Researchers have traditionally conceptualized hazards that give rise to disasters as "natural" or "technological." An extensive literature has documented differential social consequences based on this distinction, including the emergence of corrosive community dynamics in the context of technological disasters. There is also growing recognition that many disasters can be conceptualized as "natech"—processes characterized by a combination of natural and technological hazards. On August 25, 2017, Hurricane Harvey made landfall along the central Texas Gulf Coast, causing catastrophic flooding and extensive releases of industrial toxins. We examined variation in institutional trust and future storm worry in the aftermath of Harvey, paying special attention to differences between those who viewed the disaster as being primarily natech and natural. Drawing on the Survey of Trauma, Resilience, and Opportunity in Neighborhoods in the Gulf, we analyzed two waves of cohort panel data collected from households on the Texas Gulf Coast in 2016 and 2018 (before and after Hurricane Harvey). Our findings showed that those who perceived Harvey as natech (compared to natural) were significantly more likely distrust major institutional actors and be worried about the impacts of future storms, even after accounting for pre-hurricane characteristics. Elastic wave propagation experiments were performed on a thin bar sample composed of Texas "moss" sandstone in order to study nonlinear elastic effects in the time domain. The present experiments utilized a pendulous hammer to produce axially propagating transient signals with strain amplitude between 15 and 130 microstrain in the mid-audio band. Particle velocity along the bar axis was measured with a laser Doppler vibrometer, focused at various locations along the bar. Nonequilibrium dynamics and nonlinear elasticity effects were observed on the propagating pulse as it reflected between the ends of the bar. The same effects were also observed at a single location along the mid-length of the bar using a continuous-wave 1 MHz probe signal, propagating transverse to the bar axis. The results demonstrate larger strain amplitudes and greater reduction in the Young's modulus than in previously reported measurements that employed narrowband excitation. Nonlinear attenuation of the axial pulse is also observed, which increases with excitation amplitude. The present results indicate significant conditioning of the sandstone, particularly "softening" of the Young's modulus of up to 20%, primarily during the tensile phase of propagation, with a "slow dynamic" memory that is similar to that reported in previous investigations. Background/aims: In our 2009 article, we showed that Bayesian methods had established a foothold in developing therapies in our institutional oncology trials. In this article, we will document what has happened since that time. In addition, we will describe barriers to implementing Bayesian clinical trials, as well as our experience overcoming them. Methods: We reviewed MD Anderson Cancer Center clinical trials submitted to the institutional protocol office for scientific and ethical review between January 2009 and December 2013, the same length time period as the previous article. We tabulated Bayesian methods implemented for design or analyses for each trial and then compared these to our previous findings. Results: Overall, we identified 1020 trials and found that 283 (28%) had Bayesian components so we designated them as Bayesian trials. Among MD Anderson–only and multicenter trials, 56% and 14%, respectively, were Bayesian, higher rates than our previous study. Bayesian trials were more common in phase I/II trials (34%) than in phase III/IV (6%) trials. Among Bayesian trials, the most commonly used features were for toxicity monitoring (65%), efficacy monitoring (36%), and dose finding (22%). The majority (86%) of Bayesian trials used non-informative priors. A total of 75 (27%) trials applied Bayesian methods for trial design and primary endpoint analysis. Among this latter group, the most commonly used methods were the Bayesian logistic regression model (N = 22), the continual reassessment method (N = 20), and adaptive randomization (N = 16). Median institutional review board approval time from protocol submission was the same 1.4 months for Bayesian and non-Bayesian trials. Since the previous publication, the Biomarker-Integrated Approaches of Targeted Therapy for Lung Cancer Elimination (BATTLE) trial was the first large-scale decision trial combining multiple treatments in a single trial. Since then, two regimens in breast cancer therapy have been identified and published from the cooperative Investigation of Serial Studies to Predict Your Therapeutic Response with Imaging and Molecular Analysis (I-SPY 2), enhancing cooperation among investigators and drug developers across the nation, as well as