Medical affairs pharmaceutical physicians (MAPPs) are at risk for low work-related quality of life (WRQoL). The aim of this study was to develop, validate and implement the first WRQoL instrument for this population. A prospective observational cohort clinical study, the Medical Affairs Pharmaceutical Physician Work-related Quality of Life (MAPPWrQoL) Instrument Development and Patient Registry (MAPPWrQoLReg), was registered in November 2021 (NCT05123846). Thirteen MAPPs and 12 non-MAPPs participated in development and validation between December 2021 and January 2022. Development used the Jandhyala method for observing proportional group awareness and consensus. Discriminant validity analysis used the WRQoL Scale as a reference standard and assessed whether the instrument could differentiate between the groups. Twelve MAPPs and 12 non-MAPPs self-reported their WRQoL in the registry each month from February 2022. Recruitment and data collection are ongoing; 6-month data between February 2022 and August 2022 are reported here. Two participants were excluded from the registry. Chi-squared analysis showed a significant difference between the MAPPWRQoL instrument and WRQoL Scale (p = 1.029e-08) with acceptable sensitivity (89.19%) and specificity (75.00%). There were significant between-group differences for total scores at each follow-up (p =.003; n = 6 questions). Chi-squared analysis showed a significant difference between MAPPs' and non-MAPPs' ability to answer MAPPWRQoL instrument items (p =.002629), with acceptable sensitivity (91.9%) and near-acceptable specificity (66.7%). MAPPs' WRQoL did not change significantly over 6 months. Discriminant validity of the 39-item MAPPWRQoL instrument was confirmed. The Jandhyala method successfully developed and validated a specific WRQoL instrument and may be applied to similar populations, such as junior doctors and UK general practitioners. Introduction: Meaning is a key part of psychological wellbeing, and the benefits of meaningful work are widely acknowledged. Many people seek meaning from their work, and some organizations aim to facilitate this through interventions. In parallel, work-related stress has become a significant occupational risk. This study seeks to understand the perspectives of those who find their work to be both meaningful and stressful, and to explore the relationships between these concepts. Methods: Eleven women and six men, aged 34-61, primarily based in the UK, from the private, public, and third sectors were interviewed about their experiences of meaning and stress in their work. Using a social constructivist grounded theory approach, data collection and analysis ran in parallel. Results: Findings indicate that meaningful work and work-related stress are inherently connected, with bi-directional relationships that can support and hinder wellbeing. Meaningfulness can both alleviate and exacerbate stress, and stress can both reinforce and reduce meaningfulness. Meaningfulness and stress can even feel co-dependent, depending on how participants perceive and make sense of their experience. Discussion: With many individuals seeking greater meaningfulness from their work, the results suggest that they--and their employers--would benefit from understanding more about the potential harmful effects of meaningfulness, including implications for stress and possible knock-on consequences for health and work. Rationale, Aims and Objectives: Critical incident debriefing is an occupational health tool for supporting healthcare workers following critical incidents. Demand for debriefing has increased following the Covid-19 pandemic. There is now a need for more trained debrief facilitators to meet demand, but there is a dearth of literature regarding how best to train facilitators. This study addressed this by exploring participant experiences of an online critical incident debrief training programme., Methods: We conducted semi-structured interviews with 14 individuals who received a 5-day training programme based on the Critical Incident Stress Management model. Participants were recruited from a range of professional disciplines including psychology, nursing and human resources within one British healthcare system. Data were analysed using thematic analysis., Results: The analysis produced three themes. Managing trainee experiences and expectations suggested that disciplinary heterogeneity in training groups supported inter-participant knowledge exchange. However, this variation also meant that training materials did not meet the learning needs of all participants. Modality of training suggested that while online learning was acceptable for some, others experienced screen fatigue and found it hard to build rapport with other participants. Systematic and organisational obstacles to training access and delivery suggested that lack of managerial support and organisational mental health stigma may be barriers to accessing training., Conclusion: A 5-day online CISM-based training programme was acceptable to participants. Organisations implementing critical incident debrief training may benefit from (1) offering both in-person and online training options, and (2) tailoring course materials according to the disciplinary make-up of groups., (© 2024 The Authors. This study examines the psychosocial hazards experienced by academic staff working in UK institutions over time. A risk assessment framework developed by the Health and Safety Executive (HSE) was used to measure seven key hazards: demands, control, support from managers and colleagues, relationships, role and change management. Data were obtained from three waves of a national survey of academic staff across the UK (2008, n = 6,203; 2012, n = 7,068; 2014, n = 3,952). Mean scores for each hazard were compared with HSE benchmarks from the UK working population and changes over the three waves were examined. Apart from job control, none of the benchmarks was met and the risk associated with demands, manager and peer support, role and change was particularly high. An increase in most of the psychosocial hazards was found over time, particularly for job demands, control, role and relationships, showing clear cause for concern. How the findings could be used to monitor the wellbeing of academic staff over time and develop targeted interventions is considered. [ABSTRACT FROM AUTHOR]

