Objectives Urea cycle disorders (UCDs) are rare hereditary diseases. This study was conducted to help identify the characteristics of UCDs in Turkey. Methods The primary outcome was to determine patient characteristics. Investigating the relationships between the patient outcomes and ammonia levels were the secondary outcomes. Eighty five patients from 79 families, diagnosed with UCD at a single metabolic referral center between 1979 and 2017, were included. Clinical and laboratory data were retrieved retrospectively from hospital records. Results Classical citrullinemia was the most common type of UCD; citrin deficiency and carbamoyl phosphate synthase 1 deficiency (CPS1D) were the rarest. One thirty one hyperammonemic episodes were recorded. The peak ammonia levels were found to be significantly associated with polycythemia and hypocalcemia at presentation. The median peak ammonia values of the patients who died were higher than those of the survivors. The highest mortality rate was in the classical citrullinemia group. The mortality rate of the first hyperammonemic crisis was 28.6%, while it was 6.7% in subsequent episodes with an odds ratio of 4.28 (95% CI: 1.67-11.0) (p=0.001). Forty-four patients underwent genetic analysis and genetic variants were detected in 42 patients (95%). Three of the detected variants have not been previously reported. Conclusions This is the largest UCD series in Turkey and may serve as a guide to clinical, biochemical and genetic features of UCDs in our country. Prevention of hyperammonemia may be the most influential measure to improve long term survival.

Background/aim: The number of elderly people (=65 years old) is increasing both in Turkey and internationally due to improved living conditions and decreased mortality rates. Knowing the characteristics of elderly patients admitted to emergency departments can provide guidance for diagnosis and treatment approaches. In this study, we analyzed the characteristics of very old patients (=85 years old) admitted to the emergency department and put together data for use in forward planning in healthcare services. Materials and methods: This retrospective study included all patients aged =85 years admitted to the Adult Emergency Department of Gazi University Medical Faculty Hospital between 01.01.2014 and 31.12.2014. Data concerning the patients' age, sex, date of admission, weekday or weekend admission, reason for admission, length of stay, number of readmissions, admission from within or outside the province, and whether they were hospitalized or not were evaluated. Results: In this study, 1105 admissions of 780 patients aged =85 years to the emergency department were analyzed. The median age of the patients was 87 years and 59% were female. The most frequent admissions were in January (11.4%). Most patients (63.1%) were admitted only once to our emergency department within the study period. A total of 40.5% of patients were hospitalized in the clinics or intensive care units. The rate of mortality at first admission was 1.5% (12 patients). The most common complaints at admission were infection (13.3%) and soft tissue traumas (crashes, sprains, fractures, dislocations) related to the musculoskeletal system (11.8%). Readmissions were more frequent in males. It was seen that male patients were more frequently admitted due to deterioration of general health status and genitourinary system symptoms, despite pain being more common among women. Conclusion: Due to the changing population structure, emergency staff in Turkey as well as internationally encounter old and very old patients more frequently. It is now imperative that emergency departments acquire the necessary infrastructure, human resources, knowledge, and equipment needed to meet the needs of these patients. [ABSTRACT FROM AUTHOR]

