A considerable number of adults who are currently living with posttraumatic stress disorder (PTSD) are also parents caring for at least one biological child. Evidence suggests that parental PTSD can be associated with impairments to certain parenting behaviours, particularly increasing the use of more negative practices. However, most of the evidence to date has been collected using quantitative methodology, which gives limited insight into why such effects might occur. The current study qualitatively explored study the lived experiences of parents currently living with PTSD, within the United Kingdom. Interviews were conducted with 30 parents (16 mothers, 14 fathers) who were recruited via a PTSD research registry, and who had children living at home under the age of 18 years at the time they experienced their trauma. Three main themes were identified: key impacts of the trauma to the parent personally; negative changes to specific parenting outcomes; impact of these parenting changes on the parent's sense of parental efficacy; and recovery and coping. These findings provide novel insight into the experiences of both mothers and fathers with PTSD, and highlight the multiple challenges faced by parents living with PTSD that extend beyond impairments to themselves as individuals. Potential implications for the implementation of effective support for parents and their families following trauma exposure are considered. The current study offers a unique insight into the experiences of both mothers and fathers, as 47% of the sample were fathers. Three themes were identified perceived negative changes to specific parenting outcomes; impact of these parenting changes on the parent's sense of parental efficacy; and recovery and coping. Background. The Rewind Technique (Rewind) is a psychological therapy for people with posttraumatic stress disorder (PTSD), which is already used regularly in the National Health Service (NHS), the third sector and private practice across the UK. This study set out to explore the potential efficacy and feasibility of remotely delivered Rewind for the treatment of PTSD. Methods. This study was a two-armed, exploratory RCT to assess the preliminary efficacy, adherence, feasibility, and factors affecting outcome of Rewind versus a waitlist control group. Results. The entire trial was delivered remotely using video calls for treatment sessions and outcome assessments. A total of 40 participants were randomised with 80% retention at the primary endpoint of 8-week postrandomisation. The difference in Clinician-Administered PTSD Scale for DSM-5 scores between the immediate and delayed Rewind arms was 12.64 (95% CI, 2.29 to 22.99, F = 6.38 , df = 1 , p = 0.02) at 8 weeks. The Cohen's d was 1.05 indicating a large effect size at 8 weeks with maintenance in symptom improvement at 16 weeks. Conclusion. Rewind, delivered remotely, demonstrated a large effect size in treating symptoms of PTSD within this trial cohort. Background: Multi-modular motion-assisted memory desensitization and reconsolidation therapy (3MDR) is a new psychological intervention for people with post-traumatic stress disorder (PTSD). 3MDR is immersive, delivered in a virtual reality environment, and emphasises engagement, recollection and reprocessing. Objective: Through a theory-driven examination of data relating to 10 out of 42 UK military veterans taking part in a trial of 3MDR, the principal objective was to explore the complex interrelationships between people, interventions and context and to investigate how factors within these domains interacted in specific outcome typologies. Method: Quantitative and qualitative data relating to 10 trial participants were derived from: researcher-assessed and self-report clinical measures; interviews; physiological recordings; words describing thoughts and feelings during therapy; and subjective unit of distress scores. Using a convergent mixed methods approach, data were tabulated using a person, intervention and context model. Participant summaries were grouped into outcome typologies, followed by an analysis of data convergence and divergence within each and an interpretation of identified patterns. Results: Three outcome response typologies were identified: dramatic improvement, moderate improvement and minimal improvement. Within the person domain, factors associated with outcomes included walking capacity, commitment and ability to complete therapy, and levels of subjective distress. Within the intervention domain, factors associated with outcomes related to image selection and use, therapeutic alliance and orientations towards the tailoring of sessions. Within the context domain, factors associated with outcomes included reactions to the therapy environment. The patterning of secondary outcomes broadly corresponded with primary outcomes within each typology. Alongside patterned data differentiating aspects of the person, intervention and context domains, within the three response typologies data also existed where no obvious patterning was detected. Conclusions: The model developed here may have novel value in evaluating a range of personalised interventions, but further work is needed before confident assertions can be made of who is likely to benefit from 3MDR specifically. Objectives: To explore in‐depth the views on Internet‐based psychological therapies and their implementation from the perspective of National Health Service (NHS) commissioners and managers. Design: Qualitative interview study. Method: Ten NHS commissioners and managers participated in a semi‐structured, co‐produced interview. Each transcribed interview was double‐coded and thematically analysed using The Framework Method. Results: Interviews generated three main themes. (1) Capacity issues across psychological therapy services create barriers to face‐to‐face therapies, and Internet‐based interventions offer a solution. (2) Despite reservations, there is growing acceptance of Internet‐based therapies. Different ways of connecting with patients are required, and Internet‐based treatments are accessible and empowering treatment options, with guided self‐help (GSH) preferred. Internet‐based interventions may however exclude some individuals and be a threat to the therapeutic relationship between patient and practitioner. (3) Successful roll‐out of Internet‐based interventions would be facilitated by a strong empirical‐ or practice‐based evidence, a national coordinated approach and timely training and supervision. Barriers to the roll‐out include digital intervention set‐up costs and delays due to NHS inflexibility. Conclusions: The study highlights factors influencing access to Internet‐based therapies, important given the rapid evolution of e‐therapies, and particularly timely given increasing use of remote therapies due to