advancing information needed for personalized medicine. Many software programs and Shiny applications for Bayesian trial design and calculations are available from our website which has had more than 21,000 downloads worldwide since 2004. Conclusion: Bayesian trials have the increased flexibility in trial design needed for personalized medicine, resulting in more cooperation among researchers working to fight against cancer. Background: Nearly 50% of gastric cancer patients are diagnosed with advanced gastric cancer (AGC). Therapy is palliative but results in ill effects. The median overall survival (OS) of AGC patients is often <12 months. It is unclear if the early initiation of therapy in all AGC patients is beneficial. Methods: A retrospective analysis of AGC patients in our database was carried out. The patients were divided into two groups: asymptomatic or symptomatic. We sought to assess whether the delay of systemic therapy was harmful in asymptomatic patients. Results: A total of 135 patients were analyzed. Most patients were symptomatic (68%), males (67%), and had low ECOG scores (0-1; 85%). In univariate analyses, ECOG performance status 0 (p = 0.005), delayed initiation of therapy (p = 0.03), and lack of symptoms (p = 0.03) were associated with a longer OS. The multivariate model for OS identified only ECOG performance status as an independent prognosticator of longer OS (p = 0.02). Asymptomatic patients who had delayed (≥4 weeks) systemic therapy had an OS rate of 77% at 1 year compared to 58% for patients treated within 4 weeks (p = 0.47). Conclusion: Symptomatic AGC patients had a poor outcome compared to asymptomatic AGC patients. Background/Aim: Malignant nodes in patients with localized esophageal adenocarcinoma (L-EAC) portend a poor prognosis. We assessed the correlation of the distribution of nodes with the outcome of patients undergoing chemoradiation/surgery (trimodality therapy). Methods: We studied 209 L-EAC patients who had confirmed or suspicious nodes at baseline staging. All patients received trimodality therapy and were grouped according to the nodal geography: above the diaphragm (AD), below the diaphragm (BD), or above and below the diaphragm (ABD). Survival estimates were calculated using the Kaplan-Meier method, and the outcomes of the groups were assessed by the log-rank test. Results: Patients were primarily Caucasian (91%) and male (93%), with a baseline stage III L-EAC (89%). The median follow-up was 2.8 years (range, 0.4-11.7). Of the 209 patients, 35% (n = 73) had AD nodes, 20% (n = 41) had BD nodes, and 45% (n = 95) had ABD nodes. ABD patients had a 5-year overall survival rate of 33%, whereas this rate was 55% in AD patients and 60% in BD patients (p = 0.02). Patients with a higher histology grade were also at a higher risk of relapse and had a poor survival (p < 0.01 for both). Conclusions: L-EAC patients in the ABD group had the worst outcome after trimodality treatment compared to those in the AD or BD group. Novel strategies are needed for ABD patients. © 2015 S. This study examines the structural changes that occurred between 1990 and 2000 in the two poorest regions in the United States—the Texas Borderland and the Lower Mississippi Delta—and analyzes the associations between these changes and the substantial reduction of poverty witnessed in these areas over the decade. Results show that both regions experienced noteworthy changes during the 1990s in the aggregate characteristics commonly associated with poverty. Further, results from multivariate models indicate that counties with high initial poverty rates saw poverty decline the most, with job growth and net in-migration contributing to this decline. Conversely, smaller declines in the population younger than age 15 and greater increases in the share of families headed by females were associated with lesser reductions in poverty over the decade. The results also reveal that the underlying mechanisms influencing the reduction of poverty differed between these regions in important respects. Increases in finance, insurance, and real estate employment and educational attainment were significantly less beneficial for the Borderland, whereas the growth of the minority population was linked to significantly better outcomes for that region in comparison to the Delta. We provide a comparative analysis of county-level poverty in the two poorest regions of the United States--the Texas Borderland and the Lower Mississippi Delta--with a special focus on differences by family type. Our results reveal important regional variation in both the prevalence of poverty and the composition of the poor population across major family types. Using OLS regression models of family type-specific poverty we demonstrate three key findings: 1) There are no significant regional differences in poverty levels by family type between the Borderland and the Delta, net of important structural