5. Retrospective examination of complications observed in orthognathic surgical surgery in 85 patients.

BACKGROUND: The aim of our study is to examine the possible complications, risk factors, and solutions encountered in orthognathic surgery in the light of the cases; we performed in our clinic. METHODS: This study includes a retrospective analysis of the records of 85 patients who underwent orthognathic surgery between 2015 and 2020 in Istanbul Pendik District Hospital Oral and Maxillofacial Surgery Service. The types of complications encountered during the operations were recorded in the study. Independent variables such as gender, age, number of operations, surgical site, and type of osteotomy were evaluated. Complications were evaluated according to the Clavien--Dindo classification. The data were presented for statistical analysis with a significance level of 0.05. RESULTS: Of the 85 patients included in the study, 40 were male and 45 were female. Of these patients, 65 had double chin operation and 20 had single chin operation. A total of 150 jaw osteotomies were performed, 78 of them in the maxilla and 72 in the mandible. While the maxilla was operated in 13 of the cases, in which single jaw operation was performed, only the mandible was treated in 7 of them. Complications were observed in 24 (10.57%) of a total of 227 osteotomies. Among the complications encountered, bleeding (8), nerve damage (7), malocclusion (3), infection (2), TMJ problems (2), bad split (1), and deviation at the tip of the nose (1) stand out, while complications were observed equally in men and women, complications were observed more frequently in cases with longer operative time (p<0.05). Complications were observed more frequently in bilateral sagittal split osteotomies (p<0.001) compared to Le Fort 1 osteotomies. Clavien--Dindo grade I complications were most common (72.04%) depending on the treatment. According to the Clavien--Dindo classification, there was no relationship between gender, age, duration of surgery, length of hospital stay, or surgical site, and the degree of complications (p≥0.05). CONCLUSION: Post-operative malocclusion, bleeding, inferior alveolar nerve injury, infection, poor division, and infection are the most common complications in orthognathic surgery. It can be associated with factors such as the duration of the operation, the number of operations, the site of the operation, and the type of osteotomy which performed. It is thought that positive contributions can be made to the success of the surgery by considering these factors in the treatment planning, during the operation and in the post-operative follow-up part. - Abstract
This study reviews and analyzes family planning legislation in seven countries of the Mediterranean region: Greece, Italy, Morocco, Portugal, Spain, Tunisia, and Turkey. Part 1, a general review, specifically focuses on the role of religion in the development of family planning programs, laws with an indirect effect on family planning (minumum age of marriage, polygamy, divorce, incentives and disincentives), laws with a direct effect on family planning (contraception, voluntary sterilization, induced abortion), and teenagers and family planning (family planning services, voluntary sterilization, induced abortion, sex education). Part 2 contains country profiles of the seven countries involved in the study. Recommendations specific to the country conclude each profile. Part 3 offers conclusions and recommendations regarding legislation with an indirect effect on family planning, legislation with a direct effect on family planning, family planning for adolescents, and conferences. Appendices include a bibliography, a summary of laws affecting family planning, and population facts. (YLB)

Background: Affecting 106 countries, malaria is a major global burden. Though intensive antimalaria efforts in Turkey have been successful in bringing down the number of cases, historically malaria was a serious public health concern., Methods: This paper reviews the prevalence rates of malaria in Turkey over the last 85 years (1925-2010). The time series of malaria prevalence was evaluated for possible structural changes by using Chow breakpoint tests and regression models using dummy variables, with autocorrelated errors and generalized autoregressive conditional heteroscedasticity models to assess the impact of volatility in prevalence., Results: Seventy-eight cases of malaria were diagnosed in Turkey in 2010. Malaria prevalence rates in the country show a statistically significant volatility, which underlines the fragility of efforts to control the disease., Conclusions: It is necessary to analyse the national malaria control programme to evaluate to what extent its programmatic capacity, financial resources, and political commitment are sufficient to avoid eroding the gains that have already been made and, ultimately, eradicate malaria. It is essential that there should be no lessening in the long-standing efforts to reduce malaria.

Background: Anthrax is a zoonotic disease diminishing worldwide. Although a very rare disease in developed countries, anthrax is still endemic in developing countries., Aim: To evaluate the clinical history and features, treatment and outcome of our patients with anthrax and emphasize the importance of the disease in our region., Method: In this study, the records of all patients diagnosed with cutaneous anthrax admitted to Yüzüncü Yil University Medical Faculty between March 2002 and March 2007, were reviewed, and data on age, gender, occupation, clinical symptoms and findings, location and type of lesions, clinical history, laboratory findings, treatment and outcome were recorded., Results: There were 85 patients [46 (54.1%) male, 39 (45.9%) female; mean age 30.6 years, range 6-72]. All the patients had a history of contact with infected animals or animal products. The infectious agent was found using direct examination of Gram-stained smears from 17 patients (20%), and Bacillus anthracis was isolated from vesicle fluid cultures from 11 patients (12.9%). Diagnosis was based on clinical findings in the remaining 57 patients (67.1%). All patients, except for two with respiratory tract obstruction, made a full recovery., Conclusion: Anthrax continues to be an endemic disease in Turkey, and should be considered in patients with a relevant contact history having a painless ulcer and vesicular skin lesions surrounded by a zone of oedema. Gram staining and simple culture methods are useful aids to diagnosis, but diagnosis may have to be based on clinical grounds in the majority of patients.

