This paper considers critical reflection as a pedagogical strategy in UK higher education at a moment of an amplification of populist, reactionary discourses. It draws on written reflections of foundation-level students in a case study cohort and offers insights into their lived learning experiences and perceptions of the value of reflection. This is situated within the UK 'Brexit' context, alongside a proliferation of far-right populist voices, emboldened supremacies and rising fascism. Accompanying this has been a normalisation of reactionary 'anti-social justice' discourses. It is vital that HE practitioners recognise, pre-empt and interrupt such discourses, developing pedagogies and curricula in response. Yet there are inherent challenges in a climate of 'post-truth' anti-intellectualism. This paper argues that critical reflection contributes a useful approach to learning, fostering development of students' personal, intellectual and political capacities to navigate this complex socio-political terrain and engage with social justice.

COVID-19 presented a range of challenges to the delivery of school psychology services in countries around the world. The current study aimed to investigate the practices of school psychologists from the United States of America, Australia, Germany, Canada, and the United Kingdom, including changes to practice and exploration of the factors that supported the delivery of school psychology services during the pandemic. Quantitative and qualitative data were collected from 1,030 school psychologists and analyzed using a mixed methods, multiple case study design. Differing impacts of the pandemic on the working hours of school psychologists were reported across countries. Participants in all countries reported a shift to online working, with an increased focus on consultation and intervention and a reduction in psychoeducational assessments. School psychologists from all nations emphazised the importance of self-care strategies, social connections and physical activity and the role of support via supervision or professional networks. Access to appropriate technology and responsive workplace policies and procedures were also identified as important. Results have implications for the internationalization of the school psychology profession and can inform international school psychology planning in response to future crises.

Background: Individuals pursue teaching careers for numerous reasons, such as for instrumental or prosocial purposes. Aims: This study examined the personal (instrumental motivation) and social (prosocial motivation) utility of teaching as predictors of teaching quality in terms of clarity of instruction, classroom management, and cognitive activation. Sample: We used data from the Teaching and Learning International Survey (TALIS) 2018, which included 50,595 teachers from 1252 schools in 10 countries and regions. Methods: We performed a series of regression analyses to test a model of instrumental and prosocial motivation to predict three indicators of teaching quality (clarity of instruction, classroom management, and cognitive activation) while controlling for demographic characteristics (age, sex, educational level, and teaching experience). We examined this model in countries and regions from Eastern (Japan, Korea, Singapore, Shanghai and Taipei) and Western (Australia, Canada, New Zealand, United Kingdom and the United States of America) cultures. Results: Results demonstrated that instrumental motivation predicted clarity of instruction in the East and classroom management in both the East and West; prosocial motivation, however, was a more consistent predictor of all indicators of teaching quality, except classroom management in the West, across cultures. Conclusion: Teachers' prosocial motivation to benefit others and contribute to society must be considered to understand teaching quality across various cultural contexts. Implications for theory, practice and policy are discussed.

This study, based on two questionnaires directed to translation lecturers in UK Higher Education (HE), aims to explore the teacher awareness of learner autonomy in the UK university translation classroom, and the extent to which students of translation are encouraged to become autonomous learners. It covers six aspects of translation education, i.e., objective setting, learning strategies, resources, technology, learner reflection and assessment, and teacher's role. The results provide insights to the teachers' understanding of student choice, control and responsibility in autonomous translation learning. The findings suggest that the translation students in UK HE are offered the most choice in translation resources and technologies, and the least choice in co-deciding translation syllabi, specialized professional goal and teaching materials; they seem to share consistent control with their teachers over the whole learning process, from goal-setting, to translation task completion, group collaboration, final version and translation quality criteria decision, to self-evaluation and reflection; they are encouraged to take responsibility for their translation products, collaboration in class, for learning to translate, and learning to evaluate and reflect. Nevertheless, the importance of encouraging student self-evaluation, peer-evaluation and self reflection in UK translation education seems to be underestimated, and is recommended to be brought to the forefront of further research.

