Studies about effective practice in Higher Education (HE), student retention, progression and attainment suggest that student engagement is a major factor in success. A sense of belonging to a community of students and academics is seen as key to creating effective engagement. Such studies have identified interventions that have proved successful in traditional HE contexts; however, ideas of belonging and community are considered problematic in distance learning contexts. Preliminary work by the doctoral research author showed that many Open and Distance Learning (ODL) students were successful in their studies without identifying as a student or interacting socially with others, calling into question the extent to which belonging and community are relevant in part-time and distance learning settings. In 2014, The Open University developed a platform, the Student Hub Live (SHL), to facilitate academic community. This research focuses on the value of attending the live online interactive events at the SHL that support part-time distance learning students outside the curriculum, and relating to their studies. Using an ethnographic approach and grounded theory methods, chat logs of events were analysed and the emergent themes informed semi-structured interviews with six participants. The overall findings were that although the curriculum was often a primary focus for students, learning how to apply academic skills more generally and learning from other students is important in ODL. The findings are relevant to other distance and face-to-face HE providers that are keen to engage students in virtual extracurricular spaces to support learning.

The Houses of Parliament Restoration and Renewal Programme is the UK's largest ever. The cultural value, funding model, risk profile, and fragility of the building exposes an uncomfortable truth. Construction business-as-usual has become an enterprise risk. Current levels of productivity, health, and wellbeing are not acceptable on a project of such high national importance. The Programme joins calls made in the Farmer Review, Project 13, the Construction Play Book , and now the Building Safety Bill for transformative change. It is in the unique position of being able to proactively drive this change. It is an exemplar project intended to deliver a learning legacy for the sector at large. It is also the first project to show an interest in a new approach called Sociotechnical Construction. The focus of this new approach is the joint optimization of socio/human and technical/organizational factors. The evidence points to these being instrumental in resolving the construction sector's long-standing productivity and wellbeing paradox. Intensive Speech and Language Therapy (SLT) training courses for Early Childhood Educators (ECEs) can have a positive effect on their use of interaction strategies that support children's communication skills. The impact of brief SLT training courses is not yet clearly understood. The aims of these two studies were to assess the impact of a brief SLT training course on ECEs' interaction behaviour, and to explore ECEs' views and experiences of the course. In Study 1, eight ECEs took part in a multiple-baseline study of a brief SLT training course. Video-recordings of interactions with children were used to evaluate ECEs' interaction behaviour using the Conversational Responsiveness Assessment and Fidelity Tool. In Study 2, seven ECEs took part in semi-structured interviews about this training course. Template analysis was used to identify key themes. In Study 1, the group of trained ECEs showed a statistically significant increase in their use of one communication-facilitating strategy (using comments to cue turn-taking) and a statistically significant decrease in their use of one conversation-hindering behaviour (asking yes/no, testing or rhetorical questions). Analysis at the individual level showed a modest increase in some ECEs' use of language-modelling strategies and a more generalized decrease in conversation-hindering behaviours. In Study 2, ECEs more consistently reported learning and using communication-facilitating strategies than language-modelling strategies. ECEs identified several features of the training course that facilitated learning: the practical, interactive nature of the group training sessions, the use of video feedback, and the repetition of key strategies in several training sessions. We conclude that brief SLT training for ECEs can lead to increased use of some interaction strategies that help children's communication skills develop. Further research is needed to evaluate brief SLT training more thoroughly.

Interviews with 90 secondary teachers in 10 English schools identified contextual influences on teacher professional development and elements of leadership. Links between professional development and school policies and philosophy produced different results. In one school, professional development served to support the performance management system; in another, a major change in school culture was attempted. (Contains 30 references.) (SK)

