Giant ichthyosaurs with body length estimates exceeding 20 m were present in the latest Triassic of the UK. Here we report on the discovery of a second surangular from the lower jaw of a giant ichthyosaur from Somerset, UK. The new find is comparable in size and morphology to a specimen from Lilstock, Somerset, described in 2018, but it is more complete and better preserved. Both finds are from the uppermost Triassic Westbury Mudstone Formation (Rhaetian), but the new specimen comes from Blue Anchor, approximately 10 km west along the coast from Lilstock. The more complete surangular would have been >2 m long, from an individual with a body length estimated at ~25 m. The identification of two specimens with the same unique morphology and from the same geologic age and geographic location warrants the erection of a new genus and species, Ichthyotitan severnensis gen. et sp. nov. Thin sections of the new specimen revealed the same histological features already observed in similar giant ichthyosaurian specimens. Our data also supports the previous suggestion of an atypical osteogenesis in the lower jaws of giant ichthyosaurs. The geological age and giant size of the specimens suggest shastasaurid affinities, but the material is too incomplete for a definitive referral. Gaining an understanding of the concerns and aspirations of a country's diaspora can help domestic politicians to better connect with this community and gain their support in elections. The United Kingdom's diaspora is large and spread among many countries, and currently has the right to vote in UK general elections only for a limited time. However, there are proposals to abolish these time limits and this will make this community of increasing interest to politicians. This study uses signatories to the UK Parliaments e-petitions platform to gain an understanding of the foreign and domestic political concerns of this community. The analysis uses Latent Dirichlet allocation to identify common topics among the e-petitions and hierarchical clustering to identify commonalities among countries, territories and regions. This paper provides analytic focus on the productive and editorial contexts of children and young people's image-making, making visible its implications for the analysis of photographs. Drawing on participatory research in which children and young people worked alongside researchers to create a visual narrative of their lived experiences of neighbourhood, the paper suggests that greater attention to children's image-making practices brings an important dimension to the interpretative challenges generated by the visual. Through a focus on image-making and its productive and editorial contexts, the paper shifts the analytic focus away from the image as a site of meaning-making to encompass the ways in which photographs acquire multiple meanings through the lived experience of their creators. Obtaining up to date information on the number of UK COVID‐19 regional infections is hampered by the reporting lag in positive test results for people with COVID‐19 symptoms. In the UK, for 'Pillar 2' swab tests for those showing symptoms, it can take up to five days for results to be collated. We make use of the stability of the under reporting process over time to motivate a statistical temporal model that infers the final total count given the partial count information as it arrives. We adopt a Bayesian approach that provides for subjective priors on parameters and a hierarchical structure for an underlying latent intensity process for the infection counts. This results in a smoothed time‐series representation nowcasting the expected number of daily counts of positive tests with uncertainty bands that can be used to aid decision making. The size, composition, and spatial distribution of both people and households have a substantial impact on the demand for and development and delivery of infrastructure required to support the population. Infrastructure encompasses a wide range of domains including energy, transport, and water, each of which has its own set of spatial catchments at differing scales. Demographic projections are required to assess potential future demand; however, official projections are usually not provided at a high level of spatial resolution required for infrastructure planning. Furthermore, generating bespoke demographic projections, often incorporating a range of scenarios of possible future demographic change is a specialist, resource intensive job and as such is often missing from infrastructure development projects. In this paper we make the case that such demographic projections should be at the heart of infrastructure planning and present a set of open‐source models which can be used to undertake this demographic projection work, thus providing the tools needed to fill the identified gap. The census of population, conducted by three statistical offices in the United Kingdom, provides a unique resource for measuring and understanding internal migration patterns and propensities. It is the only source that asks an entire population, once every ten years, about their migration behaviour and as such provides a valuable resource for assessing the impact that migration has on the size and composition of local populations. The data are attribute rich in that they provide information about migration patterns by demographic group and are spatially detailed so provide a view of the impact that migration has on small areas. Despite their utility, the data have limitations, not least that they are only collected once per decade so