Purpose Recent intervention research for burnout amongst those working in health and social care contexts has found acceptance and commitment therapy (ACT) interventions to be of use but has provided less clarity on the role of psychological flexibility (a key ACT construct). This study further evaluated the usefulness of ACT for burnout and work‐engagement and assessed the role of psychological flexibility in contributing to therapeutic change. Procedure: A nonconcurrent multiple‐baseline across‐participants single‐case experimental design was used. Four participants were recruited from a homelessness organization in the East Midlands, England. The ACT‐intervention was split into three modules to reflect the three aspects of the ACT triflex, and the sequence of delivery was randomized for each participant in order to test the relationship between these aspects. Findings Support was found for the ACT intervention reducing exhaustion and increasing work‐engagement. Psychological Flexibility increased in all participants and was temporally related to increases in other outcome variables in some instances. Delivery of the intervention focussed on any given aspect of the ACT triflex could increase different domains of psychological flexibility. Implications: This study adds to the growing body of research in favour of ACT interventions for burnout and adds to the understanding of psychological flexibility as a mediating variable. Background: Children's hospices are a key provider of palliative care for children and young people with life-limiting and life-threatening conditions. However, despite recent policy attention to the provision of paediatric palliative care, little is known about the role of children's hospice staff and the factors that may impact on their wellbeing at work. This study explored the rewards and challenges of working in a children's hospice with an aim to identify staff support and development needs. Methods: We conducted an exploratory, qualitative study involving thematic analysis of semi-structured interviews with 34 staff and three focus groups with 17 staff working in a multi-disciplinary care team in a UK children's hospice. Results: Participants identified rewards and challenges related to the direct work of caring for children and their families; team dynamics and organisational structures; and individual resilience and job motivation. Participants described the work as emotionally intensive and multi-faceted; 'getting it right' for children was identified as a strong motivator and reward, but also a potential stressor as staff strived to maintain high standards of personalised and emotional care. Other factors were identified as both a reward and stressor, including team functioning, the allocation of work, meeting parent expectations, and the hospice environment. Many participants identified training needs for different aspects of the role to help them feel more confident and competent. Participants also expressed concerns about work-related stress, both for themselves and for colleagues, but felt unable to discuss this at work. Informal support from colleagues and group clinical reflection were identified as primary resources to reflect on and learn from work and for emotional support. However, opportunities for this were limited. Conclusions: Providing regular, structured, and dedicated clinical reflection provides a mechanism through which children's hospice staff can come together for support and learning, and demonstrates an organisational commitment to staff wellbeing and development. Being aware of children's hospice specific rewards and challenges can help to ensure that staff feel supported and competent in their role. Breaking down barriers to discussing work-related stress and enhancing awareness about early signs of burnout is also important. [ABSTRACT FROM AUTHOR]

There is evidence that fundamental changes to the context and content of academic work have increased demands, reduced support and eroded professional autonomy. Drawing on research conducted in the UK and Australia, this paper initially considers the implications of these changes for the wellbeing of academics. Particular focus is placed on a longitudinal programme of research that has utilised the UK Health and Safety Executive's Management Standards Framework to investigate the job-related stressors and strains experienced in the university sector. It is argued that this benchmarking approach has strong potential to monitor working conditions in universities over time, facilitate comparisons with the work-related wellbeing of other occupational groups, and identify priority areas for intervention. The paper also focuses on the antecedents and outcomes of work-life conflict which is particularly prevalent amongst academics and a key source of strain. Finally, ways in which the wellbeing of academic employees may be enhanced are considered. Psychological capital (PsyCap), which is a resource that comprises positive individual characteristics, has not previously been investigated in relation to the experience of bullying. The role of psychological capital and social support in relation to the impact of bullying at work was investigated in a quantitative survey of 2068 employees from a range of UK organizations. A resource model of bullying and health, with PsyCap and social support as potential mediators, was proposed and tested using structural equation modelling (SEM). Based on previous research we expected that both PsyCap and social support would mediate the impact of bullying. The model was tested with outcomes of both ill-being and well-being, and shown to be a good fit for the data. We concluded that both PsyCap and social support separately mediated the impact of bullying; however, it is their joint contribution that should be noted. The study indicates the value of PsyCap in relation