Background: Only 27 cases of equine conjunctival haemangiosarcoma have been reported in the literature over the past 37 years. Out of these, 22% of cases were lost to follow-up, 52% were euthanized, and 26% survived. A scarcity of cases and information is available for this rarely seen conjunctival tumour. Aim: To describe the clinical features, management, and outcome of conjunctival hemangiosarcoma in seven horses in the UK. Methods: Optivet medical records were reviewed for equine cases seen or advised on with a histopathological diagnosis of conjunctival haemangiosarcoma between January 2013 and March 2023. Medical records were accessed for details of signalment, history, management, and follow-up. Histopathology was used to confirm the diagnosis of haemangiosarcoma and assess the surgical margins. Immunohistochemistry was performed in a minority of cases with poorly differentiated solid tumours to support vascular lineage. Results: Seven eyes from seven horses (five geldings and two mares) with a mean age of 16 years and median of 18 years (range 10-21 years) met the criteria. Serosanguinous discharge was seen in six eyes. All eyes were managed surgically; 4 by exenteration and 3 by conjunctivectomy/keratectomy. Adjunctive cryotherapy was performed in two eyes. Metastatic disease in the ipsilateral parotid salivary gland, confirmed with histopathology, was seen in one horse. Surgical margins were clear in all but one eye. Solar elastosis was noted in five eyes. All horses were healthy at the last follow-up (0.2-5 years, mean 2.9 years, and median 2 years). Conclusion: Equine conjunctival haemangiosarcoma is rare. Serosanguinous ocular discharge is a common clinical sign. Early surgical excision is highly effective. It is a truism that whole energy system models underpin the development of policies for energy system decarbonisation. However, recent reviews have thrown doubt on the appropriateness of such models for addressing the multiple goals for future energy systems, in the face of emergent real-world complexity and the evolution of stakeholder's priorities. Without an understanding of the changing priorities of policy makers and expectations of stakeholders for future systems, system objectives and constraints are likely to be ill-defined, and there is a risk that models may be inadvertently instrumentalised. Adopting a system architecture perspective, the authors have undertaken a three-year programme of research to explore strategies for decarbonising heat in the UK, with interaction with and elicitation of needs from stakeholders at its heart. This paper presents the procedure, methods, and results of an exercise in which experts from stakeholder organisations across the energy system were interviewed. Analysis of interview data reveals two broad approaches to heat decarbonisation which can be defined as either adaptive or transformative. Specific insights gained from these interviews enabled our modelling teams to refocus their work for exploration with a wider circle of stakeholders. Results suggests that this iterative approach to formalising model-policy interaction could improve the transparency and legitimacy of modelling and enhance its impact on policy making. This study contrasts young people's predrinking in two European cultural contexts: Spain and the UK. Whilst UK predrinking typically occurs amongst small groups of individuals who already know one another, the distinctive Spanish context of the Botellón details a far larger gathering in which participants may be less likely to know each other. As such, predrinking motives which drive consumption and risk-taking may be expected to vary between these cultures. An online questionnaire (N = 397; UK = 167, Spain = 230) was used to examine a variety of drinking behaviours and associated beliefs/motivations including predrinking motivations, drinking behaviour, and risk taking. Path analysis was used to analyse both direct and indirect relationships between the measures with the aim of predicting problem alcohol consumption with the most parsimonious model. Varying (in)direct paths were observed between predrinking motives and alcohol consumption between the cultures. Most notably and pointing towards inconsistency in the drivers of young adults' drinking, fun predrinking motives featured prominently among Spanish respondents and predicted their reported consumption (not so in the UK), while conviviality was a more prevalent predrinking motive in the UK sample and associated with alcohol consumption (not the case in Spain). Further, (personal) risky behaviour and risk-taking predicted consumption in both samples, suggesting the importance of group norms and behaviours in predrinking activity, irrespective of alcohol consumption. These findings highlight the potential importance of the environment in which young people predrink. The assumedU-values of solid walls represent a significant source of uncertainty when estimating the energy performance of dwellings. The typicalU-value for UK solid walls used for stock-level energy demand estimates and energy certification is 2.1 Wm−2 K−1. A re-analysis (based on 40 brick solid walls and 18 stone walls) using a lumped thermal mass and inverse parameter estimation technique gives a mean value of 1.3 ± 0.4 Wm−2 K−1for both solid wall types. Among the many implications for policy, this suggests that standard UK solid-wallU-values may be inappropriate for energy certification or for evaluating the investment economics of solid-wall insulation. For stock-level energy modelling, changing the assumedU-value for solid walls reduces the estimated mean annual space heating demand by 16%, and causes a proportion of the stock to change Energy Performance Certification (EPC) band. The analysis shows that the diversity of energy use in domestic buildings may be as much influenced by heterogeneity in the physical characteristics of individual building components as it is by variation in occupant behaviour. Policy assessment and guidance material needs to acknowledge and account for this variation in physical building characteristics through regular grounding in empirical field data. Little detailed evidence has previously been available regarding the uptake rate or prevalence of energy efficiency interventions among specific household groups. This study uses the Home Energy Efficiency Database (HEED) to investigate both the