only provide a snapshot of the UK migration system. In order to understand the health outcomes for distinct sub-groups of the population or across different geographies, it is advantageous to be able to build bespoke groupings from individual level data. Individuals possess distinct characteristics, exhibit distinct behaviours and accumulate their own unique history of exposure or experiences. However, in most disciplines, not least public health, there is a lack of individual level data available outside of secure settings, especially covering large portions of the population. This paper provides detail on the creation of a synthetic micro dataset for individuals in Great Britain who have detailed attributes which can be used to model a wide range of health and other outcomes. These attributes are constructed from a range of sources including the United Kingdom Census, survey and administrative datasets. It provides a rationale for the need for this synthetic population, discusses methods for creating this dataset and provides some example results of different attribute distributions for distinct sub-population groups and over different geographical areas. Background: Zoological gardens contain unique configurations of exotic and endemic animals and plants that create a diverse range of developing sites and potential sources of blood meals for local mosquitoes. This may imply unusual interspecific pathogen transmission risks involving zoo vertebrates, like avian malaria to captive penguins. Understanding mosquito ecology and host feeding patterns is necessary to improve mosquito control and disease prevention measures in these environments. Methods: Mosquito sampling took place in Chester Zoo for 3 years (2017, 2018, and 2019) and for 1 year in Flamingo Land (2017) using different trapping methods. Blood-fed mosquitoes were identified and their blood meal was amplified by PCR, sequenced, and blasted for host species identification. Results: In total, 640 blood-fed mosquitoes were collected [Culex pipiens (n = 497), Culiseta annulata (n = 81), Anopheles maculipennis s.l. (n = 7), An. claviger (n = 1), and unidentifiable (n = 55)]. Successful identification of the host species was achieved from 159 blood-fed mosquitoes. Mosquitoes fed on birds (n = 74), non-human mammals (n = 20), and humans (n = 71). There were mixed blood meals from two hosts (n = 6). The proportions of blood-fed mosquitoes varied across sampling seasons and sites within the zoos. The use of resting traps and aspiration of vegetation were more efficient techniques for capturing blood-fed mosquitoes than traps for host-seeking or gravid mosquitoes. By relating the locations of zoo vertebrates to where fed mosquitoes were trapped, the minimum travelling distances were calculated (13.7 to 366.7 m). Temperature, precipitation, relative humidity, proximity to zoo vertebrate exhibits, and vegetation level were found to be significantly associated with the proportion of captured blood-fed mosquitoes by generalized linear modelling. Conclusions: Mosquito feeding behaviour in zoos is mainly influenced by time, location (sampling area), temperature, and host availability, which highlights the value of mosquito monitoring in complex settings to plan control strategies and potentially reduce inherent disease transmission risks for humans and threatened zoo vertebrates. An almost complete ichthyosaur from the historically significant collection of fossils amassed by Somerset geologist, Charles Moore, is described. Available information suggests that it was collected from the Lower Lias of Somerset, UK. However, inspection of the surrounding matrix, which seems to be a nodule, indicates that the specimen may instead hail from the Upper Lias (Toarcian) Strawberry Bank Lagerstätte of Ilminster, Somerset. The specimen had previously been identified as Leptonectes tenuirostris. Herein, further analysis of the skull and forefin, reveals closer affinities to Stenopterygius triscissus, to which the specimen is tentatively assigned. The United Kingdom faces demographic uncertainty, as negotiations for leaving the European Union (Brexit) proceed. Brexit has implications for international migration into and out of the UK, dependent on future immigration policy and on how attractive the UK will be as a labour market. At the same time, the UK population is experiencing ethnic diversification, consequent on past immigration. To explore the UK's future ethnic diversity, we run four projection scenarios. Three international migration scenarios, varying by the extent of the break with the EU, are implemented together with a reference projection assuming zero international migration. Ethnic groups are differently affected by these migration scenarios, depending on the contribution of international migration to population growth and the extent of demographic momentum. The White British and Irish lose population under all Brexit scenarios and the Black Caribbean population declines in all but one scenario. The White Other, Indian, Chinese, Other Asian and Other groups will show much lower growth under Soft and Hard Brexit scenarios. The growth of the Mixed, Pakistani, Bangladeshi, Black African and Black Other groups will only be affected marginally. Cannibalisation and trial are both potential consequences of line extension strategies with, normally, the former to be controlled and the latter encouraged. Both stem from the familiarity of the parent brand, the main driver of line extension strategy. For a brand manager, considering the launch of a new line, these two