The purpose of this study is to analyze the process of school principal selection and appointment in Turkey and some other developed countries in a comparative way. The specific purpose is to make suggestions in order to improve the school principal selection process in Turkey by comparatively analyzing school principal selection process in Turkey and some developed countries. The focus is on the principal selection processes in the USA, Germany, and Finland, and it makes a comparison with the current situation implemented in Turkey. In most of the states in the USA, in order to be a principal, a certificate, a master's degree, and being an actual teacher are required. In the study methodology, official papers and documents have been analyzed in the context of research review in order to study the existing materials that do not require collecting through a data collection tool. These documents include related official reports, graduate thesis, articles, proceedings, National Education Councils conclusions, and legislative texts consisting of official regulations. The findings indicate that there are various selection methods used for this purpose. District Board of Education is in charge at selection procedures. In Germany, there is a variety of selection procedures among the states. The states' Education Ministries and administrators are in charge of selection and appointment of principals and modifications of selection procedures. The state exams and regular official performance assessments by superiors are decisive in the centralized selection process in most federal states. In Finland, a certificate and teaching qualification are required to be a principal. Interviews and tests are used in the selection procedures. Administrators and educational committee members are in charge of the highly decentralized selection process. There are some common and varied features in principal selection process between Turkey and the mentioned countries in the study. In Turkey, school principality is regarded as a secondary role to a teacher and the principal selection processes, procedures, and rules change very frequently in time.

Objective: Despite its widespread use and safety data, the cardiac safety of lamotrigine was brought into question in October 2020 when the U.S. Food and Drug Administration issued a safety warning about its cardiac side effects. Here, we investigated whether there are differences in electrocardiogram (ECG) findings between epilepsy cases receiving lamotrigine monotherapy and those receiving duotherapy. Methods: Patients older than 16 years who were followed up with a diagnosis of epilepsy and receiving lamotrigine were retrospectively identified. Those receiving only lamotrigine and any second anti-seizure medication (ASM) in addition to lamotrigine were included in the study, and those receiving more than two ASMs were excluded. Eligible patients were asked to apply to any health institution and have an ECG performed. Heart rate, PR distance, QRS duration, QT duration, corrected QT value, and Tp-Tend value were calculated manually, and ST-T changes were evaluated. Comparisons were made between patients receiving monotherapy and dootherapy and those receiving low-dose and high-dose lamotrigine. Results: There were 19 patients receiving monotherapy and 11 receiving duotherapy. The ECG parameters of all other patients were within normal values. When ECG parameters were compared between patients receiving monotherapy and those receiving duotherapy, no significant differences were found in heart rate, PR distance, QRS duration, QT duration, QTc duration, Tp-Tend duration, and presence of ST-T changes. When the patients were divided into low-dose and high-dose lamotrigine groups, there were no significant differences in the ECG parameters between these two groups. Conclusion: The relationship between the use of lamotrigine and cardiac conduction problems in patients with epilepsy has attracted the attention of physicians since its introduction into clinical practice. Although our results did not indicate a significant relationship, there is still a need to determine the risk groups and clarify the pathophysiology of lamotrigine-related arrhythmia through genotype- and phenotype-related studies. OBJECTIVE There has not any formal education program covering geriatric oncology within Turkish Society for Radiation Oncology (TROD) residency programs similar with other countries. Developing geriatric knowledge could provide radiation oncologists to plan RT according to frailty status and geriatric syndromes. Learning specific needs of radiation oncologists (ROs) would allow TROD to arrange the existing education program according to the needs of society. The aim of this study is to determine the geriatric oncology educational needs and awareness of TROD members. METHODS We used an 18-item survey to collect cross-sectional opinion data from the members of TROD over 2 months. This quantitative survey was developed through discussions with the members of TROD Geriatric Oncology Working Group. Responses were analyzed with descriptive