Presents an analysis of recorded weekly discussions between British student language teachers and their school-based mentors. Results found that, despite individual preferences and interpretations, all mentors supported principles related to positive classroom management, building relationships with pupils, maximum use of the target language, and grammar as an essential part of language learning. Objective: To explore the role of chronic inflammation in rheumatoid arthritis (RA) on cognition. Methods and analysis: Six hundred sixty-one men and women aged ≥55 years who fulfilled the American College of Rheumatology/European League Against Rheumatism (ACR/EULAR) criteria for RA were recruited from three healthcare trusts in the United Kingdom (UK) between May 2018 and March 2020. Study participants took part in interviews which captured sociodemographic information, followed by an assessment of cognition. RA specific clinical characteristics were obtained from hospital medical records. Participants were cognitively assessed using the Montreal Cognitive Assessment (MoCA) and were classified as cognitively impaired if they scored ≤27/30 points. Linear regression analyses were conducted to identify which demographic and clinical variables were potential predictors of cognitive impairment. Results: The average age of participants was 67.6 years and 67% (444/661) were women. 72% (458/634; 95% CI 0.69 to 0.76) of participants were classified as cognitively impaired (MoCA≤27). Greater cognitive impairment was associated with older age (p =.006), being male (p =.041) and higher disease activity score (DAS28) (with moderate (DAS28 > 3.1) (p = 0.008) and high (DAS28 > 5.1) (p = 0.008)) compared to those in remission (DAS28 ≤ 2.6). There was no association between MoCA score and education, disease duration, RF status, anti-cyclic citrullinated peptide (anti-CCP) status, RA medication type or use of glucocorticoids or non-steroidal anti-inflammatory drugs (p > 0.05). Conclusion: This study suggests that cognitive impairment is highly prevalent in older adults with RA. This impairment appears to be associated with higher RA disease activity and supports the concept that chronic systemic inflammation might accelerate cognitive decline. Early years caregivers can play a key role in young children's eating and the prevention of childhood obesity. The UK National Diet and Nutrition Survey (NDNS) is a large representative survey collecting detailed food and nutrition consumption data. Using these data, the aim of this study was to investigate the relationship between dietary intake of preschool children in the UK aged 2 to 4 years and accompanying adult/s. Nutrition consumption data from 1218 preschool children from years 1 to 8 of the 2008–2016 NDNS were accessed. Dietary data were captured using 3 or 4 day estimated food diaries. Regression analyses revealed significant differences in consumption when children were not accompanied by their parents. Compared with when children were with parents, children consumed significantly more energy dense meals (0·32 kJ/g, 95% CI 0·1–0·6 kJ/g), energy (62 kJ/g, (95% CI 27–97 kJ)) Na (19 mg, (95 % CI 6, 32)), added sugars (0·6 g, (95 % CI 0·1, 1·1)), vegetables (3 g, (95 % CI 1, 4)), total grams (12 g, (95 % CI 3, 21)) and saturated fat (0·2 g, (95 % CI 0·1, 0·4)) per eating occasion when accompanied by wider family. When children were accompanied by a formal childcare provider, they consumed significantly lower energy dense meals (−0·9 kJ/g, (95% CI −1·4 – −0·3 kJ/g)), less added sugars (−1·6 g, (95 % CI −2·4, −0·8)) and more fruit (12 g, (95 % CI 3, 21)) per eating occasion than when they were with their parents. The results demonstrate that non-parental caregivers might be an important target to promote healthy eating in young children. Background: There is growing evidence that antibody responses play a role in the resolution of SARS-CoV-2 infection. Patients with primary or secondary antibody deficiency are at increased risk of persistent infection. This challenging clinical scenario is associated with adverse patient outcome and potentially creates an ecological niche for the evolution of novel SARS-CoV-2 variants with immune evasion capacity. Case reports and/or series have implied a therapeutic role for convalescent plasma (CP) to secure virological clearance, although concerns have been raised about the effectiveness of CP and its potential to drive viral evolution, and it has largely been withdrawn from clinical use in the UK. Case presentation: We report two cases in which persistent SARS-CoV-2 infection was cleared following administration of the monoclonal antibody combination casirivimab and imdevimab (REGN-COV2, Ronapreve). A 55-year-old male with follicular lymphoma, treated with B cell depleting therapy, developed SARS-CoV-2 infection in September 2020 which then persisted for over 200 days. He was hospitalised on four occasions with COVID-19 and suffered debilitating fatigue and malaise throughout. There was no clinical response to antiviral therapy with remdesivir or CP, and SARS-CoV-2 was consistently detected in nasopharyngeal swabs. Intrahost evolution of several spike variants of uncertain significance was identified by viral sequence analysis. Delivery of REGN-COV2, in combination with remdesivir, was associated with clinical improvement and viral clearance within 6 days, which was sustained for over 150 days despite immunotherapy for relapsed follicular lymphoma. The second case, a 68-year-old female with chronic lymphocytic leukaemia on ibrutinib, also developed persistent SARS-CoV-2 infection. Despite a lack of response to remdesivir, infection promptly cleared following REGN-COV2 in combination with remdesivir, accompanied by resolution of inflammation and full clinical recovery that has been maintained for over 290 days. Conclusions: These cases highlight the potential benefit of REGN-COV2 as therapy for persistent SARS-CoV-2 infection in antibody deficient individuals, including after failure of CP treatment. Formal clinical studies are warranted to assess the effectiveness of REGN-COV2 in antibody-deficient patients, especially in light of the emergence of variants of concern, such as Omicron, that appear to evade REGN-COV2 neutralisation. A growing body of literature suggests that higher engagement in a range of activities can be beneficial for cognitive health in old age. Such studies typically rely on self-report questionnaires to assess level of engagement. These questionnaires are highly heterogeneous across studies, limiting generalisability. In particular, the most appropriate domains of activity engagement remain unclear. The Victoria Longitudinal Study-Activity Lifestyle Questionnaire comprises one of the broadest and most diverse collections of activity items, but different studies report different domain structures. This study aimed to help establish a generalisable domain structure of the Victoria Longitudinal Study-Activity Lifestyle Questionnaire. The questionnaire was adapted for use in a sample of UK-based older adults (336 community-dwelling adults aged 65–92 with no diagnosed cognitive impairment). An exploratory factor analysis was conducted on 29 items. The final model retained 22 of these items in a six-factor structure. Activity domains were: Manual (e.g., household repairs), Intellectual (e.g., attending a public lecture), Games (e.g., card games), Religious (e.g., attending religious services), Exercise (e.g., aerobics) and Social (e.g., going out with friends). Given that beneficial activities have the potential to be adapted into interventions, it is essential that future studies consider the most appropriate measurement of activity engagement across domains. Lockdowns and social distancing have been important and successful strategies to limit the spread of the coronavirus disease 2019 (COVID-19) virus. However, excess deaths related to non-COVID-19 causes have been reported, suggesting issues around availability and use of health services, particularly for people with conditions needing ongoing medical support. In addition, evidence indicates that a range of age-related diseases and frailty are impacted by physical activity and social engagement, both limited in lockdown situations. It is therefore important to learn from the effects of lockdowns in order to limit any impacts, while still protecting people from the infection. This editorial summarizes two research themes at the Centre for Ageing Research at Lancaster University in the UK, one assessing impacts of lockdown for people living with a long-term neurodegenerative condition, Parkinson's disease, and one assessing longitudinal impacts on frailty and wellbeing, with older adults aged over 70, including those living with at least one long-term condition. Uncertainty related to Parkinson's disease and to COVID-19 amplified each other, and cancelled clinical appointments and limitations on physical activity had very significant impacts on wellbeing for this group. In the longitudinal study, frailty was more severe during lockdown periods. While lockdowns reduce spread of the virus, becoming frailer could make older adults more vulnerable to the effects of the virus during these periods. Regular exercise during lockdown had beneficial effects aiding recovery once restrictions relaxed. These studies suggest factors that could lessen negative impacts of future lockdowns. Maintaining physical activity and providing access to health services during periods of lockdown are suggested as priorities. [ABSTRACT FROM AUTHOR]

