This article considers how changing screen practices are changing prevailing domestic temporal routines to generate new household dynamics. Underpinned by a case study of the multiscreen home, a media time approach offers a close analysis of the temporal dimensions of household screen use. The analysis draws on data about patterns of use of screen devices and streaming services from UK's communications regulator Ofcom, combined with findings from qualitative studies of media time and second-screen uses. Explaining that second screens offer new kinds of connectivity within home, the study queries assumptions of time erosion and temporal fragmentation. The article proposes that second screens afford new temporal experiences and interactions signifying a new domestic screen culture characterised by time dilation. Digital screen relations operate within polymediated timescapes enabling an opening up and extension of time to produce new domestic screen cultures distinguished as intra-domestic and trans-domestic screen time. An impaction of the foetal head at caesarean section is a topical concern in modern obstetric practice. The management options for this problem are well described but the incidence or even definition of impaction, is unknown. The primary aim of this study was to ascertain the incidence of impacted foetal head at CS in labour. This prospective study used data from all women undergoing CS during a 12-month period in a single unit. Following completion of all CS, the surgeon completed a questionnaire covering: cervical dilation at time of CS; if the surgeon felt there was a difficulty in delivering the foetal head as an indicator of impaction, as well as the other techniques utilised. Of 440 EMCS in labour, 18% (n = 81) reported a difficulty delivering the head, which was most common at cervical dilation ≥8 cm (n = 124, 48%). A difficulty with the delivery of the foetal head was associated with 36% increased measured blood loss. Impact statement What is already known on this subject? Impaction of the foetal head at a caesarean section is a recognised complication of CS in late labour but there are no reliable data on the incidence of the problem. It is poorly defined and yet many techniques and devices have been described to overcome this problem, however, optimal management remains uncertain. What do the results of this study add? The primary aim of this study was to determine the incidence of the impacted foetal head during CS in labour as determined by whether the surgeon experienced difficulty with delivery of the head. We report that at least some difficulty in delivering the foetal head at CS is common, and most often encountered when cervical dilation is ≥8 cm. When additional manoeuvres were required, the 'push' technique was exclusively adopted with implications for training. A difficulty in delivering the foetal head was associated with a 36% increase in the measured maternal blood loss. What are the implications of these findings for clinical practice and/or further research? Further multi-centre investigation is required to ascertain incidence of this obstetric problem with predicting factors determined. This work will inform decisions about the optimal management. Objective: To determine the risk of spontaneous and medically indicated preterm birth associated with mode of birth in previous term‐born pregnancy. Design: Retrospective cohort study. Setting: Two UK maternity units. Population or sample: A total of 16 340 women with first two consecutive singleton births and the first birth at term. Methods: Retrospective cohort study using routinely collected clinical data. Main outcome measures: Incidence of spontaneous preterm birth and medically indicated preterm birth at less than 37 weeks of gestation after term birth, in relation to mode of birth in first pregnancy. Subgroup analysis on cervical dilatation at the time of first caesarean birth. Results: Compared with vaginal birth, emergency caesarean birth at full dilatation was associated with an increase in spontaneous preterm birth (2.3% vaginal birth versus 4.5% full dilatation caesarean; adjusted odds ratio [aOR] 3.29, 95% CI 2.02–5.13, P < 0.001). Elective caesarean, emergency caesarean at <4 cm dilatation, and emergency caesarean at 4–9 cm dilatation were associated with increased medically indicated preterm birth (0.8% vaginal births versus 1.9% elective caesarean, 3.3% <4 cm caesarean, 1.3% 4–9 cm caesarean; aOR 2.30, 95% CI 1.19–4.15, P = 0.009; aOR 4.68, 95% CI 2.98–7.24, P < 0.001; and aOR 2.43, 95% CI 1.43–4.00, P = 0.001, respectively). Conclusions: Term caesarean in the first stage of labour or performed prelabour is associated with medically indicated preterm birth. Term caesarean in the second stage of labour is associated with spontaneous preterm birth. Caesarean in the second stage of labour is associated with spontaneous preterm birth. The number of new infections per day is a key quantity for effective epidemic management. It can be estimated relatively directly by testing of random population samples. Without such direct epidemiological measurement, other approaches are required to infer whether the number of new cases is likely to be increasing or decreasing: for example, estimating the pathogen‐effective reproduction number, R, using data gathered from the clinical response to the disease. For coronavirus disease 2019 (Covid‐19/SARS‐Cov‐2), such R estimation is heavily dependent on modelling assumptions, because the available clinical case data are opportunistic observational data subject to severe temporal confounding. Given this difficulty, it is useful to retrospectively reconstruct the time course of infections from the least compromised available data, using minimal prior assumptions. A Bayesian inverse problem approach applied to UK data on first‐wave Covid‐19 deaths and the disease duration distribution suggests that fatal infections were in decline before full UK lockdown (24 March 2020), and that fatal infections in Sweden started to decline only a day or two later. An analysis of UK data using the model of Flaxman et al. gives the same result under relaxation of its prior assumptions on R, suggesting an enhanced role for non‐pharmaceutical interventions short of full lockdown in the UK context. Objectives: Ultra-running's psychological and physiological dimensions have been widely studied. However, the social dimensions of ultra-racing have been largely overlooked. This study aims to identify the different types of social interactions that occur during a multi-day, ultra-race and to understand how these interactions influence ultra-runners' race experience. Methods: Athletes competing in Britain's "Spine Race" were recruited for the current study. Twelve male runners aged between 32 and 66 years (M = 50; SD = 10.01) followed the qualitative protocol designed for the study. Specifically, each participant completed a modified version of the Day Reconstruction Method (DRM) instrument and underwent a post-race, self-confrontation interview. Participants were asked to recall and reconstruct their memories about their (a) actions (e.g., What did you do when you encountered other people?); (b) thoughts (e.g., What were your thoughts when you encountered other people?) and (c) feelings (e.g., What were your feelings when you encountered other people?). Results: The results show that social interactions and interpersonal relationships are common in ultra-races. Different groups of people play an important role in racers' motivation and goal achievement: fellow racers, volunteers, medics, friends and family, and even the general public. The resulting social interactions and relationships can occur before, during and after the race in both online and offline environments. These social influences have a powerful influence on the athlete's race experience. Conclusion: The vast majority of social interactions reported by runners had a positive effect on their race experience and often led to interpersonal relationships. This paper offers an overview of the most frequently abused recreational drugs, including ecstasy and ecstasy look-alikes, cocaine/crack cocaine, amphetamines, poppers, cathinones, LSD, DMT, methylphenidate, GHB, dextromethorphan, ketamine, and smart drugs/smart drinks. A survey of magic mushroom use was completed by 174 participants in 2004, a year when the sale of hallucinogenic mushrooms was not illegal in the UK. The data were collected in Edinburgh and Bristol (UK). Participants were a self-selecting convenience sample. Participants tended to be in their 20s, White-British, in education or employed; 64% were male. Participants reported a pattern of infrequent but intense consumption (47% used between 4-12 times/year, average consumption in one setting was 12 g, a high dose). Use was explained in terms of laughing, hallucinations, altering perspective (41-74%), and feelings of being closer to nature (49%). Negative experiences reported included paranoia (35%) and anxiety (32%). Mushroom use was located within a wider recreational drug and alcohol culture. Four focus groups aided the interpretation of the data. Future research is recommended into negative experiences. Implications for policy and harm minimisation literature are discussed.