effects create tensions; how can trial be maximised whilst parental share loss is minimised? We examine these two effects in different line extensions in the UK and Germany to understand how trial and cannibalisation interact to affect the success of a line extension. Our empirical research analyses the purchase patterns of buyers of a new line, before and after the launch of the extension, and compares these with the level of cannibalisation. Our aim is to understand the dynamics of consumer response to the new line. The household panel data analysis of fmcg purchases shows that line extension buyers are disproportionate purchasers of the parent brand, both before and after the launch. However these measures of cross-purchase correlate only weakly with the level of cannibalisation (rho = .42 before, .43 after). Whilst a line extension clearly encourages purchase of the new line by purchasers of the existing parent, this leads to additional portfolio purchasing as often as substitution. Numerous specimens of Ichthyosaurus are known, but only very few small examples (total length of <1 m) have been assigned beyond Ichthyosaurus sp. Here, we report on a very small specimen (preflexural length of 560 mm) that can be unequivocally assigned to Ichthyosaurus communis due to possessing a unique combination of diagnostic skull and postcranial characters that are found in larger examples of the species. Furthermore, the specimen is identified as a neonate because of the small size, large sclerotic ring relative to the orbital region, and poorly ossified (highly cancellous) bones of the skull and postcranium. It is not an embryo as it is not preserved within an adult specimen and stomach contents are clearly evident. This is therefore the first neonate Ichthyosaurus communis skeleton to be described. The specimen, in the Lapworth Museum of Geology, University of Birmingham, has no provenance data associated with it. A microfossil analysis of the matrix in which the ichthyosaur skeleton is preserved strongly suggests a stratigraphic range of uppermost Hettangian to lowermost Sinemurian age (Lower Jurassic), but does not provide any geographical information. Internal migration is one of the main drivers of local population change and therefore an important factor to consider in the provision of services across key domains such as housing, health and education. In recent years, the population of England and Wales has become increasingly ethnically diverse, with groups varying in age structure and time spent in the UK (whether at the group or individual level). However, relatively little is known about the internal migration patterns of these diverse groups, with research into internal migration propensity not accounting for the possible interplay between ethnicity, country of birth and age. Using micro‐data from the 2011 Census for England and Wales, this paper addresses this gap in knowledge, exploring whether widely reported determinants of internal migration operate differently for different ethnic groups, stratified by age and country of birth. This paper also explores whether for ethnic minority groups born overseas, time spent in the UK matters for migration propensity. When stratifying ethnic groups by age and country of birth, differences in migration propensity do not appear to be explained by socio‐economic differences, as previous research in the field suggests. However, for those born overseas, some of the differences are explained by time spent in the UK. From a policy perspective, it is evident that a more nuanced understanding of the way in which propensity to migrate may vary between ethnic groups differing in age and country of birth is needed to ensure sustained provision and equitable access across key service domains. Further research must examine the extent to which differences in migration propensity shapes the nature of migration events, whether in terms of distance or frequency of move. This paper examines differences in the propensity to move short (less than 10 km) and long (10 km or more) distance between ethnic groups differentiated by age and country of birth. Introduction: The undertreatment of acute pain presents a significant challenge in the Emergency Department. This post hoc subgroup analysis of a previously reported randomized controlled UK study reports the efficacy and safety of low-dose methoxyflurane analgesia in treating adolescent patients with moderate-to-severe trauma pain. Patients and methods: Three hundred patients (96 in the adolescent subgroup) aged ≥12 years requiring analgesia for acute trauma pain (pain score of 4–7 on the Numerical Rating Scale) at triage were randomized 1:1 to methoxyflurane (up to 6 mL) or placebo (normal saline), both administered using a Penthrox® inhaler. The patient could request rescue medication (paracetamol/opioids) at any time. The primary endpoint was the change from baseline in visual analog scale (VAS) pain intensity. Results: Mean VAS pain score for the adolescent subgroup at baseline was ~61 mm. Adjusted mean change in VAS pain intensity from baseline to 5, 10, 15, and 20 minutes was −24.5, –28.1, –31.6, and –31.7 mm for methoxyflurane and −14.6, –18.8, –19.2, and –23.7 mm for placebo, with a statistically significant treatment effect in favor of methoxyflurane overall across all four time points (–9.9 mm; 95% CI: −17.4, –2.4 mm; P=0.0104). Median time to first pain relief was significantly shorter with methoxyflurane (1 minute) than placebo (3 minutes, P<0.0001). Pain relief was reported within 1–10 inhalations in 95.7% of methoxyflurane-treated