During the COVID-19 pandemic, significant focus was placed on the benefits and challenges of online versus traditional face-to-face learning. This paper presents the findings from a project which paints a more complex picture. Differing Perceptions of Quality of Learning, a collaborative project between four United Kingdom universities, investigated student perceptions of teaching and learning during the pandemic. Mixed methods using survey and focus groups to collect data were used. Analysis was conducted on the overall sample, by subject area, and by ethnicity. Findings indicated that the focus in universities should be shifted from the dichotomy of face-to-face or online learning toward flexible and scaffolded modes and approaches that lead to quality learning and progressively help students to move appropriately between lecturer-led learning and independent learning. Implications for the sector include a focus on pedagogical principles and ensuring the quality of any medium and environment used. The priority recommendation is to provide scaffolding for independent distance learning.

We use sequence analysis to compare the different trajectories that individuals take through the education system and into work and identify the characteristics that could be used to target those who are at most risk of poorer labour market outcomes. As well as age 16 exam performance, we find that parental advice, aspirations, and attitudes towards HE are important predictors of the pathways through education and into work. However, these pathways are not strongly determined at the end of compulsory education, and thus there are still opportunities for individuals to change their trajectory even after leaving school.

Academic conferences have a central role in shaping career trajectories, reproducing or resisting exclusions and moulding relations in and to academia, thus shaping academic networks. In this paper, we consider how precarious academics subvert and navigate conference spaces, including emerging online forms. Particularly, we explore how academic conferences generate and nurture friendship as an enduring political practice that undermines neoliberal economic, political and social structures. This is contextualised in UK higher education amidst mass job losses, increased precarity and the subsequent breaking of academic bonds. In interviews with academics and our own auto/ethnographical experiences, we explore how friendship emerges in and resists the exclusions of the conference space for precarious researchers. Finally, we explore how the nurture of friendship by precarious scholars at academic conferences promotes collective action to resist neoliberalism within the academy.

Previous research exploring how working-class women experience UK Higher Education (HE) work has made evident recurring themes around social segregation and corresponding difficulties with feeling they belong. This paper develops this work by exploring the ways in which UK, HE based working-class women lecturers talk about their sense of belonging. It was found that, in contemporary UK HE, lecturing work is located within a marketised space where caring for students is central and the deployment of emotional labour to seen to be a necessary requirement to meet those demands. In addition, this labour is understood to be work that working-class women can readily take up, and as one of the few vehicles to enable feelings of value and belonging. However, this work is also devalued, unaccounted for and potentially harmful to those who do engage in it, therefore shoring up/ reinforcing a class and gender stratified UK academy.

Education is influenced by a broad range of factors but there has been limited research into the role that early school enjoyment plays in pupil's educational achievement. Here we used data from a UK cohort to answer three research questions. What is the association between early school enjoyment and later academic achievement? To what extent do family background factors underlie this association? Do sex differences in school enjoyment underlie sex differences in achievement? School enjoyment was self-reported in two questionnaires completed at age 6. We used multiple imputation to account for missing covariates in this study, giving an imputed sample size of 12,135. Children's school enjoyment at age 6 associated with sex and cognitive ability but not family socioeconomic background. For example, girls were twice as likely to report enjoying school than boys (OR: 1.97; 95% CI: 1.56, 2.48). School enjoyment strongly associated with later achievement in age 16 compulsory GCSE exams even after adjustment for socioeconomic background and cognitive ability; pupils who reported enjoying school scored on average 14.4 (95% CI: 6.9, 21.9) more points (equivalent to almost a 3-grade increase across all subjects) and were 29% more likely to obtain 5 + A*-C GCSE's including Maths and English (OR: 1.29; 95% CI: 0.99, 1.7) than those who did not enjoy school. These results highlight the importance of school enjoyment for educational achievement. As a potentially more modifiable factor than socioeconomic background, cognitive ability or sex, school enjoyment may represent a promising intervention target for improving educational outcomes.