combination of measures that have been installed, and in which dwellings, according to key neighbourhood socio-demographic variables, including income and tenure. Analysis of 2000–07 data indicates that approximately 40% (9.3 million) dwellings in England had approximately 23.7 million efficiency measures installed, with an average of 2.5 measures per dwelling. Building fabric-related measures were the most frequent (e.g.cavity wall insulation, loft insulation and glazing) with an average of 2.1 million installed each year. Dwellings with the highest number of fabric interventions (the top 20%) were more likely to be found in areas with low income, with more owner-occupied dwellings, experiencing lower winter temperatures, having a lower proportion of flats, and having a slightly higher proportion of older adults and children. Energy efficiency installations have tended to occur among specific types of households or parts of the building stock. These findings have implications for the design of future government programmes for targeting energy efficiency measures to specific household groups or dwelling types. [ABSTRACT FROM PUBLISHER]

• Emergent phenomena can affect energy system resilience. • A System Architectural perspective is therefore needed to support decision-making. • High rates of change will make evolvability a desirable property of energy systems. • A definition of evolvability is offered. Whole energy system modelling is a valuable tool to support the development of policy to decarbonise energy systems, and has been used extensively in the UK for this purpose. However, quantitative insights produced by such models necessarily omit potentially important features of physical and engineering reality. The authors argue that important socio-technical insights can be gained by studying critical events such as the loss of 2.1 GW generation from the electricity system of Great Britain on 9th August 2019, in conjunction with literature on the behaviour of complex systems. Among these insights is the idea that models of the operation and evolution of energy systems can never be complete. Both system behaviour (operation) and the emergence and evolution of structure in such systems are formally uncomputable. This provides a starting point for a discussion of the need for additional tools, drawn from the System Architecture literature, to support the design and realisation of future, fully-decarbonised systems with high penetrations of renewable energy. Desirable properties of System Architectures, including current and future Energy System Architectures, are discussed. These include resilience and flexibility, for which there is an extensive literature. Energy models have been widely applied to the analysis of energy system decarbonisation to assess the options and costs of a transition to a low carbon supply. However, questions persist as to whether they are able to effectively represent and assess heat decarbonisation pathways for the buildings sector. A range of limitations have been identified, including a poor spatio-temporal resolution, limited representation of behaviour, and restricted representation of the full technical option set. This paper undertakes a review of existing energy models for heat decarbonisation in the UK, applying the novel perspective of energy system architecture (ESA). A set of ESA-related features are identified (including evolvability, flexibility, robustness, and feasibility), and models are reviewed against these features. The review finds that a range of models exist that have strengths across different features of ESA, suggesting that multiple modelling approaches are needed in order to adequately address the heat decarbonisation challenge. The sizing of district energy systems involves a trade-off between reliability and continuity of service, and avoidance of capital and running costs associated with oversizing. Finding the most appropriate sizing requires a thorough understanding of energy demand. However, empirical data necessary to support such an understanding is scarce, and district energy systems are typically oversized. This study uses smart meter data from the largest field trial to analyse residential energy consumption in the UK. It presents graphically the seasonal and daily variations in energy consumption patterns, the weather dependence of energy loads, and peak hourly demand during particularly cold weather conditions. It also explores the diversity effect in residential energy consumption and computes the after diversity maximum demand at different levels of aggregations. Results show that, peak hourly gas consumption was nearly seven times higher than electricity consumption during the cold spells, while diversity reduced gas and electricity maximum demand per dwelling up to 33% and 47%, respectively. In comparison to the efforts required to bring a new drug or formulation to the clinic, bestowing a name on a medicine is relatively simple. However, if the name we choose causes confusion-by making its contents ambiguous or if it is too alike another drug-it can precipitate clinical errors. This prompted the World Health Organization to set up the International Nonproprietary Naming Committee in the 1970s to select unambiguous names for drugs. Unfortunately, multidrug products-which are becoming increasingly popular-do not fall under the remit of conventional International Non-proprietary Nomenclature. We have identified 26 combination formulations that have been historically named with the co-drug format in the United Kingdom. Most of them have also been prescribed in the United Kingdom in the past year, and although several of them are not prescribed very often, 11 were prescribed more than 2000 times. In this paper, we have explored the literature to identify prescribing errors with co-drug products and found several idiosyncrasies that have caused drug errors in the past. We advocate for a standard nomenclature (state the international nonproprietary name [INN] of each component followed by dose information in the x + y format) for these products on the box and in prescribing resources. We hope that this will enhance clarity and safety during prescribing and administration, particularly for high-volume drugs like paracetamol + codeine (co-codamol), amoxicillin + clavulanic acid (co-amoxiclav) and trimethoprim + sulfamethoxazole (co-trimoxazole)., (© 2024 The Author(s).