13. The Turkish version of the Skin Tear Knowledge Assessment Instrument (OASES): Validity and reliability study.

This study was conducted to adapt the Skin Tear Knowledge Assessment Instrument (OASES) into Turkish and to verify its validity and reliability. This study was conducted on 314 nurses in Türkiye between November 2023 and February 2024 to test the psychometric properties of OASES. The instrument consists of 20 items clustered into six domains. The cultural adaptation process was carried out according to the International Testing Commission guidelines: Turkish translation, expert panel, content validity, translation back to English, preliminary study and the final version of the instrument. To check the validity of the multiple‐choice test, item difficulty and discriminating index were analysed. The reliability of the instrument was evaluated to the retest 14 days after the first test. Scale level content validity by 11 experts in wound care was 0,97 (I‐CVI = 0.8–1.0). In the item analysed of the OASES, the item difficulty index was 0,51 (p‐value = 0.34–0.76) and the discriminating index was 0.40 (D‐value = 0.26–0.51). The 2‐week test‐retest intraclass correlation coefficient of the overall instrument was 0.90 (95% CI = 0.79–0.95). The Turkish version of OASES is a valid and reliable measurement instrument to evaluate nurses' knowledge levels regarding skin tears with acceptable psychometric properties. It can be applied in nursing education, research and practice to evaluate the knowledge of Turkish speaking nurses about skin tears. [ABSTRACT FROM AUTHOR]

14. Effects of a Multidisciplinary Therapeutic Approach to "Diabetic Foot" Complications: A Single-Center Pilot Study from Turkey.

Objectives: The prevalence of diabetes mellitus is rapidly increasing in society. Every year, millions of people suffer from diabetic foot complications. This study was conducted to evaluate the effectiveness of a multidisciplinary approach in diabetic foot treatment. Methods: A total of 85 cases with diabetic foot complications were analyzed in the study. Besides, we evaluated patients' personal information, any morbidity, the consultations they asked for while planning their treatment, the surgical options they preferred, the SINBAD and PEDIS scores they had when they were admitted to the hospital, and how these scores related to each other. Results: The number of male patient was higher (66, 77%) and the average age of all patients was 67.4 (40-93) years. The frequency of chronic ischemic heart disease was found to be 96% among the patients and also 51 patients (60%) underwent peripheral vascular interventional angiography. Wound debridement was performed on 38 patients (44%), and various levels of amputations were conducted on 47 patients. While 39 of the patients received hyperbaric oxygen therapy (HBOT) and 34 received vacuum-assisted wound closure (VAC) system treatment. Conclusion: This single clinical study demonstrates that a multidisciplinary approach is an effective strategy for treating diabetic foot complications. [ABSTRACT FROM AUTHOR]