patients and 64.6% of placebo-treated patients. Rescue medication was requested by two (4.3%) methoxyflurane-treated patients and three (6.3%) placebo-treated patients. Over 95% of patients, physicians, and nurses rated methoxyflurane treatment as "Excellent", "Very Good" or "Good" compared with between 64% and 68% for placebo. The incidence of adverse events was higher with methoxyflurane (51%) than placebo (42%), mostly comprising mild/transient dizziness and headache. Conclusion: This subgroup analysis shows that low-dose inhaled methoxyflurane is a rapid-acting and effective analgesic in adolescent patients presenting with moderate-to-severe trauma pain. Ichthyosaur fossils are abundant in Lower Jurassic sediments with nine genera found in the UK. In this paper, we describe the partial skeleton of a large ichthyosaur from the Lower Jurassic (lower Sinemurian) of Warwickshire, England, which was conserved and rearticulated to form the centrepiece of a new permanent gallery at the Thinktank, Birmingham Science Museum in 2015. The unusual three-dimensional preservation of the specimen permitted computed tomography (CT) scanning of individual braincase elements as well as the entire reassembled skull. This represents one of the first times that medical imaging and three-dimensional reconstruction methods have been applied to a large skull of a marine reptile. Data from these scans provide new anatomical information, such as the presence of branching vascular canals within the premaxilla and dentary, and an undescribed dorsal (quadrate) wing of the pterygoid hidden within matrix. Scanning also revealed areas of the skull that had been modelled in wood, clay and other materials after the specimen's initial discovery, highlighting the utility of applying advanced imaging techniques to historical specimens. Additionally, the CT data served as the basis for a new three-dimensional reconstruction of the skull, in which minor damage was repaired and the preserved bones digitally rearticulated. Thus, for the first time a digital reconstruction of the skull and mandible of a large marine reptile skull is available. Museum records show the specimen was originally identified as an example of Ichthyosaurus communis but we identify this specimen as Protoichthyosaurus prostaxalis. The specimen features a skull nearly twice as long as any previously described specimen of P. prostaxalis, representing an individual with an estimated total body length between 3.2 and 4 m. An analysis of the UK and German detergent markets provides useful evidence for the development of a model to predict the share changes that occur when a new product enters the market. A simple share order effect (SOE) model based on Luce's Axiom of the Independence of Irrelevant Alternatives (IIA) is tested on four different new product launches. Of these, only the product with both a new brand name and a new product format is consistent with the model. In the remaining three cases, all line extensions, the empirical data deviate from the model's predictions. The two UK line extensions appear to be affected by product format; the German line extension cannibalizes the parent. From these studies, it is suggested that radical line extensions do not cannibalize whereas line extensions that have a close fit with the parent are more likely to take sales from the core brand. As a non-invasive imaging technique, this study explores the application of Computed Tomography (CT) in microplastics research, assessing its potential to distinguish different types and sizes of microplastics (polypropylene, polyethylene terephthalate, polyethylene, and polyvinyl chloride) from homogenised river-estuarine sediment. When examined in layers within artificial cores, all microplastic types could be observed by CT imagery, with good contrast in X-ray attenuation (based on image gray level intensity) against background sediments. Large microplastics (4 mm diameter) were also detectable when distributed randomly amongst the sediment. These spiked cores had sufficient difference in attenuation to allow segmentation between type, and therefore isolate individual microplastics. Due to limitations on scan resolution, smaller microplastics (≤125 μm diameter) could not be detected in spiked cores. Scans of two sediment cores from a Thames River tributary (UK) revealed two distinctive sediment structures which could influence microplastic accumulation. This information would be lost using conventional recovery procedures. Many e-health projects fail to be implemented. We aimed to find a sustainable process of service user and health professional online discussions about e-health to facilitate implementation and identification of needed research. A previously piloted course compared Mental Health participants’ views with publications, identifying ‘quick wins’ and barriers to e-health implementation. This study explored this approach further in eight domains including Health Promotion, Mental Health, and Carers. Courses comprised webinar, 1-week closed discussion forum, and final webinar. Participants discussed 12 e-health topics. Course analysis identified that five out of eight domains ‘worked’. Participation was appreciated and service users influenced health professional thinking. The principle of service user–health professional online discussions to prepare for e-health implementation works for most domains, but the work of participant recruitment and forum management may make other methods, such as Tweetchats or courses hosted by existing forums where service users