Educational institutions have been among the most active social organisations responding to and facilitating processes associated with globalisation. This has primarily been undertaken through the attempts of schools and universities to 'internationalise' their student intake, staffing, curricula, research, and assessment systems. Amongst the many benefits associated with the promotion of 'internationalisation' is that it will provide students with attributes such as global citizenship, skills or competencies that will contribute to improving tolerance, respect and harmony between nations and cultures. Various nations and global agencies actively promote such goals and global citizenship is included in the United Nations' Sustainable Development Goals. Positioned as a response to the Sustainable Development Goals, the Organisation for Economic Co-operation and Development has developed a metric to compare the 'global competency' of 15-year-old pupils, which was incorporated into the Programme for International Student Assessment 2018. We analyse the rationales for this decision, the conception of 'global competence' adopted by the Organisation for Economic Co-operation and Development and how these have changed since its inception in 2013. We also explore how it will be measured and how the organisation deals with what it describes as 'the most salient challenge affecting PISA'. We argue: (i) the official conception of 'global competence' finally adopted was strongly influenced by the organisation's quest to position itself as the agency responsible for monitoring progress on the Sustainable Development Goals, and then amended to match what could be easily measured; and (ii) although the organisation presents its global competencies using a humanitarian discourse, it is framed by its economic mission.

A global health archive consisting of podoconiosis tissue slides and blocks (which was collected and imported into the UK before the introduction of the Human Tissue Act), was donated to Brighton & Sussex Medical School in 2014. There is little guidance on the socioethical and legal issues surrounding the retrospective use of archived or 'abandoned' tissue samples, which poses a number of questions relating to the ethical standing of the archive. There is a great deal of interpretation in the guidelines that are currently in existence; however, modern ethical principles cannot be applied as it is not feasible to either reconsent or retrospectively seek approval. Our research team believed that it was unethical to leave the archive in storage, as this option favours neither researcher nor subjects. Permission was obtained from the Human Tissue Authority and a local ethics board for the tissues to be utilized in on-going research on podoconiosis aetiology. There is a delicate balance between the benefits gained by society relating to the development and progress of scientific research and the risks to the donor regarding the reuse of their tissues. Clearer guidelines should be made available to ensure that researchers are able to reuse tissue archives in contemporary research.

Introduction: This paper reports on the findings of a study investigating the potential effects of discipline (sciences and engineering versus humanities and social sciences) on the application of the Institute of Electrical and Electronic Engineers learning object metadata elements for the description of learning objects in the Jorum learning object repository. Method: A survey was conducted that examined a stratified sample of 470 metadata records and surrogates used for the description of learning objects in the Jorum repository. Analysis: A quantitative data analysis was performed based on the sample. This included descriptive statistics as well as a set of Chi squared tests for the identification of differences in the application of learning object metadata elements between disciplines. Results: The results of this study showed that some metadata elements tended to be more frequently applied for the description of learning objects in the humanities and social sciences than in sciences and engineering. These were: interactivity type, difficulty, aggregation level, coverage, structure and semantic density. Conclusions: The findings of this study could have implications for the design of metadata editors and annotation tools, as well as the development of metadata training programmes. Background: The issue of violence in secure services has long been recognized both in the UK and worldwide. However, there is currently scarce literature available about violence within learning disability (LD) secure settings. Methods: Secondary data analysis was conducted on violent incidents, using information routinely collected by the staff over a 1-year period. Results: Physical assaults were the most frequent type of incident, and the distribution in terms of days or months was homogenous and incidents were concentrated in the corridors, lounges and dining rooms of secure facilities. Antipsychotic medication was not regularly prescribed. Generalized linear modelling analyses revealed significant predictors that increased the chances of seclusion and physical restraint, such as being female or directing the violence towards staff. Conclusions: These findings can inform staff training on violence prevention and suggest that increased ward-based supervision and enhanced use of psychological formulations may help in reducing violence within this service context.

Pressure on evaluators has been investigated recently by surveys in the USA, the UK, Germany, and Switzerland. This study compares the results of those studies regarding pressure on evaluators in different countries. The findings suggest that independence of evaluations does not exist for many respondents. Moreover, the person who commissioned the evaluator for evaluation is identified by all studies as the primary influencing stakeholder in the evaluation process. In terms of differences, Germany seems to be more prone to pressure on evaluators. However, German evaluators do not show stronger tendencies to surrender to pressure than the other countries' respondents. We suggest that this pattern may be explained by the strong state tradition in Germany as opposed to the U.S. and Switzerland, in conjunction with evaluators' profession-based, principled resistance to such pressure.

