Background: Diabetes self-management education (DSME) is generally considered to be a key determinant of the treatment outcomes and related costs of diabetes mellitus. While DSME programmes generally have positive outcomes, their effects may depend on certain factors, such as the type of programmes provided and patients' level of health literacy (HL). Low HL has been associated with poorer self-management behaviours and poor medication adherence in diabetic patients, but its impact on the effects of DSME has not yet been systematically investigated. This study aimed to investigate the influence of HL on the self-reported effects of DSME programmes while taking the type of programme into consideration. Method: A total of 366 diabetic patients from nine countries completed a questionnaire measuring HL, self-management behaviours, problem perception, coping, perceived general health and well-being, before and after participating in a DSME programme. Results: DSME programmes were found to have positive effects on self-reported self-management behaviours and almost all psychological and health outcomes, regardless of HL level. Patients with high HL scored better on several diabetes outcomes than those with low HL, but all patients described benefiting from DSME. Individual and group-based programmes resulted in more positive effects on several diabetes outcomes than self-help groups, but no interaction with HL was found. Conclusion: Our findings confirm those of previous studies showing that DSME programmes have positive effects and that low HL is associated with lower diabetes outcomes but do not support the assumption that the effects of DSME programmes are influenced by the patient's HL. However, due to the limitations of this study, further investigation is necessary to support these findings and improve our understanding about the impact of HL on DSME programmes' effectiveness.

Hoarding Disorder (HD) is under recognised and under-treated. Though HD develops by early adulthood, patients present only later in life, resulting in research based largely on samples of predominantly older females. Whilst formerly associated with Obsessive-Compulsive Disorder (OCD), it is now recognised that individuals with HD often have inattention symptoms reminiscent of Attention Deficit/Hyperactivity Disorder (ADHD). Here, we investigated HD in adults with ADHD. Patients in an ADHD clinic (n = 88) reported on ADHD, HD and OCD-related symptoms, and compared with age, gender and education matched controls (n = 90). Findings were assessed independently in an online UK sample to verify replication using a dimensional approach (n = 220). Clinically significant hoarding symptoms were found in ∼20% versus 2% of ADHD and control groups, respectively, with those with hoarding being on average in their thirties and with approximately half being male. Greater hoarding severity was noted even in the remaining patients compared with controls (d = 0.89). Inattention was the only significant statistical predictor of hoarding severity in patients. Similarly, inattention, alongside depression and anxiety were the greatest predictors of hoarding in the independent sample where 3.2% identified as having clinically significant hoarding. Patients with ADHD had a high frequency of hoarding symptoms, which were specifically linked to inattention. HD should be routinely assessed in individuals with ADHD, as they do not typically disclose associated difficulties, despite these potentially leading to impaired everyday functioning. Research in HD should also investigate adults with ADHD, who are younger and with a greater prevalence of males than typical HD samples. [ABSTRACT FROM AUTHOR]

Under British law, the court has the inherent authority to set aside an indictment which, under the circumstances of the case, constitutes ‘abuse’ of the defendant. This unwritten rule had been accepted also by Israeli courts, and then came a completely new, not to say surprising, Act in 2007. The Israeli Parliament (the Knesset) has thereby recognised a preliminary argument which exploits concepts of ‘justice’ and ‘legal fairness’, and the granting of pro-discretion to the court, which may decide whether or not it is fitting and proper to conduct the trial against the defendant. How has the court reacted to that? As I will try to emphasise, judicial review upon prosecutorial discretion is rare, just a drop in the bucket, and the court is quite reluctant to implement the new tool. [ABSTRACT FROM PUBLISHER]

The article provides the 'missing dimension' in the historiography of Syria and Lebanon in the Second World War. It is based on secret British and Syrian documents obtained by the French intelligence from their agents in the British Legation in Beirut and the Syrian government in Damascus, never published before. These documents, recently discovered by the author, shed new light on the activities of the British intelligence agencies in the Middle East during and after the war. They reveal that these agencies played an important role in shaping Britain's policy in the region by securing the tacit collaboration of prominent Arab nationalists in Syria and Lebanon and other Arab countries. In Syria (and Palestine), Britain conducted a 'dual policy': one purported to mediate between the French and the Syrians, details of which are found in British archives, and a tacit policy aimed to evict France, of which few traces remain in official documentation. Hence de Gaulle's accusations that Britain secretly engineered the expulsion of France from the Levant were indeed justified, and that the Syrians' claim that their country was the first Arab state to secure complete independence is questionable. The article also discloses that Britain was behind the Hashemite schemes to integrate Syria in a Greater Syria or an Iraqi-led Hashemite confederation. Copies of more than one hundred of the documents are annexed to the article, including a secret agreement from 29 May 1945 revealing that President Quwatli was coerced into granting Britain a dominant position in Syria.