predominate, easier to sustain in the long term. This study investigated mothers' experiences surrounding stillbirth in the United Kingdom, their memory making and sharing opportunities, and the effect these opportunities had on them. Qualitative data were generated from free text responses to open-ended questions. Thematic content analysis revealed that "stigma" was experienced by most women and Goffman's (1963) work on stigma was subsequently used as an analytical framework. Results suggest that stillbirth can spoil the identities of "patient," "mother," and "full citizen." Stigma was reported as arising from interactions with professionals, family, friends, work colleagues, and even casual acquaintances. Stillbirth produces common learning experiences often requiring "identity work" (Murphy, 2012). Memory making and sharing may be important in this work and further research is needed. Stigma can reduce the memory sharing opportunities for women after stillbirth and this may explain some of the differential mental health effects of memory making after stillbirth that is documented in the literature. The purpose of this cross-sectional study was to ascertain whether undergraduate dental students in the United Kingdom and Ireland are receiving formal teaching on recognizing and managing domestic violence (DV) as part of their curricula. A questionnaire was sent to all dental schools in the UK and Ireland in 2007 and again in 2012, requesting information on whether the subject was taught, by which specialty it was taught, and whether schools felt it was important to include in the curriculum. In 2007, twelve of the fifteen dental schools completed and returned the questionnaire, for a response rate of 80 percent; in 2012, eleven of the sixteen dental schools responded, for a response rate of 69 percent. The main findings were that, in 2007, 50 percent of the responding schools were providing teaching about DV and the majority of this teaching was delivered by oral surgery and pediatric dentistry departments. In 2012, only 45 percent of the responding schools were teaching DV, with 60 percent of this teaching being delivered by pediatric dentists. This study's findings suggest that DV is an undertaught area in UK and Irish undergraduate dental curricula. Some schools recognized the importance of DV teaching; however, they have been unable to implement it because of a full curriculum and lack of appropriately trained staff amongst other reasons. Purpose – This paper aims to examine the implementation of the Eco-Management and Audit Scheme (EMAS) in a non-profit, small-to-medium sized-enterprise (SME) in the UK. Design/methodology/approach – A four-year participatory action research study is made upon knowledge transfer partnerships between the University of the West of England and the Royal Bath and West Society. Findings – Through the adoption of EMAS, the organisation was able to identify operational improvements as well as make significant efforts to improve its environmental performance, reducing its carbon footprint by 30 tCO2e per annum and gaining new business. Research limitations/implications – The study is made upon a single not-for-profit organisation in the UK. Practical implications – It presents the costs, benefits and challenges that the organisation faced. Techniques that were used to successfully manage the environmental management systems (EMS) development are also discussed. The investigation identifies deficiencies in the materials that are provided to support companies that are seeking EMAS certification. To improve the uptake of these EMS and assist companies in their successful pursuit of ISO 14001 and EMAS, this supporting documentation requires enhancement. Originality/value – There has been relatively little empirical research around the development and benefits of organisational EMS. Even less has focussed upon the specific constraints and opportunities that face non-profit organisations when implementing EMAS. There is no single framework in place in the United Kingdom for collating and harmonizing the data on migration taking place between the subnational administrative units that constitute the home nations, together with the flows of international migration between these areas and the 'rest of the world'. This paper proposes the construction of complete matrices of subnational migration statistics on an annual basis which can then be used to monitor migration trends throughout the UK in a comprehensive manner. The paper reviews how various 'known' flows are estimated by the national statistics agencies and develops estimates to fill in the gaps in the matrices. It then looks at changing patterns of migration in the UK between 2001/2002 and 2010/2011, showing that migration exhibits most activity in the middle of the decade. Flows within the home nations are the most consistent over time, while the patterns of international migration and migration across the borders of the home nations exhibit more substantial change. The latter flows, although relatively small in magnitude, have not previously been studied at the subnational level in any detail. Specific remedial works designed to improve sound installation and reduce the noise level produced by helicopters inside dwellings are described. The theoretical problems and practical solutions to installing high-performance acoustic insulation to a traditional property in the United Kingdom are presented. A novel application of ISO 140-5 is presented using real helicopters to measure sound insulation in situ in the presence of multiple