School-based mental health prevention programmes can be effective but their adoption within schools will depend on their social acceptability. We report a qualitative evaluation summarising the views of children (115), parents (20) and school staff (47) about a universal school-based anxiety prevention programme FRIENDS. This study was conducted as part of a large scale randomised controlled trial (n = 1362) involving 40 schools in the UK providing primary education to children aged 7-11. Reported overall experience of the programme was very positive, with all three major components of the cognitive behaviour therapy programme (emotional, cognitive, and behavioural) being accepted well and understood by children. The programme was considered to be enjoyable and valuable in teaching children important skills, particularly emotional regulation and coping. Children provided examples of using the skills learned during FRIENDS to manage their emotions and solve problems. However, teachers were concerned that the programme overlapped with the current school curriculum, required additional time and almost half were unable to identify any tangible changes in the children's behaviour. Whilst this paper provides evidence to support the social validity of the FRIENDS anxiety prevention programme, the concerns raised by teachers question the longer-term sustainability of the programme.

Purpose: To date, the literature on perception of affective, pragmatic, and grammatical prosody abilities in autism spectrum disorders (ASD) has been sparse and contradictory. It is interesting to note that the primary perception of syllable stress within the word structure, which is crucial for all prosody functions, remains relatively unexplored in ASD. Thus, in the current study, we explored syllable stress perception sensitivity and its relationship to speech production abnormalities and communicative ability in adults with ASD. Method: A same-different syllable stress perception task using pairs of identical 4-syllable words was delivered to 42 adults with/without high-functioning ASD, matched for age, to investigate primary speech perception ability in ASD. Speech production and communicative ability in ASD was measured using the Autism Diagnostic Observation Schedule (Lord et al., 2000). Results: As predicted, the results showed that adults with ASD were less sensitive in making judgments about syllable stress relative to controls. Also, partial correlations revealed a key association of speech production abnormalities with stress perception sensitivity, rather than communicative ability. Conclusions: Our findings provide empirical evidence for deficits on primary syllable stress perception in ASD and its role on socio-communicative difficulties. This information could facilitate the development of effective interventions for speech and language therapy and social communication.

Work experience placements are mandatory in the UK for all young people aged 16-18 in education, and their employability effects and associated wage premia are well noted in the literature. Through Bourdieu's lens, this article analyses and conceptualises a unique data set of reflective testimonies submitted in response to a YouGov survey of over one thousand young people. Different capitals are detected, their distributions are observed, and correlations with key variables such as social background and school type are reported. Emerging through young people's perceptions of employer engagement is a complex web of human, social and cultural capital accumulation. Overlaps are frequent, with newly acquired forms of capital often activating others. We argue that because of the cyclical nature of employer engagement gains, benefits accumulate exponentially for some young people while leaving others increasingly detached from the capitals that are most important for labour market success.

The use of podcasting in higher education has escalated in recent years. The aim of this case study was to analyse undergraduate student use and perceptions of lecture audio recordings in the School of Biomedical Sciences at the University of Leeds. Students completed an online survey over a two-week period based on their use of lecture audio recordings in semester one of the 2011/2012 academic session (n = 131 responses). Of the podcasts available, 77% of students reported to have listened to at least 50% of recordings. The majority of students accessed podcasts using the virtual learning environment (91%) often in combination with either a media player (30%) or podcatching software (21%). Students usually listened to podcasts in private accommodation (83%) on a personal computer or laptop, with fewer listening whilst undertaking other activities e.g. walking (16%). Interestingly, 93% of respondents perceived podcasts to be important or very important to their studies, with 90% stating that concentration in lectures was improved as a result of access to podcasts. Furthermore, 73% of students indicated that the availability of podcasts did not influence lecture attendance. These data suggest that students make extensive use of lecture audio recordings, with a positive impact on study behaviour.

