taxes should be used to fund at least one of the services. Boat ramps, campgrounds, and special exhibits drew die most support for users fees with over 80 percent of respondents indicating that user fees should be charged to cover at least part of the costs of providing these services. User fees were least favored for picnic areas, historic sites, and restrooms. Restrooms were the only case in which less than 50 percent of the public favored the use of fees to cover at least part of the provision costs. The only service for which at least 50 percent of the public did not feel taxes should be used at least in part to cover the costs of provision was for boat ramps. Logistic regression models were used to examine the socioeconomic factors explaining support for fees. The model included variables on age, ethnicity, income, household size, education, gender, and region of the country'. Findings were somewhat mixed. However, in a general fee model and in a number of specific service models, income and ethnicity surfaced as significant explanatory variables, indicating that concerns about fairness in the implementation of fees are not unfounded. Background: Influenza circulation during the 2022-2023 season in the United States largely returned to pre-coronavirus disease 2019 (COVID-19)-pandemic patterns and levels. Influenza A(H3N2) viruses were detected most frequently this season, predominately clade 3C.2a1b.2a, a close antigenic match to the vaccine strain., Methods: To understand effectiveness of the 2022-2023 influenza vaccine against influenza-associated hospitalization, organ failure, and death, a multicenter sentinel surveillance network in the United States prospectively enrolled adults hospitalized with acute respiratory illness between 1 October 2022, and 28 February 2023. Using the test-negative design, vaccine effectiveness (VE) estimates against influenza-associated hospitalization, organ failures, and death were measured by comparing the odds of current-season influenza vaccination in influenza-positive case-patients and influenza-negative, SARS-CoV-2-negative control-patients., Results: A total of 3707 patients, including 714 influenza cases (33% vaccinated) and 2993 influenza- and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-negative controls (49% vaccinated) were analyzed. VE against influenza-associated hospitalization was 37% (95% confidence interval [CI]: 27%-46%) and varied by age (18-64 years: 47% [30%-60%]; ≥65 years: 28% [10%-43%]), and virus (A[H3N2]: 29% [6%-46%], A[H1N1]: 47% [23%-64%]). Importance: On June 21, 2023, the Centers for Disease Control and Prevention recommended the first respiratory syncytial virus (RSV) vaccines for adults aged 60 years and older using shared clinical decision-making. Understanding the severity of RSV disease in adults can help guide this clinical decision-making., Objective: To describe disease severity among adults hospitalized with RSV and compare it with the severity of COVID-19 and influenza disease by vaccination status., Design, Setting, and Participants: In this cohort study, adults aged 18 years and older admitted to the hospital with acute respiratory illness and laboratory-confirmed RSV, SARS-CoV-2, or influenza infection were prospectively enrolled from 25 hospitals in 20 US states from February 1, 2022, to May 31, 2023. Clinical data during each patient's hospitalization were collected using standardized forms. Data were analyzed from August to October 2023., Exposures: RSV, SARS-CoV-2, or influenza infection., Main Outcomes and Measures: Using multivariable logistic regression, severity of RSV disease was compared with COVID-19 and influenza severity, by COVID-19 and influenza vaccination status, for a range of clinical outcomes, including the composite of invasive mechanical ventilation (IMV) and in-hospital death., Results: Of 7998 adults (median [IQR] age, 67 [54-78] years; 4047 [50.6%] female) included, 484 (6.1%) were hospitalized with RSV, 6422 (80.3%) were hospitalized with COVID-19, and 1092 (13.7%) were hospitalized with influenza. Among patients with RSV, 58 (12.0%) experienced IMV or death, compared with 201 of 1422 unvaccinated patients with COVID-19 (14.1%) and 458 of 5000 vaccinated patients with COVID-19 (9.2%), as well as 72 of 699 unvaccinated patients with influenza (10.3%) and 20 of 393 vaccinated patients with influenza (5.1%). In adjusted analyses, the odds of IMV or in-hospital death were not significantly different among patients hospitalized with RSV and unvaccinated patients hospitalized with COVID-19 (adjusted odds ratio [aOR], 0.82; 95% CI, 0.59-1.13; P = .22) or influenza (aOR, 1.20; 95% CI, 0.82-1.76; P = .35); however, the odds of IMV or death were significantly higher among patients hospitalized with RSV compared with vaccinated patients hospitalized with COVID-19 (aOR, 1.38; 95% CI, 1.02-1.86; P = .03) or influenza disease (aOR, 2.81; 95% CI, 1.62-4.86; P < .001)., Conclusions and Relevance: Among adults hospitalized in this US cohort during the 16 months before the first RSV vaccine recommendations, RSV disease was less common but similar in severity compared with COVID-19 or influenza disease among unvaccinated patients and more severe than COVID-19 or influenza disease among vaccinated patients for the most serious outcomes of IMV or death.

