Objective: The objective of this study is to present the experience of 3 institutions performing renal autotransplantation (RAT) and to discuss surgical techniques employed and the results in the light of the medical literature. Materials and Methods: A total of 14 patients (11 male and 3 female) with a mean age of 47 ± 8 years (35-61 years), who underwent RAT procedure at 3 different institutions between October 2006 and November 2014, in Turkey, were evaluated retrospectively. Indications for RAT procedure are ureteral avulsion, renal artery aneurysm and intimal dissection caused by percutaneous transluminal renal artery angioplasty (PTRA). Twelve patients with ureteral avulsion, 1 patient with renal artery aneurysm and 1 patient with intimal dissection caused by PTRA were followed-up for 103 months. Seven (50%) open and 7 (50%) laparoscopic nephrectomies were performed. Nine patients (64.3%) were right-sided and 5 patients (35.7%) were left-sided. Complications of grade III and above as per Clavien-Dindo classification were assessed. Results: Mean time from injury to RAT was 21.2 ± 40.1 days. However, 5 (35.7%) patients were treated on the same day of the injury. As per Clavien-Dindo classification, 2 (14.2%) grade IVa and 1 (7.1%) grade IIIa complications were reported. However, no significant correlation was observed between the complications and graft loss regarding type and side of the nephrectomy performed (p = 0.462 and p = 0.505, respectively) and timing of the intervention (p = 0.692). Conclusion: RAT is a safe procedure in combination with minimally invasive laparoscopic technique in carefully selected patients; however, it requires expertise and proficiency in laparoscopy, reconstructive urology and transplantation. We investigated the association of prostate cancer (PCa) and benign prostatic hyperplasia (BPH) with genetic polymorphisms in prostate-specific antigen (PSA) (−158 G/A) and 17-hydroxylase (CYP17) (−34 T/C) genes in a Turkish population. In this study, we investigated the distribution of these polymorphisms in 148 PCa patients, 136 BPH patients, and 102 healthy individuals as controls. The polymorphisms were analyzed using polymerase chain reaction–restriction fragment length polymorphism assay. Genotype and allele frequencies were calculated, and their associations with PCa or BPH risk are assayed. The frequency of PSA gene GA and GG genotypes was significantly higher in PCa patients than in controls ( p = 0.017 and p = 0.019, respectively). GG genotype was also associated with BPH ( p = 0.033). In a case analysis, according to Gleason score, the association of PSA gene GG genotype with Gleason score >7 was near to statistical significance (odds ratio, 2.94; 95% confidence interval, 0.95–9.28). There was also an association between CYP17 polymorphism and BPH ( p = 0.004). No association was observed between PCa and CYP17 gene polymorphism. These data demonstrate that PSA gene promoter variation may play a significant role in the development of PCa and BPH, and that CYP17 gene polymorphism may be associated with BPH in the Turkish population studied. Purpose: We evaluated factors affecting complication rates of ureteroscopy for pediatric ureteral calculi., Materials and Methods: We retrospectively evaluated 642 children who underwent ureteroscopy at 16 Turkish centers between 2000 and 2010. Semirigid ureteroscopy was used with various calibers to treat 670 ureteral units in 660 sessions. Complications were evaluated according to the Satava and Clavien classification systems. Univariate and multivariate analyses were done to determine predictive factors affecting complication rates., Results: A total of 367 females and 265 males were studied. Mean±SD patient age was 90.2±51.4 months (range 4 to 204). Mean±SD stone size, operative time and postoperative hospital stay were 8.9±4.7 mm, 45.8±23.8 minutes and 1.8±2.8 days, respectively. At a mean±SD followup of 13.3±17.6 months 92.8% of patients were stone-free and efficacy quotient was 90.3%. Complications, which occurred in 8.4% of patients (54 of 642), were intraoperative in 25 (Satava grade I to II in 22), early postoperative in 25 (Clavien grade I to II in 23) and late postoperative in 4 (all grade III). While operative time, age, institutional experience, orifice dilation, stenting and stone burden were statistically significant on univariate analysis, multivariate analysis revealed that operative time was the only statistically significant parameter affecting the complication rate., Conclusions: Semirigid ureteroscopy is effective, with a 90% stone-free rate and efficacy quotient. Most complications are low grade and self-limiting. Purpose: We retrospectively compared morbidity and success rates in children who underwent percutaneous nephrolithotomy with 3 different sizes of instruments., Materials and Methods: Percutaneous nephrolithotomy was performed using adult instruments via a 26Fr tract in 23 patients, using pediatric instruments via a 20Fr tract in 13 and using minimal access (14Fr) in 10. Percutaneous nephrolithotomy was performed using general anesthesia with parenteral antibiotic prophylaxis. Cystoscopy and retrograde pyelography were performed, and 5Fr to 6Fr soft ureteral catheters were placed during each procedure initially. Percutaneous access was established via fluoroscopic guidance with the patient in the prone position. Pneumatic, ultrasonic and holmium laser lithotriptors were used for in situ lithotripsy. A 14Fr nephrostomy catheter was placed at the end of the procedure in the 26Fr and 20Fr groups, and in the minimal access group the ureteral stent was left indwelling in cases of atraumatic procedure with no residual fragments., Results: Mean patient age was 13.2, 5.9 and 6.3 years, respectively, in the 26Fr, 20Fr and minimal access groups (p=0.000). Sex distribution, previous surgery, stone size, surgery and fluoroscopy times, mean hemoglobin decrease and hospitalization time did not differ between the groups. However, blood transfusion rate was higher in groups 1 and 2. Stone-free rates were 69.5%, 80% and 90%, respectively, in groups 1 to 3. Overall, adjuvant shock wave lithotripsy treatment was needed in 3 children., Conclusions: Smaller instrument size to 20Fr did not significantly increase the operative time, and resulted in the same success rates as the adult sized devices. However, low blood transfusion rates were only reached in the minimal access percutaneous nephrolithotomy group.