flanking transmission paths. Dedicated field trials to evaluate the performance of such acoustic double-glazing and associated modifications systems were performed and the precautions taken to minimise measurement uncertainties over the extended time period of the trials are detailed. The field trials involved the use of military training helicopters following selected flight paths around the property, while noise-level measurements were made internally and externally, before and after replacement of the existing single-glazed windows and attenuated ventilation units were installed. The results show that after replacing the main windows with acoustic insulated glazing units, sound insulation levels of 40 dB or above are achieved in most rooms. The results also illustrate the importance of effectively addressing ventilation when windows are replaced. It is concluded that despite complications due to sound flanking and regulatory ventilation, the use of acoustic double-glazing units and properly attenuated ventilation units can effectively reduce helicopter noise in suitable dwellings.Practical application: This article provides an evaluation of ways in which the sound insulation of dwellings can be practically improved to reduce the impact on everyday living of noise from helicopter operations. It addresses concerns about the practical use of high-performance acoustic insulated glazing units (IGUs) used in combination with acoustic through-the-wall ventilation. Objective: This study examined whether the experience of creating and sharing memories of their babies is associated with mothers’ mental health after stillbirth, taking account of factors previously shown to be important. Background: Mothers of stillborn babies are usually offered the opportunity to spend time with and create memories of their babies. However, evidence on whether this leads to better mental health outcomes is equivocal. One possible explanation is that the impact of making memories is mediated by the extent to which women subsequently share these memories. Methods: Cross-sectional questionnaire study. Mothers (N = 162) of stillborn babies completed online questionnaires of how memories were made and shared, satisfaction with memory-making and sharing, professional and social support, and symptoms of depression, anxiety and PTSD. Results: The majority of mothers made and shared memories. The number of different memory-making activities was not associated with mental health outcomes. However, the degree to which mothers shared their memories was associated with fewer PTSD symptoms. Regression analyses showed that good mental health was most strongly associated with time since stillbirth, perceived professional support, sharing of memories and less wish to talk more about the baby. Conclusion: This study confirms research showing that time since stillbirth and perceived professional support is associated with better mental health following stillbirth and for the first time shows the importance of opportunities to share memories of the baby. β2-1 fructans are considered to be prebiotics. Current literature indicates that β2-1 fructans may modulate some aspects of immune function, improve the host's ability to respond to certain intestinal infections, and modify some inflammatory outcomes in human subjects. However, there is a need to find out more about the modulation of immune markers by β2-1 fructans in humans. Healthy human subjects aged 45–65 years were randomly allocated to β2-1 fructans (Orafti® Synergy1; 8 g/d; n 22) or the digestible carbohydrate maltodextrin as placebo (n 21) for 4 weeks. Blood, saliva and faecal samples were collected at study entry and after 4 weeks. Immune parameters were measured using the blood and saliva samples and bifidobacteria were measured in the faecal samples. Faecal bifidobacteria numbers increased in the Orafti® Synergy1 group (P < 0·001) and were different at 4 weeks from numbers in the placebo group (P = 0·001). There was no significant effect of Orafti® Synergy1 on any of the immune parameters measured (blood immune cell subsets, total serum Ig, salivary IgA, neutrophil and monocyte phagocytosis of Escherichia coli and respiratory burst in response to E. coli or phorbol ester, natural killer cell activity, T cell activation and proliferation, production of six cytokines by T cells). It is concluded that, compared with maltodextrin, Orafti® Synergy1 has a bifidogenic effect in healthy middle-aged human subjects but does not alter immune responses examined in the absence of an in vivo immune challenge. The objectives of this study are to determine self-assessed knowledge and skills in sexual health and HIV medicine in preregistration house officers and to explore undergraduate experiences of teaching and assessment in these subjects prior to the launch of National Core Learning Outcomes in Sexual and Reproductive Health and HIV. The study was designed as a postal questionnaire survey. The participants were all UK medical graduates of August 2004. The response rate 1737/4746 (36%). The main outcome measures were Doctors' views on their preparedness to manage patients with sexual health and HIV-related problems. Since graduation, 90% of respondents had seen at least one patient with a sexually transmitted infection or HIV-related issue. Seventy-six percent felt confident to take a sexual history. In all, 63% and 53% felt competent in male and female genital examination, respectively. Forty-three percent felt they could conduct an appropriate HIV pretest discussion and 59% felt they could recognize