Objective: Acute appendicitis (AA) is the most common cause of acute abdominal and emergency surgery worldwide. Over the past decades, the incidence of AA has been increasing, and with minimally invasive technology, treatment modalities are changing. This study aimed to examine the incidence of AA in Istanbul, Turkey over the years and seasons and investigate the rates of open and laparoscopic appendectomy (LA). Methods: This retrospective cohort study reviewed the archives of the Turkish Association of Trauma and Emergency Surgery between January 2012 and December 2019. Data from 11 tertiary hospitals with heavy patient flow from Istanbul were included. Descriptive statistics and univariate analysis of variance tests were performed. Statistical significance was defined as p-values of <0.05. Results: A total of 39,932 AA cases were evaluated. The cumulative incidence of AA in Istanbul was 123/100,000 between 2012 and 2019, with increasing incidence over the years (p=0.01). LA rates have increased, and 68.5% of the cases were laparoscopically managed in 2019. AA rates were slightly higher in spring than in other seasons (p>0.05). Conclusion: The incidence rate of AA and its laparoscopic management are significantly increasing in Istanbul. [ABSTRACT FROM AUTHOR]

Background: Acute appendicitis (AA) is one of the most common emergency surgical pathologies in patients presenting to emergency department. with abdominal pain. In patients diagnosed with Acute appendicitis, rapid and accurate diagnosis is necessary to reduce morbidity and mortality. Hemogram parameters are important in diagnosis of acute appendicitis. In this study, we aimed to investigate the place of acute appendicitis, in histopathological diagnosis by examining the hemogram parameters of the patients who were first examined in the emergency department. Materials and Methods: Hemogram parameters and postoperative pathology results of 116 patients who presented with abdominal pain to the Emergency Department of SBÜ Ş.Urfa Mehmet Akif İnan SUAM hospital between January 2018 and June 2020 and operated with the preliminary diagnosis of acute appendicitis were analyzed retrospectively. Patients were divided into three groups according to their pathology results and were questioned whether there was a relationship between hemogram parameters and acute appendicitis. Results: Of the 116 patients who underwent appendectomy, the average age was 12.0 (9.0-15.0), of which 60.3% (N=70) were male and 39.7% (n=46) were female. In the study, there were 60.3% (n=70) male patients and 39.7% (n=46) female patients. Abdominal Ultrasonography (USG) was performed to 76.7% (n=89) patients and abdominal Computed Tomography (CT) was performed to 18.1% (n=21) the patients before surgery. Both USG and abdominal CT were performed in 5.2% (n=6) patients. After postoperative pathological analysis, 11.2% (n=13) of the patients had normal appendix, 70.1% (n=82) of the patients had simple inflamed appendicitis, 18.7% (n=21) of the patients had Complicated (phlegmenous, gangrenous, perforated) appendicitis. Hemogram parameters of non- acute appendicitis and acute appendicitis patients were compared. There was statistically significant difference between Neutrophil/lymphocyte ratio (NLR) (p<0.05). There was no statistically significant difference between Red blood cell width (RDW), Platelet mass index (PMI), White blood cells (WBC), neutrophil count (p>0.05). Conclusion: White blood cells (WBC), neutrophil count, NLR, RDW, PMI, are easily accessible and fast assessable hemogram parameters, available for diagnosis of acute appendicitis patients. However, these parameters alone are not enough for diagnosis of acute appendicitis. A good anamnesis, physical examination and appropriate radiologic imaging studies are needed to make a timely and accurate diagnosis. [ABSTRACT FROM AUTHOR]

It is very important to make accurate and early diagnosis to reduce both the rate of negative laparotomy and the morbidity and mortality associated with acute appendicitis. Numerous scoring systems have been described in the diagnosis of acute appendicitis. We aimed to determine the accuracy rates of these scoring systems in the diagnosis of acute appendicitis by applying Alvarado, Eskelinen, and Ohmann clinical scoring systems to patients who underwent appendectomy. This study was performed on patients who underwent appendectomy for acute appendicitis at Başkent University Alanya Hospital and Alanya Alaaddin Keykubat University Education and Research Hospital between September 2017 and December 2018. The sensitivity and specificity of Alvarado, Eskelinen, and Ohmann scoring systems as well as the compatibility analysis of scoring systems with pathology results were investigated. A total of 554 patients undergoing appendectomy were included in the study. The sensitivity of Eskelinen scoring method was found to be 53.4%, the specificity was 68.1%; the sensitivity of Alvarado scoring method 41.6%, the specificity 79.7%; and the sensitivity of Ohmann scoring system 94.0%, the specificity was 8.7%. In conclusion, Ohmann scoring system has the highest sensitivity; it could be more efficient in exclusion of the appendicitis diagnosis due to its low specificity. It is also concluded that some clinical parameters that are scored in Eskelinen and Alvarado scoring systems may decrease the sensitivity and specificity of these methods depending on the patient population and atypical clinical presentations. Aim: We investigated the predictive value of Alvarado scoring system, which is considered a reliable, practical and inexpensive tool, in the diagnosis of acute appendicitis, and whether it can be an alternative to ultrasonography and help reduce the negative laparotomy rates. Methods: A total of 327 patients, who were referred to İstanbul Haseki Research and Training Hospital General Surgery Clinic for abdominal pain and who received the diagnosis of acute appendicitis using both compression ultrasonography and Alvarado scoring system were retrospectively evaluated. Results: Two hundred and nine patients with abdominal pain and acute appendicitis were operated and 108 patients who did not require surgery were discharged after being placed in observation. The sensitivity and the specificity of the Alvarado score were found to be 92% and 28%, respectively The sensitivity and specificity of ultrasonography were 84% and 39%, respectively. Ultrasonographic findings were consistent with acute appendicitis in 180 (82.2%) of 219 operated patients, whereas appendix was normal in 39 (17.8%). Ultrasonography showed acute appendicitis in 92 (85.2%) and normal appendix in 16 (14.8%) of the 108 patients in the unoperated group. Conclusion: Alvarado score is not superior to ultrasonography in reducing negative laparatomy rates, but it can be used safely in the diagnosis of acute appendicitis andsurgical decision making. Objective: Most common disorders confused with acute appendicitis are gynecological pathologies. Here, we aimed to investigate the patients with negative acute appendicitis and determined gynecological pathology. Methods: Between 2006-2010, 25 patients who underwent laparatomy with the diagnosis of acute appendicitis and determined gynecological pathologies were analyzed retrospectively. Results: The mean age was 28.87±8.63 years. The most common complaint and finding on physical examination of the patients were abdominal pain and tenderness, respectively. In 15 patients, intraabdominal fluid was determined with abdominal ultrasonography. Appendectomy and additional procedures, such as bleeding control and cyst excision in ovarian cyst rupture, drainage in pelvic inflammatory disease and/or tubo-ovarian abscess, were performed. Ovarian cyst rupture was the most common gynecological pathology (76%). Conclusion: The female patients presenting with acute abdominal symptoms must be evaluated more carefully. Gynecological pathologies which may mimic acute appendicitis should be kept in mind in terms of differential diagnosis. [ABSTRACT FROM AUTHOR]