factors that characterize the regions; 2) with the exception of the employment rate, the structural factors associated with poverty among married couple and single female-headed families are quite different; and 3) paradoxically, areas in the Borderland with high in-migration are especially likely to suffer from high married-couple family poverty. Summary This article is a recalling of the feelings that I experienced when I participated in two interviews with Carl Rogers in a summer workshop at Texas Woman's University. After the last day of the workshop, I viewed video tapes of the two interviews and then noted my impressions and feelings about the entire experience, both as a participant and as a spectator. Padre Island National Seashore provides important habitat for declining wintering grassland birds, yet oil and gas development is widespread in the Park. Our objective was to evaluate the effects of resource extraction on the wintering grassland birds of the Park. In January-March 2007 and 2008, we surveyed 5 active and 4 abandoned well sites and 4 road sites to investigate the relationship between distance from disturbance (well pads, access roads) and bird abundance. At road sites, bird abundance was positively correlated with increased distance from road edge, but we found no linear relationship at active or abandoned well sites. However, mean bird abundance in the first (0-30 m) distance interval of active well transects was less than half that at the second interval, and was the lowest value for all active intervals except the ninth. First-interval abundance at active wells was lower than abundance at any abandoned well interval. Road transects likewise showed low abundance in the initial interval, although unlike at active wells, abundance increased steadily with distance from the center of disturbance. This trend of lower overall numbers at the first interval of active well transects was driven largely by Sturnella magna (Eastern Meadowlark) and S. neglecta (Western Meadowlark). A combination of high noise levels near active well pads and lack of tall vegetation from which to sing may have contributed to low numbers of meadowlarks, which were the only birds to sing regularly during the study period. We recommend: (1) reducing noise at active sites, (2) limiting vegetation disturbance near pads and roads, (3) maintaining existing perch sites, and (4) limiting road construction. • Anti- Trypanosoma cruzi antibodies were detected in 11% of stray cats from South Texas. • T. cruzi DNA distributed in multiple cat organs. • Significant lesions were observed more frequently in seropositive cats. • Cats may be important in the eco-epidemiology of Chagas disease. Trypanosoma cruzi is a zoonotic protozoan parasite transmitted by triatomines that infects a wide range of mammals. South Texas is a hotspot for triatomines, T. cruzi -infected dogs and wildlife, and local transmission to humans also occurs. However, little is known about the infection of domestic cats (Felis catus) in the United States. Given the role cats play in the ecology of T. cruzi in Mexico and South America, we hypothesized that T. cruzi infection occurs in cats from south Texas, sometimes associated with cardiac pathology. In 2017, 167 euthanized cats from a south Texas shelter were sampled across winter, spring, and summer. We collected whole blood and hearts from all cats, with additional tissues from a subset. Serum samples were screened for T. cruzi antibodies using two independent rapid immunochromatographic tests and an indirect fluorescent antibody test. Cats were considered seropositive if they were positive on at least two independent serological tests. Blood clot, heart tissue and other tissues were subjected to qPCR for parasite detection and discrete typing unit (DTU) determination. Tissues from selected seropositive or PCR-positive animals and a subset of negative animals were processed routinely for histopathology and examined by a board-certified pathologist. A total of 19 cats (11.4%) were seropositive and three cats (1.8%) – one of which was seropositive - had one or more PCR-positive tissues. Infected tissues included heart, bicep femoris muscle, sciatic nerve, esophagus, and mesentery. Genotyping of the parastite to the level of DTU showed that exclusively DTU TcI was present, despite past studies showing both TcI and TcIV in vectors of the region. Eight of 19 (42.1%) seropositive cats exhibited lymphoplasmacytic inflammation, sometimes with fibrosis, in cardiac tissue compared to 28.6% of 28 seronegative cats (P = 0.10). Domestic cats are affected hosts in the eco-epidemiology of Chagas disease. Future prospective studies are needed to understand disease progression. Veterinarians in the southern United States should consider T. cruzi in their index of suspicion in cats with exposure to vectors and undetermined cardiac abnormalities. [ABSTRACT FROM AUTHOR]