Objectives: This study aimed to evaluate the neuropathic component of chronic musculoskeletal pain in post-coronavirus disease 2019 (COVID-19) and examine the relationship between neuropathic pain and clinical and demographic characteristics. Patients and methods: This cross-sectional study included 163 adult patients (85 females, 78 males; mean age: 41.7±4.3 years; range, 22 to 50 years) with post-COVID-19 musculoskeletal pain between February 1, 2021, and April 30, 2021. Demographic and clinical characteristics, including age, sex, affected site, duration, and severity of post-COVID-19 musculoskeletal pain using the Visual Analog Scale (VAS), as well as a neuropathic component of pain using the Leeds assessment of neuropathic symptoms and signs (LANSS), were collected. The most common post-COVID-19 symptoms, presence of hospitalization, and length of hospital stay during active COVID-19 infection were recorded from the patient records. Results: The mean duration and severity of pain were 7.85±1.53 months and 5.09±1.95, respectively. Half of the patients were hospitalized, and the mean length of hospital stay was 12.15±18.06 days. The most common pain sites were upper and lower back pain, followed by leg and arm pain. A total of 92 (56.4%) patients had previously received pharmacological or nonpharmacological treatment for post-COVID-19 musculoskeletal pain. Based on the LANSS (scores >12), 31 (19%) patients had neuropathic pain. There was a significant correlation between the presence of neuropathic pain and pulmonary involvement/symptoms. The presence and length of hospital stay were correlated with LANNS scores (p<0.05). The frequency, LANSS scores, and VAS-pain scores of the patients with and without neuropathic pain were similar between male and female patients (p>0.05). Conclusion: The neuropathic component of chronic musculoskeletal pain may be common, as one-fifth of our patients had neuropathic pain as assessed by the LANNS. Therefore, the awareness of post-COVID-19 chronic neuropathic musculoskeletal pain should be increased. We believe that focusing on the identification of pain phenotypes would provide adequate and tailored chronic neuropathic musculoskeletal pain management in the post-COVID-19 period. Aim: This article aims to assess Mental Health Literacy (MHL) in dialysis patients and examine their impact on anxiety, depression and physical health. Material and Method: The study was conducted in the Hemodialysis Unit of Karaman Training and Research Hospital between March and June 2024. A total of 87 patients who met the inclusion criteria and agreed to participate were included in the study. Data were collected using a semi-structured sociodemographic form, Mental Health Literacy Scale, Hamilton Depression Scale and Hamilton Anxiety Scale. Biochemical parameters were obtained from electronic patient records. Results: The frequency of depression was 45%. There was no significant difference in MHL points among individuals with and without depression. Furthermore, no correlation was found between MHL and duration of disease or dialysis. A significant relationship was found between Total and Resource subscale scores of MHL and estimated glomerular filtration rate (eGFR). Conclusion: The frequency of depression was higher in dialysis patients than in the general population. Albeit no significant difference in MHL was observed between patients diagnosed with depression and those without, the significant relationship between MHL and eGFR suggests that interventions to increase MHL may have positive effects on disease progression. Aim: The aim of this study was to compare inpatient and outpatient cases diagnosed with autism spectrum disorder (ASD) and to show which factors were more associated with hospitalization in children and adolescents. Materials and Methods: We included 85 outpatient and 34 inpatient children and adolescents with ASD. Psychiatric diagnoses were assessed with the K-SADS-PL. Factors relevant to the hospitalization of ASD cases were evaluated with a logistic regression model. Results: The Childhood Autism Rating Scale scores (p=0.002), comorbid psychopathology (p=0.04), multiple psychotropic medication (p=0.001), and psychopathology of the parents (p<0.001) and siblings (p=0.029) were higher, and the education levels of the mothers (p=0.017) were lower in the inpatient-ASD group. The logistic regression model showed that the psychopathology of the mother (odds ratio=10.293, p<0.001) was associated with psychiatric hospitalization. Conclusion: It is very important to provide psychiatric and psychosocial support especially to mothers during the inpatient treatment process in addition to outpatient family support. As global migration and transnational mobility have increased steadily in the recent few decades, interests in equity-based theories and pedagogies have intensified to respond to racially and linguistically diverse student needs in today's classrooms. Raciolinguistic ideology is a theoretical framework challenging monoglossic language ideologies and the 'White gaze' that privileges White speaking and listening subjects (Flores, N., & Rosa, J. (2015). Undoing appropriateness: Raciolinguistic ideologies and language diversity in education. Harvard Educational Review, 85(2), 149–171. ). While raciolinguistic ideologies aim to critically examine the idealized monolingualism, the framework is typically adopted to analyze racial realities in Western or European educational contexts. In this study, we intend to (re)imagine raciolinguistic ideologies situated in non-Western educational contexts by examining language-in-education policies in Turkey and South Korea. Using raciolinguistic ideologies and Kaplan and Baldauf's (Kaplan, R. B., & Baldauf, R. B., Jr. (2005). Language-in-education policy and planning. In E. Hinkel (Ed.), Handbook of research in second language teaching and learning (pp. 1013–1034). Lawrence Erlbaum) language-in-education planning framework as our analytical lenses, we interrogate raciolinguistic ideologies manifested through language-in-education policy and planning in Turkey and South Korea, where transnational mobility and diversity are drastically increasing to shape language education. The analyses suggest broadening the definition and viewpoint of race to understand space-specific and localized interpretations of raciolinguistic ideologies while dismantling new contemporary racism in non-Western educational contexts. The current biodiversity of 14 phyla, namely Platyhelminthes, Xenacoelomorpha, Nematoda, Acanthocephala, Brachiopoda, Kinorhyncha, Nemertea, Chaetognatha, Tardigrada, Gastrotricha, Rotifera, Phoronida, Echinodermata, and Chordata (invertebrates only, including the subphyla Tunicata, Cephalochordata, and Hemichordata) from the coasts of Türkiye is revisited. It was found that a total number of 695 species occur along all the coastlines of Türkiye, with the Black Sea being represented by 297 species, the Sea of Marmara by 305 species, the Aegean Sea by 308 species, and the Levantine Sea by 204 species. The most species-rich phyla in these regions were Platyhelminthes (249 species), Nematoda (131), Echinodermata (98), and Chordata (85). A total of 26 alien species have been reported from the regions to date, of which one species belongs to Chaetognatha, 8 species to Echinodermata, and 17 species to Tunicata. Background: Many studies on multiple sclerosis (MS) reveal different prevalence and epidemiologic results. Objectives: In this study, we aimed to determine the epidemiologic profile of MS using official health records in Turkey. Methods: Patients diagnosed with MS from the official health data of the Ministry of Health, representing the entire population of Turkey, were included in the study. Prevalence and incidence calculations were performed using the data on gender, age, year of birth, city of residence, and year of diagnosis. Results: As a result of the study, the number of patients with the ICD code G35 was determined as 201,061 and the number of patients with this code entered at least three times was determined as 82,225. The prevalence of MS in Turkey was calculated as 96.4 per 100,000 and the female/male ratio as 2.1/1. The incidence of MS in 2022 was 6.2 per 100,000 and the mean patient age was 43.1 ± 13.3 years (female: 43.0 ± 13.1 vs male: 43.2 ± 13.7) while the mean age at first diagnosis was 34.0 ± 13.0 (female: 33.6 ± 12.6 vs male: 34.9 ± 13.7). Conclusion: The research was conducted via Official Database of Turkey, which includes population of 85 million and provides valuable insights into the prevalence and incidence rates of this chronic disease. 42. Prevalence, characterization and antibiotic resistance of Staphylococcus aureus isolated from traditional cheeses in Turkey.

