Uveitis is inflammation of the iris, ciliary body, and/or choroid and accounts for 10% to 15% of legal blindness cases in Europe and the United States, and particularly affects working-age people. Systemic steroids are the first-line treatment for intermediate and posterior non-infectious uveitis (NIU), whereas topical steroids are reserved for anterior uveitis. However, systemic steroids can lead to complications such as hypertension, diabetes, and secondary infection. Local use of steroids can suppress intraocular inflammation and cystic macular edema with fewer systemic risks. Intravitreal dexamethasone, fluocinolone acetonide, and/or triamcinolone acetonide implants can improve visual acuity and reduce central retinal thickness. Additionally, suprachoroidal injections of triamcinolone acetonide can reduce central subfield thickness in patients with uveitis. Subtenon injections of triamcinolone acetonide reduce central macular thickness more than periocular injections. Systemic intravenous methylprednisolone pulses can rapidly suppress uveal tract inflammation. Topical use of dexamethasone-cyclodextrin nanoparticles can penetrate the anterior segment to resolve vitritis and macular edema, but cataract progression, elevated intraocular pressure, and deterioration of glaucoma may still occur. During the COVID-19 pandemic, uveitis specialists preferred to use local corticosteroids rather than systemic immunosuppressants and biologics. They advocated dose tapering in patients taking high-dose corticosteroids to reduce their susceptibility to viral infection. NIU is increasingly managed with local steroids to reduce infection risk. This article highlights different routes for steroidal treatments of NIU, including intravitreal, suprachoroidal, periocular, systemic, and topical routes. Background: Triamcinolone acetonide (TAA) has been reformulated as an hydrofluoroalkane (HFA) aerosol for intranasal use in patients with seasonal allergic rhinitis (SAR). This study compared the TAA HFA formulation with the previously available chlorofluorocarbon (CFC) nasal inhaler in a dose-ranging study. Methods: This was a double-blind, parallel-group, multicenter study in 780 adults with SAR. Patients had a history of fall seasonal rhinitis and positive skin tests to ragweed. After meeting minimum symptom requirements during the run-in phase, patients were randomized to one of eight groups: TAA CFC or HFA at 14, 110, or 440 micrograms once daily or matching placebo. Treatment was continued for two weeks and patient completed a daily diary for reflective and instantaneous rating of nasal and ocular allergy symptoms. Results: All active treatment groups were statistically superior to placebo with respect to the primary outcome variable, total nasal symptoms. Furthermore, the TAA HFA and TAA CFC formulations were statistically comparable over the dose range. Within each formulation, there was a significant mean reduction from baseline in the symptoms of rhinitis that increased with increasing dose. Ocular symptoms were also reduced with both formulations. Both preparations were well tolerated without any safety concerns. Conclusion: In conclusion, a new formulation of TAA with a HFA propellant was found to be effective in the treatment of SAR and comparable with the previously available TAA CFC formulation. The article investigates the benefits, risks and side effects of triamcinolone acetonide (TCA) treatment among children with capillary hemangioma in the U.S. Many surgeons are aware of the complications on the removal of capillary hemangiomas so they prefer intralesional injections using long-acting glucocorticoids such as the TCA. Other suggestion is the intralesional injection of steriods for patients who have lesions that obstruct vision. Its effectivity is due to the acetonide ester it contains that decreases its solubility at the site of injection. It is emphasized that patients treated in this manner should be monitored for potential toxicity including Cushing syndrome and adrenal insufficiency.

Ragweed pollen is the most frequent cause of allergic rhinitis in the late summer-fall in North America. The objectives of this multi center, randomized, single-blind study are to assess the efficacy and safety of triamcinolone acetonide aqueous (TAA AQ) and beclomethasone dipropionate aqueous (BDP) in the treatment of adult patients with seasonal ragweed-induced allergic rhinitis. Patient's self-assessment of comfort with medication and health-related quality of life were also assessed. A total of 152 patients were randomized to treatment with TAA AQ 200 micrograms once daily in the morning (n = 75) or BDP twice-daily 336 micrograms/day (n = 77). Both treatments were shown to be effective in alleviating the symptoms of allergic rhinitis, with no significant differences between the two treatments in mean reduction from baseline of nasal stuffiness, discharge, nasal index (sum of discharge, stuffiness, and sneezing), itching, and total eye symptoms. There were no significant differences between treatments in quality of life scores.