clinical indicators suggestive of HIV. Seventy-eight percent had been formally assessed in sexual health and 55% in HIV medicine. Increased confidence in sexual history taking, HIV pretest discussion and recognition of HIV indicators was associated with a longer duration of teaching and formal examination. In conclusion, although the proportion of recent graduates confident in sexual history taking is encouraging, their lack of skill in discussing HIV testing, risk assessment and recognition of possible HIV presentations must be addressed. Objectives The British HIV Association (BHIVA) audit subcommittee aimed to survey UK clinic policy and practice regarding baseline assessment and immunization of newly diagnosed HIV-positive patients, and frequency of follow-up and testing in established patients in the UK. Methods UK centres providing HIV care were requested to complete an online survey between October 2006 and March 2007. Results 111 centres participated in the survey. 89.2% of centres routinely performed baseline HIV resistance testing. 99% of centres had a policy of routine screening for hepatitis B. Only 91% of centres were routinely offering a sexual health screen at diagnosis. Frequency of routine follow-up for patients not requiring antiretroviral therapy (ART) and stable on ART varied between three and six months. Discussion This review showed variations in practice regarding the post diagnosis assessment and routine monitoring of HIV patients. It is of concern that not all centres perform baseline HIV resistance testing. It has also been noted that hepatitis B vaccination is not being offered to non-immune patients at diagnosis. Purpose- As staff are vital to successful re-branding, particularly in the charity sector where restricted budgets limit reliance on external marketing, it is important to understand the impact of re-branding on staff. This study aims to examine the effect of time on staff knowledge, attitudes and behaviour and, in addition, the interaction of time with seniority, tenure and level of support for re-branding. Design/methodology/approach - The paper first explores the literature from both the for-profit and non-profit sectors. A quantitative study was undertaken in nine leading, UK charities that had re-branded two, three and four years ago; n = 345. The data were analysed using one-way and two- way ANOVAs. Findings - A negative relationship was found between time since re-branding and the three constructs of knowledge, attitudes and behaviour. But this consistency was not mirrored by a consistency in the impact of interaction effects. Practical implications - Re-branding is not a one-off event. To sustain its benefits, organizations need to re-visit its outcomes on a regular basis to ensure staff retain new knowledge, remain positively motivated and maintain their recently adapted behaviours over time. Originality/value - Thought to be the first empirical paper to explore the effects of re-branding over time. Furthermore, the findings contradict those from the extant literature that claim that organizational change requires a Äsettling in" period. • Despite the costs of charity re-branding, there is little research in the public domain of its effect on staff. This study addresses that gap in knowledge by evaluating the effects of re-branding large UK charities on staff knowledge, attitudes and behaviour. • A quantitative survey of 465 charity staff was carried out in 10 large UK charities. The study shows that knowledge has benefited most from re-branding although unevenly across different levels of seniority. Length of service has no effect on levels of knowledge but level of support was positively correlated. • The impact of re-branding on attitude approved positive with staff at all levels feeling more motivated, involved and valued as a consequence. However, the study also demonstrates that not all staff feel engaged with the re-branding process for the organisation to capitalise fully on enhanced staff performance. • Behaviour was less affected by re-branding with the exception of a very positive impact on staff retention. The study's aim was to investigate the cost-effectiveness of an NHS/Social Services short-term residential rehabilitation unit (a form of intermediate care) for older people on discharge from community hospital compared with ‘usual’ community services. An economic evaluation was conducted alongside a prospective controlled trial, which explored the effectiveness of a rehabilitation unit in a practice setting. The aim of the unit was to help individuals regain independence. A matched control group went home from hospital with the health/social care services they would ordinarily receive. The research was conducted in two matched geographical areas in Devon: one with a rehabilitation unit, one without. Participants were recruited from January 1999 to October 2000 in 10 community hospitals and their eligibility determined using the unit's strict inclusion/exclusion criteria, including 55 years or older and likely to benefit from a short-term rehabilitation programme: potential to improve, realistic, achievable goals, motivation to participate. Ninety-four people were recruited to the intervention and 112 to the control group. Details were collated of the NHS and Social Services resources participants used over a 12-month follow-up. The cost of the resource use was compared between those who went to the unit and those who went straight home. Overall, costs were very similar between the two groups. Aggregated mean NHS/Social Services costs for the 12 