This paper explores the potential of Semantic Web technologies to support teaching and learning in a variety of higher education settings in which some form of case-based learning is the pedagogy of choice. It draws on the empirical work of a major three year research and development project in the United Kingdom: "Ensemble: Semantic Technologies for the Enhancement of Case-Based Learning" which has been oriented toward developing a better understanding of the nature of case-based learning in different settings, but also exploring the potential for Semantic Web technologies to support, enhance, and transform existing practice. The experience of working in diverse educational settings has highlighted Semantic Web technologies that may be particularly valuable, as well as some of the enablers and barriers to wider adoption, and areas for further research and development. We previously reported that foetal valproate exposure impairs intelligence quotient. In this follow-up investigation, we examined dose-related effects of foetal antiepileptic drug exposure on verbal and non-verbal cognitive measures. This investigation is an ongoing prospective observational multi-centre study in the USA and UK, which has enrolled pregnant females with epilepsy on monotherapy from 1999 to 2004. The study seeks to determine if differential long-term neurodevelopmental effects exist across four commonly used drugs (carbamazepine, lamotrigine, phenytoin and valproate). This report compares verbal versus non-verbal cognitive outcomes in 216 children who completed testing at the age of three years. Verbal and non-verbal index scores were calculated from the Differential Ability Scales, Preschool Language Scale, Peabody Picture Vocabulary Test and Developmental Test of Visual-Motor Integration. Verbal abilities were lower than non-verbal in children exposed in utero to each drug. Preconceptional folate use was associated with higher verbal outcomes. Valproate was associated with poorer cognitive outcomes. Performance was negatively associated with valproate dose for both verbal and non-verbal domains and negatively associated with carbamazepine dose for verbal performance. No dose effects were seen for lamotrigine and phenytoin. Since foetal antiepileptic drug exposure is associated with lower verbal than non-verbal abilities, language may be particularly susceptible to foetal exposure. We hypothesize that foetal drug exposure may alter normal cerebral lateralization. Further, a dose-dependent relationship is present for both lower verbal and non-verbal abilities with valproate and for lower verbal abilities with carbamazepine. Preconceptional folate may improve cognitive outcomes. Additional research is needed to confirm these findings, extend the study to other drugs, define the risks associated with drug treatment for seizures in the neonates, and understand the underlying mechanisms.

This paper considers the results of an online questionnaire survey that collected user data on e-book collections held at Essex County Libraries over a two-year period between April 2004 and June 2006. The research was funded by the Laser Foundation, 1 and specifically considered e-book collections held on Personal Digital Assistant (PDA) devices, as well as collections viewable on Personal Computers with Internet access.2 Questionnaire responses from 58 individuals indicated clearly that marketing of e-book collections needs serious consideration, that certain genres were popular with users (notably Science Fiction), and that the largest number of respondents were aged between 21 and 30. As we discuss, the findings of this questionnaire survey offer few insights into e-book usage that have not been reported in other research. However, what the findings do illustrate clearly is that publicity afforded to e-book and other electronic collections remains crucial. From perusal of the data, 47 of the 58 respondents found out about the e-book collections after browsing the website. Whilst this obviously indicates that the website is indeed useful, the fact that only nine respondents investigated the collections having seen publicity material tells another story. This paper reports on an evaluation of Net-Detectives, a creative online role play activity aimed at 9-12 year olds. Net-Detectives forms part of Kidsmart, an Internet awareness programme aimed at school children. It was evaluated through a multiple method data collection using questionnaire surveys, follow up telephone interviews with teachers and observation in use in three UK schools during the summer term of 2003. All sources of data collected agreed that a number of benefits arise from participation in the Net-Detectives online role play. The pupils learned about Internet safety in a motivating and challenging environment. They empathised with others gaining an understanding of their motivations and practiced the ICT skills being taught. They particularly enjoyed being "detectives" and interacting with others outside their school. However, the role of the "hosts" in preparing participants and in moderating their communication is key to the success of the online role play. This preparation needs to be thoroughly planned for both the teachers whose classes are participating and for their pupils as uncertainty leads to extra communication that interferes with the smooth running of the role play exercise. It was very clear that first the technical issues have to be resolved and then, issues to do with the learner's comprehension and expectations of the task, before social interaction online can result in learning. The paper concludes by presenting the recommendations suggested by this study for hosting an online role play in the school environment.

Leisure has been contested as a valid role of public libraries in the UK since their introduction in 1850 yet it has been a major reason for the public's use of libraries. Where the educational role and information provision function of libraries have been seen as purposeful and worthwhile, the leisure role has been generally valued less with the library profession. This article is the first of two that explore the leisure role of public libraries in the UK and provides a historical overview of leisure reform, public library acts, the great fiction debate, the popularization of libraries, the educational and information role and the recognition of the leisure function.