Aims: Selection for optimal root system architecture (RSA) is important to ensure genetic gains in the sustainable production of wheat (Triticum aestivum L.). Here we examine the hypothesis that past wheat breeding has led to changes in RSA and that future breeding efforts can focus directly on RSA to improve adaptation to target environments. Methods: We conducted field trials using diverse wheat varieties, including modern and historic UK varieties and non-UK landraces, tested under contrasting tillage regimes (non-inversion tillage versus conventional ploughing) for two trial years or different seeding rates (standard versus high rate) for one trial year. We used field excavation, washing and measurement of root crowns ('shovelomics') to characterise RSA traits, including: numbers of seminal, crown and nodal roots per plant, and crown root growth angle. Results: We found differences among genotypes for all root traits. Modern varieties generally had fewer roots per plant than historic varieties. On average, there were fewer crown roots and root angles were wider under shallow non-inversion tillage compared with conventional ploughing. Crown root numbers per plant also tended to be smaller at a high seeding rate compared with the standard. There were significant genotype-by-year, genotype-by-tillage and genotype-by-seeding-rate interactions for many root traits. Conclusions: Smaller root systems are likely to be a result of past selection that facilitated historical yield increases by reducing below-ground competition within the crop. The effects of crop management practices on RSA depend on genotype, suggesting that future breeding could select for improved RSA traits in resource-efficient farming systems. [ABSTRACT FROM AUTHOR]