to bullying, health and well-being in the workplace, and supports the inclusion of psychological capital and social support in more comprehensive models of bullying at work. The Management Standards Indicator Tool (MSIT) is a 35-item self-report measure of the psychosocial work environment designed to assist organizations with psychosocial risk assessment. It is also used in work environment research. Edwards and Webster presented a 25-item version of the MSIT based on the deletion of items having a factor loading of < .65. Stress theory and research suggest that psychosocial hazard exposures may result in harm to the health of workers. Thus, using data collected from three UK organizations (N= 20,406) we compared the concurrent validity of the brief and full versions of the MSIT by exploring the strength of association between each version of the instrument and a measure of psychological wellbeing (GHQ-12 and Maslach Burnout Inventory). Analyses revealed that the brief instrument offered similar but not always equal validity to that of the full version. The results indicate that use of the brief instrument, which would be less disruptive for employees, would not elevate the risk of false negative or false positive findings in risk assessment. The Health and Safety Executive Stress Indicator Tool is a short questionnaire developed in the UK for the screening of common psychosocial risk factors leading to work-related stress. Previous research showed that in both the UK and Italy this test fits the same seven-factor solution. Building upon previous findings and by using multiple-group confirmatory factor analysis we investigated the degree of measurement equivalence of the Italian version of the test with its original UK version. Furthermore, we advanced the validation of the tool in Italy by exploring the correlations of its subscales with stress-related outcomes. Two samples of workers were available for analyses: 1,298 Italian employees of private-sector organizations and a sample of 7,589 UK employees from organizations of the same type. Results replicated previous findings: A 7-factor solution of the Indicator Tool was found to be equivalent across the Italian and UK samples. Furthermore, the analyses supported factor variance and factor covariance equivalence in addition to metric equivalence. The Indicator Tool subscales showed significant correlations in the expected direction with stress-related outcomes. The Italian version of the Indicator Tool behaves very consistently with its UK version and may be used for the screening of psychosocial risks in Italian workplaces. [ABSTRACT FROM AUTHOR]

This study provides evidence to support the validity of the UK Health and Safety Executive Management Standards Indicator Tool, which is widely used in the UK by organizations to test for stress-related working conditions. A large pooled data set was collected from 137 UK organizations (N=67,347) to test the factor structure of both the original 35-item seven-factor instrument and a new shorter 25-item version. The results showed that the tool was a good fit to the data for both versions. Further analysis examined the factor structure of the Indicator Tool by splitting the data set into both public and private sector organizations. Tests for measurement invariance showed that both versions of the Indicator Tool provided a good fit to the data. The final sequence of analysis showed that the measurement structure of both the 25-item and 35-item scales was also invariant across small-, medium- and large-sized organizations. The current study provides percentile tables for both the public and private sectors so that organizations can compare their scores against UK national benchmarks. Overall, this study validates both the full and the short versions of a valuable and reliable diagnostic instrument for use in a variety of organizations. [ABSTRACT FROM PUBLISHER]

Exploring work stress using a transactional perspective requires researchers to consider not just the role of appraisal of a stressful encounter but also the relationship of this appraisal with emotions. This research sets out to explore the appraisal-emotion relationship in a work setting. Using data from 174 civic administrators from New Zealand, sequential tree analysis (which presents patterns in a system of hierarchical ordering) was used to create the pattern of appraisals of stress associated with each of three emotions: anger, anxiety and frustration. The results suggest that if we are to advance our understanding of the appraisal-emotion relationship then future research needs to explore what common characteristics bind together and help shape appraisal patterns, whether some appraisals are more complex than others, and whether some appraisals are more potent than others. The results also raise the question of how best such relationships should be investigated in order to understand the nature of a stressful encounter. The Demand-Control (D-C) (Karasek, 1979) and the Demand-Control-Support (D-C-S) (Johnson & Hall, 1988; Johnson, Hall, & Theorell, 1989; Karasek & Theorell, 1990) models of work stress suggest that jobs with high demands and low control (and low support) are stressful. In line with the support in the literature for context-specificity in occupational stress research (Sparks & Cooper, 1999) and the limited and even contradictory support for interaction effects, the main aim of the present study was to examine how the D-C-S model applied in a well-defined occupational group. Using hierarchical regression analyses, and controlling for negative affect, the D-C-S model accounted for 26%, 6%, and 8% of the variance in job satisfaction, psychological distress and burnout, respectively, among 166 academics in a UK university. No two-way or three-way interactive effects were evident, but additive effects of job demands and control on psychological well-being and of job demands and support on both burnout and job satisfaction were shown, corroborating research showing that high job strain is linked to ill health and job dissatisfaction in this homogenous occupational sample.