This is the second of two articles examining the leisure role of public libraries. The first article examined the leisure role of public libraries in the UK from a historical perspective. This examines the value of the public library service in providing leisure opportunities from a user point of view. The results demonstrate that public libraries have a significant impact on people's leisure time. Benefits include providing relaxation, improving health and well-being, opening up choices and opportunities despite financial constraints, alleviating boredom and allowing people to socialize. Public libraries are, therefore, providing a worthwhile and necessary function through their leisure role. (Contains 4 tables.)

This article reports on one aspect of a Laser Foundation-funded research project that tested different models of e-book delivery and offered guidelines for developing e-book collections in UK public libraries. An e-book collection was offered to library users (primarily, users relying on a mobile library service) on Personal Digital Assistant (PDA) devices at Essex County Libraries. The article considers reaction from both library users and staff. A number of positive and negative reactions to e-books were noted, and the indicative results point to the conclusion that dedicated PDA collections still offer questionable benefits for use in public libraries. Reported here are the results of a research project that examined the role of UK public libraries in addressing adult literacy including approaches and issues. Eight public libraries were selected as case studies and adult literacy provision was investigated using staff interviews. The interviews provided support for the role of public libraries in promoting the pleasure of reading to adults with poor literacy. The re-branding of adult literacy collections was recognized as the ideal starting point for the rejuvenation of adult literacy work. Approaches used to address poor adult literacy included the use of reading groups, talking books, themed activities and events, and student book buying. A role for public libraries in identifying adults with poor literacy was acknowledged as part of multi-agency work. The main issues facing public libraries in the delivery of adult literacy services concerned establishing dedicated staff time and identifying funding opportunities. Recommendations are provided for public libraries involved in establishing or reviewing adult literacy services.

A cross-sectional survey of 54 nursing lecturers and 244 students using modularized curriculum and analysis of curriculum documentation indicated that only 3 of 13 modules refer explicitly to reflective practice. Students identified lectures and group work as strategies for learning reflection; lecturers cited tutorials, groups, and cases. Most lecturers were unclear about the place of reflection in the curriculum. (Contains 22 references.) (SK)

This article aims to add to the emerging empirical mitigation deterrence literature by drawing on ongoing research into a particular form of greenhouse gas removal technology – biochar – and associated biochar carbon markets. As such, the aim of this paper is to explore whether the UK carbon market for biochar, in practice is likely to contribute to delays in emissions reduction. In other words, we explore whether UK biochar carbon markets are (likely to be) as much of a problem for mitigation deterrence as imagined or envisaged within the mitigation deterrence literature. We draw from original data collected in 2022 from 33 semi-structured interviews with mostly UK based experts who have an interest or potential interest in biochar, supplemented with a document analysis. Conceptual approaches from the social studies of finance have guided the analysis, an approach that enables systematic investigation of experts' understandings of the biochar carbon market landscape in the UK. Although biochar proponents forwarded narratives – or fictional expectations – about how the future trading of carbon credits in carbon markets could lower the cost of producing biochar on a large scale, other experts doubted the credibility of these narratives. Whilst the construction and sustainable functioning of a UK-based biochar carbon market remains a speculative, rather than credible proposition, it nevertheless constitutes a threat of mitigation deterrence because of the assumption that a UK biochar carbon market will become a reality. This article examines a recent British government report on the role of educational psychology services in better promoting the development and learning of all students. It notes positive aspects of the report, especially its description of what educational psychologists can do to make a difference, but also describes problems of impact and implementation associated with the report. Set against the new chronological framework for the introduction and spread of the Neolithic into Britain this paper considers the logistics of introducing domestic cattle from continental Europe. Cattle were the most extensively utilised domestic animal at the start of the Neolithic and understanding the processes behind their introduction is critical for exploring early Neolithic farming systems. We begin by exploring the realities of creating a viable new animal population and then moving that population to Britain across the water from Continental Europe. Using a series of simulated models we demonstrate that only a very small number of cattle need to have been introduced into Britain in order to enable the uptake and spread of domestic cattle across the whole island. This, in turn, may effect how we consider the mechanisms involved in the adoption of the Neolithic, and can better inform our understanding of the kinds of processes involved in the transition to the Neolithic. Most surgical specialties have attempted to address concerns about unfair comparison of outcomes by "risk-adjusting" data to benchmark specialty-specific outcomes that are indicative of the quality of care. We are building on previous work in head and neck surgery to address the current need for a robust validated means of risk adjustment. A dataset of care episodes, which were recorded as a clinical audit of complications after operations for squamous cell carcinoma (SCC) of the head and neck (n = 1254), was analysed with the Waikarto Environment for Knowledge Analysis (WEKA) machine learning tool. This produced 4 classification models that could predict complications using data on the preoperative demographics of the patients, operation, functional status, and tumour stage. Three of them performed acceptably: one that predicted "any complication" within 30 days (area under the receiver operating characteristic curve (AUROC) 0.72), one that predicted severe complications (Clavien-Dindo grade 3 or above) within 30 days (AUROC 0.70), and one that predicted a prolonged duration of hospital stay of more than 15 days, (AUROC 0.81). The final model, which was developed on a subgroup of patients who had free tissue transfer (n = 443), performed poorly (AUROC 0.59). Subspecialty groups within oral and maxillofacial surgery are seeking metrics that will allow a meaningful comparison of the quality of care delivered by surgical units in the UK. For these metrics to be effective they must show variation between units and be amendable to change by service personnel. Published baseline data must also be available. They should be modelled effectively so that meaningful comparison, which takes account of variations in the complexity of the patients' needs or care, is possible. [ABSTRACT FROM AUTHOR]