statistics and common themes. RESULTS Six hundred TROD members were contacted and 201 responded (34%). The most common age group was the ages of 40-49 years with a rate of 31.8%; this was followed by 23,8% aged <30 year. They stated that geriatric patients constitute 40.9% of their daily practice. It was observed that 62.4% had difficulty in making decisions in geriatric patient group, with whom they had such intense communication in daily clinic practice. More than half (62.4%) of responders had difficulty when managing the geriatric patients; 75% agreed additional geriatric training would be helpful. CONCLUSION There are significant deficiencies related to geriatric management of geriatric cancer patients within the TROD members. This study aimed to determine the diagnostic accuracy of the American College of Radiology Thyroid Imaging Reporting and Data System (ACR TI-RADS) classification and shear-wave elastography (SWE) for the diagnosis of benign and malignant thyroid nodules. This retrospective study enrolled 141 patients (18-84 y of age) undergoing thyroidectomy between January 2015 and August 2020. All statistical analysis was based on pathologic results of patients. The cut-off value was found as category 4 for ACR TI-RADS classification and 5 m/s for shear-wave velocity (Vs) by the receiver operator characteristic curve analysis (area under the curve [AUC] = 0.684, p = 0.020 and AUC = 0.715, p = 0.005, respectively). SWE has higher diagnostic accuracy than the ACR TI-RADS classification system and can improve thyroid nodule discrimination in all sizes of the nodules. Purpose: This article addresses the lack of formal design guidance that supports flexibility within the architectural and engineered systems of manufacturing facilities through the development of a taxonomy and associated terminology. Design/methodology/approach: This research performed a comparative analysis of 15 manufacturing facilities located both within the United States (73 percent of cases) and internationally (27 percent of cases). These case studies provided details on how and where flexibility was incorporated into the design of a manufacturing facility. Specific consideration was given to the primary design features that enabled a decoupling of the facility from the manufacturing process. These design features were then clustered to identify main design strategies that enable flexibility. Findings: By grouping the design features together and creating a common vocabulary, three coherent design approaches for flexible facilities were identified, each having a different potential for responding to short-term and long-term changes. These include general purpose, scalable, and dedicated facilities. Research limitations/implications: By delineating three high-level strategies for early flexible facility design, this research synthesizes a conceptual understanding of flexibility with practical and implementable designs. This synthesis provides an incremental advance to a complex challenge for researchers. It also provides decision support to design teams by aiding in project definition, when flexibility is desirable. This research is primarily limited by the number of cases reviewed. With more cases, additional facility design strategies may be identified. Practical implications: The findings in this research allow for a basic understanding of how a flexible facility can be designed with only limited or vague information about the product and manufacturing processes contained within. The development of terminology associated with each facility design strategy provides standardization for the discussion and implementation of flexibility early in the design process. In doing so, flexible designs become easier to create and more efficient to implement. Introduction:  Despite the fact that necrotizing enterocolitis (NEC) is one of the reasons for morbidity and mortality in the newborn intensive care units, the literature indicates no bibliometric studies that made a holistic evaluation of the publications on this issue. This study aims to make a holistic evaluation of NEC publications to reveal the latest developments and trend topics.Materials and Methods:  Bibliometric analyses were performed by retrieving all the publications in Web of Science (WoS: Web of Science Core Collection database maintained by Clarivate Analytics) database between 1980 and 2018 using the "necrotizing enterocolitis" keyword. The correlations between economic productivity, humanity index, and performances of the countries on the topic of NEC were investigated with Spearman's correlation coefficient.Results:  A review of the related literature indicated 2,968 publications on NEC between 1980 and 2018. Of these publications, 1,690 (56.9%) were indexed in the article document category in WoS. There was an important increasing trend in the number of publications after 2006. Results of the present study showed that the Journal of Pediatric Surgery and Journal of Pediatrics were the top effective journal that contributed to the literature in terms of publication productivity. The top productive