Summary: Venetoclax is a BCL2 inhibitor with activity in relapsed/refractory (R/R) chronic lymphocytic leukaemia (CLL). We conducted a multi‐centre retrospective analysis of 105 R/R CLL patients who received venetoclax pre‐National Health Service commissioning. The median age was 67 years and median prior lines was 3 (range: 1–15). 48% had TP53 disruption. At ≥2 lines, 60% received a Bruton Tyrosine Kinase inhibitor (BTKi) and no prior phosphoinositide 3‐kinase inhibitor (Pi3Ki), 25% received a Pi3Ki and no prior BTKi, and 10% received both. Patients discontinued B cell receptor inhibitor (BCRi) because of toxicity in 44% and progression in 54%. Tumour lysis syndrome risk was low, intermediate or high in 27%, 25%, and 48% respectively. Overall response was 88% (30% complete response [CR]). The overall response rate was 85% (CR 23%) in BTKi‐exposed patients, 92% (CR 38%) in Pi3Ki‐exposed patients and 80% (CR 20%) in both (P = 0·59). With a median follow‐up of 15·6 months, 1‐year progression‐free survival was 65·0% and 1‐year overall survival was 75·1%. Dose reduction or temporary interruption did not result in an inferior progression‐free or discontinuation‐free survival. Risk of progression or death after stopping a prior BCRi for progression was double compared to those stopping for other reasons (predominantly toxicity) (Hazard Ratio 2·01 P = 0·05). Background: Current non-invasive methods of assessing disease activity in systemic lupus erythematosus (SLE) are of limited sensitivity and specificity. Testing includes acute phase markers, autoantibodies and complement levels. Although measurements of dsDNA antibodies and complement C3/C4 levels are routine, they remain of limited value. Improved blood and urine markers may help in early detection of flare, distinction between flare and chronic damage, and monitoring response to therapy. Methods: A total of 87 patients with SLE were tested for the following cytokines in serum and urine: monocyte chemoattractant protein 1 (MCP-1), regulated upon activation, normal T cell expressed and secreted (RANTES), soluble tumour necrosis factor receptor 1 (sTNF-R1), interferon-inducible protein 10 (IP-10), monocyte inhibitory protein 1α (MIP-1α) and vascular endothelial growth factor (VEGF). Patients attending the Lupus Unit at St Thomas' Hospital, London, UK were divided into active lupus nephritis (LN), inactive LN and non-renal SLE groups based on their renal pathology and SLE disease activity index (SLEDAI). Cytokine testing was performed using the FIDIS multiplex bead assay. Results: The mean level of serum sTNF-R1 was higher in the active LN group compared with both inactive LN and non-renal SLE groups (p < 0.001). For urine measurements there were significant differences between active LN and non-renal SLE for VEGF (p = 0.016), after statistical correction for multiple testing. Both urinary and serum sTNF-R1 and IP-10 levels correlated with SLEDAI scores (p < 0.001), while serum VEGF correlated weakly with SLEDAI (p = 0.025). The optimum combination for differentiating active from inactive LN patients was serum VEGF, sTNF-R1, MCP-1 and glomerular filtration rate plus urinary sTNF-R1 and protein-creatinine ratio. Conclusion: These results indicate that for active LN, sTNF-R1 could be a useful serum cytokine marker, with potential for VEGF in the urine. This study has confirmed the ability of the multiplex bead technique to detect cytokines in a good analytical range, including very low and high levels, in both serum and urine. It expresses the view on evaluation of effectiveness of policy changes in widening participation. An overview of the efforts of WP practitioners, and experienced researchers, to explore institutional developments for the success of WP.