This paper engages with the debate on niche-regime interactions in sustainability transitions, using a study of plant-based milk and its struggles against the entrenched liquid dairy-milk regime, which has various sustainability problems. Plant-based milk is under-studied, so our empirical contribution consists of an exploration of its diffusion in the UK. We make three conceptual contributions. The first calls for a bidirectional analysis that addresses niche-oriented activities by incumbent actors, in addition to the outward-oriented activities by niche advocates presented in most studies of niche-regime interaction. The second contribution nuances Smith and Raven's fit-and-conform and stretch-and-transform typology: using a societal embedding framework which distinguishes four environments, we suggest that hybrid patterns are possible in which innovations follow a 'fit' pattern in one environment but 'stretch' in another. The third contribution highlights the potential role of cultural meanings in galvanizing transitions by eroding positive associations that support the regime and stabilise consumer purchasing. Aims:: To assess the performance of the referral management system (RMS) compared to a previous paper-based referral system and to determine whether referrals reflected the patients' malocclusion and met current guidelines. Design:: Three-cycle audit. Setting:: Orthodontic Department, Liverpool University Dental Hospital, UK. Participants:: Consecutive new orthodontic patient referrals. Methods:: Data were collected prospectively from orthodontic referral letters and new patient clinic proformas (2016-2017). Cycle 1 assessed the original paper-based referral form, Cycle 2 assessed the first RMS online form and Cycle 3 assessed a modified RMS form. Results:: Cycles 1, 2 and 3 audited 83, 84 and 81 referrals, respectively. Agreement between the reason for referral and the new patient clinic findings was moderate for Cycles 1 and 3 (Kappa = 0.47 and 0.60, respectively) and fair for Cycle 2 (Kappa = 0.40). In Cycles 1, 2 and 3, the proportion of new patients appropriate for hospital orthodontic care reduced from 52% to 51% and 40%, respectively. None of the three cycles reached the 90% target for compliance with current referral guidelines. Conclusions:: Cycle 3's RMS form gave a truer reflection of the patients' malocclusion but reduced the proportion of appropriate referrals. Abstract This paper presents the final phase of a national project exploring grading of practice in programmes leading to registration as a midwife in the United Kingdom. The aim was to develop a generic framework for grading practice, enhancing standardisation while enabling flexibility in application of current and new educational standards. A mixed method on-line survey considered existing practice assessment tools, factors contributing to robust and reliable assessment and perceptions of two assessment tools developed by the research team: a 'Lexicon Framework' and 'Rubric' , which were tested through scenarios. Participants included 170 midwifery and nursing academics, clinicians and students, representing 20 universities in the UK. Seven key themes emerged, from which an 'Evidence Based Model for Professional Practice Assessment' was developed. The proposed tools were overall positively evaluated and demonstrated a good level of reliability. A national tool to standardise midwifery practice assessment is recommended, and scope for transferability of our tools to all midwifery programmes and to nursing was identified. Other recommendations include engagement of key stakeholders in development of practice assessment documentation, and maintaining the professional purpose of grading practice as central to the process. A set of key principles for assessing practice is presented.