months of follow-up were £8542.28 for the intervention group and £8510.68 for the control. However, there was a clear ‘seesaw’ effect between the NHS and Social Services: the cost of the unit option fell more heavily on Social Services (£5011.56, whereas £3530.72 to the NHS), the community option more so on the NHS (£5146.74, whereas £3363.94 to Social Services). This suggests that residential rehabilitation for older people is no more cost-effective over a year after discharge from community hospital than usual community services. This paper reviews the role that model-based forecasts play in the monetary policy process in the United Kingdom, with particular reference to the Bank of England's new quarterly model and continuing research into other statistical approaches. The Bank's models provide a consistent framework for considering alternative scenarios and risks but judgement always plays a large role in constructing forecasts. It is hard to say precisely how important forecasts are in driving policy decisions, but there is some evidence that the rethink of key issues during the forecast round has been a source of policy news. Forecasts also play a central part in communicating the Monetary Policy Committee's thinking to the outside world. But forecasts are highly fallible. So the MPC's forecast centered approach to inflation targeting has gone hand in hand with a determined effort to illustrate the wide range of uncertainties around its central projections. In making this argument, we draw particularly upon the voluminous literature which amplifies the seminal work of Foucault, especially as it relates to government. We concentrate first on the construction of legislation and its legal aftermath. Many researchers are beginning to adopt the use of wrist-worn accelerometers to objectively measure personal activity levels. Data from these devices are often used to summarise such activity in terms of averages, variances, exceedances, and patterns within a profile. In this study, we report the development of a clustering utilising the whole activity profile. This was achieved using the robust clustering technique of k-medoids applied to an extensive data set of over 90,000 activity profiles, collected as part of the UK Biobank study. We identified nine distinct activity profiles in these data, which captured both the pattern of activity throughout a week and the intensity of the activity: "Active 9 to 5", "Active", "Morning Movers", "Get up and Active", "Live for the Weekend", "Moderates", "Leisurely 9 to 5", "Sedate" and "Inactive". These patterns are differentiated by sociodemographic, socioeconomic, and health and circadian rhythm data collected by UK Biobank. The utility of these findings are that they sit alongside existing summary measures of physical activity to provide a way to typify distinct activity patterns that may help to explain other health and morbidity outcomes, e.g., BMI or COVID-19. Demand variation has some benefits for retailer. Fluctuations in demand invite two types of response from management: first, to adjust the supply of the service to the level of demand. Supermarket managers do this on a regular basis by varying staffing levels and by opening and closing checkouts to fit the daily and weekly patterns of demand and second, to try to influence the timing of demand. In some services this is achieved using reservations or price differentials. In supermarkets, promotions have occasionally been used to raise traffic in off-peak periods. A mail survey on use of supermarkets and post offices explored a number of the questions raised above. The results supported the patterns of store usage and also showed that the majority of people claimed to have regular times for shopping by day and time of day. This study divided respondents according to whether they disliked waiting at the checkout or not, there was evidence that people who disliked waiting were less likely to shop in the busy period in the morning, suggesting that some people may try to avoid congestion.

The chief purpose of the research is to improve the programs of teacher preparation which are provided in university departments and colleges of education. Those who have been involved in research on teacher education have therefore found it necessary to seek greater understanding of teaching behavior and greater knowledge of its influence on student learning. It is the purpose of this chapter to review some aspects of British research into teacher education to try to suggest problems for future investigations.

Recent analysis of area-level COVID-19 cases data attempts to grapple with a challenge familiar to geovisualization: how to capture the development of the virus, whilst supporting analysis across geographic areas? We present several glyphmap designs for addressing this challenge applied to local authority data in England whereby charts displaying multiple aspects related to the pandemic are given a geographic arrangement. These graphics are visually complex, with clutter, occlusion and salience bias an inevitable consequence. We develop a framework for describing and validating the graphics against data and design requirements. Together with an observational data analysis, this framework is used to evaluate our designs, relating them to particular data analysis needs based on the usefulness of the structure they expose. Our designs, documented in an accompanying code repository, attend to common difficulties in geovisualization design and could transfer to contexts outside of the UK and to phenomena beyond the pandemic.