Prescribing errors made by junior doctors are rare only due to lack of knowledge or technical skills. Final Year medical students were given the opportunity to investigate medication incidents involving junior doctors. However long-term sustainability of this activity requires evidence about return on investment for NHS Tayside who assist in supporting medical students. Kirkpatrick's model for evaluating training programmes has been adapted for evaluation of medical education. We address all four levels of Kirkpatrick's model for training medical students in incident reporting, discuss other models and emphasis the importance of return on investment. Examples of organizational learning highlighted in the pilot study suggest that incident review by medical students can provide fresh insights into achieving the "not-seeking-to-blame" culture. Incident Review is now a core activity for all final year medical students at the University of Dundee. The historical geography of the British motorcycle industry (its distribution over time and the rate at which motorcycle marques entered and exited) shows two major agglomerations or clusters (West Midlands and Greater London) to have been the driving force behind the industry. Motorcycle marque exits from the industry (shakeout) had a net positive effect on the West Midlands and a net negative effect on the Greater London area. Spin-off of new companies was primarily an internal process in the West Midlands and an external process in the Greater London area. The high level of spatial concentration, the rapid rate of technology diffusion, and large number of spin-off firms helped the British motorcycle industry to maintain a wide variety of marques over a long time. However, each successive shakeout period resulted in an industry that was increasingly innovation-averse and stagnant, eventually ending in its near total collapse. [ABSTRACT FROM AUTHOR]

The article describes the unprecedented influx of Iceland Gulls Larus glaucoides into the northern and northwestern Britain between January and February 2012. It elaborates on the number of birds, its age composition, and the presence of Kumlien's Gulls L. g. kumlieni. It notes that these surge of gulls was dwarfed by the influx of gulls in the Faroe Islands.