country that produced most publications about NEC was the United States (863, 51.1%).Conclusion:  Research on NEC is conducted in a limited number of countries. There seem to be more research opportunities in the developed countries because survival rates of premature babies having a disease like NEC are lower in the undeveloped countries, and survival rates are higher in developed countries due to appropriate intensive care conditions. Circumcision is one of the most common surgical operations in newborns, babies, and children due to cultural, religious, and medical reasons. Religious beliefs are known to have an important role in circumcision. Although bibliometric analyses have been performed about several topics with a lot of publications in literature, no studies in the literature were found to focus on male circumcision. This study aims to make a comprehensive bibliometric analysis of circumcision and investigate the relationship between publication productivity and religious beliefs of the countries. Web of Science was utilized to obtain the documents needed for bibliometric analyses. "Circumcision" keyword was used for search. The search included studies published between 1980 and 2018. The literature review indicated that there were 3694 publications about circumcision published between 1980 and 2018. Of these publications, 1770 (47.9%) were articles. This study provided a bibliometric summary of 1770 articles. The top five active countries about circumcision were the USA, England, South Africa, Turkey, and Kenya. The present study found that the religious beliefs of a country were directly related to publication productivity about circumcision. The top-cited study was the article entitled "Male circumcision for HIV prevention in young men in Kisumu, Kenya: a randomized controlled trial" written by Bailey et al. and published in the journal of Lancet. The keyword analysis results showed that HIV was the top keyword used in all articles about circumcision. This study is believed to help researchers interested in circumcision topic to access a summary of the literature, see contemporary and more important topics, and discover new ideas about the issue. Objective: Infections caused by bacteria growing in blood cultures are important medical problems that cause high morbidity and mortality. The aim of this study is to examine the changes in antibiotic susceptibility and the bacteria isolated from blood samples from various clinics in the five-year period and to determine the antimicrobials that can be chosen in empirical treatment in blood stream infection in our hospital. Methods: Blood samples from our laboratories were evaluated retrospectively between during 2013- 2017. Samples were incubated in an automated system of BACTEC-FX(BD,USA). Conventional methods and automated systems (Phoenix,BD,USA) have been used to identify bacteria. Antibiotic susceptibilities of isolated bacteria were evaluated using the automated systems (Phoenix,BD,USA). Results: A total of 4597 bacterial reproductions that isolated from the blood samples including 1972 (42.9%) gram positive and 2625 (57.1%) gram negative were included in the study. The most frequently isolated gram positive bacteria were Staphylococcus aureus (15.3%) and the gram negative bacteria were Escherichia coli (18,6%). Staphylococcus aureus and coagulase negative staphylococci (CNS) isolates no resistance to glycopeptide or linezolid was found. Resistance of vancomycin and teicoplanin in enterococci was determined as 9.7% while linezolid resistance was 2%. Extended spectrum beta lactamase production was detected in E. coli isolates 46.7%, Klebsiella pneumoniae isolates 63.4%, an increase has been observed in E. coli isolates over the last three years. Over the years it has been determined that resistance of trimethoprim-sulfamethoxazole in S. aureus and CNS isolates were decreases. However, it has been determined that increased resistance rates of netilmisin in E. coli and K. pneumoniae isolates, aztreonam, imipenem and meropenem resistance in Pseudomonas aeruginosa isolates. The most effective antimicrobials were linezolid and glycopeptides in staphylococci and enterococci, carbapenems and amikacin in E. coli, and colistin in K. pneumoniae, Acinetobacter baumannii and P. aeruginosa isolates in our study. Conclusion: The diversity of bacteria isolated from blood cultures and susceptibility to antibiotics varies according to geographical regions, hospital flora and antibiotics used in the hospital. For this reason, it was think that each hospital should determine its own bacterial distribution and antibiotic susceptibility at certain intervals and rational drug use policies have to be designated according to these results to provide benefits in fighting with resistant microorganisms. Background/aim: This study aimed to compare radiological and functional outcomes of patients who had single-bundle anterior cruciate ligament (ACL) reconstruction with autologous hamstring tendon grafts using transtibial (TT) versus anteromedial (AM) femoral tunnel drilling techniques. Materials and methods: Sixty patients who had been operated on between 2010 and 2013 were enrolled in this study. Tunnel positions and widenings in the femur and tibia were evaluated with radiographs, arthrometric measurements with a Rolimeter arthrometer (Aircast, Summit, NJ, USA), stability assessment with Lachman and reverse pivot shift tests, and functional assessment with the International Knee Documentation Committee and Tegner and Lysholm scoring systems. Results: Tunnel enlargement in the sagittal and coronal planes of the femur was higher with the TT technique (P < 0.0001) and that of the coronal planes of the tibia was also higher with the TT technique (P = 0.01). During the assessment with the Rolimeter, the difference between sides was significant with the TT technique (P = 0.013). Positive results of the Lachman and reverse pivot shift tests were more frequent with the TT technique (P < 0.05), and the Lysholm scores were higher with the AM technique (P = 0.001). Conclusion: ACL reconstruction with hamstring autografts by either TT or AM technique demonstrated similar and excellent results in terms of functional outcomes at the end of the first postoperative year. We examine ‘Trumpism’ as a contemporary form of colonial domination, showing how this discourse represents both a crisis of coloniality and a stimulus for a movement of ‘decoloniality’. A critical discourse analysis is applied to seven speeches delivered by Donald Trump between his announcement of his presidential candidacy in June 2015 and his inauguration in January 2017. In assessing Trump's arguments, we focus mainly on those concerning national security, illegal immigration, and the threats posed by various foreign countries. Although these arguments sit within a long colonial tradition, they also indicate a crisis of modernity, as witnessed in the growing challenges to colonial masculinity, nationalism, and rationality. We conclude that Trumpism articulates a reaction to these challenges, and that Trump's rise to power is a symptom of the crisis of post-territorial coloniality in contemporary global society. The sublethal toxicological and genotoxic potential of propoxur, a widely used carbamate insecticide against household pests, in veterinary medicine, and in public health, was evaluated on carp as a model species ( Cyprinus carpio L., 1758) using the erythrocyte micronucleus test. Based on the 96-h lethal concentration, 50% (LC50) data from the U.S. Environmental Protection Agency ECOTOX Database (10 mg/L), a sublethal exposure concentration of 5 mg/L was used under static bioassay laboratory conditions. Histopathological evaluation showed no significant changes in spleen, intestine, muscle, or skin tissues. However, the following conditions were recorded: hyperemia, branchitis in primary lamella, and telangiectasis, hyperplasia, fusion, epithelial lifting, and epithelial desquamation in secondary lamella of gill tissues; hemorrhage, destruction, prenephritis, and inflammation and desquamation in the tubules; edema in the kidney; passive hyperemia, albumin, and hydropic degeneration in the liver; and hyperemia, chromatolysis, and glial proliferation in brain tissues. No statistically significant increases in micronuclei frequencies were found. Hematological parameters showed decreased hematocrit values and mean corpuscular volume values, as well as increased erythrocyte and leukocyte counts compared with the control group ( p < 0.01). Plasma glucose, cholesterol, triglycerides, phosphorus, sodium, total plasma protein, chloride, and aspartate aminotransferase levels were increased ( p < 0.01). Abstract: Emergency Department (ED) crowding is a major problem in the U.S. like in many other countries world-wide. This problem is adversely affecting the safety of the patients who rely on receiving a timely treatment in EDs. As a part of solving this problem, a triage process is utilized. Triage is a pre-hospital process by which patients are sorted according to the severity of their illnesses or injuries. Improvements to this process would affect the patient flow positively, and in turn would enhance patient satisfaction and quality of care. In a previous study, we developed a triage algorithm that uses Fuzzy Analytic Hierarchy Process (FAHP) and Multi-Attribute Utility Theory (MAUT) to rank the patients according to their characteristics: chief complaint, age, gender, pain level, and vital signs. The main purpose of this study is to compare two triage systems using Discrete Event Simulation (DES); one system uses the typical Emergency Severity Index (ESI), and the other uses the FAHP and MAUT algorithm. Overall, there was no strong statistical evidence that either system would do better than the other for all the performance measures when the average is taken across all ESI levels. On the other hand, the collected simulated data by each ESI level showed that the FAHP–MAUT algorithm tends to balance the time-to-bed (TTB) and length of stay (LOS) for ESI levels 2–5. In terms of the percentage of tardy patients, FAHP–MAUT system significantly outperforms the ESI system for ESI levels 