Background: In this study we investigated how the incidence and course of acute appendicitis (AA) changed in children during the pandemic., Methods: Children diagnosed with AA during the 1-year pandemic period after the first COVID-19 case in Turkey and the previous 1 year were included in the study. Children were divided into two groups: those hospitalized during the pandemic (group A) and those hospitalized in the year before the pandemic (group B). Furthermore, we compared the findings obtained for COVID-19-positive and COVID-19-negative children in the whole study group and within group A., Results: A significant difference was found between the two groups in terms of gender, the rate of vomiting and the number of days of vomiting. Complicated AA was more frequent in group B than in group A. In addition, the hospital stay was significantly longer, the mean number of days with fever was significantly higher, and mean body temperature was significantly higher in COVID-19-positive patients in the whole study group and within group A., Conclusions: Contrary to most studies in the literature, in the present study the patients in the pre-pandemic period were admitted to hospital later, and this may have been associated with the higher frequency of complicated AA in these patients. During the pandemic, however, the arrangements and warnings of health authorities might have reduced the anxiety and the hesitancy of families to go to the hospital, and hence this may have been associated with the lower rate of complicated AA in this period., (© 2022 Japan Pediatric Society.)

Background: Acute appendicitis is the most common reason for abdominal surgery in children. The aim of this study was to evaluate the utility of biomarkers in predicting complicated appendicitis (CA)., Methods: Patients having a diagnosis of acute appendicitis who underwent operations were retrospectively determined, and the utility of biomarkers in predicting CA was evaluated., Results: A total of 251 patients were included in the study. The mean age was 130.9 ± 48.8 months, 148 of the cases (59%) were simple appendicitis, and 103 (41%) were CA. The C-reactive protein (CRP) levels, immature granulocyte (IG) percentage, white blood cell counts, and absolute neutrophil counts were significantly higher in the CA patients. The bilirubin levels and neutrophil to lymphocyte ratios were not useful for predicting CA. The best area under the curve (AUC) values to predict CA were with the IG percentage and CRP level (0.82), the IG percentage [odds ratio (OR) 9.36, 95% CI (4.94-17.75), p < 0.001 ] and CRP [OR 8.42, 95% CI (4.72-15.02), p < 0.001 ] were the best marker in predicting CA. The sensitivity of the IG percentage and CRP level were higher than other markers., Conclusion: To predict CA, the best AUC values were associated with the IG percentage and the CRP level. Because it is easy, fast to measure, does not require taking extra blood, and does not lead to additional costs, IG percentage may be preferred in the diagnosis of patients with CA.

Background: Appendiceal diverticular disease (ADD) is a rare pathology which is associated with an increased mortality risk due to rapid perforation and high rates of neoplasm. In our study, we aimed to evaluate the clinical and histopathological characteristics of ADD with differences from acute appendicitis (AA) diagnosis and to determine the association with neoformative processes., Methods: The 4279 patients who underwent appendectomy were evaluated retrospectively. ADD patients histopathologically classified into four groups. Patients' demographic characteristics, imaging and preoperative laboratory findings, additionally postoperative histopathology results were compared between groups., Results: The prevalence of ADD was 2.29% (n = 98). In addition, the male/female ratio was 2.37 in ADD patients who were found to be significantly older than those with AA patients. Type III was the most frequently (62.2%) identified sub-group of ADD. The incidence of neoplasms, plastrone, and Littre's hernia was found statistically higher in ADD group than AA group. Mucinous adenomas (10.2%) was the most common neoplasm while the carcinoid tumor (1%) and precancerous serrated adenomas (4.1%) were also reported., Conclusions: As a result, high neoplasm in ADD patients can be shown with incidence of perforation and plastron, and in order to avoid possible neoplasm or major complications, it is necessary to carry out new studies for the right diagnosis of ADD whether the diagnosis is done preoperatively or ıntraoperatively. We recommend surgical resection of the ADD, which may even be incidentally detected during any surgical procedure, due to its high risk of neoplasm and rapid perforation.