Background: Prolonged SARS-CoV-2 infections in people who are immunocompromised might predict or source the emergence of highly mutated variants. The types of immunosuppression placing patients at highest risk for prolonged infection have not been systematically investigated. We aimed to assess risk factors for prolonged SARS-CoV-2 infection and associated intrahost evolution., Methods: In this multicentre, prospective analysis, participants were enrolled at five US medical centres. Eligible patients were aged 18 years or older, were SARS-CoV-2-positive in the previous 14 days, and had a moderately or severely immunocompromising condition or treatment. Nasal specimens were tested by real-time RT-PCR every 2-4 weeks until negative in consecutive specimens. Positive specimens underwent viral culture and whole genome sequencing. A Cox proportional hazards model was used to assess factors associated with duration of infection., Findings: From April 11, 2022, to Oct 1, 2022, 156 patients began the enrolment process, of whom 150 were enrolled and included in the analyses. Participants had B-cell malignancy or anti-B-cell therapy (n=18), solid organ transplantation or haematopoietic stem-cell transplantation (HSCT; n=59), AIDS (n=5), non-B-cell malignancy (n=23), and autoimmune or autoinflammatory conditions (n=45). 38 (25%) participants were real-time RT-PCR-positive and 12 (8%) were culture-positive 21 days or longer after initial SARS-CoV-2 detection or illness onset. Compared with the group with autoimmune or autoinflammatory conditions, patients with B-cell dysfunction (adjusted hazard ratio 0·32 [95% CI 0·15-0·64]), solid organ transplantation or HSCT (0·60 [0·38-0·94]), and AIDS (0·28 [0·08-1·00]) had longer duration of infection, defined as time to last positive real-time RT-PCR test. There was no significant difference in the non-B-cell malignancy group (0·58 [0·31-1·09]). Consensus de novo spike mutations were identified in five individuals who were real-time RT-PCR-positive longer than 56 days; 14 (61%) of 23 were in the receptor-binding domain. Mutations shared by multiple individuals were rare (<5%) in global circulation., Interpretation: In this cohort, prolonged replication-competent omicron SARS-CoV-2 infections were uncommon. Within-host evolutionary rates were similar across patients, but individuals with infections lasting longer than 56 days accumulated spike mutations, which were distinct from those seen globally. Introduction: Demonstration of competence in neonatal resuscitation is critical for health care providers who provide newborn care because each year, approximately 400,000 (10%) newborns require resuscitation in delivery rooms across the United States. Therefore, neonatal resuscitation skills certification is provided to graduates entering specific specialties at the beginning of residency and renewed biennially through the American Academy of Pediatrics and the American Heart Association. However, there is no formal curriculum to prepare medical (MD) and physician assistant (PA) students prior to graduation., Methods: We developed a neonatal resuscitation curriculum for MD and PA students using a blended learning approach. The curriculum included web-based interactive teaching modules, a demonstration video of a mock neonatal resuscitation, and a neonatal resuscitation simulation. Final-year MD and PA students were evaluated using pre-/posttest, a neonatal resuscitation checklist, and an optional survey on interprofessional communication skills, roles, and responsibility., Results: Eighteen health professions learners completed the neonatal resuscitation curriculum. A paired-samples t test was conducted to compare pretest and posttest scores. There was a significant difference in pretest ( M = 61.4, SD = 15.3) and posttest ( M = 78.6, SD = 8.0) scores, t (17) = -4.7, p < .001. The mean score on the checklist was 79%. Learners strongly agreed/agreed that the simulation improved skills performance (100%), communication skills (92%), and understanding of roles and responsibilities (83%) during a neonatal resuscitation., Discussion: There were improvements in knowledge, procedural, and interprofessional skills in learners who received the neonatal resuscitation curriculum., (© 2020 Johnson et al.)

Introduction: We examined racial/ethnic heterogeneity in self-reported diabetes prevalence over 15 years., Methods: We used National Health Interview Survey data for 1997 through 2012 on 452,845 adults aged 18 years or older. Annual self-reported diabetes prevalence was estimated by race/ethnicity and education. We tested for trends over time by education and race/ethnicity. We also analyzed racial/ethnic and education trends in average annual prevalence., Results: During the 15 years studied, diabetes prevalence differed significantly by race/ethnicity (P < .001) and by Hispanic subgroup (P < .001). Among participants with less than a high school education, the 5-year trend in diabetes prevalence was highest among Cubans and Cuban Americans (β(5YR) = 4.8, P = .002), Puerto Ricans (β(5YR) = 2.2, P = .06), non-Hispanic blacks (β(5YR) = 2.2, P < .001), and non-Hispanic whites (β(5YR) = 2.1, P < .001). Among participants with more than a high school education, non-Hispanic blacks had the highest average annual prevalence (5.5%) and Puerto Ricans had the highest 5-year trend in annual diabetes prevalence (β(5YR) = 2.6, P = .001)., Conclusions: In this representative sample of US adults, results show ethnic variations in diabetes prevalence. The prevalence of diabetes is higher among Hispanics than among non-Hispanic whites, unevenly distributed across Hispanic subgroups, and more pronounced over time and by education. Findings support disaggregation of data for racial/ethnic populations in the United States to monitor trends in diabetes disparities and the use of targeted, culturally appropriate interventions to prevent diabetes.

Background: Community Networks Program (CNP) centers are required to use a community-based participatory research (CBPR) approach within their specific priority communities. Not all communities are the same and unique contextual factors and collaborators' priorities shape each CBPR partnership. There are also established CBPR and community engagement (CE) principles shown to lead to quality CBPR in any community. However, operationalizing and assessing CBPR principles and partnership outcomes to understand the conditions and processes in CBPR that lead to achieving program and project level goals is relatively new in the science of CBPR., Objectives: We sought to describe the development of surveys on adherence to and implementation of CBPR/CE principles at two CNP centers and examine commonalities and differences in program-versus project-level CBPR evaluation., Methods: A case study about the development and application of CBPR/CE principles for the Missouri CNP, Program for the Elimination of Cancer Disparities, and Minnesota CNP, Padres Informados/Jovenes Preparados, surveys was conducted to compare project versus program operationalization of principles. Survey participant demographics were provided by CNP. Specific domains found in CBPR/CE principles were identified and organized under an existing framework to establish a common ground. Operational definitions and the number of survey items were provided for each domain by CNP., Conclusion: There are distinct differences in operational definitions of CBPR/CE principles at the program and project levels of evaluation. However, commonalities support further research to develop standards for CBPR evaluation across partnerships and at the program and project levels.