4 and 5; 34% vs. 61% and 25% vs. 70%, respectively. Both systems were performing about equally for ESI level 1 and level 3 patients; 25% vs. 26% and 64% vs. 67%, respectively. While ESI system slightly outperforms FAHP–MAUT system for ESI level 2 patients, 56% vs. 66%. Based on these results, we recommend using FAHP–MAUT not only because it performs better in terms of minimizing the number of patients with longer than the allotted upper limits of wait times, but also it reduces potential bias and errors in decision making in clinical settings; and thus, it can be used as the basis of an expert system to advise triage nurses. This article aims to re-evaluate the visit of the battleship USS Missouri to Turkey on 5–6 April 1946, to ascertain whether or not it might be considered an early attempt on the part of the United States to challenge the Soviet Union. Greater historical clarity than previously possible has been achieved through the use of hitherto unreleased documents from the archives of the Ministry of Foreign Affairs of the Turkish Republic. Moreover, this article aims to achieve a more balanced and comprehensive analysis by integrating the systemic factors with the actions of crucial actors at the individual level based on new archival evidence. By doing this, the main argument emerges whereby the United States displayed its power against the Soviets via the visit of the USS Missouri only in hindsight; while the decision to send home the exhumed body of the late Turkish ambassador Mehmet Münir Ertegün was the action of an individual. However, due to the advent of the Cold War, the dynamics shaping Turkey's search for security and ultimately the containment policy of the United States, has loaded the event with more meaning than was originally intended, turning a ‘diplomatic courtesy’ into ‘gunboat diplomacy’. Abstract: The use of prefabrication offers significant advantages, yet appropriate criteria for applicability assessments to a given building have been found to be deficient. Decisions to use prefabrication are still largely based on anecdotal evidence or simply cost-based evaluation when comparing various construction methods. Holistic criteria are needed to assist with the selection of an appropriate construction method in concrete buildings during early project stages. Following a thorough literature review and comprehensive comparisons between prefabrication and on-site construction method, a total of 33 sustainable performance criteria (SPC) based on the triple bottom line and the requirements of different project stakeholders were identified. A survey of U.S. experienced practitioners including clients/developers, engineers, contractors, and precast concrete manufacturers was conducted to capture their perceptions on the importance of the criteria. The ranking analysis of survey results shows that social awareness and environmental concerns were considered as increasingly important in construction method selections. Factor analysis reveals that these SPCs can be grouped into seven dimensions, namely, economic factors: “long-term cost,” “constructability,” “quality,” and “first cost”; social factors: “impact on health and community,” “architectural impact”; and environmental factor: “environmental impact.” The resultant list of SPCs provides team members a new way to select a construction method, thereby facilitating the sustainable development of built environment. This article explains why Turkey is a leading ally of the United States. Not only does it share with the United States the goal of long-term stability in Europe, the Middle East, and Central Asia, but Turkey, like its major ally, is committed to democracy and other forms of development as demonstrated by its remarkable economic, political, social, and cultural achievements. Moreover, it has worked side by side with the United States and its allies in Iraq and Afghanistan, has sought to advance peace and justice between the Israelis and the Palestinians in the context of a two-state solution in the Middle East, and has contributed personnel and equipment to the UN forces stationed in Lebanon. This analysis of Turkish-U.S. relations sounds a warning about calling for a referendum on the status of Kirkuk and passing a congressional resolution about historic events in Armenia. Investigates whether inclusions of humate and probiotic into diets of hens during the late laying period increases egg production and improves egg quality in the U.S. Ingredient and chemical composition of the experimental diets; Linear effects of supplemental humate and probiotic on performance and production parameters of hens during the late laying period, including reduced mortality, feed conversion efficiency and increased egg production; Absence of consistent effects on egg quality parameters; Advantage of the supplementation of humate or probiotic for the profitability of a layer flock.