Background: In the United Kingdom (UK), the National Treatment Agency for Substance Misuse (NTA) considers retention to be the best available measure of drug treatment effectiveness. Accordingly, the NTA has set local treatment systems the annual target of retaining 75% of clients for 12 weeks or more, yet little assessment of this target or factors that improve retention has occurred. This study aims to quantify the proportion of people retained in treatment for 12 weeks in the North West of England and to identify factors associated with premature drop out. Methods: The North West National Drug Treatment Monitoring System (NDTMS) was used to identify treatment durations for everyone beginning a treatment episode between 1st April 2005 and 31st March 2006 (N = 16626). Odds ratios, chi-square and logistic regression analyses compared clients retained for 12 weeks to clients whose discharge record showed they had prematurely dropped out before 12 weeks. Individuals with other outcomes were excluded from analyses. Results: 75% of clients (N = 12230) were retained for 12 weeks and 10% (N = 1649) dropped out prematurely. Multivariate analysis showed drop out was more likely among Asian drug users (adjusted odds ratio 1.52, 95% CI 1.12 to 2.08) than their white equivalents. Drop out was more likely among residents of Cumbria and Lancashire (adjusted odds ratio 1.80, 95% CI 1.51 to 2.15) and Greater Manchester (adjusted odds ratio 2.00, 95% CI 1.74 to 2.29) than Cheshire and Merseyside and less likely among alcohol users (adjusted odds ratio 0.73, 95% CI 0.59 to 0.91). A significant interaction between age and deprivation was observed. For those aged 18 to 24 years and 25 to 34 years, drop out was significantly more likely among those living in affluent areas. For those in the older age groups the converse effect was observed. Conclusion: In combination, the drug treatment systems of the North West achieved the Government's retention target in 2005/06. A number of factors associated with drop out were identified; these should be considered in strategies that aim to improve retention. This article reports on Continuing Vocational Education (CVE) in higher education in England. The findings described here were collected from interviews, documentary evidence and observations of activity. The evidence suggests the importance of a 'supply-side' model to chart the development of CVE. First, HEIs must audit and then use the resources they have to best effect. Second, HEIs must have effective managerial systems in place. Third, the organisation of the HEI will determine the development of CVE. Complementary to these stages is the capacity and willingness of HEIs to undertake economic evaluation of their provision. Internal and external efficiency must be considered. This paper revisits the causes of product deletion, an important if somewhat neglected part of product management theory. The causes of product deletion are important in the way they compromise a product manager's ability to pursue the organisation 's product objectives. Without a knowledge of when or why a product may become sick, it is doubtful that proactive product management can be successfully accomplished. The documented causes of why products become terminally ill are explored to provide a conceptual background for the reported study. The findings of a qualitative study into the factors that cause product deletion in the financial services sector are presented. The key issues relating to both the financial services and the physical goods s ector are analysed and discussed. The outcome of this is a realisation that the development of a universal model for the identification of why products become weak is unsafe. The results of this study suggest that there is a clear need for research that explores the relationship between the causes of product decline and the formulation of triggers of deletion. The article revisits the contribution of Robert William Thomson to the development of rubber tyres. Thomson is responsible for redesigning the mobile steam crane in 1852 and in designing a novel dry dock in London, Englanf in 1860. His steam tractors with solid rubber tyres were sold to the Indian government and in Europe. In Great Britain, a series of test on the Thomson steamer on suitability for use in military transport were conducted.

The authors assert the importance of clarifying the definition and regulation of veterinary specialists then developing appropriate specialist practice standards in Great Britain, highlighting the stakeholders meeting held on March 22, 2011 of the Working Party on Veterinary Specialisation formed by the Royal College of Veterinary Surgeons (RCVS). They state the requirements for individuals to be RCVS recognised specialists. They also mention the 2009 Lowe Report on veterinary expertise in food animal production.

The article presents a case study on the occurrence of chromosomal abnormalities using the oocytes of a 39 year old woman in Great Britain. The study intends to determine the potential of the maternal chromosome in the development of the down syndrome or trisomy 21 on children. The microsatellite analysis was used in the cytogenetic diagnosis of the trisomy 21.

The article reports on the increased prevalence of equine atypical myopathy in Great Britain. It notes that several equine atypical myopathy cases were reported in England and Northern Europe. It mentions that equine atypical myopathy affects both humans and horses wherein young horses are highly susceptible to the disease. Affected horses exhibit discoloured brown urine, stiff, and weak and with increased level of creatine kinase and aspartate transaminase and urine myoglobin concentration.

• Coordination of scientific research communities supported mathematical modelling of COVID-19. • Pre-existing research collaborations contributed to the success of coordination initiatives. • National infrastructure is key to support nationwide research networks. • Evidence synthesis important in supporting scientists providing advice to government. In March 2020 mathematics became a key part of the scientific advice to the UK government on the pandemic response to COVID-19. Mathematical and statistical modelling provided critical information on the spread of the virus and the potential impact of different interventions. The unprecedented scale of the challenge led the epidemiological modelling community in the UK to be pushed to its limits. At the same time, mathematical modellers across the country were keen to use their knowledge and skills to support the COVID-19 modelling effort. However, this sudden great interest in epidemiological modelling needed to be coordinated to provide much-needed support, and to limit the burden on epidemiological modellers already very stretched for time. In this paper we describe three initiatives set up in the UK in spring 2020 to coordinate the mathematical sciences research community in supporting mathematical modelling of COVID-19. Each initiative had different primary aims and worked to maximise synergies between the various projects. We reflect on the lessons learnt, highlighting the key roles of pre-existing research collaborations and focal centres of coordination in contributing to the success of these initiatives. We conclude with recommendations about important ways in which the scientific research community could be better prepared for future pandemics.