COVID‐19 restrictions. Interviewees were open to Internet‐based approaches, particularly GSH interventions, so long as they do not compromise on therapy quality. Interviewees acknowledged implementation may be challenging, and recommendations were offered. Practitioner points: There is a shift in practice and increasingly positive views from NHS staff around remote psychological therapies and different ways of connecting with patients, particularly since the COVID‐19 pandemic.There is a strong preference for Internet‐based psychological interventions that are guided and that include built‐in outcome measures co‐produced with service users.There is a need to raise awareness of the growing evidence base for Internet‐based psychological therapies, including research examining therapeutic alliance across Internet‐based and face‐to‐face therapies.Challenges implementing Internet‐based psychological therapies include therapist resistance to changing working practices in general, and inflexibility of the NHS, and national, coordinated implementation efforts are encouraged. Background: Following the recently published 11th version of the WHO International Classification of Diseases (ICD-11), we sought to examine the risk factors and comorbidities associated with posttraumatic stress disorder (PTSD) and complex PTSD (CPTSD).Method: Cross-sectional and retrospective design. The sample consisted of 1,051 trauma-exposed participants from a nationally representative panel of the UK adult population.Results: A total of 5.3% (95% confidence interval [CI] = 4.0-6.7%) met the diagnostic criteria for PTSD and 12.9% (95% CI = 10.9-15.0%) for CPTSD. Diagnosis of PTSD was independently associated with being female, being in a relationship, and the recency of traumatic exposure. CPTSD was independently associated with younger age, interpersonal trauma in childhood, and interpersonal trauma in adulthood. Growing up in an urban environment was associated with the diagnosis of PTSD and CPTSD. High rates of physical and mental health comorbidity were observed for PTSD and CPTSD. Those with CPTSD were more likely to endorse symptoms reflecting major depressive disorder (odds ratio [OR] = 21.85, 95 CI = 12.51-38.04) and generalized anxiety disorder (OR = 24.63, 95 CI = 14.77-41.07). Presence of PTSD (OR = 3.13, 95 CI = 1.81-5.41) and CPTSD (OR = 3.43, 95 CI = 2.37-4.70) increased the likelihood of suicidality by more than three times. Nearly half the participants with PTSD and CPTSD reported the presence of a chronic illness.Conclusions: CPTSD is a more common, comorbid, debilitating condition compared to PTSD. An impediment to conducting high‐quality quantitative research studies in education is the paucity of valid measures of learning gains. Studies often seek to investigate students' deep, conceptual understanding yet many measures assess only surface, procedural understanding. One reason is that the development of validated measures of conceptual understanding is resource intensive and time consuming, and success is not guaranteed. We evaluated a novel and efficient technique, based on comparative judgement, for assessing conceptual understanding. We applied the technique to a randomised controlled trial in which students were taught simple algebra based on either the Grid Algebra or the MiGen software package. The participants were Year 5 students (N = 188) drawn from four primary schools who had not encountered algebra previously. An instrument from the literature (Concepts in Secondary Mathematics and Science: Algebra Scale) and a novel comparative judgement assessment were administered following the intervention. Students in the Grid Algebra condition outperformed those in the MiGen condition on both post‐test measures. The comparative judgement technique performed similarly to the standard instrument but was far more efficient to design and implement. The technique can, in principle, be quickly applied to any target concept of interest. We conclude that comparative judgement is a valid, reliable and practical tool that could help to increase both the quantity and quality of quantitative research in education. [ABSTRACT FROM AUTHOR]

Foreign language (FL) films with subtitles are becoming increasingly popular, and many European countries use subtitling as a cheaper alternative to dubbing. However, the extent to which people process subtitles under different subtitling conditions remains unclear. In this study, participants watched part of a film under standard (FL soundtrack and native language subtitles), reversed (native language soundtrack and FL subtitles), or intralingual (FL soundtrack and FL subtitles) subtitling conditions while their eye movements were recorded. The results revealed that participants read the subtitles irrespective of the subtitling condition. However, participants exhibited more regular reading of the subtitles when the film soundtrack was in an unknown FL. To investigate the incidental acquisition of FL vocabulary, participants also completed an unexpected auditory vocabulary test. Because the results showed no vocabulary acquisition, the need for more sensitive measures of vocabulary acquisition are discussed. Finally, the reading of the subtitles is discussed in relation to the saliency of subtitles and automatic reading behavior. [ABSTRACT FROM PUBLISHER]

The ICD-11 reconceptualized Posttraumatic Stress Disorder (PTSD) as a narrowly defined fear-based disorder, and introduced Complex PTSD (CPTSD) as a new diagnosis comprised of PTSD symptoms and symptoms of 'Disturbances in Self-Organization' (DSO) that are more reflective of general dysphoria. Previous research suggests that PTSD symptoms mediate the association between childhood trauma and physical health problems, including cardiovascular disease. No study has yet assessed how posttraumatic stress symptoms, as outlined in the ICD-11, influence the association between childhood trauma and somatic problems in adulthood.Objective: This cross-sectional descriptive study examined whether PTSD and DSO symptoms mediated the associations between childhood physical and sexual abuse and childhood emotional abuse and neglect and somatic problems and cardiovascular diseases (CVD) load in adulthood.Methods: General adult population samples from Ireland (N = 1020) and the United Kingdom (N = 1051) completed self-report questionnaires online.Results: Structural equation modelling results indicated that PTSD and DSO symptoms fully mediated the association between both forms of childhood trauma and somatic problems, and that PTSD symptoms but not DSO symptoms fully mediated the association between childhood trauma and CVD load.Conclusion: Psychological interventions that effectively treat CPTSD symptoms may have the added benefit of reducing risk of physical health problems.