This article presents information about a study which focuses on industrial purchasing managers' perceptions of foreign and national industrial suppliers in Turkey. Buyers' perceptions and attitudes towards products influence their behavior. The increasing competition in international marketing makes the examination of these perceptions inevitable. To determine the Turkish industrial purchasing managers' perceptions of foreign and national industrial suppliers' marketing approaches, three foreign countries, namely the U.S., Germany and Japan, which have the greatest export volume to Turkey, were chosen. From the list of 500 largest industrial corporations in Turkey, 222 Turkish firms were selected randomly. A deliberate attempt was made to exclude strictly service-oriented firms. Thus the sample emphasized manufacturing firms. Results of the study indicate that Turkish industrial purchasing managers perceive German and Japanese suppliers more favorably than U.S. and national suppliers on most of the marketing quality dimensions. Japanese suppliers were perceived as the most sensitive supplier to the customers. Purchasing managers found it easier to co-operate and establish personal friendships with national suppliers. Toxic insult to the gonads by chemotherapy or radiotherapy can lead to permanent infertility. It's an important health concern because each year more than 4000 male patients are at risk of azoospermia in the United States due to gonadotoxicity of the regimens used. There are also several benign/genetic diseases whose natural course can result in infertility without gonadotoxic therapy. Considering the fact that most of these people are cured and survive with the advent of modern medicine, infertility is related to serious psychological and relationship implications and parenthood is a significant issue for those patients. Semen cryopreservation option is available for postpubertal adolescent and adult men, while children do not have this storing option since they do not have mature spermatozoa. However, their testes contain spermatogonial stem cells (SSCs), which are initiators of spermatogenesis. Promising findings in animal studies and human cell lines have encouraged scientists that SSCs may be hope for restoring fertility option of patients who cannot produce functional sperm and who have no other choice to preserve their future fertility. For this reason, several centers around the world already began to collect and cryopreserve testicular tissue or cells with anticipation that SSC-based therapies will be available in the near future; however, an optimal transplantation design in humans is yet to be developed. Here we propose an allogeneic testicular stem cell transplantation with an encapsulation device to restore fertility in patients with infertility. The self-help culture, in the context of the U.S. medical system, demands proactive patient behavior as more responsibility for good health falls on the patient. The presence of health/medical information online alters the dynamics of medical encounters and transforms patients into reflexive consumers or consumer-patients who are engaged in personal health management. This paper examined whether using health information obtained from the Internet to manage personal health care (referred as e-health information consumerism) is associated with subjectively reported negative health outcomes in a representative sample of Internet users in the U.S. These outcomes are conceptualized as experiencing (a) adverse affect (feeling worried and anxious) and (b) health problem due to using Internet information. An online survey (n = 710) was conducted with study participants who were recruited from the largest online probability U.S. research panel developed by a non-profit academic research firm, Knowledge Networks. Analyses included multivariate regressions that included the statistical interactions between e-health information consumerism and the communicational and interactional aspects of medical encounters. Parallel models were tested in the gender-stratified subsamples. Results indicate that robust associations exist between e-health information consumerism and patient-reported health outcomes. Respondents who indicated greater satisfaction with medical communication reported lower averages on experiencing worry and a health problem due to e-health information consumerism. While e-health information consumerism is significantly associated with experiencing a self-reported health problem in both men and women, the association with adverse affect is significant only among women. The moderating role of patient satisfaction with medical communication is stronger among women. Interestingly, the moderating role of patient satisfaction with a medical interaction is stronger among respondents who reported higher averages on experiencing a health problem due to e-health information consumerism. e-Health information consumerism can translate into health benefits if the Internet information is deployed to promote a satisfactory medical partnership. • The growth of online health information challenges the parameters of the sick role. • Information pluralism encourages a consumerist orientation towards health. • The Internet may pose a health risk for physical and emotional well-being.