This study aimed to provide information about the characteristics of Staphylococcus aureus strains isolated from traditional Turkish cheese samples in Ankara (Turkey), focusing on their prevalence, phenotypic and genotypic characteristics, and antibiotic resistance. To this end, 387 cheese samples were randomly collected from various local bazaars and supermarkets. A total of 85 S. aureus isolates were identified by morphological, biochemical and molecular methods. In addition, the strains were screened for enterotoxin production. None of the strains was confirmed as being enterotoxigenic. The strains were tested for resistance to 29 different antimicrobial agents using the phenotyping agar diffusion method. The strains showed a high level of resistance (60-84.71%) to penicillin, ampicillin, and oxacillin. Furthermore, resistance to lincomycin, cefoxitin, clindamycin was also observed, although at slightly lower levels (50.59-57.65%). Additionally, low-level resistance (1.4-48.8%) was observed against gentamicin, methicillin, kanamycin, streptomycin, cefepime, erythromycin, teicoplanin, rifampicin, vancomycin, cefazolin, netilmicin, sulfadimethoxazole, ofloxacin, enrofloxacin, tetracycline, ciprofloxacin, azithromycin, trimethoprim, and norfloxacin. None of the strains was resistant to doxycycline, linezolid, furazolidone, and chloramphenicol. About 72.94% of S. aureus strains were resistant to multiple drugs. Our results suggest that the presence of a high level of multidrug-resistant S. aureus strains in traditional Turkish cheese is not safe for human consumption. Menopausal status affects the prognoses and consequences of breast cancer. Therefore, this retrospective study aimed to reveal the molecular variation profile differences in breast cancer patients according to their menopausal status, with the hypothesis that the molecular variation profiles will be different at premenopausal and postmenopausal ages. Breast cancer patients (n = 254) who underwent molecular subtyping and QIAseq Human Breast Cancer NGS Panel screening between 2018 and 2022 were evaluated retrospectively. Their menopausal status was defined by age, and those aged 50 years and above were considered postmenopausal. Of the subjects, 58.66% (n = 149) were premenopausal and 41.34% (n = 105) were postmenopausal. The mean age at the time of diagnosis for all patients was 49.31 ± 11.19 years, with respective values of 42.11 ± 5.51 and 59.54 ± 9.01 years for the premenopausal and postmenopausal groups, respectively (p = 0.000). Among premenopausal patients, the percentages of patients in BCa subtypes (luminal A, luminal B-HER2(−), luminal B-HER2(+), HER2 positive, and triple-negative) were determined to be 34.90%, 8.05%, 26.17%, 10.74%, and 20.13%, respectively, while in the postmenopausal group, these values were 39.05%, 16.19%, 24.76%, 6.67%, and 13.33%, respectively (p > 0.05). Considering menopausal status, the distribution of hormone receptors in premenopausal patients was ER(+)/PgR(+) 63.76%, ER(−)/PgR(−) 23.49%, ER(+)/PgR(−) 10.74%, and ER(−)/PgR(+) 2.01%, respectively, while in postmenopausal women, this distribution was observed to be 74.29%, 23.81%, 1.90% and 0.00% in the same order (p = 0.008). The most frequently mutated gene was TP53 in 130 patients (51.18%), followed by PIK3CA in 85 patients (33.46%), BRCA2 and NF1 in 56 patients (22.05%), PTEN in 54 patients (21.26%), and ATR and CHEK2 in 53 patients (20.87%). TP53, PIK3CA, NF1, BRCA2, PTEN, and CHEK2 mutations were more frequently observed in premenopausal patients, while TP53, PIK3CA, BRCA2, BRCA1, and ATR mutations in postmenopausal patients. These findings contribute to a deeper understanding of the underlying causes of breast cancer with respect to menopausal status. This study is the first from Turkey that reflects the molecular subtyping and somatic mutation profiles of breast cancer patients according to menopausal status. [ABSTRACT FROM AUTHOR]
